Effect of straw particle size on the behavior, health, and production of early-lactation dairy cows.

The objective of this study was to determine the effect of reducing the particle size of wheat straw in a total mixed ration (TMR) on cow behavior, health, and production in early lactation. For 28 d after calving, 41 multiparous Holstein cows were individually provided either 1 of 2 TMR with 9% wheat straw (dry matter basis) chopped (1) using a 2.54-cm screen (short; n = 21) or (2) using a 5.08-cm screen (long; n = 20). Cows were housed in freestall pens during both the dry and lactating period. Enrollment in the trial was on a rolling basis and cows were evenly distributed by parity and milk

Use of physician education and computer alert to improve targeted use of gastroprotection among NSAID users.

BACKGROUND: Gastrointestinal (GI) hemorrhage accounts for 200-400,000 admissions in the United States annually. Around 50% of patients with bleeding ulcer have used aspirin and/or nonsteroidal anti-inflammatory drugs (NSAIDs). Misoprostol and proton pump inhibitors (PPIs) may reduce NSAID-related upper GI tract complications in high-risk patients, but their targeted use may be suboptimal. AIM: To determine the impact of physician education, a computer alert, or both on the targeted use of GI prophylaxis in high-risk patients discharged from hospital. METHODS: To target high-risk patients, we

Knowledge Performance, User Experience, and Cybersickness With Immersive Virtual Reality for Tai Chi Movement-Encoding in Older Adults: Randomized Comparison Study.

BACKGROUND: Immersive virtual reality (VR) is increasingly used for embodied training, but its practical value for older adults depends on whether the experiential benefits can be achieved without unacceptable discomfort or overstated claims about motor learning. OBJECTIVE: This randomized study compared head-mounted immersive VR with standardized, content-matched 2D video instruction for Tai Chi movement-encoding in community-dwelling older adults. METHODS: A total of 106 older adults were randomized to immersive VR (n=56) or video instruction (n=50). The primary analysis included 92 partic

Chemogenetic activation of cholinergic intrinsic cardiac ganglia improves border zone oxygenation and reduces arrhythmias during acute local ischemia.

BACKGROUND: Current therapeutic strategies for acute myocardial infarction rely on reperfusion and pharmacological management, which are typically administered hours after an event. Activation of the cardiac cholinergic efferents superior to the heart soon after a coronary occlusion has shown promise as a potential therapy to reduce arrhythmias and improve ventricular function. We tested whether selective activation of cholinergic neurons within the intrinsic cardiac ganglia (ICG) would also reduce arrhythmias and improve oxygenation of ischemic border zone tissue after an acute coronary occlu

RIMKLA promotes hypertension by activating PKM2 to trigger VSMC phenotype switch.

BACKGROUND: Hypertension affects around one billion adults worldwide, with abnormal glucose metabolism and vascular smooth muscle cell (VSMC) phenotype switch playing crucial roles in its pathogenesis. Pyruvate kinase M2 (PKM2) is a key glycolytic enzyme, but its regulation and roles in VSMC phenotype switch and hypertension are unknown. Using the Gene Importance Calculator (GIC) to predict gene essentiality, we identified ribosomal modification protein rimK-like family member A (RIMKLA) as a highly relevant gene and explored its regulatory contributions to hypertension. METHODS: Internal mam

Delayed ischemic stroke ten days after aluminum phosphide ingestion in a previously healthy 21-year-old Ethiopian woman: a case report.

BACKGROUND: Aluminum phosphide poisoning is a highly lethal toxicologic emergency classically dominated by cardiovascular collapse. Delayed ischemic stroke after apparent clinical stabilization is exceptionally rare and mechanistically uncertain. CASE PRESENTATION: A 21-year-old previously healthy Ethiopian woman intentionally ingested five aluminum phosphide tablets. After early gastric lavage, she remained hemodynamically stable during the initial 24 hours and was discharged. Ten days after ingestion, she developed sudden right facial weakness, right hemiparesis, and mild dysarthria. On day

Incidence, implications, and management of sense-B-noise failure in subcutaneous cardioverter-defibrillator patients: insights from a large multicentre registry.

AIMS: Subcutaneous implantable cardioverter-defibrillators (S-ICDs) offer potentially distinct advantages over transvenous defibrillator systems. Recent randomized trials showed significantly lower lead failure rates than transvenous ICD. Still, S-ICDs remain associated with the risk of inappropriate shocks (IAS). While previous studies have reported varying causes of IAS, this study explores a rare cause of IAS, referred to as 'sense-B-noise.' It was recently described in case series, but its incidence has not been studied in a large cohort of S-ICD patients. METHODS AND RESULTS: We retrospe

Cognitive and Salience Network Connectivity Changes following a Single Season of Repetitive Head Impact Exposure in High School Football.

BACKGROUND AND PURPOSE: During a season of high school football, adolescents with actively developing brains experience a considerable number of head impacts. Our aim was to determine whether repetitive head impacts in the absence of a clinically diagnosed concussion during a season of high school football produce changes in cognitive performance or functional connectivity of the salience network and its central hub, the dorsal anterior cingulate cortex. MATERIALS AND METHODS: Football players were instrumented with the Head Impact Telemetry System during all practices and games, and the helm

Contribution of macrophages to intracochlear tissue remodeling responses following cochlear implantation and neural survival.

INTRODUCTION: Cochlear implants (CIs) restore hearing to deafened patients. The foreign body response (FBR) following cochlear implantation (post-CI) comprises an infiltration of macrophages, other immune and non-immune cells, and fibrosis into the scala tympani; a space that is normally devoid of cells. This FBR is associated with negative effects on CI outcomes including increased electrode impedances and loss of residual acoustic hearing. This study investigates the extent to which macrophage depletion by an orally administered CSF-1R specific kinase (c-FMS) inhibitor, PLX-5622, modulates t

Role of sympathetic pathway in light-phase time-restricted feeding-induced blood pressure circadian rhythm alteration.

Disruption of blood pressure (BP) circadian rhythm, independent of hypertension, is emerging as an index for future target organ damage and is associated with a higher risk of cardiovascular events. Previous studies showed that changing food availability time alters BP rhythm in several mammalian species. However, the underlying mechanisms remain largely unknown. To address this, the current study specifically investigates (1) the relationship between rhythms of food intake and BP in wild-type mice; (2) effects of light-phase time-restricted feeding (TRF, food only available during light-phase

Circadian Differences in the Contribution of the Brain Renin-Angiotensin System in Genetically Hypertensive Mice.

Objective: Genetically hypertensive BPH/2J mice are recognized as a neurogenic model of hypertension, primarily based on sympathetic overactivity and greater neuronal activity in cardiovascular regulatory brain regions. Greater activity of the central renin angiotensin system (RAS) and reactive oxygen species (ROS) reportedly contribute to other models of hypertension. Importantly the peripheral RAS contributes to the hypertension in BPH/2J mice, predominantly during the dark period of the 24 h light cycle. The aim of the present study was to determine whether central AT1 receptor stimulation

Retrospective Chart Review of Synthetic Cannabinoid Intoxication with Toxicologic Analysis.

INTRODUCTION: Use of synthetic cannabinoids (SC) has recently emerged as a new drug epidemic. Our emergency departments (EDs) received a surge of SC users presenting with lethargy and bradycardia, contrasting prior reports of SC-induced tachycardia and agitation. Our goal was to describe these novel presentations and characterize the compounds. METHODS: We present a case series of patients with SC intoxication who presented to our toxicology service covering two tertiary care EDs between 2/11/2015 and 6/23/2015. A retrospective chart review recorded initial vital signs, chief complaint and cl

γδ T Cells Mediate Angiotensin II-Induced Hypertension and Vascular Injury.

BACKGROUND: Innate antigen-presenting cells and adaptive immune T cells have been implicated in the development of hypertension. However, the T-lymphocyte subsets involved in the pathophysiology of hypertension remain unclear. A small subset of innate-like T cells expressing the γδ T cell receptor (TCR) rather than the αβ TCR could play a role in the initiation of the immune response in hypertension. We aimed to determine whether angiotensin (Ang) II caused kinetic changes in γδ T cells; deficiency in γδ T cells blunted Ang II-induced hypertension, vascular injury, and T-cell activation; and γ

Circadian rhythms are disrupted in patients and preclinical models of Machado-Joseph disease.

Machado-Joseph disease (MJD) is caused by an abnormal CAG repeat expansion in the ATXN3 gene, leading to the expression of a mutant ataxin-3 (mutATXN3) protein. Patients with MJD exhibit a wide range of clinical symptoms, including motor incoordination. Emerging evidence highlights circadian rhythm disruptions as early indicators and potential risk factors for the progression of neurodegenerative conditions. Circadian rhythms are regulated by internal clocks, with the suprachiasmatic nucleus (SCN) acting as the master pacemaker to synchronize timing across the body's behavioural and physiologi

An Intelligent Medical Isolation Observation Management System Based on the Internet of Things.

BACKGROUND: Since COVID-19 (coronavirus disease 2019) was discovered in December 2019, it has spread worldwide. Early isolation and medical observation management of cases and their close contacts are the key to controlling the spread of the epidemic. However, traditional medical observation requires medical staff to measure body temperature and other vital signs face to face and record them manually. There is a general shortage of human and personal protective equipment and a high risk of occupational exposure, which seriously threaten the safety of medical staff. METHODS: We designed an int

Magnetic Resonance Imaging in Patients With Cardiac Implantable Electronic Devices With Abandoned Leads.

IMPORTANCE: Magnetic resonance imaging (MRI) is the modality of choice for many conditions. Conditional devices and novel protocols for imaging patients with legacy cardiac implantable electronic devices (CIEDs) have increased access to MRI in patients with devices. However, the presence of abandoned leads remains an absolute contraindication. OBJECTIVE: To assess if the performance of an MRI in the presence of an abandoned CIED lead is safe and whether there are deleterious effects on concomitant active CIED leads. DESIGN, SETTING, AND PARTICIPANTS: This cohort study included consecutive CI

A Rare and Severe Presentation of Henoch-Schönlein Purpura in an Adolescent With Crescentic Glomerulonephritis, Arrhythmia, Acute Gastrointestinal Bleed, and Neurological Complications.

Henoch-Schönlein purpura (HSP) is a childhood vasculitis disorder that involves the skin, joints, gastrointestinal (GI) tract, and kidneys. It is related to immunoglobulin A (IgA) antibody deposition in small blood vessels. HSP is a self-limiting disorder, but its morbidity is primarily associated with renal involvement. GI pathologies like intussusception, gastritis, duodenitis, ileitis, or ulcer have been reported to be associated with this disease. However, cardiac and neurological complications are rarely reported. We present the case of a 16-year-old, previously healthy male who was diagn

Iron-deficiency and dopaminergic treatment effects on RLS-Like behaviors of an animal model with the brain iron deficiency pattern of the restless legs syndrome.

BACKGROUND: Brain iron deficiency (BID), especially for the substantia nigra (SN), without peripheral iron deficiency (ID) has been well documented as a ubiquitous finding for restless legs syndrome (RLS) patients. This close association suggests the biology of RLS BID can produce RLS symptoms. Association, however, cannot establish such a direct relationship. Instead, the BID of RLS could be experimentally produced to determine if it then produces significant RLS-like biological or behavioral features. Forward genetics approach led to identification from the BXD strains the BXD40 females (BXD
