The role of predisposition to hallucinations on non-clinical paranoid vs. socially anxious individuals after hearing negative affective-laden sounds: an experimental investigation.

BACKGROUND: Research suggested that negative affective-laden sounds act as environmental stressors that elicit negative affect (Bradley and Lang, 2000a). AIMS: We tried to test for the role of an interaction between predisposition to hallucinatory experiences and exposure to negative affective laden sounds for the presence of paranoid ideation. METHOD: We used an experimental design that followed the vulnerability × stress model. We defined three groups from a sample of students: paranoia group vs. social anxiety group vs. control group. Their psychological characteristics were measured thro

Delusional parasitosis: a rare cause of pruritus in hemodialysis patients.

Uremic pruritus is a common symptom in patients undergoing hemodialysis (HD) or peritoneal dialysis, but its exact pathogenesis remains rather unclear. However, severe or "intractable" pruritus may be the manifestation of another underlying disease or disorder other than uremia. Delusional parasitosis, or Ekbom syndrome, is a rare psychiatric disorder characterized by the false conviction of being infested with parasites, and it can be primary, or secondary to several medical and psychiatric disorders. We report 2 elderly HD patients who presented one after another, with delusional parasitosis

Ziprasidone-induced tardive laryngeal dystonia: a case report.

Tardive laryngeal dystonia, a rare form of dystonic syndrome, was only reported to be induced by typical antipsychotics. Here, we report one case of ziprasidone-induced tardive laryngeal dystonia in a schizophrenic female patient, who showed dysphonia, hoarseness and dyspnea after taking ziprasidone 120 mg/day for 8 months. These symptoms were significantly improved after discontinuing ziprasidone and increasing the dose of trihexyphenidyl for 1 week. Although atypical antipsychotics are associated with a lower risk of extrapyramidal symptoms, caution should be taken for any tardive dystonic m

Impairment in visual and spatial perception in schizophrenia and delusional disorder.

The Judgment of Line Orientation Test, a visuospatial processing task, was administered to normal subjects, to schizophrenic patients and to patients with delusional disorder. Significantly better performance was seen in the normal subjects than in the schizophrenic and delusional patients. Delusional patients, in turn, showed better performance than the schizophrenic patients.

[Duties and liabilities for psychiatrists].

This paper evaluates forensic duties and liabilities for psychiatrists, based on the current literature and based on our experience over several years of forensic medicine practice, involving both criminal and civil cases. We evaluated different scenarios, including cases of patients dangerous to themselves or others. We highlighted the importance to keep adequate and detailed clinical records, both in the inpatient and outpatient setting, given the absence of other objective items (e.g., laboratory records or other instrumental assessments) that may be of help for the judge and his/her counse

Social anxiety disorder in first-episode psychosis: incidence, phenomenology and relationship with paranoia.

BACKGROUND: Social anxiety disorder constitutes a significant problem for people with psychosis. It is unclear whether this is a by-product of persecutory thinking. AIMS: To compare the phenomenology of social anxiety disorder in first-episode psychosis with that in a group without psychosis. The relationship between social anxiety and psychosis symptoms was investigated. METHOD: A sample of people with first-episode psychosis (FEP group) was compared with a sample with social anxiety disorder without psychosis (SaD group). RESULTS: Of the individuals in the FEP group (n = 80) 25% were diag

Amelioration of psychiatric symptoms through exposure to music individually adapted to brain rhythm disorders - a randomised clinical trial on the basis of fundamental research.

INTRODUCTION: This pilot study examined, whether long-term exposure of psychiatric patients to music that was individually adapted to brain rhythm disorders associated with psychoticism could act to ameliorate psychiatric symptoms. METHODS: A total of 50 patients with various psychiatric diagnoses were randomised in a 1:1 ratio to listen to CDs containing either music adapted to brain rhythm anomalies associated with psychoticism - measured via a specific spectral analysis - or standard classical music. Participants were instructed to listen to the CDs over the next 18 months. Psychiatric sym

Cognitive and emotional differences between abusive and non-abusive fathers.

OBJECTIVE: Abusive fathers perpetrate a substantial portion of child physical abuse. Despite this, little is known about how they differ from non-abusive fathers. This study compared a broad range of cognitive and affective factors between physically abusive and non-abusive fathers. METHODS: Abusive (n=24) and non-abusive (n=25) fathers completed standard measures assessing their experience and expression of anger, mental health, parenting stress, and their empathy and perceptions of children's socio-emotional signals. RESULTS: Abusive fathers differed from comparisons on almost all construc

Psychometric properties of a performance-based measurement of functional capacity, the UCSD Performance-based Skills Assessment - Brief version.

The UCSD Performance-based Skills Assessment - Brief version (UPSA-B) describes the functions of patients without negative influences of environmental factors such as unemployment or shortage in housing. The aim of the present study is to further explore the psychometric properties of the UPSA-B as well as to ensure that the Swedish version can be used in clinical practice and for research purposes. Participants were 211 patients, 135 men and 76 women, diagnosed with schizophrenia, schizoaffective disorder, or delusional disorder. Results indicate that the UPSA-B is a reliable instrument with

Anxiety and negative self-schemas mediate the association between childhood maltreatment and paranoia.

This study examined affective and psychological routes from childhood maltreatment to increased paranoia in adulthood. Recent anxiety and negative beliefs about the self partially accounted for the associations between emotional or physical abuse and paranoia. However, as full mediation did not occur, other psychological, social and biological pathways require exploration.

Male genital self-mutilation as a psychotic solution.

BACKGROUND: Male genital self-mutilation (GSM) is a rare, but serious phenomenon. Some of the risk factors for this act are: presence of religious delusions, command hallucinations, low self-esteem and feelings of guilt associated with sexual offences. Other risk factors include failures in the male role, problems in the early developmental period, such as experiencing difficulties in male identification and persistence of incestuous desires, depression and having a history of GSM. The eponym Klingsor Syndrome, which involves the presence of religious delusions, is proposed for GSM. DATA SECT

A cognitive model of dangerous delusional misidentification syndromes.

The hallmark of the delusional misidentification syndromes is the presence of a misidentification delusion of the self or others. Delusional misidentification may present with an increased risk for dangerous behaviors. Individuals suffering from delusional misidentification syndromes may express hostility in ways ranging from serious verbal threats to homicidal acts. The causes of dangerous misidentification delusions remain for the most part undetermined. In this article, we report a series of six cases of individuals who harbored dangerous misidentification delusions. These individuals were

An assessment of hostility in a population of adolescents.

In the course of an epidemiological survey of adolescents, the Hostility and Direction of Hostility Questionnaire was administered to 6,709 persons aged 13 to 18 years, representing 89% of secondary school students in Hobart, Tasmania. Extrapunitiveness was found to decrease substantially between the ages of 15 and 17 years in both sexes. Intropunitiveness also decreased, although to a lesser extent. Girls were less extrapunitive and more intropunitive than boys over the whole age range. These findings are discussed in relation to their clinical relevance and to the understanding of psychiatri

Myxedema Psychosis: Neuropsychiatric Manifestations and Rhabdomyolysis Unmasking Hypothyroidism.

Background. Hypothyroidism is a prevalent endocrine disorder, often presenting with a spectrum of symptoms reflecting a hypothyroid state. It is also generally linked to causing mood swings, psychomotor slowing, and fatigue; however, in rare instances, it may lead to or induce acute psychosis, a condition referred to as myxedema psychosis (MP). We report a case of myxedema psychosis and present a literature review discussing its presentation, diagnosis, management, and prognosis. Case Presentation. A 36-year-old lady presented with one-week history of persecutory and paranoid delusions, along

The mood-instability hypothesis in the origin of mood-congruent versus mood-incongruent psychotic distinction in mania: validation in a French National Study of 1090 patients.

BACKGROUND: Although included in successive editions of the DSM since its introduction in the IIIrd, the subtyping of manic episode into 'with mood-congruent (MCP) versus mood-incongruent (MIP) psychotic features' is yet to be fully validated. METHODS: One thousand and ninety consecutive manic patients were recruited at 19 medical centers in France from December 2000 to April 2002. Patients were systematically assessed for sociodemographic characteristics, illness course, phenomenology and had to fill out biphasic mood charts during the first study week. RESULTS: Five hundred forty-four mani
