The term scleritis refers to a heterogeneous group of disorders characterized by a chronic inflammatory response centered on the sclera and often involving adjacent structures such as the episclera, cornea, and uvea. It is an uncommon but severe, painful, and potentially blinding ocular disease and may occasionally result in perforation of the globe. Through a case report of severe scleritis that occurred after pterygium surgery, the authors expose diagnostic difficulties and emphasize the need for a meticulous diagnostic approach in such a condition.

Systemic diseases as rheumatoid arthritis and ankylosing spondylitis have unknown etiologies, unclear pathogenic mechanisms and multiorgan involvements. Ocular involvement is variable and may effect several levels: sclera, cornea, uvea and conjunctiva. Because an ocular event signify an enhancement of inflammatory-immune reactions, the patient may undergo a complete investigation and general therapy must complete local treatment. Primary malignant melanoma most commonly originates from the skin; other less common extra cutaneous sites include squamous mucous membranes, uvea, retina, leptomeninges, genitourinary tract, digestive tract, biliary tract, and upper respiratory tract. Primary melanoma of the gastrointestinal tract is exceedingly rare. We are reporting a histo-pathologically proven rare case of primary malignant melanoma of the esophagus and its findings on fluorodeoxyglucose positron emission tomography and computed tomography.

Scleritis is typically a severe painful inflammatory process centered in the sclera that may involve the cornea, adjacent episclera, and underlying uvea; it poses a significant threat to vision. Careful clinical history taking, detailed ocular examination, appropriate investigation for ocular disease with or without underlying systemic disease, and timely intervention with the use of immunosuppressant drugs when necessary, has improved the long-term outcome for patients with this disease.

White diseases are a heterogenous group characterized by hypopigmentation or depigmentation. Skin and eye color are determined by the number and size of melanosomes present. Melanin is produced by melanosomes in the melanocytes present within the epidermis of the skin, uvea, and retinal pigmented epithelium (RPE). Conditions altering the number of melanocytes or concentration of melanin result in a lack of pigmentation, appearing as "white diseases" ranging from the well-known albinism and vitiligo to more esoteric white hand syndrome and Degos disease.

This monograph has considered the clinical features, differential diagnosis, diagnostic approaches, treatment and prognosis for primary and secondary tumors of the uveal tract. Tumors metastatic to the uvea are the most common intraocular malignant lesions, although primary uveal melanomas are recognized more frequently clinically. These tumors differ considerably with regard to clinical features and in the recommended approach to systemic evaluation and treatment. The diagnosis and management of such tumors should be a joint effort between the ophthalmologist and other physicians.

Spontaneously occurring ocular neoplasia is rarely reported in rabbits. This case series presents four cases of rabbits diagnosed with iridociliary tumors, which have not been previously reported in this species. Major pathological findings include epithelial tumors affecting the anterior uvea with variable pigmentation and basement membrane formation. Follow-up information was only available for two cases, but neither showed evidence of metastasis, suggesting that the prognosis for these tumors in rabbits, as in other species, may be very good.

Two cases (four eyes) of medullary thyroid carcinoma with metastases in the choroid, ciliary body and iris are described for the first time. In one eye the choroidal metastases were destroyed clinically by photo- and cryocoagulation. In another eye coagulation of a tumor may have facilitated transscleral extension into the orbit. The clinical conclusions arising from the pecularities of these tumors are discussed. It is particularly striking that in 2 patients with this rare tumor metastases developed in the anterior uvea in all four eyes.

Conjunctival melanoma is an uncommon tumor that is likely to recur and carries an overall mortality rate of approximately 30%. Melanoma arises from melanocytes, most often in sun-exposed skin. Less commonly, melanoma originates from other tissues such as the uvea, rectum, mouth, respiratory tract, and conjunctiva. Conjunctival melanoma represents only 1.6% of all noncutaneous melanoma. Herein, we reported a case of conjunctival melanoma followed by reviewing the literature to provide an optimal diagnostic approach.

Herpes zoster becomes latent in the sensory ganglia after the primary infection and may reactivate many years later to produce shingles in adults. Involvement of the ophthalmic branch of the trigeminal nerve with this virus is called Herpes zoster ophthalmicus (HZO). The eyelid skin, cornea, uvea, retina, optic nerve and other cranial nerves can be affected by HZO. Isolated internal ophthalmoplegia can rarely be seen. Clinicians should be aware of this condition and HZO must be kept in mind in the differential diagnosis of anisocoria.

PURPOSE: To describe a technique for reducing and reinforcing scleral thinning in cases of necrotizing scleritis with ectasia.
METHODS: Descriptive report of a surgical technique in 2 surgical cases with a video.
RESULTS: Successful repair and reduction in uveal prolapse from scleral thinning in 2 cases of necrotizing scleritis with ectasia with a suturing technique using overlapping (weave) compressive suturing over a scleral patch.
CONCLUSIONS: In cases of necrotizing scleritis with ectasia in which there is perforation or prolapsing uvea, this technique should be considered.

Human ocular infestation by live filarial worm is a rare occurrence and has been reported mostly form South-East Asia. It involves the eyelids, conjunctiva, cornea, anterior chamber and uvea. No case of intravitreal Brugian microfilaria / adult worm has been found by Medline search. Here we report a case of live intravitreal adult Brugian filaria, where the parasite was successfully removed by pars plana vitrectomy. Identification of the worm was done by light microscopy and confirmed by immuno chromatographic test.
