Impact of the human immunodeficiency virus infection on emergency medicine department in a tertiary care hospital in India.

PURPOSE: To assess the impact of HIV infection on emergency medicine department services. METHODS: The demographic and clinical data of all the HIV reactive individuals admitted to the emergency medicine department (EMD) were analyzed by reviewing the case records retrospectively. RESULTS: Of the 1187 HIV reactive cases detected during a period of 10 years, 435 cases were admitted to the EMD. 90% of the patients were unaware of their HIV reactive status. Prolonged perplexing pyrexia was the commonest presentation (73.4%) followed by Tuberculosis (53%). Neurological complications, AIDS entero

[Hepatitis C infection among patients receiving hemodialysis].

In France, the prevalence of hepatitis C virus (HCV) infection in chronic hemodialysis patients is high, about 15 to 42%. Although the incidence and the prevalence of HCV infection decreased during the last decade, de novo cases of HCV infection still occur in hemodialysis units. To date the nosocomial transmission is the principal cause of contamination. The strict application of universal precautions and the separation of chronic hemodialysis patients according to their virological status may decrease the incidence of HCV infection in hemodialysis units. The transaminase level and HCV viremi

Epidemiological study on knowledge, attitudes and behaviour of health care workers with respect to HIV infection.

To analyse knowledge, perception of risk, attitudes and behaviour towards HIV infection among health workers in two hospitals in the North-East of Italy, we sent all health care workers who were working in direct contact with patients an anonymous self-administered questionnaire covering personal and occupational data, perception and knowledge of risk, behaviour in routine activities, attitudes towards and care of HIV patients. The response rate was 70.06%. The perception of the risk of acquiring HIV infection was influenced by occupational qualification, by work unit and by having cared for a

Mode of hepatitis C virus infection, epidemiology, and chronicity rate in the general population and risk groups.

Since the discovery of the hepatitis C virus (HCV), it has become evident that this infectious agent is a primary cause of posttransfusion and sporadic non-A, non-B hepatitis. Identification and introduction of surrogate markers for posttransfusion hepatitis and later introduction of anti-HCV screening has decreased the incidence of posttransfusion hepatitis. Community-acquired HCV infection is less common than posttransfusion HCV hepatitis. HCV infection may lead to liver cirrhosis without prior evidence of laboratory or histologic infection. Populations at risk for HCV infection include pati

Radiation exposure to the surgeon during closed interlocking intramedullary nailing.

During interlocking intramedullary nailing of twenty-five femoral and five tibial fractures, the primary surgeon wore both a universal film badge on the collar of the lead apron and a thermoluminescent dosimeter ring on the dominant hand to quantify the radiation that he or she received. When distal interlocking was performed, the first ring was removed and a second ring was used so that a separate recording could be made for this portion of the procedure. At the conclusion of the study, all of the recorded doses of radiation were averaged. The average amount of radiation to the head and neck

Does the rise in eating disorders lead to increasing risk of orthorexia nervosa? Correlations with gender, education, and body mass index.

Investigating eating disorders and orthorexia nervosa, especially in the young population, is an important step in taking protective precautions and identifying disease. This study was carried out to determine the relationship of eating disorders and orthorexia nervosa to gender, BMI, and field of study in a population of university students in Turkey. In all, 900 university students aged 17-23 years participated in this study. EAT-40 and ORTO-15, which are validated instruments for the screening of participants with anormal eating behaviors and orthorexia nervosa, respectively, were used. The

Low health literacy impact on headache diagnosis.

Health literacy has a broad range of impacts, including whether and where a person seeks treatment and how well that patient interprets and applies clinical recommendations. Patients with headache may not seek treatment, may fail to disclose even severe pain to their physician, and may not understand or adhere to treatment. Universal precautions can be a helpful initial approach, and information styles can be modified as needed over time. Patients should be asked about headache and explanations about headache causes and treatment should be given in everyday language. The teach-back method can

Impact of Sterilization Methods on the Seeding Ability of Human Tau Proteopathic Seeds.

The protein tau, when misfolded in neurodegenerative diseases, has several prion-like properties including being able to spread by cell-to-cell transfer, induce templated seeding, and exist in distinct conformational strains. These properties of transmission may present health hazards when lesion-containing biospecimens are used in research and neuropathology laboratories. We evaluated the impact standard sterilization and cleaning methods have on the capacity of tau seeds to induce aggregation. We employed a previously developed, highly sensitive FRET-based biosensor assay to assess remnant t

Non-infectious status indicated by detectable IgG antibody to SARS-CoV-2.

A key tenet of protection from infection for dentists is to know who is not infectious. The evidence base regarding protection from respiratory pathogens in dentistry is poor. Those with a positive SARS-CoV-2 IgG antibody are non-infectious (>99% certainty) and can be safely treated with good universal precautions, even for aerosol generating procedures. Viral infectivity with SARS-CoV-2 lasts eight days, unlike viral polymerase chain reaction (PCR) swab tests which can persist for as long as seven weeks. SARS-CoV-2 IgG antibody becomes detectable from 11 days after infection. SARS-CoV-2 IgG a

Challenge of hepatitis C in Egypt and hepatitis B in Mauritania.

Egypt has one of the highest prevalence rates of hepatitis C virus (HCV) in the world, mostly with genotype 4 that is highly associated with severe fibrosis. As a consequence, hepatocellular carcinoma has become the leading cause of cancer in this country. Mauritania is a highly endemic area for hepatitis B virus (HBV). HBV and HCV could both be iatrogenically transmitted through infected blood products, infected needles, and medical equipment improperly sterilized. Adequate and efficient healthcare and public health measures with good surveillance programs, access for screening, prevention st

Plague.

In the United States, plague poses a threat to humans from the infected animals in the endemic areas of the Western states. Plague may also be used in the near future as an agent of warfare or terrorism. Although the presentation of bubonic plague may be less of a problem, the septicemic and pneumonic forms present challenges to early diagnosis and prompt treatment. The major threat of plague as an agent of terrorism will probably be through the inhalational route. which could result in many cases of the pneumonic form, requiring early recognition and initiation of appropriate therapy. Imported viral haemorrhagic fever with a potential for person-to-person transmission: review and recommendations for initial management of a suspected case in Belgium.

Viral haemorrhagic fevers are caused by a wide range of viruses. There are 4 types of viruses well known to spread from person to person and able to cause nosocomial outbreaks with a high case fatality rate: an arenavirus (Lassa fever and more exceptionally the Junin and Machupo virus), a bunyavirus (Crimean-Congo haemorrhagic fever) and the Filoviridae (Ebola and Marburg viruses). So far there have been only a limited number of imported cases of viral haemorrhagic fever in industrialized countries. In recent years an increasing number of outbreaks of filovirus infections have occurred in Afri

HIV infection in the parturient.

As the incidence of AIDS continues to increase in the female population, there will be more cases of HIV-positive women in the obstetric suite requiring anesthesia services. Many may be asymptomatic, but there will be patients with active opportunistic infections and malignancies who may or may not have been treated. The impact of both the disease AIDS and the drugs used to treat it on anesthetic technique can be significant. Hematological, neurological, respiratory, and immune dysfunction can be expected in a significant proportion of these women. Universal precautions should be utilized for

Otolaryngologic aspects of acquired immunodeficiency syndrome.

Currently, most patients with AIDS present with a manifestation involving the head, neck, or respiratory tract. The otolaryngologist may be the first physician to evaluate the patient with HIV infection and, therefore, must be aware of the various manifestations. The otolaryngologist may also be involved in diagnosing and managing the disease through modalities such as biopsy, endoscopy, tracheotomy, and other surgical procedures. In all phases of care, it is important to retain a medically appropriate and psychologically supportive environment. Although universal precautions should be followe

Rehabilitation dimensions of AIDS: a review.

With the spread of human immunodeficiency virus (HIV) infection and of the acquired immune deficiency syndrome (AIDS), many rehabilitation professionals are faced with new challenges. This report covers two basic problems that are becoming common in rehabilitation practice: the management of the patient who suffers from disability resulting from HIV infection or AIDS, and the management of the traditional rehabilitation patient who may coincidentally be infected with HIV. Common manifestations of HIV infection and associated secondary infections and neoplasms are discussed, as well as are the

Admission of AIDS patients to a medical intensive care unit: causes and outcome.

As the number of cases of AIDS increases, it is important to determine whether ICUs can be productively and safely used for this patient population. From July 1981 to March 1987, 216 patients were admitted to the medical ICU: 166 (77%) were admitted for procedures and 50 (23%) were admitted for life-sustaining support. Most of the patients were admitted for respiratory failure (36 of 50), primarily as a result of Pneumocystis carinii pneumonia. Other patients were admitted for cardiovascular instability (six of 50 patients), CNS dysfunction (four patients), or other reasons (four patients). Of

Clinical effectiveness and risk of emerging resistance associated with prolonged use of antibiotic-impregnated catheters: more than 0.5 million catheter days and 7 years of clinical experience.

OBJECTIVES: Catheters coated with minocycline and rifampin are proven to decrease the rates of central line-associated bloodstream infection; however, it is unclear whether success occurs independent of other infection control precautions. We evaluated the effect of catheters coated with minocycline and rifampin with and without other infection control precautions on our rates of central line-associated bloodstream infection in critically ill patients and on antibiotic resistance throughout the hospital and in the intensive care unit. DESIGN: Retrospective clinical cohort study conducted duri

Commentary: the importance of education and vaccination in reducing the risk of hepatitis infection among nursing students.

BACKGROUND: Healthcare workers are at risk of contracting blood-borne viral infection during contact with blood and body fluids. Nursing students may be at particular risk because of their inexperience. Adoption of suitable precautions, hepatitis B vaccination and appropriate post-exposure prophylaxis will significantly reduce the risk of infection. AIM: This commentary puts into context the findings of a study by Yamazhan et al. who explored nursing students' knowledge on hepatitis together with their immunization status. EVALUATION: Knowledge of hepatitis, its mode of transmission and the

Suggestions for the care of patients with liver disease during the Coronavirus 2019 pandemic.

This document, written by the French Association for the Study of the Liver (AFEF) board, aims to provide information to physicians involved in the care of patients with liver disease during the Coronavirus disease (COVID-19) epidemic. These are not based on a systematic review of the literature and a rigorous evaluation using the GRADE method. These are recommendations based on feedback from China available in the form of original articles or letters - for which the scientific evidence is often modest - and the rules put forward by American (1) and European (Boettler et al, 2020) hepatology s

A managed multidisciplinary programme on multi-resistant Klebsiella pneumoniae in a Danish university hospital.

BACKGROUND: Bacteria-producing extended spectrum β-lactamase (ESBL) enzymes are resistant to commonly used antimicrobials. In 2008, routine monitoring revealed a clonal hospital outbreak of ESBL-producing Klebsiella pneumoniae (ESBL-KP). METHODS: At a 510-bed Danish university hospital, a multidisciplinary change project inspired by Kotter's Eight Steps of Change was designed. In addition to revision of antimicrobial guidelines and restriction of selected antimicrobials, the complex, managed, multi-faceted intervention comprised training and education, enhanced isolation precautions, and a se
