Which continuous positive airway pressure system is best for the preterm infant with respiratory distress syndrome?

Various commercial and home-made continuous positive airway pressure (CPAP) systems are described in this article. CPAP may be delivered via a range of device-patient interfaces; nasal CPAP is most common, and short binasal prongs impose the least extrinsic load impedance on the infant. The source of pressure generation is categorized as either constant pressure or constant flow. The efficacy of different systems may vary according to whether lung volume recruitment, airway patency, minimization of work of breathing, or central nervous system stimulation are the primary goal of the clinical de

Use of heliox in children.

For over 70 years, helium-oxygen mixture (heliox) has been promoted as adjunctive therapy to overcome airflow-obstructive disorders and lesions. In the past 2 decades heliox has gained widespread support in many pediatric emergency departments and intensive care units, in treatment of infants and children with both upper and lower airway obstruction. Because heliox is less dense than air or oxygen, it provides more laminar flow in obstructed airways, and it is purported to reduce work of breathing, respiratory distress, and postextubation stridor. Clinical evidence of the effectiveness of heli

Infant sleeping position and inspiratory pressures in the upper airways and oesophagus.

Inspiratory pressures were measured at three levels in the upper airways and in the oesophagus in different sleeping positions. Thirty one consecutive healthy infants were examined at the age of 1-3 days, 25 of these at 6 weeks, and 23 at 12 weeks. Oxygen saturation, an electrocardiogram, transthoracic impedance, nasal airflow, and inspiratory pressure in the epipharynx, oropharynx, hypopharynx, and in the intrathoracic part of the oesophagus were measured. There was no significant change in the heart rate, oxygen saturation, or respiratory rate in the four sleeping positions. The peak negativ

Ventilatory work and oxygen consumption during exercise and hyperventilation.

The work of breathing (WB), and thus the energy requirement of the respiratory muscles, is increased any time minute ventilation (VE) is elevated, by either exercise or voluntary hyperventilation. Respiratory muscle O2 consumption (VRMO2) in humans has generally been estimated by having subjects breathe at a level comparable to that during exercise while the change in O2 consumption (VO2) is measured. The difference between VO2 at rest and during hyperventilation is attributed to the respiratory muscles and is assumed to be similar to VRMO2 during exercise at the same VE. However, it has been

Volume-assured pressure support ventilation (VAPSV). A new approach for reducing muscle workload during acute respiratory failure.

This study reports the preliminary clinical evaluation of a new mode of ventilation--volume-assured pressure support ventilation (VAPSV)--which incorporates inspiratory pressure support (PSV) with conventional volume-assisted cycles (VAV). This combination optimizes the inspiratory flow during assisted/controlled cycles, reducing the patient's respiratory burden commonly observed during VAV. Different from conventional PSV, VAPSV assures precise control of tidal volume (VT) in unstable patients. Eight patients with acute respiratory failure (ARF) were submitted to assisted ventilation under VA

[Nutrition in chronic obstructive bronchopneumopathy].

The respiratory system and nutrition are linked. Obesity is sometimes seen in chronic obstructive pulmonary disease (COPD), but its prevalence, the morbidity and mortality induced by it are not known. In addition, the prevalence of malnutrition is high in COPD and the more severe the COPD is, the higher percentage of malnutrition is present. Emphysematous patients are more frequently undernourished than those suffering from chronic bronchitis. Malnutrition is the consequence of the hypermetabolism induced by the higher cost of breathing in emphysema. The survival rate of these patients is nega

Expiratory timing in obstructive sleep apnoeas.

Diaphragmatic electromyogram was recorded during NREM sleep in 4 patients affected by obstructive sleep apnoea (OSA) syndrome in order to evaluate the behaviour of expiratory time (TE) in the course of the obstructive apnoea-ventilation cycle. The two components of TE, i.e. time of post-inspiratory inspiratory activity (TPIIA) and time of expiratory phase 2 (TE2) were separately analysed. TPIIA showed a short duration, with only minor variations, within the apnoea, while its duration was more variable and longer in the interapnoeic periods: the longest TPIIA values were associated with the hig

Association of Breathing Effort With Survival in Patients With Acute Respiratory Distress Syndrome.

OBJECTIVES: Invasive mechanical ventilation (IMV) is crucial for acute respiratory distress syndrome (ARDS) management, but mortality remains high. While spontaneous breathing is key to weaning, excessive respiratory effort may injure the lung and diaphragm. Most existing data on respiratory effort during IMV are based on brief periods of observation, potentially underestimating the burden of inappropriate efforts. This study aims to characterize the evolution of respiratory effort over time in ARDS patients and its relation to survival. We hypothesized that nonsurvivors would spend a greater

Heated Humidified High-Flow Nasal Oxygen in Adults: Mechanisms of Action and Clinical Implications.

Traditionally, nasal oxygen therapy has been delivered at low flows through nasal cannulae. In recent years, nasal cannulae designed to administer heated and humidified air/oxygen mixtures at high flows (up to 60 L/min) have been gaining popularity. These high-flow nasal cannula (HFNC) systems enhance patient comfort and tolerance compared with traditional high-flow oxygenation systems, such as nasal masks and nonrebreathing systems. By delivering higher flow rates, HFNC systems are less apt than traditional oxygenation systems to permit entrainment of room air during patient inspiration. Comb

Nocturnal evolution of respiratory effort in obstructive sleep apnoea syndrome: influence on arousal threshold.

It has been recently described that the overnight increase in maximal end-apnoeic oesophageal pressure (P(oes, max)), considered as an index of the arousal threshold to occlusion, mostly contributes to apnoea lengthening during the night. However, the rise in apnoea duration could also be caused by changes in hypoxaemia, chemosensitivity and upper airway resistance. To better define the relative contributions of each of these factors, we examined the recordings of nine patients. Before apnoea, the mean pulmonary resistance at peak inspiratory flow (RPIF) was computed. During apnoea, all swings

The cost of breathing in stable chronic obstructive pulmonary disease.

1. The hypermetabolism frequently observed at rest in patients with chronic obstructive pulmonary disease has been attributed to a high cost of breathing. However, measurement of the cost of breathing by the usual hyperventilation procedure is fraught with methodological problems. The purpose of this study was to measure more directly the cost of breathing in a group of ambulatory patients with stable chronic obstructive pulmonary disease. 2. The cost of breathing was calculated as the difference in oxygen consumption measured by indirect calorimetry between spontaneous breathing and noninvasi

Indoor and outdoor pollutants and the upper respiratory tract.

The health effects of both indoor and outdoor air pollutants are of increasing concern. The health effects of outdoor air pollutants traditionally have been assessed through measurements of lower respiratory tract changes. However, it has been shown that one outdoor air pollutant, sulfur dioxide, decreases nasal mucus flow and increases nasal airway resistance. Along with cigarette smoke, indoor air pollutants such as formaldehyde, cadmium, and ammonium or sulfate ions have been shown to alter upper airway mucociliary function. Emissions from wood stoves are known to irritate the upper airways

Non-vagal reflex effects on medullary inspiratory neurons during inspiratory loading.

Studies were conducted to compare the first-breath responses of medullary Dorsal and Ventral Respiratory Group inspiratory (I) neurons to the mechanical loading (tracheal occlusion, TO) of inspiration in unanesthetized (decerebrate) and anesthetized (Dial) vagotomized cats, and to determine the sources of the sensory activity causing the changes in I-neuron activity. In decerebrate cats, TO resulted in a prolongation of the firing duration in 49% of the I-neurons. There was a delayed onset of firing in 7% of the I-neurons. The responses of I-neurons to TO in anesthetized cats were similar to t

Sex and APOE genotype influence respiratory function under hypoxic and hypoxic-hypercapnic conditions.

The apolipoprotein E (APOE) gene has been studied due to its influence on Alzheimer's disease (AD) development and work in an APOE mouse model recently demonstrated impaired respiratory motor plasticity following spinal cord injury (SCI). Individuals with AD often copresent with obstructive sleep apnea (OSA) characterized by cessations in breathing during sleep. Despite the prominence of APOE genotype and sex as factors in AD progression, little is known about the impact of these variables on respiratory control. Ventilation is tightly regulated across many systems, with respiratory rhythm for

Role of breath phase and breath boundaries for the classification between asthmatic and healthy subjects.

Asthma is an inflammatory disease of the airways which causes cough, chest tightness, wheezing and other distinct sounds during breathing. Spirometry is a golden standard lung function test, is used to monitor and diagnose asthma. Spirometry is very time-consuming and requires a lot of patient's efforts. Therefore, an alternate method which can overcome spirometry limitations is required. Sound based method can be one such alternative as it is less tedious, less time consuming and suitable for patients of all ages. It has been shown in the past that breath, among other vocal sounds, performs t

Awareness of breathing: the structure of language descriptors of respiratory sensations.

OBJECTIVE: Recent research suggests that dyspnea is not a single sensation but a multidimensional construct reflected in different verbal descriptors that can provide useful diagnostic information. In this study superordinated clusters of dyspnea were investigated in combination with a dimensional approach. DESIGN: We examined the use of 20 respiratory symptom descriptors by healthy volunteers who completed a protocol of seven experimental conditions: Quiet breathing, breath holding, paced breathing, climbing stairs, resistive load breathing, voluntary hyperinflation, and voluntary hyperventi

Modeling alveolar volume changes during periodic breathing in heterogeneously ventilated lungs.

A simplified model of periodic breathing, proposed by Whiteley et al. (Math. Med. Biol. 20:205-224, 2003), describes a non-uniform breathing pattern for a lung with an inhomogeneous gas distribution, such as that observed in some subjects suffering from respiratory disease. This model assumes a constant alveolar volume, and predicts incidence of irregular breathing caused by small, poorly ventilated regions of the lung. Presented here is an extension to this work which, by allowing variable lung volume, facilitates the investigation of pulmonary collapse in poorly ventilated compartments. A we
