Pars plana vitrectomy. Microsurgical pars plana lensectomy.

Pars plana lensectomy, wherein the entire lens within the pupillary area (the anterior capsule, anterior cortex, nucleus, posterior cortex, and posterior capsule) is removed, is now a well-established technique. It is to be considered as a method of choice for the removal of presenile cataracts in patients under the age of 40, developmental cataracts, cataracta complicata secondary to uveitis, subluxated and luxated cataracts. Pars plana lensectomy of the senile cataract is limited to a grade 2 nuclear sclerosis on a scale of 4. The use of ultrasonic instrumentation via the pars plana in conju

27‑Gauge vitrectomy vs. 25‑gauge vitrectomy in the management of proliferative diabetic retinopathy with preoperative intravitreal injection of conbercept.

Small-gauge vitrectomy has become popular due to its notable advantages, including less trauma, shortened convalescence and improved manoeuvrability. The aim of the present study was to compare the surgical outcomes of 27-gauge (27-G) vitrectomy with those of 25-gauge (25-G) vitrectomy in the management of proliferative diabetic retinopathy (PDR) with preoperative intravitreal injection of conbercept. The data of 48 consecutive patients with PDR (48 eyes) were retrospectively collected. The patients underwent conbercept intravitreal injection and pars plana vitrectomy with a 27-G group (23 eye

Vitrectomy in penetrating eye injury. Results of a controlled trial of vitrectomy in an experimental posterior penetrating eye injury in the rhesus monkey.

A controlled trial of pars plana vitrectomy for posterior penetrating eye injury was done in rhesus monkeys. After standard injury, each eye was allocated randomly into a treatment or control group. Treatment groups (ten eyes each) had vitrectomy at 1, 14, or 70 days after injury. The control group had 15 eyes. The same techniques were employed as in human eyes. Surgical objectives were lensectomy, then complete posterior vitrectomy and excision of the anterior vitreous to within 1 or 2 mm of the surface of the peripheral retina and pars plana. Results from this controlled trial provide strong

How much vitrectomy is enough? A narrative review.

The optimal extent of vitreous removal in modern vitreoretinal surgery remains controversial. While extensive vitrectomy has traditionally been performed to minimize traction and reduce postoperative complications, emerging evidence challenges its routine use across all indications. This narrative review critically evaluates whether extensive vitreous removal provides superior outcomes compared to limited, vitreous-sparing approaches. Current data do not demonstrate a consistent or clinically meaningful advantage of extensive vitrectomy. Although broader vitreous removal may reduce residual tr

Enhancing the outcomes of diabetic vitrectomy with pharmacological adjuvants.

This editorial offers insights from a minireview by Venkatesh et al, who explored pharmacological adjuvants for diabetic vitrectomy. Specifically, they synthesized current knowledge and evaluated the efficacy of various adjunctive therapies in improving the outcomes of diabetic retinopathy and managing associated complications. Herein, we highlight the key roles of pharmacological adjuvants in optimizing surgical techniques, minimizing intraoperative challenges, and enhancing postoperative recovery. We further discuss the potential implications of this approach for clinical practice and future

Vitreous function and intervention of it with vitrectomy and other modalities.

The vitreous body, the largest intraocular component, plays a key role in eye development, refraction, cell barrier function, oxygen metabolism and the pathogenesis of assorted diseases. Age, refraction and systemic diseases can cause vitreous metabolic abnormalities. With the continuous development of vitrectomy techniques and equipment, vitreous injections and vitrectomies have increased over the recent decades. However, the normal oxygen tension gradient in the vitreous helps to protect the lens and anterior chamber angle from oxidative stress damage, whereas the increased vitreous oxygen t

Current concepts and techniques of vitrectomy for retinopathy of prematurity.

Retinopathy of prematurity (ROP) is a leading cause of childhood blindness worldwide. ROP screening and interventions (e.g., laser ablation and anti-vascular endothelial growth factor [VEGF] therapy) at the right time can reduce disease activity and prevent retinal detachment. However, sometimes, ROP is refractory to treatment, leading to tractional retinal detachment (TRD), requiring surgical intervention, such as vitrectomy. Vitrectomy for Stage 5 ROP (total retinal detachment) is beneficial in preventing total blindness in some patients. However, it has poor anatomical and functional result
