Are guidelines for non-invasive ventilation during weaning still valid?

Noninvasive ventilation (NIV) has gained increasing acceptance over the years to reduce endotracheal intubation, pneumonia and to prevent or treat respiratory failure in patients with different diagnoses. The international consensus conference, and the British society guidelines on NIV ventilation have analyzed its use during the weaning phase concluding that there were still conflicting results of its use. However, recent clinical trials have shown clear clinical benefits on the use of NIV in several patient populations during the weaning period. Acute respiratory failure (ARF) during the wea

Post-traumatic stress disorder after weaning from prolonged mechanical ventilation.

PURPOSE: Weaning from prolonged mechanical ventilation may be associated with mental discomfort. It is not known whether such discomfort is linked with the development of post-traumatic stress disorder (PTSD). Accordingly, we investigated whether PTSD occurs in patients after weaning from prolonged ventilation. We also determined whether administering a questionnaire would identify patients at risk for developing PTSD. METHODS: A prospective longitudinal study of patients transferred to a long-term acute-care hospital for weaning from prolonged ventilation was undertaken: 72 patients were stu

Determinants of weaning success in patients with prolonged mechanical ventilation.

INTRODUCTION: Physiological determinants of weaning success and failure are usually studied in ventilator-supported patients, comparing those who failed a trial of spontaneous breathing with those who tolerated such a trial and were successfully extubated. A major limitation of these studies was that the two groups may be not comparable concerning the severity of the underlying disease and the presence of comorbidities. In this physiological study, we assessed the determinants of weaning success in patients acting as their own control, once they are eventually liberated from the ventilator. M

Hemodynamic changes during weaning from nasal continuous positive airway pressure.

BACKGROUND: Nasal continuous positive airway pressure is frequently used to support preterm infants with respiratory distress syndrome. Little is known about the hemodynamic changes that occur, particularly during the weaning phase when lung compliance has improved and most of the airway pressure can be transmitted to the heart and major blood vessels. METHODS: We conducted a prospective study on preterm infants (gestational age <or=32 weeks) with resolving respiratory distress syndrome, who were receiving nasal continuous positive airway pressure of 5 cm H(2)O and 21% oxygen. While cycling n

[Effect of preterm infant position on weaning from mechanical ventilation].

OBJECTIVE: To determine the effects of prone positioning on cardiorespiratory stability and weaning outcome of preterm infants during weaning from mechanical ventilation. METHODS: From January to December 1999, a sample of 42 preterm infants, with birthweight < 2,000 g, mechanically ventilated in the first week of life, were randomly divided, in the beginning of the weaning process, into two groups according to the position: supine position (n = 21) or prone position (n = 21). Heart rate, respiratory rate, transcutaneous oxygen saturation and ventilatory parameters were recorded every one hou

[Difficult weaning from mechanical ventilation: a graduated approach and teamwork].

Weaning from mechanical ventilation in patients with chronic obstructive pulmonary disease (COPD) or left ventricular failure may be difficult. At the time of intubation and initiation of mechanical ventilatory support, this treatment is usually life-saving in the short term. Only later on, the condition which necessitated such support may prove irreversible. COPD patients often require positive end-expiratory pressure to enable them to trigger the ventilator comfortably. Patients with left ventricular failure need pharmacological support to reduce the circulating volume and to reduce left ven

Oxygen cost of breathing and weaning process in newborn infants.

Newborn infants may have a high oxygen cost of breathing (OCB) at the time of being weaned from mechanical ventilation. We hypothesized that this increase in oxygen consumption (V'O2) could be reduced by using certain weaning ventilatory modes. We designed a study to assess V'O2 during three weaning ventilatory modes: patient triggered ventilation, synchronous intermittent mandatory ventilation (SIMV) and continuous positive airway pressure in 16 newborn infants before being weaned from mechanical ventilation In seven infants whose OCB was high. V'O2 was not significantly different between CV

Characteristics associated with successful weaning in ventilator-dependent preterm infants.

Eighteen ventilator-dependent preterm infants with hyaline membrane disease were studied for 24 hours before and after an attempt at extubation. All were treated with theophylline prior to weaning and achieved average levels of 8.9 +/- 1.7 micrograms/ml (49 +/- 9 mumol/liter) in 13 successfully weaned infants and 8.4 +/- 1.1 micrograms/ml (47 +/- 6 mumol/liter) in 5 infants not extubated, p > 0.05. Infants successfully weaned were significantly (p < 0.01) older, more mature (29 +/- 1 versus 26 +/- 2 weeks' gestational age) and heavier (1107 +/- 236 versus 1016 +/- 256 gm) than infants not succ

A reduced CP level without medicinal zinc oxide does not alter the intestinal morphology in weaned pigs 24 days post-weaning.

The use of medicinal zinc oxide (ZnO) to prevent diarrhoea post-weaning will be banned in the EU from 2022. Therefore, new alternatives are needed to avoid an increase in diarrhoea and higher antibiotic use. A low dietary CP level has shown to lower the frequency of diarrhoea in pigs, due to lower microbial protein fermentation in the colon as well as improved conditions in the small intestine after weaning. The objective of this study was to examine the effect of decreased CP levels post-weaning as an alternative to medicinal ZnO on gut morphology and histopathology. Five hundred and sixty pi

Early weaning from invasive mechanical ventilation via high-flow nasal oxygen versus conventional weaning in patients with hypoxemic respiratory failure: a prospective randomized controlled study.

OBJECTIVE: Although the efficacy of high-flow nasal oxygen therapy in delaying or avoiding intubation in patients with hypoxemic respiratory failure has been studied, its potential for facilitating early weaning from invasive mechanical ventilation remains unexplored. METHODS: In this randomized controlled trial, 80 adults with acute hypoxemic respiratory failure requiring invasive mechanical ventilation for > 48 hours were enrolled and divided into two groups: conventional weaning and early weaning via high-flow nasal oxygen. In the conventional weaning group, the spontaneous breathing trial

Maternal chromium supplementation increases lamb weaning weight and modulates rumen microbiota.

Maternal nutrition during gestation can program offspring development and may influence the establishment of the rumen microbiome, with implications for growth and rumen health. However, the effects of maternal chromium (Cr) supplementation on offspring rumen microbial communities and performance in sheep remain poorly characterised. This study investigated the effects of maternal Cr supplementation during late gestation and early lactation on the offspring microbiome and performance in lambs. Fifteen pregnant ewes were assigned to two treatments in a randomised design: no Cr supplementation a

Integrated lung-diaphragm ultrasound in pediatric weaning assessment: a mini review.

Weaning from mechanical ventilation remains a major challenge in pediatric critical care. Diaphragm ultrasound has emerged as a promising bedside, radiation-free tool for assessing respiratory muscle function. However, the value of diaphragm thickening fraction (DTF) in neonates and children remains heterogeneous rather than definitive. While several pediatric studies have reported potentially useful DTF thresholds, often around 20%-25%, other cohorts and physiological validation studies have shown weak or inconsistent associations with extubation outcomes or inspiratory effort. These discrepa

Fetal programming through early weaning shapes the metabotype of Nelore heifers.

This study investigated the effects of fetal programming induced by early weaning (EW) of the previous calf on the serum metabotype of Nelore heifers. A 2 × 2 factorial design was employed, considering maternal category (multiparous vs. second-parity) and weaning strategy [EW vs. conventional weaning (CW)]. A total of 55 heifers were evaluated, including 25 born to cows subjected to EW (15 multiparous, 10 second-parity) and 30 born to cows subjected to CW (15 multiparous, 15 second-parity). Serum samples were collected from heifers at an average age of 14 months prior to the initiation pf the

Intravenous immunoglobulin weaning evaluation with zero-shot large language model classification.

OBJECTIVES: The aims of this study were to determine the performance of a large language model (LLM) with a zero-shot strategy for the classification of several factors relevant to the consideration of intravenous immunoglobulin (IVIg) weaning. BACKGROUND: In many cases, IVIg should be weaned to prevent excessive resource utilisation and adverse effects. METHODS AND MATERIALS: A cohort study was conducted examining neurology outpatients receiving regular IVIg in a 2-month period. Prespecified criteria were used to determine how many individuals were suitable for IVIg weaning. A LLM was appli

Laxative Weaning Protocol for Patients With Functional Constipation: A Pilot Study.

BACKGROUND: Children with functional constipation require prolonged laxative administration for proper emptying. Whether these laxatives can be weaned after better functioning is achieved is unknown. We aim to describe a standardized protocol for stimulant laxative weaning and its early outcomes. METHODS: Patients were candidates for weaning if they had been on a stable laxative dose for six months, defined as one bowel movement per day with no soiling, impaction, or enemas. Laxative dose was decreased by 10-25% with re-evaluation every two weeks. If patients remained well without constipatio
