Sagittal Alignment Reciprocal Changes After Thoracolumbar/Lumbar Anterior Vertebral Body Tethering.

Background/Objective: The anterior vertebral body tethering (AVBT) technique, which preserves spinal mobility and avoids possible fusion problems in adolescent idiopathic scoliosis (AIS) patients, continues to be increasingly used in spine surgery. The study aims to report the early-to-early-mid postoperative radiological results of thoracolumbar/lumbar AVBT on sagittal alignment, and the second aim is to compare AVBT with selective thoracic fusion (STF) and non-selective fusion (NSF) groups in AIS patients. Methods: Patients with a diagnosis of AIS were retrospectively evaluated in the study.

Ingested Australasian Snapper (Chrysophrys auratus) Bone Migration to C5 Vertebral Body.

Fishbone ingestion has been reported multiple times previously as a cause of oesophageal perforation. This is a surgical emergency that needs to be identified early to ensure immediate medical attention. This report presents the case of a 70-year-old patient with laryngeal perforation and the migration of a Chrysophrys auratus (Australasian snapper) fishbone to the C5 vertebral body. It is hypothesized that the fishbone migrated from the larynx to the visceral fascia and prevertebral fascia before lodging in between the intramuscular substance of the longus coli muscle. Multiple imaging modali

Bilateral vertebral body tethering: identifying key factors associated with successful outcomes.

PURPOSE: The treatment of operative double major pediatric spinal deformities (e.g., Lenke 3 or 6) with bilateral vertebral body tethering (VBT) can be significantly more challenging when compared to other deformity patterns (e.g., Lenke 1) or treatment with a posterior spinal fusion. We aimed to identify preoperative and perioperative characteristics that were associated with successful postoperative outcomes in patients treated with both a thoracic and thoracolumbar (e.g., bilateral) tether. METHODS: We retrospectively assessed radiographic and clinical data from patients enrolled in a larg

Vertebral Body Morphology in Neuromuscular Scoliosis with Spastic Quadriplegic Cerebral Palsy.

Background/Objectives: The distorted vertebral body has been studied in scoliosis; however, there is little knowledge about the difference between neuromuscular and idiopathic scoliosis. This study aimed to investigate the vertebral body morphology in patients with spastic quadriplegic cerebral palsy and scoliosis (CP scoliosis) and compare them with those of apex- and Cobb angle-matched patients with adolescent idiopathic scoliosis (AIS). Methods: Thirty-four patients with CP scoliosis and thirty-two patients with AIS were included. The pedicle diameter, chord length, and vertebral body rotat

Associations between body composition and vertebral fracture risk in postmenopausal women.

INTRODUCTION: Osteoporosis is a metabolic bone disease with low bone mineral density (BMD) and high incidence of vertebral fractures (VFs). Postmenopausal women with osteoporosis have decreased total fat and lean mass. This study aimed to investigate the associations between body composition and VF risk and explore the potential predictor of VF risk in postmenopausal women. METHODS: Enrolled 731 postmenopausal women were referred by various departments and outpatient clinics to assess vertebral status between October 2016 and November 2017. The main measures were total body lean mass, fat mas

Factors Predictive of Outcomes in Vertebral Body Stapling for Idiopathic Scoliosis.

STUDY DESIGN: Retrospective review. OBJECTIVES: To identify factors associated with successful outcomes in patients treated with vertebral body stapling (VBS) for idiopathic scoliosis. SUMMARY OF BACKGROUND DATA: The standard of care for moderate scoliosis (20°-45°) consists of observation and bracing with the goal of halting curve progression. Although several recent studies have confirmed the efficacy of bracing in altering the natural history of scoliosis, bracing is not universally effective. Recent studies have demonstrated that VBS is a safe and viable treatment for some young patients

Vertebral body segmentation with GrowCut: Initial experience, workflow and practical application.

OBJECTIVES: Spinal diseases are very common; for example, the risk of osteoporotic fracture is 40% for White women and 13% for White men in the United States during their lifetime. Hence, the total number of surgical spinal treatments is on the rise with the aging population, and accurate diagnosis is of great importance to avoid complications and a reappearance of the symptoms. Imaging and analysis of a vertebral column is an exhausting task that can lead to wrong interpretations. The overall goal of this contribution is to study a cellular automata-based approach for the segmentation of vert

Re-irradiation of Vertebral Body Metastases: Treatment in the Radiosurgery Era.

Vertebral bodies remain one of the most common sites of metastases. In cases where surgical intervention is not indicated or appropriate, conventional external radiation therapy (cEBRT) has been the standard treatment modality. Unfortunately, cEBRT is typically limited, with low complete response and poor local control rates. Disappointing results with re-irradiation using cEBRT highlight the need for innovative salvage therapeutic strategies, such as stereotactic body radiotherapy. A detailed description of this complex treatment strategy is outlined, as is a systematic review of current lite

[Differentiation between acute osteoporotic and metastatic vertebral body fractures by imaging].

OBJECTIVES: This article discusses the morphological criteria for the differentiation between acute osteoporotic and metastatic vertebral body fractures and new imaging methods, such as diffusion-weighted and chemical shift magnetic resonance imaging (MRI) are presented. BACKGROUND: The differential diagnostics of osteoporotic and metastatic vertebral body fractures can be difficult in some cases. Both entities normally occur without adequate trauma and predominantly in elderly patients. IMAGING: Conventional X-ray examination is the initial imaging method of choice but is not able to reliab

Caudal vertebral body fractures following lateral interbody fusion in nonosteoporotic patients.

BACKGROUND: The minimally invasive lateral transpsoas approach for lumbar fusions has become increasingly popular. However, vertebral body fractures have been reported after this procedure, particularly in patients with osteoporosis and patients undergoing multilevel fusions. We evaluated the risk factors for caudal vertebral body fractures in 2 nonosteoporotic patients with single-level disease. CASE REPORTS: Two patients presented with several years' history of incapacitating chronic low back pain and intermittent radicular pain. Diagnostic imaging in both cases demonstrated grade 1 degener

A retrospective analysis of vertebral body fractures following epidural steroid injections.

BACKGROUND: Lumbar epidural steroid injections (LESIs) are frequently prescribed for the treatment of radiculopathy or neurogenic claudication arising from compression of spinal nerves. However, there is evidence suggesting that corticosteroids adversely affect bone strength by diminishing new bone formation and increasing bone resorption. Our study sought to assess whether LESIs increase the risk of subsequent vertebral body fracture. METHODS: A retrospective cohort study was conducted to compare patients receiving LESIs with a control group. A total of 50,345 patients with ICD-9 (Internatio

Changes in spinal canal diameter and vertebral body height with age.

PURPOSE: All structures of the spine, including the spinal canal, change continuously with age. The purpose of this study was to determine how the spinal canal of the lumbar spine changes with age. The L4/5 is the most common site of spinal stenosis and has the largest flexion-extension motion, whereas the T5/6 has the least motion. Therefore, we measured the spinal canal diameter and vertebral body height at T5, T6, L4, and L5 with age. MATERIALS AND METHODS: This was a retrospective study of aged 40 to 77 years. We reviewed whole spine sagittal MRIs of 370 patients with lumbar spinal stenos

Vertebral compression fracture risk after stereotactic body radiotherapy for spinal metastases.

OBJECT: The aim of this study was to identify potential risk factors for and determine the rate of vertebral compression fracture (VCF) after intensity-modulated, near-simultaneous, CT image-guided stereotactic body radiotherapy (SBRT) for spinal metastases. METHODS: The study group consisted of 123 vertebral bodies (VBs) in 93 patients enrolled in prospective protocols for metastatic disease. Data from these patients were retrospectively analyzed. Stereotactic body radiotherapy consisted of 1, 3, or 5 fractions for overall median doses of 18, 27, and 30 Gy, respectively. Magnetic resonance i

Prevention of vertebral body-splitting fractures after multilevel ProDisc-L implantation.

BACKGROUND: Prior studies of multilevel ProDisc-L (PD-L) implants (Synthes Spine, Inc., West Chester, Pennsylvania) using the standard US technique have used conventional radiography postoperatively. We found vertebral body-splitting fractures (VB-SFs) in interposed vertebral bodies after 5 sequential multilevel PD-L device implantations using the standard US technique. These were identified with postoperative computed tomography (CT) but were not visible on plain radiographs. In an additional patient, we found that a stress-relieving, pilot holes-only technique did not prevent VB-SFs. The 5 p

Thoracoscopic vertebral body stapling for treatment of scoliosis in young children.

PURPOSE: The management of juvenile idiopathic scoliosis (JIS) in young children can be challenging. Vertebral body stapling (VBS) is a new alternate to bracing and is intended to reduce or eliminate the need for subsequent spinal fusion. In concept, VBS of the anterior spine inhibits curve progression, thus stabilizing the spine as the child grows. There is a paucity of data in the pediatric population on its effectiveness. Therefore, we reviewed our experience with thoracoscopic VBS in children with juvenile scoliosis. SUBJECTS AND METHODS: After Institutional Review Board approval was obta

Vertebral body reconstruction for thoracolumbar spinal metastasis--a review of techniques.

The spine is the most common site of skeletal metastases with its involvement occurring in up to 40% of patients. Metastatic spinal involvement can cause a number of sequelae like pain, instability and neurologic compression. About 10% of patients with involvement of the vertebral column will subsequently develop neurologic compression. The metastatic spinal lesions mostly affect the vertebral body and pedicle (85%). Management of spinal metastases remains controversial. Recent reports attest to the beneficial role of surgery. The role of decompressive laminectomy without stabilization, has bee
