Chronic Venous Insufficiency and Lymphedema With Papillomatosis Cutis Lymphostatica, Hyperkeratosis, and Skin Ulcers: A Case Report.

Chronic venous insufficiency (CVI) is a common condition characterized by lower extremity edema, discomfort, and skin changes due to venous hypertension caused by incompetent or obstructed venous valves. We report a case of chronic venous insufficiency and lymphedema with papillomatosis cutis lymphostatica, hyperkeratosis, and skin ulcers with proteus superinfection. A 67-year-old male presented to the emergency department (ED) for wound evaluation and was found to have severe hyperkeratosis, multiple ulcers with purulent discharge, and "tree bark" skin changes. Prophylactic treatment for deep

A New Technique for Creation of Femoral Vein Monocusp Neovalve for Patients With Primary Deep Vein Incompetence.

Chronic venous insufficiency due to deep vein incompetence (DVI) is difficult to treat and is prone for failure and recurrence of venous ulcers. A variety of techniques have been tried to create valve competency but majority of them have been difficult to perform and have not shown long term results. Opie reported a technique for construction of a monocusp neo valve. We have modified this procedure and successfully corrected the deep vein reflux in a patient with primary DVI.MethodsMonocusp valve was created by folding the vein wall flap so that both sides of the valve will have intimal surfac

Anatomical basis of retrograde thoracic veins flow and its implications in complex thoracic wall reconstructive surgery.

PURPOSE: Internal thoracic veins are increasingly used as recipient's vessels in chest wall reconstructive surgery due to their predictable anatomy and to the possibility to make a double venous anastomosis, exploiting the retrograde flow within them. Over the years, retrograde flow had been explained by the absence of valves in internal thoracic veins, which have been found recently instead. Therefore, our aim is to analyze the retrograde flow and its relationship with valves in the internal thoracic veins. METHODS: We evaluated 32 internal thoracic veins of 16 fresh-frozen specimens with un

Descending phlebography in patients with venous ulceration: hemodynamic implications.

AIM: The aim of this paper is to report on the hemodynamic significance of the various degrees reflux as demonstrated on descending phlebography, by comparing the phlebographic findings with ambulatory venous pressure (AVP) measurements. METHOD: Thirty-two patients (45 affected limbs) with active or healed venous ulceration were admitted to the study. Descending phlebography with grading of reflux (0-4 using Herman's grading), AVP and refilling time 90 (RT90) were performed in all patients. In addition, the presence of deep to superficial reflux into the great saphenous vein at the sapheno-fe

Single-leaflet reconstruction surgery for severe chronic lower limb venous insufficiency caused by post-thrombotic syndrome: a case report and literature review.

BACKGROUND: A considerable number of patients with deep vein thrombosis of the lower extremities will develop post-thrombotic syndrome even after receiving standardized anticoagulation therapy. Damage to the femoral vein valves caused by post-thrombotic syndrome can lead to severe chronic lower limb venous insufficiency and currently there is a lack of effective treatment. CASE PRESENTATION: We present a patient with pigmentation and itching due to post-thrombotic syndrome, where the anterior leaflet of the first set of valves in the superficial femoral vein was completely destroyed, while th

Aortic valve replacement through left thoracotomy after esophageal operation.

A 67-year-old man was referred for aortic valve surgery due to aortic valve regurgitation. He underwent an aortic valve replacement through a left thoracotomy, since he had a history of esophageal surgery with substernal gastric tube reconstruction and lymph node dissection through a right thoracotomy 14 years ago. The aortic valve was successfully replaced with excellent visualization using vacuum-assisted venous drainage on a cardiopulmonary bypass. Although exposing the aortic valve through a left thoracotomy is difficult, the application of vacuum-assisted venous drainage helps visualize t

Altered hemodynamics in chick embryos after extraembryonic venous obstruction.

OBJECTIVE: To obtain insight into hemodynamics during abnormal cardiac development, a chick model was developed recently in which a spectrum of conotruncal anomalies, in combination with abnormal semilunar valves and/or pharyngeal arch artery malformations, was induced through extraembryonic venous obstruction (venous clip) at stage 17 (70-h incubation). METHODS: In chick embryos of stage 24 and stage 34 control (n = 8; n = 21) and with venous clip (n = 11; n = 18), we simultaneously measured dorsal flow velocities with a 20-MHz pulsed Doppler velocity meter and dorsal aortic (stage 24) and v

[Functional anatomy of the pelvic veins in women].

Pelvic veins and lower extremities veins form a functional unit resulting in the interaction of their pathology. The intrapelvic venous system consists of two independent networks, under physiological conditions: the parietal and the visceral networks. The main collectors are the internal iliac veins, the ovarian superior rectal veins. The original venous plexi, located in the sub-peritoneal pelvic connective tissue, remain open because of the adhesion of their wall to the parietal pelvic fascia. The parietal venous network, abundant and supplied with valves, includes the retro-public and sacr

Matrix Metalloproteinases in Remodeling of Lower Extremity Veins and Chronic Venous Disease.

The veins of the lower extremity are equipped with efficient wall, contractile vascular smooth muscle (VSM), and competent valves in order to withstand the high venous hydrostatic pressure in the lower limb and allow unidirectional movement of deoxygenated blood toward the heart. The vein wall structure and function are in part regulated by matrix metalloproteinases (MMPs). MMPs are zinc-dependent endopeptidases that are secreted as inactive pro-MMPs by different cells in the venous wall including fibroblasts, VSM, and leukocytes. Pro-MMPs are activated by other MMPs, proteinases, and other en

Adrenal cortical hormones in the spermatic vein of 95 patients with left varicocele.

Adrenocortical hormones were measured in the peripheral and spermatic venous blood samples, taken simultaneously at the time of a high vasoligation for left varicocele in 95 patients. Our results do not confirm the hypothesis of MacLeod that the deleterious effects of a varicocele on male fertility are caused by an higher local concentration of adrenocortical hormones, due to venous reflux through incompetent valves. If this is the case it occurs only in a minority of patients.

[Remnants of the embryologic sinus venosus valves. "Cor triatriatum dexter" (author's transl)].

Remnants of the embryologic sinus venosus valves are not uncommon. However, persistent venous valves in the right atrium, which are large enough to obstruct the blood flow through the tricuspid valve (the so called cor triatriatum dexter) are very rare. There are 3 different types according to the location of the right atrial membrane. Right heart chambers hyoplasia is the most common association. Concurrent lesions of lesser degree have also been described. Although isolated cor triatriatum dexter could be symptomatic, both the clinical features and the prognosis usually are the consequence o

Surgical pathology of the tricuspid valve: a study of 363 cases spanning 25 years.

Surgical pathologic features of the tricuspid valve were reviewed in 363 patients who had undergone tricuspid valve replacement at our institution during the period 1963 through 1987. Valves were purely regurgitant in 74%, stenotic and regurgitant in 23%, and purely stenotic in 2%; two valves were neither stenotic nor regurgitant. Among 269 purely insufficient tricuspid valves, the four most common causes were postinflammatory disease (41%), congenital disorder (32%), pulmonary venous hypertension (21%), and infective endocarditis (4%). Of 92 cases of tricuspid stenosis, with or without regurg

Great toe-to-hand transfer nourished by arterial inflow through the venous system.

Revascularization of tissues through their venous system is currently used in vascularized venous flaps and in replantation of some fingertips. A toe-to-hand transfer that suffered prolonged and unexplainable arterial spasm, unrelenting to the usual therapeutic measures, was revascularized through its venous system. Tissue perfusion in the toe began 24 hours after the vascular repairs were through, because arterial flow was hampered by the venous valves in the toe. Tissue perfusion was poor initially but became stable 72 hours postoperatively, and the toe survived. The only complications were

Early Palma procedure after iliac vein injury in abdominal penetrating trauma.

Ligation for penetrating abdominal vein trauma may have better outcome than a vascular reconstruction in an unstable patient. However, symptoms of chronic venous insufficiency may appear over time. We describe our surgical experience with 4 patients who underwent iliac vein ligation followed by venous bypass with a modified Palma derivation between 48 and 240 hours after sustaining penetrating abdominal trauma with concomitant iliac vein injury. Patients were assessed for venous symptoms and conduit patency with continuous wave Doppler and duplex scanning. One graft occluded acutely and the re

Trans-right atrial access to the left ventricle for catheter ablation of ventricular tachycardia in patients with mechanical aortic and mitral valves.

The presence of mechanical heart valves in both aortic and mitral positions is a significant limitation for traditional left ventricular (LV) access, including retrograde transaortic and/or antegrade interatrial transseptal routes. We present a case of successful catheter ablation for ventricular tachycardia via a traditional transfemoral venous approach, which involves direct puncture of the inferior and medial aspect of the right atrium adjacent to the posteroseptal process of LV (PSPLV). Percutaneous trans-right atrial access to the left ventricle appears to be a safe and feasible method fo

Multiple mycotic arterial aneurysms involving the left anterior descending coronary artery following mitral and aortic valve endocarditis: A case report.

We present a 57-year-old man with recent Streptococcus viridans endocarditis on mitral and aortic valves who had a mycotic aneurysm of the left anterior descending (LAD) coronary artery and associated superior mesenteric and cerebral artery aneurysms. The patient had preoperative renal failure and the infection was controlled with ceftriaxone. Mitral and aortic valve replacement were performed using tissue valves and the LAD aortic aneurysm was ligated and the patient had saphenous venous graft to the LAD. The postoperative course was complicated by pleural effusion and the patient had antibio

Isolated right-sided varicocele as a salvage pathway for portal hypertension.

Retrograde blood flow can occur in the testicular veins and in the pampiniformis plexus in the absence of valves or if the valves are incompetent, resulting in tortuosity and dilatation of the veins. These abnormal alterations in the anatomy of the veins, termed varicoceles, are associated with infertility in the male. Most varicoceles occur on the left. We report the case of a rare isolated right-sided varicocele in a male evaluated for infertility in whom extensive work-up revealed venous anomalies and a spontaneous porto-systemic shunt. In such cases, standard approaches to infertility trea

[Acute iliofemoral deep venous thrombosis should be treated with catheter-directed thrombolysis].

Treatment of acute iliofemoral deep venous thrombosis (DVT) with catheter-directed thrombolysis (CDT) has been performed in Denmark since 1999. The purpose of CDT is to dissolve thrombus and to restore the venous lumen as fast as possible and thereby save venous valve function and prevent postthrombotic syndrome. Danish studies have shown that treatment of acute iliofemoral DVT using CDT results in good patency, preserves venous valve function, reduces the frequency of PTS, and is associated with a higher quality of life.

The importance of the angulation and termination of external jugular vein in central venous catheterization in newborn.

The purpose of this study is to evaluate the termination and angulation of the EJV (External jugular vein) and the number and the position of its valves so as to improve our understanding of the anatomy of the EJV and to make the manuplation easier in central venous catheterization in newborn. The present study was carried out on 50 newborn cadavers (31 male, 19 female). 100 Dissections were bilaterally made. The angle of the termination of the EJV was measured. Its termination angle was also radiologically measured. Its termination was classified into three types: In 72 cases (72%), EJV flowe
