PFAPA syndrome (Periodic Fever, Aphthous stomatitis, Pharyngitis, Adenitis).

This paper aims to remind paediatric clinicians to suspect and confirm 'PFAPA' syndrome (Periodic Fever, Aphthous stomatitis, Pharyngitis and cervical Adenitis syndrome). We report two cases of PFAPA syndrome: a 3-year-old healthy boy with atopic rhinitis and a boy aged 8 years 5 months who simultaneously had lymphocytic vasculitis syndrome treated with immunosuppressive drugs. Both met Marshall's criteria. The literature regarding PFAPA syndrome was complied using a Medline search for articles published between 1963 and 1998 and we then reviewed the reference lists of the articles. The Medlin

Assessment of obstructive sleep apnea risk in different types of cleft lip and palate.

OBJECTIVE: This study aimed to assess the risk of obstructive sleep apnea (OSA) in different types of cleft using the Pediatric Sleep Questionnaire (PSQ). The influence of sex, age, body mass index (BMI), and history of adenotonsillectomy surgery were also evaluated. METHODOLOGY: Parents or legal guardians of 195 patients with nonsyndromic cleft lip, lip and palate, and isolated palate aged from 5 to 18 years (9.11±3.09 years) were invited to answer the PSQ. The sample included 94 individuals with unilateral or bilateral complete cleft lip and palate, 56 individuals with cleft lip, and 45 ind

Pediatric obstructive sleep apnea: a prospective observational study of respiratory events in the immediate recovery period after adenotonsillectomy.

BACKGROUND: Adenotonsillectomy is often curative for pediatric obstructive sleep apnea, yet children remain at high risk of respiratory complications in the postoperative period. We sought to determine the incidence and risk factors for respiratory depression and airway obstruction, as well as clinically apparent respiratory events in the post-anesthesia care unit (PACU) in high-risk children after adenotonsillectomy. METHODS: In this prospective cohort study, we enrolled 60 high-risk children having adenotonsillectomy. Our primary outcome was respiratory depression and airway obstruction in

Obstructive sleep apnea in children with down syndrome - Prevalence and evaluation of surgical treatment.

INTRODUCTION: The prevalence of obstructive sleep apnea (OSA) in the pediatric population with Down syndrome (DS) is reported to be 45-76%. The criterion "gold" standard for diagnosing OSA is polysomnography (PSG). First-hand surgical treatment is adenotonsillectomy (ATE), and another option is adenopharyngoplasty (APP). The aims of this study were to determine the OSA prevalence among children with DS who had undergone PSG and to evaluate and compare the efficiency and results of ATE and APP treatments. METHODS AND MATERIALS: Children <18 years old with DS who underwent PSG at Karolinska Uni

Socioeconomic variables as a predictor of indication for pediatric adenotonsillectomy.

OBJECTIVES: Adenotonsillectomy (AT) is a common pediatric procedure performed for sleep disordered breathing (SDB) or chronic/recurrent tonsillitis. A better understanding of factors associated with clinical indications for AT would positively contribute to patient-centered care of these conditions. Our objective is to assess the relationships between race, ethnicity, and socioeconomic status (SES) and indications for adenotonsillectomy in pediatric patients. METHODS: A retrospective chart review was conducted for pediatric patients between the ages 0-18 years who underwent adenotonsillectomy

Two cases of pulsatile tinnitus: Key points for the clinician.

INTRODUCTION: Pulsatile tinnitus is a sound generated by an acoustic source from within the body, which is perceived by the auditory system of the patient. CASES SUMMARY: We report two puzzling cases of pulsatile tinnitus with normal ENT examinations: the first was due to a cervical internal carotid artery dissection, and the second to the compression of the foramina of Monro by a craniopharyngioma, leading to hydrocephalus. DISCUSSION: We review the systematic management and emphasize the decisive role of the ENT clinician regarding this pulsatile tinnitus complaint, because of the potentia

Facial nerve palsy and laryngospasm as a complication of local anaesthesia during adenotonsillectomy.

Tonsil surgeries are the most frequently performed surgical procedures in ENT departments. We would like to present the case of a 5-year-old patient who suffered from unilateral peripheral facial nerve palsy and laryngeal spasm following adenotonsillectomy. Paresis was observed immediately after the transfer of the patient to the postoperative room. The activity of facial muscles was restored within 2 hours from the beginning of the surgery. We assume that this was the direct effect of an anaesthetic on the extracranial processes of the facial nerve.

Airway Resistance in Children with Obstructive Sleep Apnea Syndrome.

STUDY OBJECTIVES: Enlarged tonsils and adenoids, the main cause of obstructive sleep apnea syndrome (OSAS) in children, results in upper airway (UA) loading. This contributes to the imbalance between structural and neuromotor factors ultimately leading to UA collapse during sleep. However, it is unknown whether this UA loading can cause elevated airway resistance (AR) during wakefulness. We hypothesized that children with OSAS have elevated AR compared to controls and that this improves after OSAS treatment. METHODS: Case control study performed at an academic hospital. Children with OSAS and

Use of the sleep clinical record in the follow-up of children with obstructive sleep apnea (OSA) after treatment.

PURPOSE: The aim of our study was to evaluate the utility of the sleep clinical record (SCR) in the follow-up of children with obstructive sleep apnea (OSA) after treatment. METHODS: SCR was completed and overnight polysomnography (PSG) was performed in all enrolled children (T0), with SCR considered positive for scores ≥6.5, as previously validated. Patients underwent adenotonsillectomy (T&A), rapid maxillary expansion (RME), and medical therapy according to severity of OSA and clinical features. Six months after completing therapy, the second overnight PSG and SCR (T1) were performed. RESU

A mixed cell culture model for assessment of proliferation in tonsillar tissues from children with obstructive sleep apnea or recurrent tonsillitis.

BACKGROUND: Recurrent infective tonsillitis (RI) and obstructive sleep apnea (OSA) are the major indications for adenotonsillectomy (T&A) in children. However, little is known on the determinants of lymphadenoid tissue proliferation in the pediatric upper airway. OBJECTIVES: To develop an in vitro culture system allowing for assessment of tonsillar or adenoidal proliferation under basal or stimulated conditions. METHODS: Tonsils surgically removed from pediatric patients with obstructive sleep apnea and recurrent tonsillitis during T&A, were dissociated using standard methods. Whole cell ton

[Obstructive sleep apnea syndrome in children].

The obstructive sleep apnoea syndrome (OSAS) in children is a clinical syndrome resulting from complete or partial obstruction of the upper respiratory tract during sleep. The pathogenesis is multifactorial; clear risk groups are children with anatomical anomalies of the upper airways, neurological abnormalities and genetic syndromes (including craniofacial syndromes). The clinical symptoms of OSAS in children vary. In partial obstructions, the most frequent forms, the patients may snore and have impaired respiration during sleep. Polysomnography contributes to definite confirmation and specif

Surgical treatment of obstructive sleep apnea in neurologically compromised patients.

Children with cerebral palsy are at risk of developing obstructive sleep apnea, which is initially managed by medical therapy but often requires tracheostomy for stabilization of the airway. We report preoperative and postoperative polysomnographic findings in a prospective series of 18 patients with cerebral palsy and obstructive sleep apnea who were refractory to medical management and underwent aggressive surgical treatment of upper airway obstruction. Fifteen of the 18 children (83 percent) in whom tracheostomy was recommended were spared the procedure. Eighteen children with cerebral pals

Are thymus-derived lymphocytes (T cells) defective in the nasopharyngeal and palatine tonsils of children?

The present study was conducted to evaluate the response of adenoidal T cells and B cells in the production of immunoglobulins. There appears to be a consistent inability of adenoidal T cells to turn on B cells to mature into immunoglobulin-secreting plasma cells. This phenomenon did not appear to be due to suppressor activity of adenoidal T cells because T cells from other sources appeared to effectively result in adenoidal B cell maturation, even in the presence of adenoidal T cells. Both tonsils and adenoids appear to have defective IL-2 production, in response to both mitogens and specific
