Unilateral congenital ptosis: compensatory head posturing and amblyopia.

Patients with unilateral congenital ptosis often adopt chin elevation to maintain fusion. This compensatory head posturing has been considered a sign of fusion, thus indicating a low risk for amblyopia. To evaluate this sign, we reviewed the charts of 11 preverbal children with unilateral congenital ptosis who had straight eyes and appropriate compensatory head posturing to maintain binocular vision. Four additional patients with unilateral ptosis and no compensatory head posturing were tested for comparison. All patients had undergone an examination and had been tested by pattern Visual Evoke

Fixational eye movements and binocular vision.

During attempted visual fixation, small involuntary eye movements-called fixational eye movements-continuously change of our gaze's position. Disagreement between the left and right eye positions during such motions can produce diplopia (double vision). Thus, the ability to properly coordinate the two eyes during gaze fixation is critical for stable perception. For the last 50 years, researchers have studied the binocular characteristics of fixational eye movements. Here we review classical and recent studies on the binocular coordination (i.e., degree of conjugacy) of each fixational eye move

Adaptation to prism-induced heterophoria in subjects with abnormal binocular vision or asthenopia.

In a previous study we investigated how normal subjects adapted to prism-induced heterophoria. In this study we have continued this work by investigating this phenomena in 15 patients with abnormal binocular vision and/or asthenopia. It was found that the majority of subjects lacked or had a deficient adaptation system to base-in and/or base-out prisms, but all except two subjects examined adapted normally to vertical prism. These results suggest that it may be possible to treat the horizontal adaptation system independently from the vertical one. Prism vergences, heterophoria measurements, an

The Impact of Simulated Anisometropia and Bilateral Defocus on Reflex Vergence Responses of Children and Adults.

PURPOSE: This study assessed the impact of computationally-simulated defocus on reflex vergence movements responsible for maintaining binocularly aligned fixation in adults and children. This question is clinically relevant for uncorrected anisometropia during childhood and the use of monovision to manage presbyopia. METHODS: Reflex vergence step and continuous tracking responses were recorded from 28 adults (15-53 years) and 32 children (four to eight years) viewing a small spatially broadband stimulus with disparity changing in steps from 0° to ±4° for 0.32 second or following a random walk

Binocular dysfunction in Parkinson's Disease: Decoding near triad dynamics and divergence deficits.

Parkinson's Disease (PD) is a progressive neurodegenerative disorder that often affects the oculomotor system, causing strabismus and vergence impairments, particularly convergence insufficiency when focusing on nearby objects. This study evaluated eye deviation, vergence, accommodation, and pupil responses in PD during converging and diverging gaze shifts, correlating these findings with neurologic severity using the Unified Parkinson's Disease Rating Scale (UPDRS). We recruited 19 participants with varying severity of PD and 10 age-matched controls. 26 % of PD participants (PD Group 1) exhib

The clinical near gradient stimulus AC/A ratio correlates better with the response CA/C ratio than with the response AC/A ratio.

AIM: To provide evidence that a near clinical gradient AC/A ratio could instead reflect the CA/C relationship (the accommodation driven by response to disparity). DESIGN: Case control study. METHODOLOGY: 27 emmetropic participants with heterophoria <4 PD, 19 with intermittent distance exotropia, and 17 with near exophoria >6 PD were tested. A remote haploscopic photorefractor, which can measure simultaneous convergence and accommodation to a range of targets containing all combinations of presence or absence of binocular disparity, blur, and proximal (looming) cues, was used to assess respon

Pediatric ophthalmology and childhood reading difficulties: Overview of reading development and assessments for the pediatric ophthalmologist.

Reading difficulties are common in the pediatric population, and large socioeconomic disparities exist. In the United States 46% of white children achieved expected reading proficiency by the end of fourth grade, while only 21% of Hispanic and 18% of African American children were reading at the expected level. Reading is an involved cognitive process with many subskills; likewise, development of reading proficiency is a complex and continuous process. Failure to achieve reading proficiency or even early difficulty with reading can affect a child's academic performance for years to come. Some

Controversies and Disparities in the Management of Age-Related Macular Degeneration.

Age-related macular degeneration is a leading cause of blindness in patients aged 50 years and older. Prior to the 21st century, there were no effective treatments for this devastating disease. However, the last 20 years have heralded the development of treatments for both the nonexudative and exudative forms. The invention of AREDS vitamin supplements and anti-VEGF therapies forever changed the treatment of dry and wet age-related macular degeneration, respectively. The rapid adoption and expansion of these vision preserving treatments has created controversy regarding their cost, burden of a

Symptomatology associated with accommodative and binocular vision anomalies.

PURPOSE: To determine the symptoms associated with accommodative and non-strabismic binocular dysfunctions and to assess the methods used to obtain the subjects' symptoms. METHODS: We conducted a scoping review of articles published between 1988 and 2012 that analysed any aspect of the symptomatology associated with accommodative and non-strabismic binocular dysfunctions. The literature search was performed in Medline (PubMed), CINAHL, PsycINFO and FRANCIS. A total of 657 articles were identified, and 56 met the inclusion criteria. RESULTS: We found 267 different ways of naming the symptoms

[Do prism corrections according to H.-J. Haase affect ocular prevalence?].

BACKGROUND: For prism correction of associated phoria (at 5-6 m viewing distance), Hans-Joachim Haase developed--among other tests--the prevalence test, which consists of a central fixation target and two triangles that appear stereoscopically about 1.5 m in front of or behind the fixation target; the vertex of each triangle is directed towards the centre of the fixation target. Only when both eyes contribute equally to the perception of visual directions do the triangles appear centred (equivalence), while any perceived horizontal offset between the triangles and fixation target indicates a p

The Near Mallett Unit Foveal Suppression Test: a cross-sectional study to establish test norms and relationship with other optometric tests.

When the binocular system comes under prolonged stress in decompensated heterophoria, a small suppression area can develop at the fovea. This foveal suppression (FS) may be an adaptation to eliminate symptoms. The near Mallett unit (NMU) is a hand-held instrument that contains a binocular status test for detecting and quantifying FS. We measured FS in a cross-sectional study of 131 patients to investigate the normal range of responses. A ratio scoring method was devised which considers the number of letters read with dichoptic viewing when binocularly fused (b) and under monocular (m) conditio

Global Gender Disparities in Access to Refractive Error Services: A Systematic Review and Meta-Analysis.

TOPIC: Summarize existing evidence on global gender disparities in access to refractive error (RE) correction, among adults and children. CLINICAL RELEVANCE: Uncorrected RE remains the leading cause of vision impairment worldwide. Women and girls experience disproportionate levels of vision impairment, but gender disparities in access to RE correction are not well understood. Lower coverage among women may lead to prolonged vision impairment and functional limitations, impacting education, employment, and quality of life. METHODS: We searched MEDLINE (Ovid), Embase (Ovid), Global Health, and

[Disturbances of binocular vision in macular heterotopia].

BACKGROUND: Tractive translocation of the macula (secondary macular heterotopia) may result in disturbance of binocular vision. The report of a case shall discuss the sensorial problems of these patients. HISTORY AND SIGNS: We report of a 40-years old male who had decreased visual acuity and loss of binocular vision for several years due to episodes of uveitis with intravitreous hemorrhage and cataract formation. After bilateral vitrectomy and cataract extraction a good visual acuity was restored in both eyes. Postoperatively, the patient monocularly complained about disturbed egocentric loca

Unmet eye care needs among U.S. 5th-grade students.

BACKGROUND: There is substantial evidence of a disparity in access to eye care services among adults in the U.S.; however, little is known about health disparities for children's eye care. PURPOSE: The goal of the study was to assess the prevalence of and risk factors for 5th-grade students' unmet eye care needs. METHODS: Data were collected from 5147 5th-grade students (aged 10-11 years) and their parents and primary caregivers (hereafter "parents") participating in the Healthy Passages study between fall 2004 and summer 2006 (analyzed in 2011). The lightness/pitch crossmodal correspondence modulates reported dominance in binocular rivalry.

Crossmodal correspondences (CMCs: systematic associations between feature dimensions across sensory modalities) are known to facilitate performance in simple detection and discrimination tasks and can bias reports of ambiguous figures. The present study tested whether a lightness/pitch CMC biases reported dominance in binocular rivalry. Across three experiments, participants viewed dichoptically presented black and white circles accompanied by relatively high- or low-pitched tones. Dominance reports were coded as congruent when they matched the lightness/pitch correspondence (white/high, black

Topographical representation of binocular depth in the human visual cortex using fMRI.

We used binocular stimuli to define how the visual location of stereoscopic depth structure maps topographically onto the human visual cortex. The main stimulus consisted of a circular disk of dots, most at zero-disparity, against which a single quadrant was defined with changing disparity ('correlated' disparity), and moved around the visual field. The second stimulus had exactly the same structure, except that the disparity in the quadrant was 'anticorrelated,' that is black dots in one eye were paired with white dots in the other. Unlike the correlated stimulus, this 'anticorrelated' stimul

Acquired central fusion disruption following cataract extraction.

Central sensory and motor fusion of images, once established, is usually maintained throughout life. Under certain circumstances, this ability may be lost. This gives rise to the syndrome known as horror fusionis, or acquired disruption of central fusion. We report the development of central fusion disruption in three patients with uniocular mature senile cataract. We postulate that this is the result of prolonged sensory deprivation in the cataractous eye. The loss of fusion results in a particularly troublesome type of intractable diplopia, characterized by the inability to either fuse or su

Custom optimization of intraocular lens asphericity.

PURPOSE: To investigate the optimal amount of ocular spherical aberration (SA) in an intraocular lens (IOL) to maximize optical quality. METHODS: In 154 eyes of 94 patients aged 40 to 80 years, implantation of aspheric IOLs was simulated with different amounts of SA to produce residual ocular SA from -0.30 microm to +0.30 microm. Using the VOL-CT program (Sarver & Associates, Carbondale, Illinois), corneal wavefront aberrations up to 6th order were computed from corneal topographic elevation data (Humphrey Atlas, Carl Zeiss Meditec, Inc, Dublin, California). Dissociated vertical divergence: a righting reflex gone wrong.

Dissociated vertical divergence (DVD) is an ocular motor disorder characterized by a slow, upward drift of 1 eye when the other eye is fixating a target. I propose that DVD is a dorsal light reflex in which asymmetrical visual input to the 2 eyes evokes a vertical divergence movement of the eyes. This primitive visuo-vestibular reflex functions as a righting response to restore vertical orientation in lower lateral-eyed animals by equalizing binocular visual input. The dorsal light reflex is suppressed in humans but can manifest as DVD when early-onset strabismus precludes normal binocular dev
