Navigating Health Insurance Selection for in Vitro Fertilization (IVF) Benefits: A Study Protocol.

INTRODUCTION: A large public university added health insurance coverage of 50% co-insurance for up to two cycles of in vitro fertilization (IVF) to eligible faculty and staff. METHODS: We describe the design and conduct of a randomized controlled trial to evaluate the effectiveness of a health insurance educational intervention on health insurance literacy and IVF benefit utilization. The intervention materials included 1) Key insurance terms; 2) Examples of premiums and deductibles across the insurance plan options; 3) Examples of how premiums and deductibles affect out-of-pocket costs; and

Determining the efficiency path to universal health coverage: cost-effectiveness thresholds for 174 countries based on growth in life expectancy and health expenditures.

BACKGROUND: Assessment of the efficiency of interventions is paramount to achieving equitable health-care systems. One key barrier to the widespread use of economic evaluations in resource allocation decisions is the absence of a widely accepted method to define cost-effectiveness thresholds to judge whether an intervention is cost-effective in a particular jurisdiction. We aimed to develop a method to estimate cost-effectiveness thresholds on the basis of health expenditures per capita and life expectancy at birth and empirically derive these thresholds for 174 countries. METHODS: We develop

Analysis and interpretation of the heterogeneity of community-based health insurance attributes and preferences in Senegal: evidence from a discrete choice experiment.

INTRODUCTION: Community-based health insurance (CBHI) schemes have been widely promoted to improve financial protection and expand access to healthcare in low- and middle-income countries (LMICs), particularly among populations working in the informal sector who lack access to formal health insurance. In Sub-Saharan Africa, CBHI has been implemented as part of broader efforts to advance universal health coverage (UHC), yet enrolment often remains low. In Senegal, CBHI schemes aim to expand insurance coverage among informal sector populations, but participation remains limited and uneven across

Health disparity and healthcare utilization inequity among older adults living in poverty in South Korea: a cross-sectional study.

BACKGROUND: Korea has a two-tiered universal health security system: the wage-based National Health Insurance (NHI) program and government-subsidized Medical Aid (MA) program. Beneficiaries of the MA program belong to the lowest economic class. This study aims to investigate the association between economic status-defined as NHI or MA enrollment-and health disparity of older people aged ≥ 65 years in South Korea. METHODS: The claims records of 672,525 older age population from the 2017 Health Insurance Review and Assessment Service-Adult Patient Sample were used to estimate adjusted odds rati

Access to primary health care among homeless adults in Toronto, Canada: results from the Street Health survey.

BACKGROUND: Despite experiencing a disproportionate burden of acute and chronic health issues, many homeless people face barriers to primary health care. Most studies on health care access among homeless populations have been conducted in the United States, and relatively few are available from countries such as Canada that have a system of universal health insurance. We investigated access to primary health care among a representative sample of homeless adults in Toronto, Canada. METHODS: Homeless adults were recruited from shelter and meal programs in downtown Toronto between November 2006

Barriers to health insurance uptake in Rwanda: a nationwide cross-sectional survey.

INTRODUCTION: achieving universal health coverage by 2030 is a key objective for Rwanda, ensuring equitable access to health insurance. However, approximately 10% of the population remains uninsured. This study aims to identify socio-economic and demographic factors associated with health insurance uptake in Rwanda. METHODS: we analyzed secondary data from the Fifth Integrated Household Living Conditions Survey (EICV 5), comprising 14,580 households. Multivariable logistic regression was performed to assess factors associated with health insurance uptake. Descriptive statistics were used to s

Kerala's Aardram Mission and universal health coverage: integrated time series evidence on mortality reduction and assessment of financial protection and equity gaps.

INTRODUCTION: Universal health coverage (UHC) requires simultaneous progress across health outcomes, financial protection, and service delivery dimensions. Kerala has achieved favorable mortality indicators, yet implementation gaps and financing barriers persist despite policy reforms. The Aardram Mission (2017) represents a major primary care strengthening intervention aimed at advancing UHC, but its association with multiple UHC dimensions has not previously been examined in an integrated way. OBJECTIVE: The study aimed to assess the association between the implementation of the Aardram Mis

Addressing the burden of mental, neurological, and substance use disorders: key messages from Disease Control Priorities, 3rd edition.

The burden of mental, neurological, and substance use (MNS) disorders increased by 41% between 1990 and 2010 and now accounts for one in every 10 lost years of health globally. This sobering statistic does not take into account the substantial excess mortality associated with these disorders or the social and economic consequences of MNS disorders on affected persons, their caregivers, and society. A wide variety of effective interventions, including drugs, psychological treatments, and social interventions, can prevent and treat MNS disorders. At the population-level platform of service deliv

Ningen Dock: Japan's unique comprehensive health checkup system for early detection of disease.

Japan's comprehensive health checkup - Ningen Dock - is a unique system for early detection of disease that has developed over the years along with the country's universal health insurance system. Ningen Dock is currently offered at 1,727 facilities nationwide, involving about 3.7 million people annually. The development of the comprehensive health checkup system may be one reason for Japan's long life expectancy. The major purpose of the comprehensive health checkup system is to maintain health in three main ways: early detection of cancer, detection of lifestyle-related diseases, and confirm

Re-envisioning contributory health schemes to achieve equity in the design of financial protection mechanisms in low- and middle-income countries.

Universal health coverage has emerged as a global health priority, requiring that financing strategies that ensure low-income and medically and financially at-risk individuals can access health services without the threat of financial catastrophe. Contributory financing schemes and social health insurance (SHI) schemes, in particular, predominate in low- and middle-income countries (LMICs), despite evidence that suggests the most vulnerable remain excluded from such schemes. In this commentary, we discuss the need to re-envision schemes to prioritize equity, offering 3 concrete recommendations

Socioeconomic status and access to mental health care: The case of psychiatric medications for children in Ontario Canada.

We examine differences in the prescribing of psychiatric medications to lower-income and higher-income children in the Canadian province of Ontario using rich administrative data that includes diagnosis codes and physician identifiers. Our most striking finding is that conditional on diagnosis and medical history, low-income children are more likely to be prescribed antipsychotics and benzodiazepines than higher-income children who see the same doctors. These are drugs with potentially dangerous side effects that ideally should be prescribed to children only under narrowly proscribed circumsta

The other health care system in Germany: care for people without health insurance.

BACKGROUND: A large number of people in Germany have no health insurance. Their access to the official healthcare system is significantly more difficult or impossible. Charitable institutions try to provide medical care and create parallel healthcare structures. Their possibilities and limits are still unknown. METHODS: This study identified 128 institutions for people without health insurance. A data query was used to determine the status of medical care for uninsured patients. RESULTS: Sixty-eight of 128 identified institutions participated in the study. They provided care to 26,300 people

Organ procurement in Munich: financial and organizational aspects.

The Transplantation Center at the University of Munich is like all other German centers financially supported by the Insurance Companies via a non-profit organization (Kuratorium für Heimdialyse e.V.). The financial modalities are based on the arrangement that the Insurance Companies pay a certain amount for each transplantation to the non-profit organization. This institution, in cooperation with the University, uses this money to cover all extra expenses arising in the field of organ procurement as well as clinical transplantation (providing additional staff, equipment, etc).
