An experimental investigation of the resolution of tympanometry for micro-perforations of the tympanic membrane.

Tympanometry is well established as a useful tool in the field of otological diagnosis. There have been sporadic reports, however, of bizarre tympanograms in the presence of small perforations of the tympanic membrane. A model of the outer and middle ears was fabricated which was able to produce normal and pathological tympanograms. This model was used to investigate tympanometry in the presence of microperforations. A hypothesis for the mechanism of generation of sawtooth tympanograms in the presence of micro-perforations is postulated.

Outcome of 22 cases of perforated tympanic membrane caused by otomycosis.

Twenty-two cases of perforated tympanic membrane due to fungal otitis externa were observed over a five-year period. The diagnosis of fungal otitis externa was made on clinical grounds due to the obvious presence of fungal bloom in the external ear canal. Some perforations were noted at the first treatment after the fungal debris had been removed from the external ear canal using a microscope. Other perforations were observed to develop over a few days. Initially, a discrete area of the tympanic membrane appeared white and opaque. As time progressed the white area disintegrated, forming a perf

The relationship between traumatic tympanic membrane perforations and pneumatization of the mastoid.

We evaluated the possible relationship between tympanic membrane perforations resulting from blast trauma or slap and pneumatization of the mastoid cells. A total of 25 male patients with tympanic membrane perforations resulting from blast injury (n = 7), slap (n = 17), and football hit (n = 1) and 20 healthy male volunteers without any ear problem had temporal bone computed tomographic scans in the axial plane, parallel to the infraorbitomeatal line, with 2 mm slice thickness and 2-mm intervals using bone algorithm with a ProSpeed Spiral tomography machine. The area of air cells in each slice

Extended high-frequency audiometry in traumatic tympanic membrane perforations.

Air- and bone-conduction audiometry in the frequency ranges 0.125-18 kHz and 0.25-16 kHz respectively were performed in 38 patients with unilateral traumatic tympanic membrane perforation. Sensorineural threshold elevation was found in 16 ears. In nine of these this was permanent and in four restricted to the frequency range > 8 kHz. Both sensorineural threshold elevation and tinnitus (n = 16) diminished with time. A temporary, mean 5 dB, bone-conduction threshold elevation > or = 8 kHz was seen in 26 ears before spontaneous tympanic membrane closure. Closure resulted in a 7-20 dB improvement

Treatment of chronic tympanic-membrane perforations with a platelet-derived releasate.

PURPOSE: Tympanic membrane perforations are very common and often require surgical treatment. Recent studies have suggested that growth factors may be an effective nonsurgical alternative for treating chronic perforations. The purpose of this study was to assess the efficacy of a platelet releasate in the treatment of chronic nonhealing perforations in the chinchilla model. METHODS: Bilateral perforations were created in 47 chinchillas by excising 80% of the tympanic membrane with a thermal myringotomy knife. Bilateral perforations > 50% of its surface area persisted for 10 weeks in 34 animal

Real ear unaided responses in ears with tympanic membrane perforations.

This study evaluated 13 adult patients with tympanic membrane (TM) perforations in one or both ears. Probe microphone real ear unaided responses (REURs) for mean peak frequency and peak dB gain showed no substantial differences among patients, across ears with intact TMs, ears with TM perforations (regardless of size), or between measures taken before and after closure of the perforations. Inspection of individual differences in the patients' REURs, however, produced an interesting finding. The REURs of ears having small TM perforations did not differ from those with intact TMs, but the REURs

Hydrocortisone delay of tissue repair of experimental tympanic membrane perforations.

The effects of topically applied hydrocortisone on experimental tympanic membrane (TM) perforations in rats were evaluated by otomicroscopy and light microscopy. Standardized TM perforations, occupying the whole upper rear quadrant, were performed, and a 2% hydrocortisone suspension (CORT) was applied once daily for 10 consecutive days. Application of CORT caused a delayed healing pattern, and even at 1 month only 2 of 12 perforations had healed. The keratinizing squamous epithelial cells of the TM appeared hypertrophic, whereas hyperplasia, characterizing a nontreated healing TM perforation,

Tympanic membrane perforations secondary to ventilation devices: special considerations.

Tympanic membrane perforations are uncommon following the use of ventilation devices; however, persistent perforations should be given special attention for squamous epithelial growth through the perforation to the undersurface of the tympanic membrane. Surgical management is indicated in these cases. Tympanic membrane grafting procedures are adequate for early involvement. Successful grafting depends on excision of the tympanic membrane involved with the undersurface growth of squamous epithelium. Mastoid extension of the squamous tissue ingrowth requires greater surgical exposure. Cases and

Otitis media with perforation of the tympanic membrane: a longitudinal experimental study.

Despite the high incidence and prevalence of otitis media, its pathogenesis is not thoroughly understood. In an effort to provide a better understanding of this disease, experimental animal models have been developed which corroborate the changes observed in humans. In this study a new factor was added: tympanic membrane perforation 1 week after Eustachian tube obstruction. Twelve cats were divided in 4 even groups, and sacrificed at 1 and 2 weeks, 1 and 3 months after perforation, and their temporal bones were studied. Findings revealed an early massive reaction of the mucoperiosteum with gra

Outcomes of regenerative treatment for over 200 patients with tympanic membrane perforation.

OBJECTIVE: To evaluate outcomes of a regenerative treatment (RT) for over 200 patients with tympanic membrane perforation (TMP). The RT-TMP method involves a gelatin sponge, basic fibroblast growth factor (bFGF) and fibrin glue. METHODS: The study population included 216 patients and 234 ears (male: female =100:116; age 1-93 years). All enrolled patients were treated with RT-TMP in which TMP edges were disrupted mechanically and a gelatin sponge immersed in bFGF was inserted into the perforation. Fibrin glue was then dripped over the sponge. Patient outcomes including TMP closure rates, chang

An insight to tympanic membrane perforation pressure through morphometry: A cadaver study.

INTRODUCTION: A cadaveric experimental investigation aimed to show the rupture pressure of the tympanic membrane (TM) for otologists to evaluate its tensile strength. METHODS: Twenty adult ears in 10 fresh frozen whole cadaveric heads (four males, six females) mean age 72.8 (SD 13.8) years (range 40-86) were studied. The tensile strength of the TM was evaluated with bursting pressure of the membrane. The dimensions of the membranes and perforations were measured with digital imaging software. RESULTS: The mean bursting pressure of the TM was 97.71 (SD 36.20) kPa. The mean area, vertical and

Mesenchymal stem cell-laden hybrid scaffold for regenerating subacute tympanic membrane perforation.

Tympanic membrane (TM) perforation is one of the most common otology complications. To date, there has not been reported TM regeneration using bioprinted scaffold. The purpose of this study was to evaluate the efficacy and feasibility of bioprinted polycaprolactone/collagen/alginate-mesenchymal stem cell (PCAMSC) scaffolds for the regeneration of subacute TM perforation. Sprague-Dawley rats were used in an animal model of subacute TM perforation. In the experimental group (n=7), bioprinted 3D PCAMSC scaffold was placed on the perforation. The control group (n=7) were treated with polycaprolact
