Is iron deficiency modulating physical activity in COPD?

There is evidence that iron plays a key role in the adequate functioning of skeletal muscle. While it has been demonstrated that nonanemic iron deficiency (NAID) affects exercise tolerance and response to exercise training in patients with COPD, the impact on daily physical activities (DPAs) remains unknown. Eighteen COPD patients with NAID (ferritin <100 ng/mL or ferritin 100-299 ng/mL with a transferrin saturation <20%) and 18 COPD patients without this abnormality, matched for age, gender, and the degree of airflow limitation (control group), were enrolled to the study. The primary outcome

Quantitative characterization of biological age and frailty based on locomotor activity records.

We performed a systematic evaluation of the relationships between locomotor activity and signatures of frailty, morbidity, and mortality risks using physical activity records from the 2003-2006 National Health and Nutrition Examination Survey (NHANES) and UK BioBank (UKB). We proposed a statistical description of the locomotor activity tracks and transformed the provided time series into vectors representing physiological states for each participant. The Principal Component Analysis of the transformed data revealed a winding trajectory with distinct segments corresponding to subsequent human d

A Web-Based Pedometer-Mediated Intervention Maintains Benefits After Pulmonary Rehabilitation: A Pilot Study.

RATIONALE: The optimal content and delivery method of pulmonary rehabilitation (PR) maintenance programs are unknown. We developed a web-based, pedometer-mediated intervention, based on the Behavioral Theory of Self-Regulation, to promote physical activity in persons with COPD. In this pilot single-arm cohort study, we examined whether it can maintain benefits after PR completion. METHODS: US Veterans with COPD who completed PR were enrolled for 12 months. At baseline and follow-up every 3 months, the Fitbit Inspire-2 pedometer objectively measured daily step count. The 6-minute walk test (6M

Development of a Standard Set of Outcome Domains and Proposed Measures for Chronic Obstructive Pulmonary Disease in Primary Care Physical Therapy Practice in the Netherlands: a Modified RAND/UCLA Appropriateness Method.

BACKGROUND: Standardization of measures in a common set opens the opportunity to learn from differences in treatment outcomes which can be used for improving the quality of care. Furthermore, a standard set can provide the basis for development of quality indicators and is therefore useful for quality improvement and public reporting purposes. The aim of this study was to develop a standard set of outcome domains and proposed measures for patients with COPD in Dutch primary care physical therapy practice, including a proposal to stratify patients in subgroups. MATERIAL AND METHODS: A consensu

Patterns of physical activity and survival following cardiac resynchronization therapy implantation: the ALTITUDE activity study.

AIMS: Cardiac resynchronization therapy with implantable defibrillator backup (CRT-D) improves outcomes, but predictors and markers of response remain limited. Physical activity information collected by CRT devices may provide insights to CRT response and the relationship between activity changes and survival. METHODS AND RESULTS: Patients entered into the LATITUDE remote monitoring system from 2008 to 2012 after receipt of a new CRT-D were eligible. Mean daily activity was calculated from LATITUDE uploads at baseline (first 3-10 days following implant) and 6 months (180-210 days). Pairwise d

Factors Associated with Sedentary Behavior in Patients with Intermittent Claudication.

OBJECTIVES: Time spent in sedentary behavior has been associated with cardio-metabolic risk factors in the general population and in patients with symptomatic peripheral artery disease (PAD). Given the association of sedentary behavior and poor health outcomes, it is important to identify factors associated with sedentary behavior in these patients. The aim of this study was to identify factors associated with the sedentary time in patients with symptomatic PAD. METHODS: The sample included 297 patients with symptomatic PAD. Sedentary behavior was assessed using a step activity monitor and th

Sensor-Based Monitoring of Knee Osteoarthritis Symptoms in Free-Living Settings: Scoping Review.

BACKGROUND: Knee osteoarthritis is a heterogeneous condition characterized by chronic pain, stiffness, and fatigue that fluctuate rapidly over time. Traditional clinical assessments provide only static diagnoses of disease severity, failing to capture the dynamic, day-to-day symptom variability that impacts patient quality of life. While wearable technologies offer the potential for continuous, high-frequency monitoring, previous reviews have examined general technological interventions for knee osteoarthritis management, yet they lack a specific synthesis of technologies for symptom monitorin

Design and Rationale of Prolonged Nightly Fasting for Multiple Myeloma Prevention (PROFAST): Protocol for a Randomized Controlled Pilot Trial.

BACKGROUND: Obesity is an established, modifiable risk factor of multiple myeloma (MM); yet, no lifestyle interventions are routinely recommended for patients with overweight or obesity with MM precursor conditions. Prolonged nightly fasting is a simple, practical dietary regimen supported by research, suggesting that the synchronization of feeding-fasting timing with sleep-wake cycles favorably affects metabolic pathways implicated in MM. We describe the design and rationale of a randomized controlled pilot trial evaluating the efficacy of a regular, prolonged nighttime fasting schedule among

Interpretable depressive symptoms screening via statistical reasoning-augmented large language models using wearable and environmental data.

Early assessment of depressive symptoms is essential for scalable and personalized mental health care. We developed a hybrid clinical decision support system (CDSS) that combines interpretable logistic regression with fine-tuned large language models (LLMs) to classify depressive symptom status using wearable-derived behavior, clinical features, and environmental exposures. This study aimed to develop a hybrid clinical decision support system by fine‑tuning a GPT‑based large language model with odds ratio-derived statistical reasoning to classify depressive symptom status from wearable‑derived

Accelerometer-Derived Rest-Activity Rhythm Amplitude, Genetic Predisposition, and the Risk of Ischemic Heart Disease: Observational and Mendelian Randomization Study.

BACKGROUND: The rest-activity rhythm amplitude (RARA), as a fundamental human behavior, has been linked to various health conditions. However, its causal relationship with ischemic heart disease (IHD), along with the potential modification by genetic predisposition, remains unclear. OBJECTIVE: This study aimed to investigate the causal association between RARA and IHD using a triangulation approach that incorporated both observational and Mendelian randomization (MR) analyses, and to determine whether genetic predisposition modifies this relationship. METHODS: First, a prospective cohort ana

High-altitude hypoxia and periodic breathing during sleep: gender-related differences.

High-altitude exposure is characterized by the appearance of periodic breathing during sleep. Only limited evidence is available, however, on the presence of gender-related differences in this breathing pattern. In 37 healthy subjects, 23 male and 14 female, we performed nocturnal cardio-respiratory monitoring in the following conditions: (1) sea level; (2) first/second night at an altitude of 3400 m; (3) first/second night at an altitude of 5400 m and after a 10 day sojourn at 5400 m. At sea level, a normal breathing pattern was observed in all subjects throughout the night. At 3400 m the apn

Management of residual excessive daytime sleepiness in depression: An AFPBN consensus using the nominal group technique.

BACKGROUND: Excessive daytime sleepiness and broader hypersomnolence symptoms may persist despite improvement of core depressive symptoms, yet no specific clinical framework currently guides their assessment and management in mood disorders. This work aimed to establish a national expert consensus on the definition, assessment, management and follow-up of residual excessive daytime sleepiness associated with depression (rEDS-dep). METHODS: A nominal group consensus process was conducted from September 2025 to April 2026. A Steering Committee comprised eleven French experts with clinical and s
