Symptomatic contralateral subdural hygromas after decompressive craniectomy: plausible causes and management protocols.

OBJECT: Contralateral subdural hygromas are occasionally observed after decompressive craniectomies (DCs). Some of these hygromas are symptomatic, and the etiology and management of these symptomatic contralateral subdural collections (CLDCs) present surgical challenges. The authors share their experience with managing symptomatic CLSDCs after a DC. METHODS: During a 10-month period, 306 patients underwent a DC. Of these patients, 266 had a head injury, 25 a middle cerebral artery infarction (that is, a thrombotic stroke), and 15 an infarction due to a vasospasm (resulting from an aneurysmal

Neurosurgical injuries resulting from the 2011 tornados in Alabama: the experience at the University of Alabama at Birmingham Medical Center.

OBJECT: The April 27, 2011, tornados that affected the southeastern US resulted in 248 deaths in the state of Alabama. The University of Alabama at Birmingham (UAB) Medical Center, the largest Level I trauma center in the state, triaged and treated a large number of individuals who suffered traumatic injuries during these events, including those requiring neurosurgical assessment and treatment. METHODS: A retrospective review of all adult patients triaged at UAB Medical Center during the April 27, 2011, tornados was conducted. Those patients who were diagnosed with and treated for neurosurgic

Costs and benefits of neurosurgical intervention for infant hydrocephalus in sub-Saharan Africa.

OBJECT: Evidence from the CURE Children's Hospital of Uganda (CCHU) suggests that treatment for hydrocephalus in infants can be effective and sustainable in a developing country. This model has not been broadly supported or implemented due in part to the absence of data on the economic burden of disease or any assessment of the cost and benefit of treatment. The authors used economic modeling to estimate the annual cost and benefit of treating hydrocephalus in infants at CCHU. These results were then extrapolated to the potential economic impact of treating all cases of hydrocephalus in infant

Management of subarachnoid hemorrhage patients who presented with respiratory arrest resuscitated with bystander CPR.

BACKGROUND AND PURPOSE: The sudden death rate from aneurysmal subarachnoid hemorrhage (SAH) is 10%. Since 1989, 26 SAH patients who were witnessed to collapse into coma with respiratory arrest and required cardiopulmonary resuscitation (CPR) at the scene survived to reach the hospital and be diagnosed. Although reports on hospital management of grade V SAH suggest improved outcome, no report has previously addressed the issue of respiratory arrest after acute SAH. We analyze our experience with this unique subgroup of aneurysmal SAH patients. METHODS: This is a retrospective analysis of 26 co

Do Complication Rates of Ventricular Drain Placement Differ between Twist Drill and Burr Hole in Acute Hydrocephalus?

OBJECTIVE: The aim of the study was to compare two techniques for external ventricular drainage (EVD) placement with respect to their complication rates. METHODS: A retrospective descriptive study was performed to analyze all patients who had undergone EVD implantation for acute hydrocephalus between January 2010 and December 2013 with a focus on surgical technique and rate of complications. The burr hole technique (BHT) was used in one group and the twist-drill technique (TDT) in the other. Particular attention was paid to malposition, hemorrhage, and catheter-associated infection. RESULTS:

Long subcutaneous tunnelling reduces infection rates in paediatric external ventricular drains.

PURPOSE: The aim of this study is to report the efficacy of long subcutaneous tunnelling of external ventricular drains in reducing rates of infection and catheter displacement in a paediatric population. METHODS: In children requiring external ventricular drainage, a long-tunnelled drain was placed and managed according to a locally agreed guideline. End points were novel CSF infection incurred during the time of drainage and re-operation to re-site displaced catheters. Data were compared to other published series. RESULTS: One hundred eighty-one long-tunnelled external ventricular drains (

The evolution of neurosurgery: how has our practice changed?

INTRODUCTION: Neurosurgery is a rapdily evolving speciality and has often taken a lead in adopting new technologies. Advancing technology however is not the only force driving change in this discipline. Other factors include an escalation in patient expectations, an increasing (and ageing) population and the prevailing economic conditions. The aim of our study was to analyse the trends in neurosurgical practice over a 10-year period. MATERIALS AND METHODS: The details of every neurosurgical procedure performed at Wellington Regional Hospital from 2001 to 2011 were prospectively recorded in a

[Congenital medulloblastoma associated with intracranial arachnoid cyst].

Arachnoid cysts are very common lesions in paediatric patients, with treatment depending on their location and symptomatology. They are usually solitary cysts but may be associated with other central nervous system diseases such as tumours and congenital deformities. We describe the case of a neonate diagnosed with an arachnoid cyst of the quadrigeminal cistern treated by endoscopy. After the operation, the child's condition worsened; a CT scan revealed a midline posterior fossa tumour not visible in the preoperative neuroradiological tests. The tumour, a medulloblastoma, was partially removed

Colloid cyst: a case study.

A colloid cyst, also called a neuroepithelial cyst, is a slow-growing, benign tumor that occurs in the anterior third ventricle. The cyst typically blocks the foramen of Monro, causing obstructive hydrocephalus involving only the lateral ventricles. The most common presenting sign and symptom of a colloid cyst is headache. Less common presenting signs and symptoms include nausea, vomiting, memory loss, mental status changes, gait disorder, and visual disturbances. Surgical resection is the recommended treatment. Meticulous baseline and ongoing neurological assessments are of paramount importan

Cerebellar hemorrhage caused by remote neurological surgery.

INTRODUCTION: Intracranial hemorrhage usually occurs as a complication of hypertension, coagulopathy, or trauma. In rare instances, remote cerebellar hemorrhage (RCH) may complicate supratentorial or spinal surgery. METHODS: Retrospective case series study. RESULTS: We describe three cases of RCH complicating spinal and cranial surgical procedures associated with cerebrospinal fluid hypotension. CONCLUSIONS: Remote cerebellar intracranial hemorrhage may complicate neurosurgical procedures in which cerebrospinal fluid hypotension occurs.

Relationships of virtual reality neuroendoscopic simulations to actual imaging.

Advances in computer technology have permitted virtual reality images of the ventricular system. To determine the relevance of these images we have compared virtual reality simulations of the ventricular system with endoscopic findings in three patients. The virtual fly-through can be simulated after definition of waypoints. Flight objects of interest can be viewed from all sides. Important drawbacks are that filigree structures may be missed and blood vessels cannot be distinguished clearly. However, virtual endoscopy can presently be used as a planning tool or for training and has future pot

Case Demonstrating the Nuances of Acute Cortical Venous Thrombosis Anticoagulation Guidelines.

BACKGROUND: The American Stroke Association and the European Stroke Organization have established guidelines on cerebral venous thrombosis (CVT); however, questions remain when an individual case does not fall within the inclusion criteria on which these guidelines are based. This is relevant when considering the use of anticoagulation in cases of CVT regarding whether or not associated hemorrhage is present and whether the hemorrhage is currently expanding. CASE DESCRIPTION: A 16-year-old right-handed female G2P2 (gravidity 2 [2 pregnancies] and parity 2 [2 live births after at least 24 week

Shunt failure clusters: an analysis of multiple, frequent shunt failures.

OBJECTIVE: Repeated failure of ventriculoperitoneal shunts (VPSs) is a problem familiar to pediatric neurosurgeons and patients. While there have been many studies to determine what factors are associated with the first shunt failure, studies of subsequent failures are much less common. The purpose of this study was to identify the prevalence and associated risk factors of clustered shunt failures (defined as 3 or more VPS operations within 3 months). METHODS: The authors reviewed prospectively collected records from all patients who underwent VPS surgery from 2008 to 2017 at their institutio

Hydrocephalus associated with vestibular schwannomas: management options and factors predicting the outcome.

OBJECT: The current, generally accepted optimal management for hydrocephalus related to vestibular schwannomas (VSs) is primary tumor removal, with further treatment reserved only for patients who remain symptomatic. Previous studies have shown, however, that this management can lead to an increase in surgery-related complications. In this study, the authors evaluated their experience with the treatment of such patients, with the aim of identifying the following: 1) the parameters correlating to the need for specific hydrocephalus treatment following VS surgery; and 2) patients at risk for dev

Small callosal fenestration: anatomical and clinical study.

BACKGROUND: Traditional callosotomy is associated with neurologic deficits due to surgical damage of corpus callosum and nearby brain structures. To reduce this damage, we studied a small callosal fenestration in cadaver heads and clinical patients. METHODS: Fifteen cadaver heads were used to study the anatomy of corpus callosum and nearby brain structures. Based on anatomical study, small callosal fenestration was first tested in 5 cadaver heads and then applied to 7 consecutive clinical patients. RESULTS: In cadaver heads, a 1.5 x 1.5-cm (length x width) small callosal fenestration could p

Splenic artery embolization in the management of an acute immune thrombocytopenic purpura-related intracranial hemorrhage.

Although relatively rare, intracranial hemorrhage remains the most common cause of immune thrombocytopenic purpura-related mortality [Medeiros D. Current controversies in the management of idiopathic thrombocytopenic purpura during childhood. Pediatr Clin North Am . 1996;43:757-72]. The required decompressive treatment has the potential for substantial blood loss and must often be delayed because of resistant thrombocytopenia responsive only to splenectomy. Splenic embolization is a novel approach to this problem that can expedite definitive neurosurgical care and minimize permanent sequelae.

Short-term medical management of hydrocephalus.

Hydrocephalus is an excess accumulation of cerebrospinal fluid in or around the brain that can be produced by a broad spectrum of disorders. It can develop at any age and its incidence is increasing, both in infants and adults. Although the standard treatment of hydrocephalus is cerebrospinal fluid shunting, there are certain circumstances in which medical treatment, alone or in combination with shunting, has been suggested as an alternative. This review aims to present and discuss the indications for pharmacological treatment in the medical management of hydrocephalus, and the drugs most freq
