An atropinized heat-stressed rat model: dose response effects and pharmacokinetics.

Atropine and other anticholinergic drugs are widely used in common medications and in the treatment of organophosphate poisoning. Man dissipates heat by the evaporation of sweat. Analogously, rats spread saliva over their bodies for evaporative cooling. Atropine inhibits both sweating and salivation. Therefore, we sought to quantitate the effects of atropine in our rat heatstroke model. While heat-stressing adult male rats of 500 or 250 g at 41.5 degrees C, we measured the effects of i.v. atropine (10-4000 micrograms X kg-1) on the following: heating rate (HR), % wt loss (saliva production), a

Orthostatic tolerance of normal Indians and those with suspected abnormal cardiovascular reflex status.

There were 143 healthy Indian males subjected to 70 degrees headup tilt (HUT) for 20 min on 166 occasions to evaluate their cardiovascular response. They responded with a significant rise in heart rate, diastolic pressure, and mean arterial pressure. Pulse pressure fell significantly. Heart rate and diastolic pressure during the tilt were significantly (p less than 0.05) and positively correlated to the height of the subjects. None of the subjects had syncope, which has been reported in normal individuals elsewhere. Of the 57 subjects with suspected abnormality of the cardiovascular reflex sta

Dietary manipulation and exercise in the heat: thermoregulatory and metabolic effects in rats.

To determine the effects of dietary manipulation on the metabolic, physical, and physiological responses to exercise in the heat, adult male rats (n 5 16/group) were food deprived for 72 h followed by ad lib consumption of a high protein (HIPROT) or high carbohydrate (HICARB) diet for 96 h. Each dietary regimen was followed immediately by exercise (9.14 m . min-1) in the heat (35.58C) to hyperthermic exhaustion (TCO approximately 43 8C). Animals consuming the HICARB or HIPROT diet had an increased mean TCO prior to the acute heat/exercise protocol when compared with a control (CONT) group with

Effect of age, weight, and metabolic rate on endurance, hyperthermia, and heatstroke mortality in a small animal model.

Since exercise performance in man is a function of the age, body weight, and metabolic efficiency interrelationship, this investigation was designed to investigate in rats the effects of these variables on endurance, thermoregulation, and heatstroke mortality at a relatively mild environmental temperature (26 degrees C). The results indicate that light rats (250 g) are able to run (11 m X min-1, 6 degrees incline) longer (221 min), have the lowest rate of heat gain (0.019 C degrees X min-1) than the other groups and thereby experience low heatstroke mortality (20%) despite a large proportion o

Subjective effects of combined-axis vibration: III. Comparison of Y-axis and Y-plus-yaw vibrations.

Two psychophysical matching experiments (Exps. III and IV in a series) were conducted to compare the perceived intensity of Y-axis and Y-plus-yaw vibrations. Seated subjects matched their perceptions of the intensity of single-axis vibrations in the Y-axis, or combined-axis vibrations made up of Y-axis and yaw motions, by adjusting the intensity of a sinusoidal, 5 Hz, Z-axis response vibration. Stimulus vibrations were sinusoidal at 3.15, 4, 5, 6.3, and 8 Hz. For each frequency, both types of stimulus vibrations were presented at three acceleration levels related to three axis-to-seat distance

Aseptic necrosis in caisson workers: a new set of decompression tables.

There is a high incidence of aseptic necrosis and decompression sickness among caisson workers due to inadequate decompression using the current OSHA decompression tables (1-7). Because of this, a new set of tables--Autodec III-O2--was developed which more effectively eliminates nitrogen from the body and, therefore, should decrease the incidence of both bends and aseptic necrosis. The Autodec III-O2 schedule's superiority was statistically significant at a level of 0.08 compared to the OSHA table. It is our conclusion that OSHA should adopt the Autodec III-O2 schedule as a replacement for the

Effect of different ascent profiles on performance at 4,200 m elevation.

Two groups of sea level residents were studied at the summit of Mauna Kea (4,200 m elevation) following ascent by vehicle. "Commuters" spent 6 h at the summit, while "shiftworkers" lived on the mountain for 5 d. Although PaO2 levels were lower in commuters, they experienced fewer altitude sickness symptoms than shiftworkers on the first day at 4,200 m. After 5 d, shiftworkers reported fewer symptoms and performed better at tests of numerate memory and psychomotor ability than commuters. At high altitude, pulse rates were increased in both groups, but only shiftworkers exhibited an elevation in

A misconception of motion sickness leads to false therapeutic expectations.

The emetic chemoreceptor trigger zone (CTZ), located in the area postrema of the medulla oblongata, is generally believed to be indispensable for the vomiting in motion sickness and, by extrapolation, also in space sickness. Accordingly, it has been postulated that a "motion vomiting substance" is secreted into the cerebrospinal fluid in the emetic process. Furthermore, certain therapeutic measures against motion sickness are aimed at preventing the presumed chemical stimulation of the CTZ. This concept originated from laboratory experiments in which ablation of the area postrema protected som

Efficacy of phosphatidylcholine in the modulation of motion sickness susceptibility.

This study evaluated the efficacy of pharmacological doses of phosphatidylcholine (lecithin) in the modulation of motion sickness induced by exposure to coriolis stimulation in a rotating chair. Subjects received daily dietary supplements of 25 grams of lecithin (90% phosphatidylcholine) and were tested for their susceptibility to motion sickness after 4 h, 2 d, and 21 d. A small but statistically significant increase in susceptibility (+ 15%) was noted 4 h after supplemental phosphatidylcholine, with four of nine subjects demonstrating a marked increase in susceptibility. We attributed this f

Psychological differences noted in aircrewmembers undergoing systematic desensitization and their subsequent functioning.

This study presents the results of a 3-year follow-up of patients treated for flight anxiety via relaxation/desensitization. Noted differences between successfully and unsuccessfully treated patients are discussed as well as the long-range effectiveness of relaxation/desensitization. Significant differences were noted on MMPI (Minnesota Multiphase Personality Inventory) scale elevations and major presenting symptoms. There was a significant relationship between specific MMPI profiles and failure to successfully treat flight anxiety. Individuals most likely to benefit from therapy were those su

Otolith tilt-translation reinterpretation following prolonged weightlessness: implications for preflight training.

Observations with three astronauts yielded two major findings. First, perceived self-motion during sinusoidal roll differed immediately postflight from preflight. Between 70 and 150 min after landing, roll was perceived primarily as linear translation. Secondly, more horizontal eye movement was elicited by roll stimulation immediately postflight relative to both preflight and later postflight observations. These results support an "otolith tilt-translation reinterpretation" hypothesis, which has clear implications for understanding astronaut reports of space motion sickness during the early pe

Serum levels of eleven steroid hormones following motion sickness.

In order to grade motion sickness objectively, the following 11 adrenal hormones were investigated in subjects with different motion sickness susceptibility: Aldosterone, corticosterone, 11-deoxycorticosterone, progesterone, 17-OH-progesterone, 11-deoxycortisol, cortisol, cortisone, testosterone, androstendione, dehydroepiandrosterone sulfate. Motion sickness was induced by the coriolis effect on a rotary chair. Both severe kinetosis after short rotation time and mild motion sickness after 30 min of rotation occurred together with small hormonal changes. Androstendione and 11-deoxycortisol app

Endocrine correlates of susceptibility to motion sickness.

Motion sickness releases ACTH, epinephrine, and norepinephrine. We are interested in endocrine responses to motion sickness, in adaptive responses leading to the resolution of the syndrome, and in how antimotion-sickness drugs influence the endocrine responses. Susceptible or insusceptible subjects were administered antimotion-sickness drugs prior to stressful stimulation. Insusceptible subjects displayed more pronounced elevations of ACTH, epinephrine, and norepinephrine after stressful motion. Predrug levels of ACTH were higher in insusceptible subjects (p less than 0.01). Acute blockade of

Self-control of psychophysiologic response to motion stress: using biofeedback to treat airsickness.

Investigators of the Neuropsychiatry Branch, Clinical Sciences Division, U.S. Air Force School of Aerospace Medicine (USAFSAM), provided biofeedback-moderated behavioral treatment to 53 fliers grounded for chronic, severe motion sickness, and followed each flier for 2 years after treatment completion. Success was defined as returning to and maintaining satisfactory operational flying status. Of these, 42 fliers (79%) met this criterion; 3 (6%) were partially successful, and 8 (15%) were subsequently grounded for recurrent airsickness. Follow-on studies will investigate psychophysiological mech

Analyses of maximum cardiopulmonary performance during exposure to acute hypoxia at simulated altitude--sea level to 5000 meters (760-404 mm Hg).

The maximum cardiopulmonary performance of seven healthy male subjects was studied repeatedly in graded hypoxia at ambient pressures ranging from 760 to 404 mm Hg (sea level to 5000 m of simulated altitude). Using this approach it has been possible to not only establish a reproducible value for VO2max, but to determine an equation which may be used to predict the VO2 at altitude for healthy, unacclimatized males exercising to exhaustion. Moreover, we have attempted to explain the limits to pulmonary ventilation at decreasing levels of PO2 by comparing a given VO2max (STPD) to the corresponding

Fatal heatstroke after a short march at night: a case report.

A case of heatstroke is reported in a young recruit who participated in a 4-km march at night. Body temperature was not measured at the time of collapse. Hyperventilation and aggressive behavior misled to the incorrect diagnosis of hysteric reaction. Upon arrival at the Medical Center 4h later, the patient experienced deep coma, renal failure, shock, and uncontrolled bleeding. Inspite of intensive treatment, the patient died 27 h after collapse. A positive blood culture suggests the involvement of septicemia in the fulminant picture of the disease. The importance of immediate measurement of re

Maxillary sinus barotrauma--case report and review.

A case is presented in which a mucous retention cyst that obliterated the right maxillary sinus caused symptoms due to pressure on two separate branches of the second branch of the fifth cranial nerve during a chamber dive to 112 feet. The symptoms of pain and numbness occurred at different times during and after the dive. Referred pain to the maxillary teeth was due to pressure on the posterior superior alveolar branch and paresthesia with numbness and tingling of the lip and cheek was caused from pressure on the infraorbital nerve prior to its emergence through the infraorbital foramen. Ethical dilemmas and nursing.

Professional responsibilities, tradition, and personal conscience along with legal, philosophical, and religious convictions dictate nursing interventions. Inevitably, these factors embrace life-sustaining therapies; however, in view of complications, prognosis, pain and suffering, and their own views of quality of life, some patients express wishes inconsistent with life-sustaining measures. In other situations, the health care provider as well as the patient may view heroic efforts as more debilitating than resortative.
