Medial open-wedge high tibial osteotomy demonstrates an 85.9% non-conversion rate and 76.8% patient satisfaction at a minimum follow-up of 10 years.

PURPOSE: To evaluate long-term survival and functional outcomes following medial open wedge high tibial osteotomy (MOWHTO) using a locking plate. METHODS: One hundred fifty-four patients underwent MOWHTO for isolated medial compartment osteoarthritis at Martin Luther Hospital, Berlin, Germany. After application of the predefined inclusion and exclusion criteria, 26 patients were excluded, leaving 126 eligible patients. Of these, two patients had died, and 18 were lost to follow-up. The primary endpoint was survival, defined as the absence of conversion to total knee arthroplasty (TKA). Second

Physical Therapy Management of the First-Time Patellar Instability Event.

BACKGROUND: Patellar instability, the subluxation or dislocation of the patella within the patellofemoral joint, is common in adolescents and can significantly affect their function. This study evaluates conservative management for first-time patellar instability events, discussing rehabilitation strategies and criteria for return to activity/sport. INDICATIONS: Conservative management is typically indicated for patients experiencing a first-time patellar dislocation, particularly when there are no osteochondral injuries or significant anatomic abnormalities. TECHNIQUE DESCRIPTION: Physical

Radiological evaluation of cartilage after microfracture treatment: a long-term follow-up study.

INTRODUCTION: Recent literature revealed good short-term results after microfracturing (MFX) of isolated focal cartilage defects in the knee joint. Study purpose was a long-term evaluation of patients who received MFX through a multimodal approach, correlating clinical scores and morphological pre- and postoperative MRI-scans. MATERIALS AND METHODS: Between 2000 and 2007 158 patients were treated with MFX for focal femoral or tibial defects at our department. Patients with instabilities, secondary surgical intervention, patellofemoral lesions, a plica mediopatellaris or more than one cartilag

Could kinesiology taping help mitigate pain, breathlessness and abdominal-related symptoms in cancer?

We present the case of a woman who was an amateur athlete diagnosed with primary breast cancer, and 10 years later with terminal metastatic cancer. This case report was prepared posthumously in co-operation with her next of kin (husband). The patient first presented to a sports physiotherapist (AR) for her pain-management and to help maintain physical fitness so that she could continue with sports and an active lifestyle. The patient continued with physiotherapy for several months to enable her to be active. However, when her health deteriorated significantly due to advancing cancer, the treat

Arthroscopic Findings After Osteochondral Allografts of the Knee: Proposed Classification Scheme and Associations With Graft Failure.

BACKGROUND: Limited studies exist on the gross appearance of osteochondral allografts (OCAs) at the time of second-look arthroscopy to determine whether the graft and adjacent cartilage have signs of degeneration contributing to failure. PURPOSE: To describe the gross appearance of OCAs and the adjacent cartilage at the time of a second-look knee operation to help propose an accurate definition of structural and clinical OCA failure and to standardize and classify the intraoperative findings associated with structural graft failure. STUDY DESIGN: Case series; Level of evidence, 4. METHODS:

Biplanar Proximal Tibial Opening-Wedge Osteotomy for a Chronic PCL Tear With Medial Compartment Arthritis.

BACKGROUND: Chronic posterior cruciate ligament (PCL) insufficiency leads to instability of the knee, resulting in increased loading of the medial and patellofemoral compartments. Chronic PCL insufficiency progressively damages the menisci and chondral surfaces and can accentuate and worsen varus malalignment. Biplanar proximal tibial opening-wedge osteotomy (BPTO) is a treatment option for chronic PCL tears with medial compartment arthritis which concurrently treats sagittal (flat slope) and varus coronal alignment. INDICATIONS: Opening-wedge BPTO is indicated in active patients who present

Selection of joint replacement methods based on AI cartilage model.

To explore the application value of an artificial intelligence (AI)-based cartilage model evaluation system in the decision-making of knee joint replacement procedures (total knee arthroplasty [TKA] vs. unicompartmental knee arthroplasty [UKA]), and to compare the dynamic differences in postoperative functional recovery between the two procedures. Eighty patients with advanced knee osteoarthritis evaluated by the AI cartilage model were retrospectively included, with 40 patients in each of the TKA group and the UKA group. The AI model performed thin-slice MRI scans of the knee joints, selected

Ethnic differences in pain, function, and catastrophizing in South Florida adults with knee osteoarthritis.

ObjectiveEthnicity is associated with varying reporting of pain, coping mechanisms, and disease severity in patients with knee osteoarthritis (KOA). However, few studies have evaluated its importance in ethnicity, particularly the Hispanic population. This study compares pain intensity (VAS), function (WOMAC), and pain catastrophizing (PCS) between Hispanic (HP) and non-Hispanic White patients (NHWP) stratified by socio-economic status (SES) and osteoarthritis radiographic K-L grade.MethodsA cross-sectional study of patients from a tertiary care clinic between July 2021 and December 2022 was p

Cruciate-retaining TKA using a third-generation system with a four-pegged tibial component: a minimum 10-year followup note.

BACKGROUND: A third-generation TKA system was designed to address problems encountered with earlier designs including a high rate of patellofemoral complications. At a minimum of 5 years, we previously reported survivorship of 98.7% using revision for any reason as the endpoint for a cohort that includes the patients described in this report. That cohort was unique in that a tibial component that uses four short pegs for fixation was used in a subset of patients undergoing cruciate-retaining TKA and the tibial and femoral components were precoated with polymethylmethacrylate. A novel classification system for anterior femoral notching in total knee arthroplasty.

BACKGROUND: Femoral notching in Total Knee Arthroplasty (TKA) is traditionally associated with an increased risk of periprosthetic fracture in cases of excessive anterior resection. Beyond fracture risk, notching can influence implant fit, patellofemoral and tibiofemoral balance, joint stability, and overall biomechanics. Despite its clinical importance, a standardized system for describing femoral notching is lacking. We propose a novel Femoral Notch Classification, based on resection depths and trochlear geometry, intended as a common language applicable both intraoperatively and on lateral

Internal versus external femorotibial rotation an anterior cruciate ligament rupture and patellofemoral instability on preoperative standing radiographs.

BACKGROUND: Previous literature has identified non-modifiable risk factors for anterior cruciate ligament (ACL) rupture and patellofemoral instability (PFI), such as sex, joint laxity, tibial slope, variations in the intercondylar notch, patella alta, trochlear dysplasia, distance from the tibial tubercle to the trochlear groove, and posterior cruciate ligament morphology. The primary aim of this study was to compare femorotibial rotation among pediatric patients with ACL rupture, PFI, and a comparison cohort. The authors hypothesized there would be differences in femorotibial rotation measure

Surgical versus non-surgical interventions for treating patellar dislocation.

BACKGROUND: Patellar dislocation occurs when the patella disengages completely from the trochlear (femoral) groove. Following reduction of the dislocation, conservative rehabilitation with physiotherapy may be used. Since recurrence of dislocation is common, some surgeons have advocated surgical intervention in addition to rehabilitation. OBJECTIVES: The purpose of this review was to assess the clinical and radiological outcomes of surgical, compared with non-surgical, interventions for treating people with primary or recurrent patellar dislocation. SEARCH METHODS: We searched the Cochrane B

The Association of Advanced Patellofemoral Chondrosis With Outcomes After Posterior Medial Meniscus Root Repair.

BACKGROUND: Meniscus root repair has been shown to improve symptoms and protect against progressive knee arthritis by restoring more normal weightbearing force distribution. While advanced tibiofemoral chondrosis is known to negatively affect root repair outcomes, there is a paucity of literature describing the impact of concomitant patellofemoral chondrosis (PFC). PURPOSE: To evaluate outcomes of meniscus root repair in patients with and without advanced PFC. STUDY DESIGN: Cohort study; Level of evidence, 3. METHODS: This single-center retrospective study included patients who underwent is

Bilateral Knee Effusions Secondary to Zoledronic Acid Infusion.

Intravenous zoledronic acid is an established and generally well tolerated form of antiresorptive therapy for osteoporosis. Although mild arthralgias are a well-documented manifestation of the acute phase response to intravenous bisphosphonates, more severe musculoskeletal reactions manifesting as debilitating pain and joint effusions have been rarely documented in the current literature. In this case report, we discuss the case of a 55-year-old woman who developed severe painful bilateral knee effusions within 1 week of her first zoledronic acid infusion for osteoporosis. Prescribing physicia

Knee Arthroscopy: A Simulation Demonstrating the Imperial Knee Arthroscopy Cognitive Task Analysis (IKACTA) Tool.

BACKGROUND: Virtual reality and cadaveric simulations are expensive and not readily accessible1. Innovative and accessible training adjuncts are required to help meet training needs. Cognitive task analysis (CTA) has been used extensively to train pilots and surgeons in other surgical specialties2-6. However, the use of CTA tools within orthopaedics is in its infancy. Arthroscopic procedures are minimally invasive and require a different skill set compared with open surgery. Residents often feel poorly prepared to perform this in the operating room because of the steep learning curve associate

The Mark Coventry Award: Patellofemoral Arthroplasty Results in Better Range of Movement and Early Patient-reported Outcomes Than TKA.

BACKGROUND: Controversy exists over the surgical treatment for severe patellofemoral osteoarthritis. We therefore wished to compare the outcome of patellofemoral arthroplasty (PFA) with TKA in a blinded randomized controlled trial. QUESTIONS/PURPOSES: In the first 2 years after surgery: (1) Does the overall gain in quality of life differ between the implants based on the area under the curve of patient-reported outcomes (PROs) versus time? (2) Do patients obtain a better quality of life at specific points in time after PFA than after TKA? (3) Do patients get a better range of movement after P

The New Bern Chondrolabral Classification Is Reliable and Reproducible.

BACKGROUND: Several classification systems have been used to describe early lesions of hip cartilage and the acetabular labrum in young adults with hip pain. Some of them were introduced before the concept of femoroacetabular impingement was proposed. Others were developed for other joints (such as the patellofemoral joint). However, these often demonstrate inadequate reliability, and they do not characterize all possible lesions. Therefore, we developed a novel classification system. QUESTION/PURPOSE: We asked: What is the (1) intraobserver reliability, (2) interobserver reproducibility, and

Higher precision and preservation of frontal plane alignment in slope-reducing infra-tubercle compared to retro-tubercle high tibial osteotomy.

PURPOSE: Slope-reducing high tibial osteotomies (SR-HTOs) correct posterior tibial slope (PTS) abnormalities in patients with anterior knee instability, as in cases of anterior cruciate ligament (ACL) deficiency. The SR-HTO techniques, including infra-tubercle and retro-tubercle approaches, provide distinct benefits: retro-tubercle techniques help preserve patellofemoral joint mechanics, while infra-tubercle techniques are effective in mitigating iatrogenic varus. However, there is limited comparative literature available. This study compares the PTS correction precision, frontal plane alignme
