Hyperectatic Herniation of the Tympanic Membrane With Adenoid Vegetation.

A 31-year-old woman presented with intermittent otalgia in the right ear, reporting severe pain during flights. The patient denied performing habitual Valsalva maneuvers. Otoendoscopic examination revealed hyperectatic herniation of the posterior portion of the right tympanic membrane (TM). Nasopharyngoscopy showed adenoid vegetation, and tympanometry demonstrated an abnormally high peak, indicating a thinned TM. The temporal bone CT was unremarkable, but a CT of the paranasal sinuses revealed a mild homogeneous lesion in the nasopharynx. The patient underwent adenoidectomy and right ventilati

Rare case of ballooning herniation of the tympanic membrane.

Herniation of the tympanic membrane is a rare benign malformation of the tympanic membrane into the external auditory canal. It may be asymptomatic or associated with symptoms such as aural fullness, tinnitus, otalgia or hearing loss. We present a case of a symptomatic herniation of the tympanic membrane and its surgical therapy with hernia excision and tympanoplasty. An internal review board exemption was obtained.

[The healing process of tympanic membrane perforations in rats].

INTRODUCTION: Tympanic membrane (TM) perforations are commonly seen in clinical practice as a result of trauma or in the course of otitis media. The TM is a unique structure suspended in air which makes its healing processes different than in the skin wounds. The aim of the study was otoscopical and histological evaluation of the rat's TM healing process. MATERIAL AND METHODS: 56 male Wistar rats were used for the study. Fifty of them had TMs perforated bilaterally using CO2 laser, additional 6 served as a controls. The animals were sacrificed on either day 1, 2, 3, 6 and 10 post injury. Proc

Repair of the tympanic membrane with urinary bladder matrix.

OBJECTIVES/HYPOTHESIS: To test urinary bladder matrix (UBM) as a potential treatment for tympanic membrane (TM) healing and regeneration. STUDY DESIGN: This prospective pilot study was designed to provide both qualitative and semiquantitative assessment of temporal and spatial healing events in the chinchilla model of chronic TM perforations with and without UBM patching. METHODS: Bilateral myringotomies were performed and repeated as necessary to create subtotal perforations over an 8-week period. Myringoplasty was then performed, with left TMs serving as controls and right TMs receiving UB

Determinants of hearing loss in perforations of the tympanic membrane.

BACKGROUND: Although tympanic membrane perforations are common, there have been few systematic studies of the structural features determining the magnitude of the resulting conductive hearing loss. Our recent experimental and modeling studies predicted that the conductive hearing loss will increase with increasing perforation size, be independent of perforation location (contrary to popular otologic belief), and increase with decreasing size of the middle-ear and mastoid air space (an idea new to otology). OBJECTIVE: To test our predictions regarding determinants of conductive hearing loss in

A hole in the drum. An overview of tympanic membrane perforations.

BACKGROUND: Tympanic membrane perforations are common and can be categorised into either acute or chronic. Acute perforations are usually traumatic or inflammatory in origin and heal spontaneously. Chronic perforations may be associated with underlying progressive disease. OBJECTIVE: To differentiate between safe and unsafe perforations and describe management principles for these conditions. DISCUSSION: The majority of acute tympanic membrane perforations heal spontaneously. Those that do not can be considered for surgical repair. Chronic perforations should be viewed as either safe or unsa

Tympanosclerosis in the rat tympanic membrane: an experimental study.

OBJECTIVES/HYPOTHESIS: Tympanosclerosis is a pathologic condition affecting the middle ear and tympanic membrane. It is a common condition in humans, most notably after grommet insertion. However, the pathogenesis of this disease is unclear. The aim of this study was to investigate the development and progression of tympanosclerosis in the pars tensa of rat tympanic membranes at various time intervals after inducing sterile middle ear effusions. METHODS: Fifty-six male-specific, pathogen-free CD Wistar rats (100-170 g) underwent unilateral eustachian tube obstruction. Contralateral ears serve

[Fat tissue in the treatment of tympanic membrane perforations].

Adipose myringoplasty is presented as a simple and effective technique in managing tympanic membrane perforations. A review of 34 fat plug myringoplasties performed in 28 patients with drum perforations over a 4-year period was done. Twenty-two myringoplasties were postinflammatory, five--posttraumatic and three--postinflammatory in temporalis fascia. Criteria for selection, operative technique and results are discussed.

Tympanosclerosis in the tympanic membrane: influence on outcome of myringoplasty.

The effect of the presence of tympanosclerotic plaques in tympanic membrane on the outcome of a myringoplasty procedure was evaluated in this retrospective study. Long-term results of 714 myringoplasties were analyzed, and of these, 555 were eligible for further study. Three groups were studied separately: ears without tympanosclerosis, ears with a plaque not exceeding one third of the tympanic membrane surface area, and ears with a plaque involving more than one third of the surface area extending to the border of the perforation. The latter group was subdivided into one in which during the o

Sodium hyaluronate in the repair of perforations of the tympanic membrane.

Hyaluronic acid is the most abundant mucopolysaccharide in connective tissue. Because of its high viscous elasticity, it lubricates joints and can hold cells together in the intercellular spaces in connective tissue. The administration of exogenous hyaluronic acid can increase the repair potential of damaged tissue. A study was conducted to verify whether or not hyaluronic acid enhances the repair process in perforations of the tympanic membrane and to evaluate the quality of the tympanic membrane after healing. The 17 patients, aged 22 to 63 years, had small, medium, or large perforations of

Tympanograms in ears with small perforations of the tympanic membranes.

It has been believed that in an ear with a perforation, the tympanogram becomes a straight line due to the lack of compliance change in response to the pressure change in the external auditory meatus. Recently, however, we found two types of tympanogram with distinctive characteristics in ears with small perforations. In the first tympanogram type, when the external auditory meatus pressure was changed in the decreasing direction, the compliance peak was formed in the positive-pressure area, whereas it was formed in the negative-pressure area when it was changed in the increasing direction. Epithelial migration on the tympanic membrane. An experimental study.

The existence of an epidermal migration pattern was studied in mice using a whole-mount autoradiographical technique. The mitotic activity appeared to be confined to an area close to the annulus tympanicus. A very slow migratory activity of the labelled cells, was observed, situated in the basal cell layer. Perforation of the tympanic membrane showed no effect on the position of the labelled cells. This slow migration described by several authors. The latter migration was suggested to represent a mechanism for cleaning the tympanic membrane.

Allogeneic composite amniotic membrane in tympanic membrane repair: a randomized parallel controlled study.

BACKGROUNDS: Composite amniotic membranes have been widely used in ophthalmology and orthopedics. For the repair of tympanic membrane perforation, the introduction of amniotic membrane is still under study. OBJECTIVES: To analyse the effectiveness of a homograft composite amniotic membrane in tympanic membrane repair. METHODS: A total of 76 patients were recruited and randomly assigned to the experimental group (n = 39) and the control group (n = 37). The test group employed a composite amniotic membrane for the repair of the tympanic membrane, while the control group utilized cartilage from
