Evaluating Reduced Use and Abstinence as Outcomes in Pharmacotherapy Trials for Stimulant Use Disorder: A Meta-Analysis of 12 Randomized Controlled Trials.

IMPORTANCE: Cocaine and methamphetamine use disorders are major public health challenges with no approved pharmacological treatments. Reliance on complete abstinence as the sole end point in randomized controlled trials (RCTs) may hinder identification of effective therapies, as it is often overly stringent, whereas reduced stimulant use is a promising alternative given its association with improved health outcomes. OBJECTIVES: To evaluate the treatment effect of pharmacological interventions tested in RCTs for stimulant use disorder, comparing reduced use to total abstinence. DESIGN, SETTIN

Inhibitory Role of Gamma-Aminobutyric Receptors in Paraventricular Nucleus on Ejaculatory Responses in Rats.

BACKGROUND: Although abnormal sympathetic nerve system (SNS) activity has been demonstrated in the pathogenesis of ejaculation disorders, few data are available on its underlying mechanism. AIM: To investigate whether differences in ejaculatory behavior of rats were associated with the state of SNS activity and gamma-aminobutyric (GABA) receptor expressions in the paraventricular nucleus (PVN) of the hypothalamus and the effects of GABA receptors in the PVN on ejaculatory behavior. METHODS: Based on ejaculatory performance, Sprague-Dawley rats were divided into "sluggish," "normal," and "rap

Role of Ventral Subiculum in Context-Induced Relapse to Alcohol Seeking after Punishment-Imposed Abstinence.

UNLABELLED: In many human alcoholics, abstinence is self-imposed because of the negative consequences of excessive alcohol use, and relapse is often triggered by exposure to environmental contexts associated with prior alcohol drinking. We recently developed a rat model of this human condition in which we train alcohol-preferring P rats to self-administer alcohol in one context (A), punish the alcohol-reinforced responding in a different context (B), and then test for relapse to alcohol seeking in Contexts A and B without alcohol or shock. Here, we studied the role of projections to nucleus ac

Spinal cord SP release and hyperalgesia in monoarthritic rats: involvement of the GABAB receptor system.

1. Monoarthritis was induced in Lewis rats by interdermal injection in the left hind paw of a suspension of Mycobacterium tubercolusis in mineral oil (500 micrograms 100 microliters-1). Controls were injected with 100 microliters mineral oil. 2. Withdrawal latencies to thermal stimuli of the inflamed paw, the contralateral and both paws of control rats were measured at daily intervals after injection by the plantar test. 3. After detection of the pain threshold, rat spinal cords were removed and horizontal dorsal slices were mounted in a 3-compartment bath to measure electrically-evoked releas

Comprehensive pediatric neurosurgical implant checklist to maintain low implant infection rates: lessons on compliance and institutional culture.

OBJECTIVE: Implant infection is a significant contributor to morbidity and mortality for children with hydrocephalus and other neurosurgical conditions requiring shunts and other neurosurgical implants. To standardize pediatric neurosurgical care and minimize the risk of infections related to implanted shunts and neurosurgical devices, the authors introduced a 23-point checklist for all pediatric neurosurgical implant procedures at Stanford University in March 2019. This protocol minimizes operating room traffic and standardizes sterile technique, preparation, and antibiotic use. METHODS: Pro

Taurine Recovery of Learning Deficits Induced by Developmental Pb2+ Exposure.

Lead (Pb2+) is a historically well-documented environmental neurotoxin that produces developmental cognitive learning and memory impairments. These early neurodevelopmental impairments cause increased brain excitability via disruption of Ca2+ mediated signaling during critical periods of synaptogenesis inducing competition with Ica2+ through NMDARs resulting in altered brain development and functioning across the lifespan. Interestingly, Pb2+ has been shown to decrease GABA transport and uptake, decrease spontaneous and depolarization-evoked GABA neurotransmission and lower the expression of g

Spasticity or periodic limb movements?

BACKGROUND: Spasticity and spasms are distressing features of the upper motor neuron syndrome (UMNS) following spinal cord injuries (SCI) or multiple sclerosis (MS), and have common therapeutic implications. Despite an increase of antispastic drugs and strategies, sometimes up to the surgical implant of intrathecal baclofen pump, some patients still complain of disabling spasms, which poses diagnostic and therapeutic challenges. Although clinically similar, flexor spasms due to pyramidal tract disruption must be clearly differentiated from periodic limb movements (PLM), often accompanying rest

Therapeutic interventions for disease progression in Huntington's disease.

BACKGROUND: Huntington's disease (HD) is an autosomal dominant neurodegenerative disease with an average onset between the fourth and fifth decade of life; it leads to death 15 to 20 years after the onset of symptoms. Although several drugs seem effective in controlling the incapacitating manifestations of HD, no specific therapy is known. The present review aims at analysing the best available data on therapeutic interventions investigated with the goal of modifying the progression of the disease as measured in terms of survival, disability or progression of HD core symptoms. OBJECTIVES: Eva

Functional GABAA receptor heterogeneity of acutely dissociated hippocampal CA1 pyramidal cells.

CA1 pyramidal cells were voltage clamped, and GABA was applied to individual cells with a modified U-tube, rapid drug application system. With Vh = -50 mV, inward currents elicited by 10 microM GABA were inhibited by GABAA receptor (GABAR) antagonists and were baclofen insensitive, suggesting that GABA actions on isolated CA1 pyramidal cells were GABAR mediated. GABA concentration-response curves averaged from all cells were fitted best with a two-site equation, indicating the presence of at least two GABA binding sites, a higher-affinity site (EC50-1 = 11.0 microM) and a lower-affinity site (

GABA transporters and GABAC-like receptors on catfish cone- but not rod-driven horizontal cells.

The effects of GABA and related agents were studied in solitary rod- and cone-driven horizontal cells, acutely isolated from the catfish retina using enzymatic and mechanical treatment. Both types of horizontal cells, which normally receive glutamatergic input from photoreceptors, responded to pressure ejection of the glutamate analog kainate (50 microM) with an inward current of 300-800 pA when voltage-clamped at -70 mV using the whole-cell patch-clamp technique. But pressure ejection of GABA (500 microM) elicited an inward current only in cone-driven horizontal cells. This current, ranging b

Release and effect of gamma-aminobutyric acid (GABA) on rat pineal melatonin production in vitro.

1. 3H-gamma-Aminobutyric acid (GABA) release elicited by a depolarizing K+ stimulus or by noradrenergic transmitter was examined in rat pineals in vitro. 2. The release of 3H-GABA was detectable at a 20 mM K+ concentration in medium and increased steadily up to 80 mM K+. 3. In a Ca2+-free medium 3H-GABA release elicited by 30 mM K+, but not that elicited by 50 mM K+, became blunted. 4. Norepinephrine (NE; 10(-6)-10(-4) M) stimulated 3H-GABA release from rat pineal explants in a dose-dependent manner. 5. The activity of 10(-5) M NE on pineal GABA release was suppressed by equimolecular amounts

Tiagabine Protects Dopaminergic Neurons against Neurotoxins by Inhibiting Microglial Activation.

Microglial activation and inflammation are associated with progressive neuronal apoptosis in neurodegenerative disorders such as Parkinson's disease (PD). γ-Aminobutyric acid (GABA), the major inhibitory neurotransmitter in the central nervous system, has recently been shown to play an inhibitory role in the immune system. Tiagabine, a piperidine derivative, enhances GABAergic transmission by inhibiting GABA transporter 1 (GAT 1). In the present study, we found that tiagabine pretreatment attenuated microglial activation, provided partial protection to the nigrostriatal axis and improved motor

Cerebellar Ataxia.

There is nothing more discouraging than for a patient to be given a specific diagnosis, then to be told that there is nothing that can be done. Physicians are equally disheartened to see exponential progress being made in the understanding of the pathophysiology of a complex disorder but few direct benefits resulting for their patients. Over the past 5 years, molecular genetic research has completely revolutionized the way in which the progressive cerebellar ataxias are classified and diagnosed, but it has yet to produce effective gene-based, neuroprotective, or neurorestorative therapies. Dendritic origin of late events in optical recordings from salamander olfactory bulb.

1. Optical recordings of membrane-potential changes were used to characterize the origin and properties of the electrical signals from the dendritic level in slices of the salamander olfactory bulb. 2. The optical events were correlated with field-potential waves recorded simultaneously. Both responses exhibited patterns similar to those found in other species. 3. Orthodromic stimulation evoked a compound action potential in the olfactory nerve fibers, followed by two additional principal waves (N1 and N2). These field-potential waves reflected excitatory postsynaptic potentials at the primary

Insights into pathophysiology and treatment of visual snow syndrome: A systematic review.

BACKGROUND: Visual snow syndrome is a debilitating disorder characterized by tiny flickering dots (like TV static) in the entire visual field and a set of accompanying visual (palinopsia, enhanced entoptic phenomena, photophobia, nyctalopia), nonvisual (e.g. tinnitus) and nonperceptional (e.g. concentration problems, irritability) symptoms. Its pathophysiology is enigmatic and therapy is often frustrating. OBJECTIVES: To summarize our current understanding of pathophysiology and treatment of visual snow syndrome. METHODS: A systematic search of PubMed database was performed using the key wor
