Incidental gossypiboma on contrast enema study.

This is a case report of a 50-year-old woman with HIV/HCV coinfection who was diagnosed with an HPV-related well-differentiated squamous cell carcinoma of the vulva. After undergoing modified radical vulvectomy and bilateral inguinofemoral lymphadenectomy, the patient developed a rectovaginal fistula and a diverting colostomy was placed. During a postoperative contrast enema study, a curvilinear hyperdense stripe was noted, representing the radiodense marker thread of a retained surgical sponge. This case report highlights the importance of awareness of this retained surgical item and its asso

Carcinosarcoma of the vulva: a case report.

Carcinosarcoma is a well-recognized tumor even if it is an uncommon entity. Neoplasms usually occur in the oral cavity, pharynx, esophagus, larynx and skin, and have been rarely documented in the female genital tract. This case reports a patient with a diagnosis of vulvar carcinosarcoma that has been treated with radical vulvectomy and a left inguinal lymphadenectomy but she died two months later of progressive disease. Because of the extreme rarity and severe prognosis of the tumor, we believe that this is a useful addition to the literature and might serves as a reminder to physicians that a

Surgical treatment of carcinoma of the clitoris.

Thirty-eight consecutive patients were treated with either vulvectomy (14) or in combination with groin dissection (24) according to the same treatment protocol. The crude 5-year survival was 50% and the corrected 5-year survival was 66%. Three patients died postoperatively. Endophytic tumor, poor degree of differentiation, and involvement of lymph nodes resulted in higher mortalities. No patient with involvement of deep inguinal or pelvic nodes could be cured. The study concludes that invasive squamous cell carcinoma of the clitoris should be treated, in the same was as the same tumor in othe

Advanced primary carcinoma of the Bartholin gland: report of 18 patients.

Conservative surgery plus radiotherapy for vulvar cancer has been established as a therapeutic alternative to extensive radical surgery and produces a similar cumulative 5-year survival. We retrospectively analyzed the cases of 18 patients with advanced primary carcinoma of the Bartholin gland treated with wide local excision (WLE) or radical vulvectomy and lymphadenectomy followed by radiotherapy (RT) at the University of Texas M. D. Anderson Cancer Center from January 1978 through December 1990. All patients have been observed for a minimum of 7 months (maximum follow-up, 15 years; median fo

Paget's disease of the vulva in a patient with scleroderma and underlying adenocarcinoma: case report.

Systemic sclerosis (SSc) is a multisystem connective tissue disease characterized by fibrosis of the skin and internal organs. Several studies have demonstrated an increased frequency of cancer in patients with SSc. We report a case of a 71-year-old woman with SSc, who presented with an eczematous lesion of the vulva. The diagnosis of Paget's disease of the vulva (PVD) was established. The patient underwent radical vulvectomy, but 18 months later died due to adenocarcinoma of unknown primary origin. SSc and PVD are associated with various types of malignancies and patients suffering from these

[Substitution of immunoglobulins in severe infections].

In cases of severe infections substitution therapy of immunoglobulins shall improve the course of the disease. At example of a case report the possibility of the immunosubstitution is demonstrated. 31 days after the radical vulvectomy a bacteriantoxic phlegmone of the cutis and the subcutis in the abdominal, genital and thigh regions has been occurred. The acute medical care consists of shock therapy, large incisions of the cutis and the subcutis and high level antibiotic therapy. In the following course the immunoglobulins (Gamma-Venin) were substituted. The serum concentrations of IgG, M and

Pelvic lymphadenectomy in women with carcinoma of the clitoris.

Thirty-three women with carcinoma of the clitoris underwent surgical therapy, and 12% subsequently developed pelvic recurrence. However, none of the 18 patients without initial evidence of inguinal node metastases developed a pelvic recurrence. Of the 15 women with initial evidence of inguinal node metastases, 26.6% developed pelvic recurrence. It is, therefore, concluded that the addition of the routine use of pelvic lymphadenectomy to radical vulvectomy and bilateral inguinal lymphadenectomy in women with carcinoma of the clitoris is not warranted and that only those patients with histologic

Spindle cell sarcoma of the vulva with myofibroblastic differentiation.

OBJECTIVE: Primary vulvar sarcomas are rare lesions of the lower genital tract. We report the case of a patient with a spindle cell sarcoma of the vulva. MATERIALS AND METHODS: A 44-year-old woman presented with a painless vulvar mass. Vulvar biopsy demonstrated a spindle cell sarcoma with myofibroblastic differentiation. RESULTS: Pretreatment evaluation revealed no evidence of metastatic disease, and magnetic resonance imaging found no local masses. The patient underwent right radical vulvectomy with negative margins and tolerated the procedure well. CONCLUSIONS: Women undergoing gynecolog

Lymphangioma circumscriptum of the vulva: Report of a rare case.

Lymphangioma circumscriptum (LC) is a form of lymphangioma characterised by benign dilation of lymphatic channels, which affects the skin and subcutaneous tissues. The most common sites of LC are mucosa of mouth, tongue, groin, axilla, trunk and proximal region of extremities. Vulva is a rare site of LC. In this report, we are presenting a case of LC of vulva occurring in a 60-year-old female without any obvious reason. The patient presented with multiple genital wart-like papular lesions in the vulva. Biopsy of lesion reveals LC. She was treated with vulvectomy and showed no sign of recurrenc

Comparison of the treatment results in the vulvar and clitoris squamous cell carcinoma.

Treatment results in 360 patients with vulvar carcinoma during the time period from 1975 to 1994 were evaluated. Out of the whole group radical vulvectomy with bilateral inguinal lymphadenectomy was performed in 215 patients. After excluding 35 patients having the Stage IV of the disease, 11 with malignant melanoma and 9 missing, in the group of remaining 160 cases with squamous cell carcinoma the survival rates of 3, 5 and 10 years were evaluated. Squamous cell carcinoma involvement of the clitoris in 40 patients out of this 160 cases has not confirmed clitoris localization as a poor prognost

Epithelioid sarcoma of the Bartholin's gland primarily diagnosed as vulvar carcinoma.

A case of vulvar epithelioid sarcoma (VES) in a 49-year-old female is described. The disease originated from Bartholin's gland and was originally diagnosed as vulvar squamous cell carcinoma. The patient was treated by radical vulvectomy and bilateral inguinal lymphadenectomy. Multidrug chemotherapy using doxorubicin, cyclophosphamide, vincristine, and dacarbazine was started, but had to be abandoned because of cardiac toxicity. Despite intensive treatment, the patient developed pulmonary metastatic disease and died after 8 months. The course of epithelioid sarcoma in this site seems to be more

[A case report of Buschke-Lowenstein tumor (giant condyloma)].

A 28-year-old woman with diabetes mellitus and alcoholic hepatitis presented a rare case of Buschke Lowenstein tumor, or giant condyloma of vulva. HPV DNA of this tumor was detected by in situ hybridization using tritium labeled HPV 6 b, 16 and 18 DNA. This tumor appeared to harbour HPV 6 b DNA, but other HPV DNAs were negative. The distribution of HPV 6 b DNA was detected in the nucleus of the squamous epithelium showing koilocytosis. Total vulvectomy and resection of the clitoris were done. The postoperative course was uneventful and there has been no recurrence of tumor so far.

Aggressive Angiomyxoma of the Vulva Mimicking Clitoromegaly in a Young Child.

Aggressive angiomyxoma (AAM) is a rare soft tissue tumor of mesenchymal origin that chiefly involves the vulvar and perineal region. In 90% of cases, AAM presents in women of reproductive age with a peak incidence in the fourth decade of life. To date, AAM of the vulva in young children and infancy has never been reported. We present a case of AAM of the vulva that was initially interpreted as clitoromegaly in a 3-year-old girl.

Therapeutic implications of lymph nodal spread in lateral T1 and T2 squamous cell carcinoma of the vulva.

From 1963 to 1993, 157 patients with primary squamous cell carcinoma of the vulva were treated by radical surgery at the University of Kentucky Medical Center. There were 84 unilateral lesions confined to the labium majus or labium minus. Thirty-seven patients had T1 lesions, median diameter 1.5 cm (range 0.5-2.0 cm), and 47 patients had T2 lesions, median diameter 3.4 cm (range 2.2-9.0 cm). Radical vulvectomy with bilateral inguinal lymphadenectomy was performed in 56 patients and radical hemivulvectomy with selective inguinal lymphadenectomy in 28 patients. An average of 8 nodes was removed
