Impact and centrality of attention dysregulation on cognition, anxiety, and low mood in adolescents.

Functional impairments in cognition are frequently thought to be a feature of individuals with depression or anxiety. However, documented impairments are both broad and inconsistent, with little known about when they emerge, whether they are causes or effects of affective symptoms, or whether specific cognitive systems are implicated. Here, we show, in the adolescent ABCD cohort (N = 11,876), that attention dysregulation is a robust factor underlying wide-ranging cognitive task impairments seen in adolescents with moderate to severe anxiety or low mood. We stratified individuals high in DSM-or

Importance of Trauma-Informed Practice in Evaluation of Children Diagnosed with Autism Spectrum Disorder.

As part of a multidisciplinary adoption support clinic, Erin, a 5-year-old girl, adopted approximately 6 months before the clinic visit, presents for postadoption evaluation. Erin was born at full term. Her birth history was significant for reported maternal treatment for liver failure during pregnancy. Her previous medical history included hospitalization for a viral illness at age 2 months, recurrent ear infections, and a fractured forearm. Family history was significant for a maternal history of bipolar disorder, depression, anxiety, borderline personality disorder, and concern for substanc

The impact of child psychiatric conditions on future educational outcomes among a community cohort in Brazil.

AIMS: Mental health problems early in life can negatively impact educational attainment, which in turn have negative long-term effects on health, social and economic opportunities. Our aims were to: (i) estimate the impacts of different types of psychiatric conditions on educational outcomes and (ii) to estimate the proportion of adverse educational outcomes which can be attributed to psychiatric conditions. METHODS: Participants (N = 2511) were from a school-based community cohort of Brazilian children and adolescents aged 6-14 years enriched for high family risk of psychiatric conditions. W

[Adversity, adolescent psychiatric disorders and complex trauma: Comorbidities or overlap?].

OBJECTIVE: In a context of debates on the place of complex trauma within psychiatric pathology, it is essential to accurately measure the extent of the effects of adversity across different disorders. Such studies are rare, however, because they often focus on a single diagnosis. The measurement of adversity is usually self-reported, retrospective, and usually includes a single dimension. Furthermore, at-risk family interactions and maltreatment constitute two distinct and central risk factors for both complex trauma and psychiatric disorders. Yet they are rarely studied together, nor through

A "line item" approach to the identification of genes involved in polygenic behavioral disorders: the adrenergic alpha2A (ADRA2A) gene.

The usual approach for using single base pair polymorphisms (SNPs) for the investigation of the genetics of behavioral disorders is to examine a single diagnostic syndrome or personality trait based on variables relating to a cluster of behavioral symptoms. However, since some of these variables may address behaviors that are associated with one allele while others are associated with the other allele, the overall association may be non-significant and significant sub-syndromal associations may be missed. Thus, we have reversed the process in a technique we term a "line item" approach. As a te

Less imitation of arbitrary actions is a specific developmental precursor to callous-unemotional traits in early childhood.

OBJECTIVE: Callous-unemotional (CU) traits in early childhood explain heterogeneity within conduct problems and are associated with higher risk for later diagnoses of childhood disruptive behavior disorders and antisocial behavior in adulthood. Emerging research implicates impairments in affiliative processes in the etiology of CU traits. The current study tests whether the imitation of intentional actions with no functional significance -a behavior that supports the acquisition of social conventions and affiliative bonds, is a specific developmental precursor to CU traits in early childhood.

Predictive associations of executive functions and oppositional defiant problems and obsessive-compulsive problems in preschoolers.

Oppositional defiant problems (ODP) and obsessive-compulsive problems (OCP) may co-occur in children, though the way they interact is not known. The aim of the study was to examine longitudinal associations between executive functions at age 3 and ODP, ODP dimensions, and OCP at age 6. The sample consisted of 622 preschoolers (50% were boys) from the general population. Executive functions were assessed by teachers using the Behavior Rating Inventory of Executive Functioning - Preschool version questionnaire when children were 3 years old, and ODP and OCP were informed by parents and teachers

Obsessive-compulsive and spectrum disorders in children and adolescents.

The available literature indicates that OCD affecting children and adolescents is highly prevalent. Pediatric-onset OCD seems to share important similarities with the adult disorder but also shows important differences. For example, the clinical phenotype of OCD is remarkably consistent at all ages with some allowances for developmental expression. Pediatric patients frequently demonstrate poor insight into the nature of their obsessions, which in association with their limited verbal expression may make the diagnosis more difficult. Obsessions involving fear of harm and separation, compulsion

Specific, Replicable Behavioral and Neural Correlates of Sensory Over-Responsivity in Childhood.

OBJECTIVE: Sensory over-responsivity (SOR), characterized by strong negative reactions to typically innocuous stimuli, is considered a symptom of autism spectrum disorder. However, SOR also affects 15% to 20% of children overall, including a majority of children with common psychiatric conditions. Despite its prevalence, the clinical specificity and neurobiological bases of SOR remain poorly understood. Our study aims to determine the specific clinical significance of SOR across diverse child samples, and to establish whether SOR is associated with replicable patterns of functional connectivit

Attention-Deficit/Hyperactivity Disorder and Mortality Risk in Taiwan.

IMPORTANCE: Few studies have investigated levels of mortality in patients with attention-deficit/hyperactivity disorder (ADHD), and findings have been inconsistent and lacking information on specific causes of deaths. OBJECTIVE: To investigate the association between ADHD and causes of death in Taiwan. DESIGN, SETTING, AND PARTICIPANTS: A nationwide population-based cohort study was conducted using a cross-national Taiwanese registry. The ADHD group comprised 275 980 individuals aged 4 to 44 years with a new diagnosis between January 1, 2000, and December 31, 2012. All individuals with ADHD

Impulsivity and Early Initiation of Sexual Behaviors in Adolescents with Externalizing Disorders.

Impulsivity, a multidimensional construct characterized by rash decision-making and difficulty delaying gratification, is a trait of externalizing disorders (e.g., attention-deficit/hyperactivity, disruptive, oppositional defiant, and conduct disorders) and has been associated with risk-taking behaviors. However, its association with risky sexual behaviors in this population remains limited. This observational study examined the association between impulsivity and early sexual behaviors in 96 adolescents (male n = 63) with externalizing disorders. Impulsivity at baseline was assessed using the

Explaining the relationship between temperament and symptoms of psychiatric disorders from preschool to middle childhood: hybrid fixed and random effects models of Norwegian and Spanish children.

BACKGROUND: Four explanations for the concurrent and prospective associations between temperament and psychopathology in children have been suggested: predisposition, complication/scar, common cause/continuity, and pathoplasty/exacerbation. Because the confounding effects of common causes have not been ruled out in prior work, the support for the various explanations is uncertain. METHODS: Screen-stratified community samples of 4-year olds in Trondheim, Norway (n = 1,042), and 3-year olds in Barcelona, Spain (n = 622), were assessed biennially for symptoms of attention-deficit/hyperactivity (

A retrospective chart review of risperidone use in treatment-resistant children and adolescents with psychiatric disorders.

Antipsychotic drugs are used to treat a wide variety of child psychiatric disorders characterized by psychotic symptoms, aggression, excitement, tics, stereotypies and hyperactivity nonresponsive to other therapies. Unfortunately, typical antipsychotics have many adverse effects limiting their long-term use. Novel antipsychotics with combined dopaminergic and serotonergic action, such as risperidone, appear to offer better safety and efficacy profiles in controlled studies of adult patients, and therefore appeared as promising pharmacotherapeutic agents in child psychiatry. The purpose of this

Stimulant and atypical antipsychotic medications for children placed in foster homes.

OBJECTIVES: The purpose of this study is to examine the use of prescribed psychoactive medications in a prospective cohort of children shortly after they entered foster homes; and to identify demographics, maltreatment history, psychiatric diagnoses including ADHD comorbidity, and level of aggression that contribute to prescribed use of stimulant and atypical antipsychotic medication over time. METHODS: The sample included N = 252 children (nested in 95 sibling groups) followed for three years up to 4 yearly waves. RESULTS: Nearly all (89%) met criteria for at least one of eight psychiatric

Trauma-Informed Understanding of Depression Among Justice-Involved Youth.

The association between adverse childhood experiences (ACEs) and depression has been the focus of a number of prevalent studies in recent years-particularly among high-risk youth. Depression remains a significant mental health issue among justice-involved youth. There is a well-established correlation between depressed mood and conduct problems (e.g., conduct disorder and oppositional defiant disorder) during childhood and adolescence, which tends to become more prevalent during adolescence. Studies of justice-involved youth reveal high prevalence rates of depression and other mood disorders.

Sensory processing abilities and their impact on disease severity in children with attention-deficit hyperactivity disorder.

OBJECTIVES: Although several studies have shown sensory processing abnormalities in pediatric subjects with attention-deficit hyperactivity disorder (ADHD), there is significant heterogeneity among their results. MATERIALS AND METHODS: This study was performed to compare the sensory processing abilities of children and adolescents with and without ADHD aged 6-15 years and to correlate the sensory processing problems in these patients, with the symptom profile and severity of ADHD. While child sensory profile-2 (SP-2) was used to assess, the sensory processing abilities of ADHD patients, revis

Robust caregiver-youth discrepancies in irritability ratings on the affective reactivity index: An investigation of its origins.

OBJECTIVE: The Affective Reactivity Index (ARI) is widely used to assess young people's irritability symptoms, but youth and caregivers often diverge in their assessments. Such informant discrepancy might be rooted in poor psychometric properties, the differential conceptualization of irritability across informants, or reflect sociodemographic and clinical characteristics. We use an out-of-sample replication approach and leverage longitudinal data, available for a subset of the participants, to test these hypotheses. METHOD: Across two independent samples (NCohort-1 = 765, 8-21 years; NCohort

Retrospective Outcome Monitoring of ADHD and Nutrition (ROMAN): The Effectiveness of the Few-Foods Diet in General Practice.

INTRODUCTION: Double-blind placebo-controlled studies investigating the effect of a few-foods diet (FFD) on attention-deficit/hyperactivity disorder (ADHD) have provided consistent evidence that ADHD can be triggered by foods, indicating the existence of a food-induced ADHD subtype. In 2001 the "few-foods" approach was included in an ADHD treatment protocol. This approach consists of (a) determining, by means of an FFD, whether food is a trigger of ADHD; (b) reintroducing, in FFD responders, foods to assess which foods are incriminated; (c) finally composing a personalised diet eliminating the
