Neuronal network-based mathematical modeling of perceived verticality in acute unilateral vestibular lesions: from nerve to thalamus and cortex.

Acute unilateral lesions of vestibular graviceptive pathways from the otolith organs and semicircular canals via vestibular nuclei and the thalamus to the parieto-insular vestibular cortex regularly cause deviations of perceived verticality in the frontal roll plane. These tilts are ipsilateral in peripheral and in ponto-medullary lesions and contralateral in ponto-mesencephalic lesions. Unilateral lesions of the vestibular thalamus or cortex cause smaller tilts of the perceived vertical, which may be either ipsilateral or contralateral. Using a neural network model, we previously explained wh

Sex and Age Differences in Motion Sickness in Rats: The Correlation with Blood Hormone Responses and Neuronal Activation in the Vestibular and Autonomic Nuclei.

Many studies have demonstrated sex and age differences in motion sickness, but the underlying physiological basis is still in controversy. In the present study, we tried to investigate the potential correlates of endocrine and/or neuronal activity with sex and age differences in rats with motion sickness. LiCl-induced nausea symptom was evaluated by conditioned gaping. Motion sickness was assessed by measurement of autonomic responses (i.e., conditioned gaping and defecation responses), motor impairments (i.e., hypoactivity and balance disturbance) after Ferris wheel-like rotation, and blood h

Sympathetic and parasympathetic neurons are likely to be absent in the human vestibular and geniculate ganglia: an immunohistochemical study using elderly cadaveric specimens.

The vestibular and geniculate ganglia of the ear in experimental animals carry both of the tyrosine hydroxylase (TH)-positive sympathetic neurons and the neuronal nitric oxide synthase (nNOS)-positive parasympathetic neurons. With an aid of immunohistochemistry, we examined these ganglia as well as the horizontal part of the facial nerve using specimens from 10 formalin-fixed elderly cadavers. The submandibular ganglion from the same cadavers was used for the positive control for both markers. Although there was a nonspecific reaction in nuclei for the present antibody of nNOS, these ganglia w

Activation of μ-opioid receptors inhibits calcium-currents in the vestibular afferent neurons of the rat through a cAMP dependent mechanism.

Opioid receptors are expressed in the vestibular endorgans (afferent neurons and hair cells) and are activated by the efferent system, which modulates the discharge of action potentials in vestibular afferent neurons (VANs). In mammals, VANs mainly express the μ opioid-receptor, but the function of this receptors activation and the cellular mechanisms by which they exert their actions in these neurons are poorly studied. To determine the actions of μ opioid receptor (MOR) and cell signaling mechanisms in VANs, we made perforated patch-clamp recordings of VANs that were obtained from postnatal

[Unilateral caloric hyporeflexis].

Seventy-one patients with an unilateral caloric hypofunction were examined in order to establish the correlation with cochlear function. We divised our cases following the subjective clinical syndromes. Groupe A (12 patients) with a history of only one crise of vertigo. Groupe B (13 patients) with repeated vertiginous attacks. Groupe C (21 patients) with a positional vertigo only. Groupe D (15 patients) with rather undefinite complaints. Groupe E (7 patients) with no vertigo at all. We believe to discern the following syndromes in our cases of unilateral vestibular hypofunction: 9 cases with a

Multifunctional redox modulator prevents blast-induced loss of cochlear and vestibular hair cells and auditory spiral ganglion neurons.

Blast wave exposure, a leading cause of hearing loss and balance dysfunction among military personnel, arises primarily from direct mechanical damage to the mechanosensory hair cells and supporting structures or indirectly through excessive oxidative stress. We previously reported that HK-2, an orally active, multifunctional redox modulator (MFRM), was highly effective in reducing both hearing loss and hair cells loss in rats exposed to a moderate intensity workday noise that likely damages the cochlea primarily from oxidative stress versus direct mechanical trauma. To determine if HK-2 could

Light microscopical study of nitric oxide synthase I-positive neurons, including fibres in the vestibular nuclear complex of the cat.

Nitric oxide is a gaseous neurotransmitter that is synthesized by the enzyme nitric oxide synthase I (NOS I). At present, little is known of NOS I-positive neurons in the vestibular nuclear complex of the cat (VNCc). The aim of the present study was to examine the morphology, distribution patterns and interconnections of NOS I-positive neurons, including fibres in the VNCc. Five adult cats were used as experimental animals. All cats were anaesthetized and perfused transcardially. Brains were removed, postfixed, cut on a freezing microtome and stained in three different ways. Every third sectio

Dual task interference during gait in patients with unilateral vestibular disorders.

BACKGROUND: Vestibular patients show slower and unsteady gait; they have also been shown to need greater cognitive resources when carrying out balance and cognitive dual tasks (DT). This study investigated DT interference during gait in a middle-aged group of subjects with dizziness and unsteadiness after unilateral vestibular neuronitis and in a healthy control group. METHODS: Fourteen individuals with subacute unilateral vestibular impairment after neuronitis and seventeen healthy subjects performed gait and cognitive tasks in single and DT conditions. A statistical gait analysis system was

Treatment of vertigo.

Vertigo is the illusion of motion, usually rotational motion. As patients age, vertigo becomes an increasingly common presenting complaint. The most common causes of this condition are benign paroxysmal positional vertigo, acute vestibular neuronitis or labyrinthitis, Ménière's disease, migraine, and anxiety disorders. Less common causes include vertebrobasilar ischemia and retrocochlear tumors. The distinction between peripheral and central vertigo usually can be made clinically and guides management decisions. Most patients with vertigo do not require extensive diagnostic testing and can be

Retro-labyrinthine disorder in cases with peripheral facial palsy.

In some previously reported cases, facial palsy, even in Bell's palsy, involved vestibular dysfunction. Vestibular dysfunction in facial palsy is considered to be due to a retro-labyrinthine disorder such as vestibular neuronitis or acoustic neuromas. For the past 9 years we have been using the galvanic body sway test (GBST) routinely for differential diagnosis of the vestibular system. This paper reports GBST detected, retro-labyrinthine disorders in facial palsy. From 1981 to 1989, equilibrium examinations were performed in 33 cases with Bell's palsy and in 12 cases with Hunt's syndrome with

[The acute vestibular paralysis (author's transl)].

Acute vestibular paralysis may not be considered as a nosologic entity but as a syndrome. Symptomatology (vertigo, spontaneous and provoked vestibular nystagmus, absence of cochlear signs) shows an uniform picture. The results of the caloric test as well as the nystagmic responses induced by galvanic stimulation and the development of central vestibular compensation however indicate that the site of the lesion is not only confined to the labyrinth but may also occur at the level of the peripheral neuron or even the vestibular nuclei. Etiology and pathology are still vague. Our own clinical obs

Vertigo: few new spins on a common problem.

Vertigo is a common balance deregulation symptom among elders. Its etiology may be peripheral vestibular disease or a central vestibular disorder. Symptom onset, severity, and duration are distinguishing characteristics. The majority of peripheral vestibular disorders include benign paroxysmal positional vertigo, vestibular neuronitis, Meniere's disease, and perilymph fistula. Medication regimen review is necessary to identify possible drug-induced etiology. Treatment options include pharmacotherapy, canalith repositioning procedures, surgery, vestibular rehabilitation therapy, and dietary int

[The perverted nystagmus (author's transl)].

The perverted nystagmus is a modification of the horizontal beating nystagmus (in classical caloric stimulation) by a vertical or rotatory mainly component. Experimentally, this disorder can be shown after an unilateral destroy of the superior or median vestibular nucleus. We show 32 cases of subjects with a perverted nystagmus. 75% of these subjects have a pure central disease and 20% a vestibular neuronitis. The perverted nystagmus had an upbeat component with the cold stimulation and downbeat component with the hot stimulation. It was accompanied by a spontaneous nystagmus in 66% (and 30% w

[Lymphocyte subpopulations and HLA-DR determinations in diseases of the inner ear and Bell's palsy].

Thymus lymphocytes-subpopulations (T-helper T4 and T-suppressorcells T8) were determined in inner ear diseases (e.g. sudden hearing loss, neuronitis vestibularis, Morbus Menière, Bell's Palsy) by specific monoclonal antibodies. T-cell subset ratio (T4/T8) was elevated (less than 3) in about 50% of all patients. DR-typifying was performed because of the well-known control of the immunoregulation through the class II HLA-DR antigens. There was a relative risk of 5.2 in peripheral vestibular lesion (neuronitis vestibularis). The relative risk of autoimmune diseases is found at this level.

Clinical Findings in a Pati ent with Large B-Cell Lymphoma andNeuroniti s: A Case Study.

BACKGROUND: Given the multifaceted nature of the balance system, an interdisciplinary approach to patient care is imperative. For surgical patients with complex medical histories, comprehensive vestibular evaluations provide information on baseline vestibular function and identify possible surgical risks and outcomes. This case study follows a patient diagnosed with probable right vestibular neuronitis immediately preceding surgical resection of a left brainstem lesion, later diagnosed as a B cell lymphoma (BCL) and provides details regarding pre- and postsurgical outcomes. PURPOSE: This case

[Detection of latent herpes simplex virus in human vestibular ganglia].

Viral infections are considered to be one of the possible etiologies of vestibular neuronitis. Herpes simplex virus type 1 (HSV-1) is a likely candidate agent as this virus has a strong neurotropism, and can establish latent infections in the nervous system. However the existence of infections in the vestibular ganglia has not been established. In this study, 31 vestibular ganglia from autopsied adults were investigated. Polymerase chain reaction (PCR) was used to detect HSV-1 viral genomes, and reverse transcription-PCR (RT-PCR) and in situ hybridization (ISH) methods were employed to demonst

[Gaze-evoked suppression of the vestibular-ocular reflex in patients with posterior cervical sympathetic syndrome].

This vestibulometric (sinusoidal rotation) study included patients with posterior cervical sympathetic syndrome (n=40) and with vestibular neuronitis (n=20) as well as 20 healthy subjects of the control group. Vestibular dysfunction was induced by the method proposed by de Klein. The patients with posterior cervical sympathetic syndrome in the decompensation phase displayed abnormal gaze-evoked suppression of the vestibular-ocular reflex when their head movements lined up with sinusoidal rotation. This symptom is considered to be of primary value for the diagnosis of vestibular dysfunction in

New clinical finding in vestibular neuritis: high-frequency audiometry hearing loss in the affected ear.

Twenty-one patients with established vestibular neuronitis were examined otoneurologically and audiologically. High-frequency audiometry up to 20,000 Hz was performed on all the patients. In 17 cases a high-frequency hearing loss was greater in the ear with vestibular neuritis. The mean hearing loss difference varied from 14 dB to 24 depending on the frequency. Our results reveal that in most cases of vestibular neuritis there are no isolated vestibular lesions, but most cases also involve an auditory end-organ lesion. This finding is in unison with the anatomic conditions of the cochlea and t

Dynamic locomotor function in normals and patients with vertigo.

Gait analysis was performed in patients with various vestibular systems using a tactile sensor. There were 4 patients with vestibular neuronitis, 6 patients with large acoustic neuroma and 6 patients with spino-cerebellar degeneration (SCD). Gait phase related parameters such as stance, swing and double support were studied to assess gait stability. Also the area ratio of trajectories of center of force during stance and progression of foot pressure were checked. The calculated value of each variable became high in pathological cases compared with normal controls, and the highest value was obt
