Early revision surgery after elective anterior cervical discectomy and fusion: incidence, risk factors, and 90-day burden in a national cohort.

PURPOSE: Early revision surgery after elective anterior cervical discectomy and fusion (ACDF) is uncommon but clinically important, as it may reflect early mechanical failure, procedure-related morbidity, or postoperative complications requiring repeat operative intervention. Contemporary national data specifically evaluating predictors of early revision after elective ACDF remain limited. The purpose of this study was to determine the incidence of revision surgery requiring readmission within 90 days after elective ACDF and to identify baseline clinical and index-admission postoperative facto

Outcomes of Press-Fit Use of a Cementable Monoblock Radial Head Prosthesis in Complex Elbow Trauma: A Retrospective Observational Study.

BACKGROUND: Radial head arthroplasty is the preferred surgical strategy for unreconstructable radial head fractures associated with complex elbow injuries. Single-diameter monoblock prostheses remain widely used and are sometimes implanted off-label as press-fit, even when the manufacturer's instructions specify cementation. Outcome data on this practice are scarce. We aimed to describe the clinical, functional, and radiographic outcomes of a single-diameter monoblock partial radial head prosthesis implanted off-label as a press-fit for complex elbow trauma. METHODS: We retrospectively review

Combined Minimally Invasive Chevron-Akin and Distal Minimally Invasive Metatarsal Osteotomy for Hallux Valgus and Metatarsalgia: 12-Month Outcomes and High Radiographic Recurrence in a Dual-Center Study.

BACKGROUND: Minimally invasive techniques for forefoot surgery, including the minimally invasive chevron-Akin (MICA) and distal minimally invasive metatarsal osteotomy (DMMO), have demonstrated favourable outcomes in the treatment of hallux valgus and central metatarsalgia, respectively. Combining these 2 procedures in a single surgical session aims to address multiple deformities concurrently, although potential biomechanical interactions may affect long-term stability. This study investigates the clinical and radiographic outcomes of patients undergoing simultaneous MICA and DMMO, with a foc

An operative approach to address severe genu valgum deformity in the Ellis-van Creveld syndrome.

BACKGROUND: The genu valgum deformity seen in the Ellis-van Creveld syndrome is one of the most severe angular deformities seen in any orthopaedic condition. It is likely a combination of a primary genetic-based dysplasia of the lateral portion of the tibial plateau combined with severe soft-tissue contractures that tether the tibia into valgus deformations. Progressive weight-bearing induces changes, accumulating with growth, acting on the initially distorted and valgus-angulated proximal tibia, worsening the deformity with skeletal maturation. The purpose of this study is to present a relati

Simultaneously anterior decompression and posterior instrumentation by extrapleural retroperitoneal approach in thoracolumbar lesions.

BACKGROUND: Anterior decompression with posterior instrumentation when indicated in thoracolumbar spinal lesions if performed simultaneously in single-stage expedites rehabilitation and recovery. Transthoracic, transdiaphragmatic approach to access the thoracolumbar junction is associated with significant morbidity, as it violates thoracic cavity; requires cutting of diaphragm and a separate approach, for posterior instrumentation. We evaluated the clinical outcome morbidity and feasibility of extrapleural retroperitoneal approach to perform anterior decompression and posterior instrumentation

Internal Fixation Construct and Defect Size Affect Healing of a Translational Porcine Diaphyseal Tibial Segmental Bone Defect.

BACKGROUND AND OBJECTIVE: Porcine translational models have become the gold-standard translational tool to study the effects of major injury and hemorrhagic shock because of their similarity to the human immunologic response to trauma. Segmental bone defects (SBDs) typically occur in warfighters with associated severe limb trauma. The purpose of this study was to develop a translational porcine diaphyseal SBD model in Yucatan minipigs (YMPs), which could be used in bone healing investigations that simulate injury-relevant conditions. We were specifically working toward developing a critical si

Posterior keyhole corpectomy with percutaneous pedicle screw stabilization in the surgical management of lumbar burst fractures.

OBJECTIVE: The authors present a new method of minimally invasive surgical management of lumbar burst fractures through the posterior approach. The method includes minimally invasive corpectomy and interbody fusion, both of which are performed through a keyhole approach, and percutaneous pedicle screw fixation of the fracture. The technique of the posterior keyhole corpectomy presented in this report is a novel and original concept of the first author (AM). The percutaneous pedicle screw stabilization is performed with the use of a percutaneous instrumentation system (Sextant; Medtronic, Inc.,

Head reduction osteotomy with additional containment surgery improves sphericity and containment and reduces pain in Legg-Calvé-Perthes disease.

BACKGROUND: Severe femoral head deformities in the frontal plane such as hips with Legg-Calvé-Perthes disease (LCPD) are not contained by the acetabulum and result in hinged abduction and impingement. These rare deformities cannot be addressed by resection, which would endanger head vascularity. Femoral head reduction osteotomy allows for reshaping of the femoral head with the goal of improving head sphericity, containment, and hip function. QUESTIONS/PURPOSES: Among hips with severe asphericity of the femoral head, does femoral head reduction osteotomy result in (1) improved head sphericity

Lateral Opening-wedge Distal Femoral Osteotomy: Pain Relief, Functional Improvement, and Survivorship at 5 Years.

BACKGROUND: Distal femoral varus osteotomy may be used to treat valgus knee malalignment or to protect a knee compartment in which cartilage restoration surgery (such as osteochondral or meniscus allografting) has been performed. Medial closing-wedge osteotomy has demonstrated good success in treatment of osteoarthritis in published series, but few studies have evaluated distal femoral lateral opening-wedge osteotomy in terms of correction of deformity, pain and function, and survivorship. QUESTIONS/PURPOSES: (1) Does lateral opening-wedge osteotomy lead to accurate correction? (2) What pain

Trans-iliosacral plating for vertically unstable fractures of sacral spine associated with spinopelvic dissociation: A cadaveric study.

BACKGROUND: The treatment algorithm for sacral fracture associated with vertical shear pelvic fracture has not emerged. Our aim was to study a new approach of fixation for comminuted and vertically unstable fracture pattern with spinopelvic dissociation to overcome inconsistent outcome and avoid complications associated with fixations. We propose fixation with well-contoured thick reconstruction plate spreading across sacrum from one iliac bone to another with fixation points in iliac wing, sacral ala and sacral pedicle on either side. Present biomechanical study tests the four fixation patter

Factors affecting length of stay after elective posterior lumbar spine surgery: a multivariate analysis.

BACKGROUND CONTEXT: Elective posterior lumbar fusion is a common surgical procedure, but reported length of hospital stay is variable (usually 3-7 days). The effect of a limited number of factors on length of stay (LOS) has previously been evaluated. However, multivariate analysis using LOS as a dependent variable to separate potentially confounding variables has not been performed. PURPOSE: To facilitate setting of realistic expectations and considering the significant costs of hospitalization, it would be ideal to have a clear understanding of the variables affecting LOS for this surgery.

Hammer Toe Correction with Proximal Interphalangeal Joint Arthrodesis.

BACKGROUND: First described by Soule in 1910, arthrodesis of the proximal interphalangeal joint is a common operative method of treatment of hammer toe, or fixed-flexion deformity of the proximal interphalangeal joint of the lesser toes1. The deformity is often caused by imbalance in intrinsic and extrinsic muscle function across the interphalangeal joint and metatarsophalangeal joint2,3, which can be effectively addressed through proximal interphalangeal joint straightening and arthrodesis in conjunction with soft-tissue balancing of the metatarsophalangeal joint. DESCRIPTION: Following long

Does Flexion Varus Osteotomy Improve Radiographic Findings Compared With Patients Treated in a Brace for Late-onset Legg-Calvé-Perthes Disease?

BACKGROUND: Legg-Calvé-Perthes disease (LCPD) is a childhood hip disease characterized by osteonecrosis of the femoral head. Because severe deformity of the femoral head can cause secondary osteoarthritis in adulthood, progressive collapse should be prevented in children with a necrotic epiphysis. The prognosis of patients with LCPD generally worsens as the age at disease onset increases, and the appropriate treatment for late-onset LCPD remains unclear. Based on the limited effect of nonoperative treatment using a nonweightbearing brace, flexion varus osteotomy (FVO) was introduced in 2010 as

Robotic-Assisted Total Hip Arthroplasty Through the Posterior Approach.

BACKGROUND: Robotic-assisted total hip arthroplasty (THA) through the posterior approach is indicated in cases of symptomatic hip arthritis. The goal of the procedure is to relieve pain and restore function while minimizing postoperative complications such as dislocation. Dislocation often occurs despite traditionally well placed components1,2. The hip-spine relationship can be a causative factor in postoperative instability, particularly in patients with altered spinopelvic kinematics as a result of spinal fusions or degenerative spine disease, in whom component placement based on anatomic la

Minimally Invasive Chevron Akin Osteotomy for Hallux Valgus Correction.

BACKGROUND: The minimally invasive chevron Akin osteotomy technique is indicated for the treatment of symptomatic mild to moderate hallux valgus deformities. The aim of the procedure is to restore alignment of the first ray while minimizing soft-tissue disruption. DESCRIPTION: Prior to the procedure, radiographs are utilized to characterize the patient's hallux valgus deformity by determining the hallux valgus angle and intermetatarsal angle. The metatarsal rotation is also assessed via the lateral round sign and sesamoid view. To begin, a stab incision is made over the lateral aspect of the

BACKGROUND: The purposes of this retrospective study were to review the results of isolated subtalar arthrodesis in adults and to identify factors influencing the union rate. The hypotheses were that (1) the overall outcome is acceptable but is not as favorable as previously reported, (2) complication rates, especially the nonunion rate, are higher than previously reported, and (3) factors contributing to a less favorable union rate can be identified. METHODS: Between January 1988 and July 1995, 184 consecutive isolated subtalar arthrodeses were performed in 174 adults (115 men and fifty-nine

What Is the Prevalence of Cam Deformity After Prophylactic Pinning of the Contralateral Asymptomatic Hip in Unilateral Slipped Capital Femoral Epiphysis? A 10-year Minimum Followup Study.

BACKGROUND: Prophylactic pinning of the asymptomatic and normal-appearing contralateral hip in patients with unilateral slipped capital femoral epiphysis (SCFE) remains controversial. Understanding the minimal 10-year clinical, functional, and radiographic outcomes of the contralateral asymptomatic hip in unilateral SCFE may be helpful in the decision regarding whether the benefits associated with potentially preventing a SCFE are outweighed by the risk of additional surgery. QUESTIONS/PURPOSES: Among patients with SCFE treated with prophylactic pinning of the asymptomatic and contralateral h
