Staged transition from veno-arterial extracorporeal membrane oxygenation to left ventricular assist device using Extra-VAD in acute myocardial infarction cardiogenic shock: a case report of individualized management.

BACKGROUND: Acute myocardial infarction (AMI) and cardiogenic shock (CS) in patients requiring veno-arterial extracorporeal membrane oxygenation (V-A ECMO) are life-threatening, with extremely high mortality and complication rates due to hemodynamic instability and multiple organ dysfunction. A durable left ventricular assist device (LVAD) is a viable therapeutic option for these patients with severely impaired cardiac function who have failed attempts to wean from V-A ECMO. However, accurate assessment of right heart function during V-A ECMO and the complication rates associated with prolonge

Treatment strategies for patients with ischemic mitral regurgitation: a systematic review and meta-analysis.

BACKGROUND: Treatments for ischemic mitral regurgitation (IMR) include coronary artery bypass grafting combined with mitral valve replacement (CABG + MVR), percutaneous coronary intervention (PCI) alone, and PCI combined with transcatheter edge-to-edge repair (TEER), but comparative evidence remains limited. We aimed to systematically evaluate perioperative characteristics, in-hospital and long-term outcomes of these strategies. MATERIALS AND METHODS: This PRISMA 2020-compliant systematic review and meta-analysis searched PubMed, EMBASE, Web of Science, Cochrane Library, and ClinicalTrials.go

Breaking the Rhythm: Extracorporeal Membrane Oxygenation (ECMO) as a Life Saver in a Case of Resistant Arrhythmia and Cardiac Arrest.

Refractory ventricular arrhythmias and cardiac arrest in younger patients are rare, life-threatening events, particularly when standard resuscitation fails. Extracorporeal membrane oxygenation (ECMO) provides temporary circulatory support, enabling hemodynamic stabilization, myocardial recovery, and time for diagnostic and therapeutic interventions. We report the case of a 40-year-old East African woman, medically free at baseline, who developed refractory ventricular fibrillation (VF) and cardiogenic shock during elective myomectomy on 12th August 2024. She underwent 45 minutes of cardiopulmo

Acute Limb Ischemia in Patients on Veno-Arterial Extracorporeal Membrane Oxygenation (VA-ECMO) Support: A Ten-Year Single-Center Experience.

BACKGROUND: Veno-arterial extracorporeal membrane oxygenation (VA-ECMO) has been increasingly utilized as a life-saving modality in patients with cardiopulmonary compromise. Acute limb ischemia (ALI) has been reported when the femoral artery was accessed, and it was associated with higher mortality in patients on extracorporeal membrane oxygenation (ECMO). However, prior studies were limited by small sample size and the lack of long-term follow-up. We aimed to investigate the predictors of ALI in ECMO and the effect of ALI on long-term mortality. METHODS: Retrospective institutional chart rev

Recent advances in Del Nido cardioplegia: A comprehensive analysis of randomized clinical trials in adult cardiac surgery.

BACKGROUND: Del Nido cardioplegia (DNC) has extensively been used for pediatric population undergoing cardiac surgery. However, its use in adult cardiac surgeries have been limited thus, its benefits are not yet fully known. This analysis was performed to evaluate the impact of DNC versus any other type of cardioplegia in adult patients who are undergoing cardiac surgery. METHODS: We systematically searched PubMed, Cochrane Library, and Scopus from database inception till March 2023, and moderate to high-quality randomized controlled trials were included which compared DNC to other cardiopleg

Prediction of Male Coronary Artery Bypass Grafting Outcomes Using Body Surface Area Weighted Left Ventricular End-diastolic Diameter: Multicenter Retrospective Cohort Study.

BACKGROUND: The presence of a high left ventricular end-diastolic diameter (LVEDD) has been linked to a less favorable outcome in patients undergoing coronary artery bypass grafting (CABG) procedures. However, by taking into consideration the reference of left ventricular size and volume measurements relative to the patient's body surface area (BSA), it has been suggested that the accuracy of the predicting outcomes may be improved. OBJECTIVE: We propose that BSA weighted LVEDD (bLVEDD) is a more accurate predictor of outcomes in patients undergoing CABG compared to simply using LVEDD alone.

The Impact of Simulation-Based Training in Cardiovascular Medicine: A Systematic Review.

Cardiovascular medicine and practice in recent times have evolved as complex procedures are performed to manage difficult cases. The majority of these interventions are done percutaneously in order to minimize patient risk. Additionally, training specialist in handling these interventions require a lot of exposure to them; as such, patients are at higher risk of errors and complications from trainees before attaining expertise. In order to avoid these possible risks to patients and ensure their safety, using simulation commonly in cardiovascular specialist education is a possible trend in the

Indicators and predictors of in-hospital mortality and survival in patients with ventricular septal rupture.

BACKGROUND: Ventricular septal rupture (VSR), a mechanical complication of myocardial infarction (MI), usually presents with rapid clinical deterioration with acute heart failure or cardiogenic shock. VSR may occur within 24 h to several days after MI and can occur in both anterior and inferior wall MI. Although guidelines recommend emergent surgery, this is associated with a high mortality rate of up to 40%. Intra-aortic balloon pump (IABP) and extracorporeal membrane oxygenation (ECMO) stabilize patients in preparation for angiography and surgery. Delayed surgery allows better septal repair

Outcomes of Renal Function in Cardiogenic Shock Patients With or Without Mechanical Circulatory Support.

BACKGROUND: The objective of the study was to compare the renal outcomes in patients presenting with all-cause cardiogenic shock who were supported by either Impella devices (Abiomed, Danvers, MA), intra-aortic balloon pump (IABP), or vasopressors alone. Outcomes of cardiogenic shock remain poor even with the advancement of early revascularization and circulatory supportive care. Percutaneous mechanical circulatory support (MCS) device has emerged as an effective strategy in protecting end organ function especially renal function during high risk percutaneous coronary intervention (PCI) and in

[Temporary percutaneous ventricular assist devices for cardiogenic shock and high-risk percutaneous coronary intervention: a systematic literature review].

BACKGROUND: Percutaneous ventricular assist devices (pVADs) are frequently used for the treatment of patients with cardiogenic shock (CS) due to acute myocardial infarction (AMI) and as a support in percutaneous coronary intervention (PCI) for high-risk patients. CS is a clinical condition characterized by inadequate tissue perfusion due to cardiac dysfunction and for 80% of cases it is caused by AMI with left ventricular insufficiency. CS is responsible for about 50% of deaths in patients with myocardial infarction. Usually, PCIs do not require hemodynamic support, which could be however nece

Improvement in left ventricular function following higher-risk percutaneous coronary intervention in patients with ischemic cardiomyopathy.

BACKGROUND: Surgical revascularization is associated with improved ventricular function and clinical outcomes among patients with ischemic cardiomyopathy. There are less extensive data on changes in ventricular function among patients with ischemic cardiomyopathy undergoing percutaneous coronary intervention (PCI). Accordingly, we sought to assess the extent and predictors of change in left ventricular ejection fraction (ΔLVEF) among patients undergoing hemodynamically-supported PCI. METHODS: We assessed ΔLVEF following hemodynamically-supported PCI (with Impella or intra-aortic balloon count

Interleukin-6 is the strongest predictor of 30-day mortality in patients with cardiogenic shock due to myocardial infarction.

INTRODUCTION: Cardiogenic shock (CS) remains the leading cause of death in patients hospitalized for myocardial infarction (MI). Systemic inflammation with inappropriate vasodilatation is observed in many patients with CS and may contribute to an excess mortality rate. The purpose of this study was to determine the predictive role of serial measurements of Nt-proBNP, interleukin-6 (IL-6), and procalcitonin (PCT) for 30-day mortality in patients with CS due to MI. METHODS: The present study is a prospective single-center study including 87 patients with MI complicated by CS treated with acute
