Conduit repair for complex congenital heart disease with pulmonary atresia or right ventricular outflow tract obstruction. Part II: early and late hemodynamic and echocardiographic findings.

The results are presented of 44 systematic cardiac catheterizations in 35 patients after successful repair of various types of congenital cyanotic heart disease using a conduit. The operations were performed from 1972 to 1981. The patients were followed from 6 months to 9 years, the mean interval between operation and catheterization was 2.5 years (4 weeks to 8 years). Different types of conduits were used: a Hancock conduit in 27 cases (1 replaced), an aortic homograft in 3 cases (all 3 replaced by a Hancock conduit), a Dacron tube with Lillehei-Kaster valve in 2 cases (1 replaced), a lonescu

Association Between Location of Pulmonary Outflow Obstruction and Right Ventricular Size and Function After Repair of Tetralogy of Fallot, Pulmonary Atresia, and Major Aortopulmonary Collaterals.

Quantification of right ventricular (RV) size and function after tetralogy of Fallot repair is critical for determining timing of reintervention and outcomes. Tetralogy of Fallot patients with pulmonary atresia and major aortopulmonary collateral arteries (TOF/PA/MAPCAs) are a unique group in which the RV is subjected to various loading conditions, allowing for direct comparison. Retrospective evaluation of RV echocardiographic indices in repaired pediatric TOF/PA/MAPCAs patients (2/2002 - 4/2018). Patients were categorized as having conduit stenosis (peak gradient ≥ 2.5 m/s) and/or distal pul

Postoperative long-term results in total correction of tetralogy of Fallot: with special reference to method and degree of relief of right ventricular outflow tract obstruction.

Late results were studied in 63 patients who underwent complete repair of tetralogy of Fallot. Mild pulmonary stenosis (PS) was present in 73%, moderate PS in 17% and severe PS in 10% of the patients. When a transannular patch was used, wider enlargement of the pulmonary annulus did not always relate to a decrease in right ventricular pressure and right ventricular to pulmonary artery pressure gradient in the late postoperative period. Pulmonary regurgitation (PR) of grade 2 or less did not relate to the late postoperative hemodynamics and cardiac function. PR of grade 3 or over led to right v

Responses of plasma norepinephrine and heart rate during exercise in patients after Fontan operation and patients with residual right ventricular outflow tract obstruction after definitive reconstruction.

To determine the exercise responses of patients with congenital heart disease, 20 patients-5 who had undergone a right ventricular outflow tract reconstruction (group R; age, 15 +/- 2 years), eight who had undergone a Fontan operation (group F; age, 13 +/- 2 years), and seven who had a history of Kawasaki disease (group C; age, 15 +/- 1 years)-performed a treadmill exercise test. Patients of group R had a significant residual right ventricular outflow obstruction. Oxygen uptake (VO2), heart rate (HR), and plasma norepinephrine (NE) concentrations were measured at rest, during warm-up, at venti

Chest pain on exertion after the Takeuchi repair of anomalous origin of the left coronary artery: right ventricular ischemia due to severe pulmonary outflow tract obstruction.

Right ventricular (RV) outflow obstruction (in the form of valvar or supravalvular pulmonary stenosis) is a well-known complication of the Takeuchi procedure. We describe a 13-year-old male with exertional chest pain, pulmonary stenosis, RV hypertrophy, and consequent RV ischemia, which was confirmed using stress echocardiography and single-photon emission tomography.

Tetralogy of Fallot without the infundibular septum-restricted growth of the pulmonary valve annulus after annulus preservation may render the right ventricular outflow tract obstructive.

OBJECTIVE: Tetralogy of Fallot (ToF) with infundibular septal deficiency (ISD) is commonly associated with postoperative right ventricular outflow tract obstruction (RVOTO), presumably caused by subpulmonary extension of ventricular septal defect (VSD) and restricted growth of the pulmonary valve annulus (PVA). We sought to determine the postoperative growth of the PVA after annulus preservation according to the presence or absence of the infundibular septum. METHODS: A retrospective review of 90 patients who underwent surgical repair of ToF between June 1997 and August 2008 was performed. Me

Impact of residual right ventricular outflow tract obstruction on biventricular strain and synchrony in patients after repair of tetralogy of Fallot: a cardiac magnetic resonance feature tracking study.

OBJECTIVES: Residual right ventricular outflow tract (RVOT) obstruction (RVOTO) is considered beneficial in patients after repair of tetralogy of Fallot (TOF) although underlying mechanisms are unknown. We sought to elucidate differences in myocardial strain and dyssynchrony parameters in patients after TOF repair with and without residual RVOTO using cardiovascular magnetic resonance (CMR) feature-tracking (CMR-FT) analysis. METHODS: Fifty-four patients (mean age 16.4 ± 8.4 years) were assessed by CMR 14.2 ± 7.3 years after repair of TOF. Residual RVOTO on echocardiography was defined as a p

Outcomes of pulmonary valve replacement in 170 patients with chronic pulmonary regurgitation after relief of right ventricular outflow tract obstruction: implications for optimal timing of pulmonary valve replacement.

OBJECTIVES: The objectives of this study were to evaluate outcomes of pulmonary valve replacement (PVR) in patients with chronic pulmonary regurgitation (PR) and to better define the optimal timing of PVR. BACKGROUND: Although PVR is effective in reducing right ventricular (RV) volume overload in patients with chronic PR, the optimal timing of PVR is not well defined. METHODS: A total of 170 patients who underwent PVR between January 1998 and March 2011 for chronic PR were retrospectively analyzed. To define the optimal timing of PVR, pre-operative and post-operative cardiac magnetic resonan

Incidence, prognosis, and perinatal outcomes of and risk factors for severe twin-twin transfusion syndrome with right ventricular outflow tract obstruction in the recipient twin after fetoscopic laser photocoagulation.

BACKGROUND: Right ventricular outflow tract obstruction (RVOTO) is the most frequently encountered congenital heart disease in patients with twin -twin transfusion syndrome (TTTS) and is especially prevalent in the recipient twin. In this retrospective study, we evaluated the incidence, prognosis, postnatal management, and perinatal outcomes of and risk factors for RVOTO in the recipient twin in severe TTTS cases which diagnosed before 26 weeks after fetoscopic laser photocoagulation (FLP) at a single center in Taiwan. METHODS: RVOTO was diagnosed using fetal or postnatal echocardiography. Th

Palliation using a self-expandable stent in a patient with obstructive right-ventricular outflow tract after total correction of tetralogy of Fallot: self-expandable stent in tetralogy of Fallot.

Total correction of tetralogy of Fallot (TOF) is typically performed within the first year of life. However, subsequently, right-ventricular dysfunction often progresses by pulmonary stenosis or regurgitation. Although most patients with corrected TOF require additional surgery for right-ventricular outflow tract, intervention could be an alternative to palliative surgery and associated morbidities, especially in children. We describe a palliative application of the self-expandable stent in a patient with pulmonary stenosis after total correction of TOF who lacked a conduit between the right v

Five-year results from a prospective multicentre study of percutaneous pulmonary valve implantation demonstrate sustained removal of significant pulmonary regurgitation, improved right ventricular outflow tract obstruction and improved quality of life.

AIMS: Percutaneous pulmonary valve implantation (PPVI) is used to treat patients with dysfunctional pulmonary valve conduits. Short- and longer-term results from multiple trials have outlined haemodynamic improvements. Our aim was to report the long-term results, including quality of life, from a multicentre trial in Europe and Canada. METHODS AND RESULTS: From October 2007 to April 2009, 71 patients (24 female; median age 19.0 [IQR: 14.0 to 25.0] years) were enrolled in a prospective cohort study. PPVI was performed successfully in 63 patients. At five-year follow-up four patients had died.

Comparison of bare metal stenting and percutaneous pulmonary valve implantation for treatment of right ventricular outflow tract obstruction: use of an x-ray/magnetic resonance hybrid laboratory for acute physiological assessment.

BACKGROUND: Treatment of right ventricular outflow tract obstruction is possible with a bare metal stent (BMS), although this treatment causes pulmonary regurgitation. In this study, we assessed the acute physiological effects of BMS versus percutaneous pulmonary valve implantation (PPVI) using an x-ray/magnetic resonance hybrid laboratory. METHODS AND RESULTS: Fourteen consecutive children (median age, 12.9 years) with significant right ventricular outflow tract obstruction underwent BMS followed by PPVI. Early initiation of peritoneal dialysis improves postoperative recovery in children with right ventricular outflow tract obstructive lesions at high risk of fluid overload: a propensity score-matched analysis.

OBJECTIVES: Postoperative fluid overload is common in children after cardiac surgery, especially for those with right ventricular outflow tract obstruction, which is associated with poor outcomes. This study was conducted to investigate whether early peritoneal dialysis (PD) was associated with improved outcomes in these children at high risk of fluid overload. METHODS: Between January 2010 and January 2015, a total of 2555 consecutive patients with right ventricular outflow tract obstruction underwent anatomical repair. Using empirical risk evaluation, 219 patients at high risk of fluid over

The aortic translocation (Nikaidoh) procedure: midterm results superior to the Rastelli procedure.

OBJECTIVE: Midterm follow-up is analyzed after the aortic translocation (Nikaidoh) procedure, an alternative to the Rastelli procedure for ventriculoarterial discordance, ventricular septal defect, and pulmonary stenosis. METHODS: Nineteen patients underwent a Nikaidoh procedure at a median age of 3.3 years (0.9-9.3 years). The native aortic valve was translocated from the right to the left ventricular outflow tract by full (n = 6) or partial (n = 13) mobilization of the aortic root. Seven patients with partial mobilization had the right coronary artery reimplanted as a button. The conal sept

Surgical substrates of postoperative junctional ectopic tachycardia in congenital heart defects.

BACKGROUND: Junctional ectopic tachycardia is a major cause of postoperative morbidity after surgery for congenital cardiac disease. To elucidate the mechanism of junctional ectopic tachycardia, surgical correlations were studied in four types of congenital heart defects involving closure of a ventricular septal defect, relief of right ventricular outflow tract obstruction, or both. METHODS: Between 1997 and 1999, a total of 343 consecutive patients underwent repair of tetralogy of Fallot (n = 114), common truncus arteriosus (n = 10), ventricular septal defect (n = 161), and atrioventricular

Aortic root translocation plus arterial switch for transposition of the great arteries with left ventricular outflow tract obstruction: intermediate-term results.

OBJECTIVES: The goal of our study was to report our intermediate-term results with aortic root translocation plus arterial switch for d-transposition of the great arteries with left ventricular outflow tract obstruction. BACKGROUND: A d-transposition of the great arteries with left ventricular outflow tract obstruction represents a difficult surgical problem. The Rastelli procedure is the usual approach to this condition. However, recurrent left ventricular outflow tract obstruction and early conduit obstruction as well as arrhythmias and troublesome late mortality are significant limitations

Slicing technique of the RV outflow tract in transatrial-transpulmonary repair for tetralogy of Fallot.

The relief of a right ventricular outflow tract obstruction is a crucial step to successful transatrial-transpulmonary repair for tetralogy of Fallot. We here describe our technique for achieving good effective relief of a right ventricular outflow tract obstruction by slicing the wall without right ventriculotomy or with minimum transannular right ventriculotomy if necessary. The right ventricular outflow tract can be widely opened by slicing the inner half of the wall both through the tricuspid valve and the pulmonary valve. This procedure can be performed safely and effectively by detecting

Complete transposition of the great arteries: visualization of left and right outflow tract obstruction by oblique subcostal two-dimensional echocardiography.

Subcostal oblique 2-dimensional echocardiography was performed in 64 infants younger than 2 years with complete transposition of the great arteries (TGA) (situs solitus, concordant atrioventricular and discordant ventriculoarterial connections). All patients examined before cardiac catheterization had a correct diagnosis by 2-dimensional echocardiography using the subcostal oblique views. Twelve patients had associated left ventricular (LV) outflow tract obstruction and 7 had right ventricular (RV) outflow obstruction. The standard parasternal views failed to diagnose obstruction in 1 patients
