Steroids and Thyrotoxicosis Precipitate Periodic Paralysis.

Thyrotoxic Periodic Paralysis (TPP) belongs to a group of muscle diseases called channelopathies, which present with painless generalized muscle weakness without exertion. TPP can be precipitated by a large carbohydrate meal, stress, strenuous exercise, alcohol, a high-salt diet, menstruation, and cold temperatures. Rarely, steroids such as dexamethasone can also precipitate a TPP attack. A 29-year-old Hispanic male, with a history of hyperthyroidism, presented to the emergency department with progressive weakness, predominantly in the lower extremities since morning. Earlier that day, the pat

Lower oxygen consumption and Complex I activity in mitochondria isolated from skeletal muscle of fetal sheep with intrauterine growth restriction.

Fetal sheep with placental insufficiency-induced intrauterine growth restriction (IUGR) have lower hindlimb oxygen consumption rates (OCRs), indicating depressed mitochondrial oxidative phosphorylation capacity in their skeletal muscle. We hypothesized that OCRs are lower in skeletal muscle mitochondria from IUGR fetuses, due to reduced electron transport chain (ETC) activity and lower abundances of tricarboxylic acid (TCA) cycle enzymes. IUGR sheep fetuses (n = 12) were created with mid-gestation maternal hyperthermia and compared with control fetuses (n = 12). At 132 ± 1 days of gestation, b

Living with diabetes: quality of life and functional impairment among adults with type 2 diabetes in Northern Brazil.

Type 2 diabetes mellitus (T2DM) is a growing public health concern in Brazil, especially in underserved regions where social and geographic barriers may affect disease management and quality of life. Functional capacity and health-related quality of life (HRQoL) are critical dimensions often overlooked in primary care approaches. To assess the functional capacity and HRQoL of individuals with T2DM receiving care in primary health units in Tocantinópolis, Northern Brazil, and to explore associations with sociodemographic and clinical variables. This is a cross-sectional, descriptive, and analyt

Associations between quantitative sensory tests and measures of pain and function in persons over 45 with meniscal tear.

OBJECTIVE: Symptomatic meniscal tear and knee osteoarthritis (OA) are commonly accompanied by pain, which may arise from nociceptive mechanisms and from processes, referred to as sensitization, that alter and amplify pain perception. Sensitization may be assessed objectively with quantitative sensory testing (QST). There is a paucity of data on the magnitude of associations between QST measures and subjective and objective outcome measures. DESIGN: We assessed cross-sectional associations between QST measures and pain, muscle strength, and performance measures in participants in the TeMPO tri

A Data-Driven Approach to Estimate Changes in Peak Knee Contact Force With Exoskeleton Assistance.

Lower-limb exoskeletons could benefit individuals with knee osteoarthritis by reducing knee loading. Real-time estimation of knee loads could accelerate the development of load-reducing exoskeletons. However, measuring or estimating internal knee forces remains challenging due to the rarity of force-sensing knee implants and complexity of simulation-based methods. We developed two data-driven models to separately estimate the peaks in knee contact force during early and late stance using a limited set of features from electromyography (EMG), ground reaction force (GRF), and knee angle recordin

The dynamic reconstruction of the medial patellofemoral ligament shows good subjective outcomes but high rates of recurrent instability: a systematic review and meta-analysis.

PURPOSE: The surgical reconstruction of the medial patellofemoral ligament (MPFL) is a commonly used treatment for recurrent patellar dislocations. A surgical method which is frequently used is the dynamic reconstruction of the MPFL (dMPFLr), which involves attaching the released end of a hamstring muscle to the medial patella using a pulley. The aim of this systematic review and meta-analysis is to evaluate the evidence of this method in terms of patient-reported outcomes, the rate of recurrent instabilities, and other complications. METHODS: MEDLINE® and Web of Science™ were used to identif

Torque Onset Angle of the Knee Extensor as a Predictor of Walking Related Balance in Stroke Patients.

OBJECTIVE: To investigate the relationship between the torque onset angle (TOA) of the isokinetic test for knee extensors in the paretic side and walking related balance in subacute stroke patients. METHODS: We retrospectively reviewed patients with first-ever strokes who have had at least two isokinetic tests within 6 months of onset. 102 patients satisfied the inclusion criteria. The characteristics of walking related balance were measured with the Berg Balance Scale sub-score (sBBS), Timed Up and Go test (TUG), 10-m Walk Test (10MWT) and Functional Independence Measure sub-score (sFIM). Consequences of Patellar Tendinopathy on Isokinetic Knee Strength and Jumps in Professional Volleyball Players.

Patellar tendinopathy (PT) in professional volleyball players can have an impact on their careers. We evaluated the impact of this pathology in this specific population in terms of isokinetic strength and jumping performances. Thirty-six professional male volleyball players (mean age: 24.8 ± 5.2) performed isokinetic knee assessments, single-leg countermovement jumps and one leg hop test. They filled out the Victorian Institute of Sport Assessment-Patella (VISA-P) score. Two groups were assessed: “PT group” (n = 15) and “control group” (n = 21). The VISA-P score was lower in the PT group (p <

Comparison of hamstring and quadriceps tendon autografts in anterior cruciate ligament reconstruction with gait analysis and surface electromyography.

BACKGROUND: Anterior cruciate ligament (ACL) tear is the most frequent ligamentous injury of the knee joint. Autografts of hamstring (HS) or quadriceps tendons (QT) are used for primary ACL reconstruction. In this study, we planned to examine whether harvesting an HS graft is related to a deficit in dynamic knee stabilisation and strength revealed by dynamic valgus as compared with QT graft or the uninjured leg. Furthermore, if this deficit exists, is it compensated by higher neuromuscular activity of the quadriceps muscle? MATERIALS AND METHODS: Adult patients who had undergone ACL reconstru

Acute changes in cortical activation during active ankle movement after whole-body vibration for spasticity in hemiplegic legs of stroke patients: a functional near-infrared spectroscopy study.

Background: A recent study revealed that whole-body vibration (WBV) tends to decrease spasticity in stroke-related hemiplegic legs. However, acute changes in cortical activation after WBV are unclear. Objective: To examine whether WBV induces acute changes in sensorimotor cortical activation in patients with stroke-related hemiplegic legs. Methods: Eleven stroke patients (mean age 52.6 [SD 15.4] years; median time after stroke 3 [25th and 75th percentiles; 3 and 10.5, respectively] months) participated in a comparative before-and-after intervention trial. Six healthy adults were also studied. WB

Compensatory mechanisms in below-knee amputee gait in response to increasing steady-state walking speeds.

Compensatory mechanisms in below-knee amputee gait are necessary due to the functional loss of the ankle muscles, especially at higher walking speeds when the mechanical energetic demands of walking are greater. The objective of this study was to examine amputee anterior/posterior (A/P) ground reaction force (GRF) impulses and joint kinetics across a wide range of steady-state walking speeds to further understand the compensatory mechanisms used by below-knee amputees. We hypothesized that amputees would rely more on their intact leg to generate greater propulsion relative to the residual leg,

Abnormalities in alpha-, beta- and gamma-sarcoglycan in patients with limb-girdle muscular dystrophy.

We have identified 12 cases from a group of 45 patients with early onset limb-girdle muscular dystrophy (LGMD), who have a deficiency of the 50 kDa dystrophin-associated glycoprotein, alpha-sarcoglycan. An additional male sibling of one case was also studied clinically. All 12 patients showed a concomitant, but variable, deficiency of alpha-, beta- and gamma-sarcoglycan. None of our patients had a defect in only one component of the sarcoglycan complex. Molecular analysis confirmed that a total absence of one sarcoglycan, associated with reduced expression of the other two, indicates a primary

Knee flexor and extensor strength ratios with deficiency of the lateral collateral ligament.

This investigation studied the peak torque and total work hamstring/quadriceps (HQ) ratios of 24 lateral collateral ligament (LCL) insufficient knees, and clarified the possible associations between different HQ ratios and the long-term outcomes of patients. Peak torque values were recorded at low (60 degrees/sec) and high (180 degrees/sec) speed of isokinetic movement, and the maximal isometric extension and flexion outputs were measured at the knee angle of 60 degrees from straight. Three standardized knee scoring scales were used to determine the subjective, functional, clinical, and radiol

Preventive efficiency of neuromuscular training in reducing lower limb injuries among youth soccer athletes: a systematic review and meta-analysis of randomized controlled trials.

UNLABELLED: Lower-limb injuries are common in youth soccer. Neuromuscular training (NMT) warm-ups are used, but the preventive effects across injury types in young players are unclear. This review quantified NMT effectiveness for lower-limb injury prevention in 8-18-year-old soccer players. A PRISMA-guided systematic review and meta-analysis of randomized controlled trials was conducted. Four databases were searched up to October 2025 using keywords including soccer/football, neuromuscular training, balance, proprioception, plyometrics, injury prevention, ACL, ankle sprain, and hamstring strai

Comparison of Dynamic Postural Stability in Autografts Versus Allografts Following Anterior Cruciate Ligament Reconstruction With Remnant Preservation: A Prospective Study With 1-Year Follow-up.

BACKGROUND: Graft selection is an important part of preoperative planning for anterior cruciate ligament reconstruction (ACLR). In addition, ACLR with the remnant preservation technique has recently gained attention due to potential benefit in bone-tendon healing, graft revascularization, and proprioceptive nerve remodeling. However, the ideal graft choice remains controversial, and there is limited research comparing autograft and allograft in ACLR with remnant preservation. PURPOSE: To compare knee muscle strength, neuromuscular control, and patient-reported outcomes between hamstring (HS)

Effectiveness of Muscle Energy Technique, Strengthening, and Patellar Mobilization in the Physiotherapeutic Management of Patellofemoral Osteoarthritis.

INTRODUCTION: Patellofemoral osteoarthritis (PFOA) is a prevalent degenerative joint disorder characterized by anterior knee pain, stiffness, reduced functional mobility, and impaired quality of life, particularly in older adults. Due to its multifactorial etiology-including muscle imbalance, altered patellar tracking, and joint degeneration-conservative physiotherapy approaches are often prioritized over surgical interventions. This study aimed to evaluate the clinical effectiveness of an integrated physiotherapeutic regimen comprising muscle energy technique (MET), targeted strengthening exe

Non-drug therapies for lower limb muscle cramps.

BACKGROUND: About one in every three adults are affected by lower limb muscle cramps. For some people, these cramps reduce quality of life, quality of sleep and participation in activities of daily living. Many interventions are available for lower limb cramps, but some are controversial, no treatment guidelines exist, and often people experience no benefit from the interventions prescribed. OBJECTIVES: To assess the effects of non-drug, non-invasive treatments for lower limb cramp. SEARCH METHODS: We searched the Cochrane Neuromuscular Disease Group Specialized Register (13 September 2011)
