Presence of dogs and proximity to a wildlife reserve increase household level risk of tungiasis in Kwale, Kenya.

INTRODUCTION: Tungiasis is a ectopic skin disease caused by some species of fleas in the Tunga genus, most notably T. penetrans. The disease afflicts poor and marginalized communities in developing countries. Transmission of tungiasis comprises a complex web of factors including domesticated animals and wildlife. This research explores animal and environmental risk factors for tungiasis in an area adjacent to a wildlife reserve in Kwale, Kenya. METHODS: A two-stage complex sampling strategy was used. Households were selected from three areas in and around Kwale Town, Kenya, an area close to t

Tungiasis, a rare case of plantar inflammatory disease, a review of travelers skin lesions for emergency providers.

Parasitic infections while common in underdeveloped nations are rarely seen in developed urban centers. We report a case of a thirty-three-year-old male with no past medical history who presented to the emergency department with a chief complaint of "eggs coming out of my foot" after returning home from Brazil. Based on clinical presentation, travel history, and appearance of the lesion, diagnosis was most consistent with tungiasis infection which was confirmed by the pathology examination. It is important to make the appropriate diagnosis when skin lesions are found in returning travelers and

[Effectiveness of 20% salicylated vaseline in the treatment of profuse tungiasis. Report of 8 cases in French Guiana].

The treatment of tungiasis involves the mechanic extraction of the gravid females of Tunga penetrans. We have observed on 8 patients profuse tungiasis diagnosed in the centre hospitalier de Cayenne. We have carried out different types of treatment, including the occlusive application of 20% of salicylated vaseline during 12 or 24 hours. This procedure causes the death of the parasites and facilitates their extraction.

Distribution of tungiasis in latin America: Identification of areas for potential disease transmission using an ecological niche model.

BACKGROUND: Tungiasis is a neglected tropical disease (NTD) found in Sub-Saharan Africa and Latin America. Despite the high frequency in marginalized populations, little information is available on the geography and estimates of the population at risk in endemic regions. Here we used a geostatistical model to map the potential geographic distribution of areas suitable for tungiasis transmission in Latin America and estimated the at-risk population. METHODS: We developed an ecological niche model (ENM) using tungiasis occurrence records and remotely sensed environmental and socioeconomic data.

Successful Treatment of Severe Tungiasis in Pigs Using a Topical Aerosol Containing Chlorfenvinphos, Dichlorphos and Gentian Violet.

BACKGROUND: In endemic communities, zoonotic tungiasis, a severe skin disease caused by penetrating female sand fleas, is a public health hazard causing significant human and animal morbidity. No validated drugs are currently available for treatment of animal tungiasis. Due to the reservoir in domestic animals, integrated management of human and animal tungiasis is required to avert its negative effects. METHODS AND PRINCIPAL FINDINGS: A topical aerosol containing chlorfenvinphos 4.8%, dichlorphos 0.75% and gentian violet 0.145% licensed to treat tick infestations, myiasis and wound sepsis in

Regression of severe tungiasis-associated morbidity after prevention of re-infestation: a case series from rural Madagascar.

Tungiasis (sand flea disease) is a neglected tropical disease. Heavy infestation results in mutilation of the feet and difficulty in walking. We identified eight individuals with extremely severe tungiasis in rural Madagascar. To prevent reinfestation, four individuals received solid shoes and four received a daily application of an herbal repellent effective against Tunga penetrans. Over a period of 10 weeks the feet were examined and the severity of tungiasis-associated morbidity was measured. Within this period, the severity score for acute tungiasis decreased 41% in the shoe group and 89%

Estimation of the global burden of tungiasis in endemic countries using a disability adjusted life-years modelling approach.

BACKGROUND: Tungiasis is a neglected tropical zoonotic skin disease which is endemic to Sub-Saharan Africa and Latin America, caused by sand fleas of the genus Tunga, which burrow into the skin of affected humans and animals, most commonly when the feet come in contact with infested soil. Sand flea infection leads to inflammation, severe itching, and pain, with the potential for secondary bacterial or viral infections, gangrene, and disfigurement. Tungiasis disproportionately affects poor rural communities, and individuals living in unhygienic settlements. To date, the existing burden of tungi

Environmental and Social Factors Associated with the Occurrence of Severe Tungiasis and Scabies in the State of Ceará, Brazil: An Ecological Study.

Scabies and tungiasis are skin-related neglected tropical diseases (NTDs) associated with poverty and poor living conditions. We performed an ecological study covering a state in northeast Brazil to identify socio-economic and environmental factors associated with the occurrence of severe scabies and severe tungiasis, respectively. Data on disease occurrence on the municipality level were derived from a previous study based on online questionnaires. A total of 47 (26.0%) of the 181 state's municipalities reported severe tungiasis, and 113 (62.4%) severe scabies. Municipalities with occurrence

Seasonal variation and persistence of tungiasis infestation in dogs in an endemic community, Bahia State (Brazil): longitudinal study.

Tungiasis is a zoonosis neglected by authorities, health professionals, and affected populations. Domestic, synanthropic, and sylvatic animals serve as reservoirs for human infestation, and dogs are usually considered a main reservoir in endemic communities. To describe the seasonal variation and the persistence of tungiasis in dogs, we performed quarterly surveys during a period of 2 years in a tourist village in the municipality of Ilhéus, Bahia State, known to be endemic for tungiasis. Prevalence in dogs ranged from 62.1% (43/66) in August 2013 to 82.2% (37/45) in November 2014, with no sig

Factors associated with tungiasis among primary school children: a cross-sectional study in a rural district in Rwanda.

BACKGROUND: Tungiasis is a relatively frequent ectoparasitosis in low-income settings, yet its morbidity and social impact are still not well understood due to the scarcity of information. In Rwanda, data on the magnitude and conditions leading to the tungiasis is rare. This study sought to determine the prevalence and factors associated with tungiasis among primary school children in Rwandan setting. METHOD: A descriptive cross-sectional study utilising systematic random sampling method was adopted to select 384 children from three primary schools. From July to October 2018, data were collec

Investigations on the biology, epidemiology, pathology and control of Tunga penetrans in Brazil: I. Natural history of tungiasis in man.

Tungiasis is an important health problem in poor communities in Brazil and is associated with severe morbidity, particularly in children. The causative agent, the female flea Tunga penetrans, burrows into the skin of its host, where it develops, produces eggs and eventually dies. From the beginning of the penetration to the elimination of the carcass of the ectoparasite by skin repair mechanisms, the whole process takes 4-6 weeks. The present study is based on specimens from 86 patients, for some of whom the exact time of penetration was known. Lesions were photographed, described in detail an

[On a new species of sand flea from Ecuador and tungiasis, a problem of Public Health in many developing countries].

The authors recently described a new species of sand flea (genus Tunga) occurred in goat, sheep, cow, pig and man, in the Andean village of S.ta Isabel (Ecuador) and named Tunga trimamillata. Its most important morphological characteristics are: slightly larger dimensions than T. penetrans, both in male and female; presence on the anterior extremity of gravid female of three rounded humps surrounding head and thorax; length of the first segment of maxillary palpi which is longer than each of the other three. The importance of this parasite is due to the fact that it can cause the same damages

Protocol for a Randomized Control Trial for Tungiasis Treatment in Homa Bay County, Kenya: Dimeticone versus Sodium Carbonate.

Tungiasis, a World Health Organization neglected tropical disease, is caused by the female sand flea. Most clinical trials for tungiasis use expensive or impractical drugs, which are difficult for residents to use. However, in western Kenya, communities successfully treat tungiasis with sodium carbonate. We hypothesise that the topical risk-difference of 5% sodium carbonate is no more than 10% non-inferior to dimeticone (NYDA®) for tungiasis treatment. This is a protocol for a non-inferiority study, which will be randomised and with an observer-blinded control. The study will have two arms: 5%

Prevalence, intensity and risk factors of tungiasis in Kilifi County, Kenya: I. Results from a community-based study.

BACKGROUND: Tungiasis is a neglected tropical disease caused by female sand fleas (Tunga penetrans) embedded in the skin. The disease is associated with important morbidity. Tungiasis is endemic along the Coast of Kenya with a prevalence ranging from 11% to 50% in school-age children. Hitherto, studies on epidemiological characteristics of tungiasis in Africa are scanty. METHODS: In a cross-sectional study 1,086 individuals from 233 households in eight villages located in Kakuyuni and Malanga Sub-locations, Kilifi County, on the Kenyan Coast, were investigated. Study participants were examine

Effectiveness of dimeticone oils versus sodium carbonate solution in the treatment of tungiasis in Kenya: a non-inferiority randomised trial.

BACKGROUND: Tungiasis is a cutaneous parasitic disease caused by the female flea Tunga penetrans. The World Health Organization recommends two-component dimeticone (NYDA®) as the sole treatment for tungiasis; however, this topical medication is not available in Kenya. In western Kenya, sodium carbonate has been adopted as a traditional village-based treatment. A pilot study found that the proportion of dead fleas on day 7 was higher with NYDA® treatment than that with 5% sodium carbonate treatment (87% vs. 64%, respectively). This study was aimed at assessing the 11-day cure rates of tungiasis

Treatment-seeking behaviours of patients with tungiasis in endemic areas of Homa Bay County, Kenya: a mixed-methods study.

BACKGROUND: Tungiasis, an ectoparasitic disease caused by sand fleas, causes suffering to millions of people in the tropics. Although the Kenyan National Policy Guidelines list tungiasis treatments as including disinfectants, flea repellents, and botanical oil, the insufficient knowledge and financial constraints of affected communities have led to neglect and inappropriate self-treatment. Current reports show insignificant progress on educational activities at the community level. Therefore, we investigated community residents' treatment-seeking behaviour concerning tungiasis, using an endemi

Modeling the impact of public health education on tungiasis dynamics with saturated treatment: Insight through the Caputo fractional derivative.

Public health education is pivotal in the management and control of infectious and non-infectious diseases. This manuscript presents and analyses a nonlinear fractional model of tungiasis dynamics with the impact of public health education for the first time. The human population is split into five classes depending on their disease status. The infected population is split into two subgroups; infected but unaware and infected but aware. The model focuses on the impacts of public health education, contact and treatment contact on tungiasis transmission dynamics. Notably, public health education
