The levels of vascular endothelial growth factor in the synovial fluid correlated with the severity of arthroscopically observed synovitis and clinical outcome after temporomandibular joint irrigation in patients with chronic closed lock.

OBJECTIVE: This study aimed to investigate the level of vascular endothelial growth factor (VEGF) in the temporomandibular joint (TMJ) synovial fluid (SF) and the severity of arthroscopically observed synovitis before and after visually guided TMJ irrigation (VGIR) in patients with chronic closed lock (CCL). In addition, the findings were correlated with the clinical outcome. STUDY DESIGN: Twenty-four patients with unilateral CCL, who underwent a second VGIR either as a repeated therapeutic TMJ irrigation or as a follow-up arthroscopy, were enrolled in the study. They were divided into either

The next step in the treatment of persistent temporomandibular joint pain following arthrocentesis: a retrospective study of 18 cases.

Temporomandibular joint disorders affect a big portion of the population. There are a variety of treatment methods currently in use. Conservative treatment modalities are followed by more invasive approaches like arthrocentesis or arthroscopy. The aim of the study is to compare the effects of intra-articular tenoxicam injection and arthrocentesis plus viscosupplementation on patients in which a previous arthrocentesis plus viscosupplementation has failed to relieve pain and restore function. The study group consists of 18 TMJs in 16 patients (15 female and 1 male) and the patients were randoml

Superficial parotidectomy for chronic parotid sialadenitis.

Chronic sialadenitis (CS) of the parotid gland is an insidious inflammatory disorder which tends to progress and may lead to the formation of a fibrous mass. This is a review of the author's experience of superficial parotidectomy (SP) with duct ligation for non-specific CS of the parotid gland not responding to conservative management. 21 patients (11 females; 10 males) with intractable non specific CS underwent SP with duct ligation. The mean duration of symptoms was 1.93 years (SD 0.48). Fine needle aspiration cytology and magnetic resonance imaging were carried out prior to SP to rule out

Randomised trial of Haemophilus influenzae type-b tetanus protein conjugate vaccine [corrected] for prevention of pneumonia and meningitis in Gambian infants.

BACKGROUND: In developing countries, pneumonia and meningitis due to Haemophilus influenzae type b (Hib) are common in children under age 12 months and the mortality from meningitis is high. Protein-polysaccharide conjugate vaccines have brought Hib disease under control in industrialised countries. We did a double-blind randomised trial in The Gambia to assess the efficacy of a Hib conjugate vaccine for the prevention of meningitis, pneumonia, and other invasive diseases due to Hib. METHODS: Between March, 1993, and October, 1995, 42,848 infants were randomly allocated the conjugate vaccine

Pleural tuberculosis: incidence, pathogenesis, diagnosis, and treatment.

Recent studies in populations with a high prevalence of tuberculosis and HIV infection report that tuberculous pleurisy occurs in approximately 30% of patients with tuberculosis. However, the fraction of patients with tuberculosis who have tuberculous pleurisy is comparable in HIV-positive and HIV-negative individuals. It appears that tuberculous pleurisy mostly develops in patients with HIV who have CD4 counts above 200/microL. Primary tuberculous pleurisy is thought to occur as a result of a delayed hypersensitivity reaction to mycobacterial antigens. Recent studies highlight the way in whic

Acquired palmoplantar keratoderma.

The management of children with end-stage chronic liver disease and acute liver failure mandates a multidisciplinary approach and intense monitoring. In recent years, considerable progress has been made in developing specific and supportive medical measures, but studies and publications have mainly concerned adult patients. Therapeutic approaches to complications of end-stage chronic liver disease and acute liver failure (e.g. refractory ascites, hepatorenal syndrome, encephalopathy, and cerebral edema) that may be applied to children are reviewed in this article.Mild-to-moderate ascites shoul

Diagnosis of periprosthetic joint infection in Medicare patients: multicriteria decision analysis.

BACKGROUND: In the setting of finite healthcare resources, developing cost-efficient strategies for periprosthetic joint infection (PJI) diagnosis is paramount. The current levels of knowledge allow for PJI diagnostic recommendations based on scientific evidence but do not consider the benefits, opportunities, costs, and risks of the different diagnostic alternatives. QUESTIONS/PURPOSES: We determined the best diagnostic strategy for knee and hip PJI in the ambulatory setting for Medicare patients, utilizing benefits, opportunities, costs, and risks evaluation through multicriteria decision a

Diagnosis and treatment of temporomandibular disorders.

Current concepts and recommended treatment for temporomandibular disorders (TMDs) and temporomandibular joint pain and dysfunction have evolved over time. This article attempts to distill the current information for this often confusing topic into relevant clinical issues that will allow the general dental practitioner to be better able to diagnose and interpret clinical findings, and institute a therapeutic regimen that will provide needed relief to patients suffering from TMD dysfunction. Current management methods, both surgical and nonsurgical, are reviewed and discussed.

Effect of arthrocentesis and hydraulic distension on the temporomandibular joint disk position.

OBJECTIVE: Recent studies have suggested arthrocentesis and hydraulic distension as an effective treatment modality in patients demonstrating clinical findings consistent with the diagnosis of disk displacement without reduction, normal range of motion thereby being restored and pain of the temporomandibular joint reduced. In view of the fact that only a few studies have been performed to verify the biologic concept of disk displacement without reduction as a diagnostic and therapeutic approach in patients with "closed-lock" symptoms, the purpose of this study was to investigate whether tempor

Early intraocular pressure kinetics after intravitreal aflibercept 8 mg (0.07 mL) versus aflibercept 2 mg (0.05 mL) in nonglaucomatous eyes: a prospective comparative study.

BACKGROUND: Transient intraocular pressure (IOP) spikes after intravitreal injection (IVI) are expected. The higher-volume aflibercept 8 mg (0.07 mL) formulation has recently entered clinical use, yet its early IOP kinetics-particularly the immediate post-injection trajectory in eyes without known glaucoma or ocular hypertension-remain incompletely characterised. METHODS: In this prospective, non-randomised, single-centre comparative study, 48 eyes of 48 patients received intravitreal aflibercept 8 mg/0.07 mL (n = 25) or aflibercept 2 mg/0.05 mL (n = 23).
