Factors associated with adherence to treatment with olanzapine and other atypical antipsychotic medications in patients with schizophrenia.

OBJECTIVES: Poor treatment response is an important factor contributing to lack of treatment adherence. The goals of this research were to determine whether improvements in Positive and Negative Syndrome Scale (PANSS) symptom domains predict the likelihood of staying on treatment and whether differential responses to treatment with various atypical antipsychotics in specific symptom domains account for differences in discontinuation rates or treatment adherence. METHODS: We conducted a post-hoc analysis of pooled data from 5 randomized, double-blind, 24- to 28-week clinical trials in 1103 ola

Atypical antipsychotics in the treatment of mania: a meta-analysis of randomized, placebo-controlled trials.

BACKGROUND: Randomized, controlled trials have demonstrated efficacy for atypical antipsychotics in the treatment of mania in bipolar disorder, either as monotherapy or adjunctive treatment. However, there are no published comparisons of individual atypical antipsychotics for mania. DATA SOURCES AND STUDY SELECTION: We conducted a systematic review and meta-analysis of randomized, placebo-controlled monotherapy and adjunctive therapy trials of atypical antipsychotics for acute bipolar mania. Studies published through 2004 were identified using searches of PubMed/MEDLINE with the search terms

Atypical antipsychotics and pituitary tumors: a pharmacovigilance study.

STUDY OBJECTIVE: To analyze the disproportionality of reporting of hyperprolactinemia, galactorrhea, and pituitary tumors with seven widely used antipsychotic drugs. DESIGN: Retrospective pharmacovigilance study. DATA SOURCE: United States Food and Drug Administration's Adverse Event Reporting System (AERS) database. INTERVENTION: We initially identified higher-than-expected postmarketing reports of pituitary tumors associated with risperidone, a potent dopamine D2-receptor antagonist antipsychotic, by analyzing reporting patterns of these tumors in the AERS database. To further examine thi

A Descriptive Analysis of Factors Leading to a 1-Year Remission in Bipolar Disorder.

BACKGROUND: Bipolar spectrum disorders are associated with symptomatic and functional disability in many patients. Other studies have examined predictors of outcome with variable results. The goal of this retrospective study was to identify medications associated with a minimum of 12 consecutive months of symptomatic, functional, and syndromal remission. METHODS: The charts of 121 patients with confirmed bipolar disorder were reviewed. Data on medical regimen and demographic and adherence factors were gathered using standardized data collection sheets approved by the University of Toledo Inst

[Hyperprolactinemia in mentally ill patients].

Hyperprolactinemia is a common, but neglected, adverse effect of conventional antipschycotics and of some of the atypical antipshycotics. It occurs in almost 42% of men and in 75% of women with schizophrenia who are treated with prolactin-raising antipshycotics, even though it has aroused minimal interest within the scientific community when compared with extra-pyramidal effects. Conventional antipsychotics and some of the atypical antipsychotics, such as risperidone, paliperidone, amisulpride and zotepine, are frequently associated with the raise in prolactin plasma levels. Because of this in

Effectiveness of add-on l-methylfolate therapy in a complex psychiatric illness with MTHFR C677 T genetic polymorphism.

The 5,10-methylenetetrahydrofolate reductase (MTHFR) gene plays a central role in folate metabolism. Many studies have demonstrated an association between MTHFR C677 T variant with depression, schizophrenia and bipolar disorder as one of them being comorbid to other. This has justified the use of folate supplement in psychiatric disorders mainly depression but still not in various other comorbid complex psychiatric disorders. Here we have tried to show how the l-methylfolate in conjunction with the conventional psychotropic drugs can be useful in a state of such complex psychiatric phenomenon

The role of second-generation antipsychotic monotherapy in the rapid control of acute bipolar mania.

A key goal of the pharmacologic treatment of acute bipolar mania is rapid symptom improvement. Medications commonly used to attain this goal include lithium, several anticonvulsants, and both first- and second-generation antipsychotics. Second-generation antipsychotics, which are associated with substantially lower rates of extrapyramidal side effects than first-generation agents, are becoming a mainstay in the treatment of acute mania. Although their efficacy appears to be comparable, second-generation antipsychotics may differ in time to onset and in their side effect profiles. Therefore, se

Pharmacological Interventions for Acute Stress Disorder and for the Prevention and Treatment of Post-traumatic Stress Disorder in Children and Adolescents: A Narrative Review.

Traumatic events are common during childhood and adolescence and may lead to acute stress disorder (ASD) or post-traumatic stress disorder (PTSD), both associated with developmental, cognitive and functional impairment. Although trauma-focused psychotherapies are recommended as first-line treatments, pharmacological interventions are frequently used in clinical practice despite limited evidence in paediatric populations. To review the evidence on pharmacological interventions for the prevention and treatment of paediatric ASD and PTSD, we reviewed the literature published up to December 2025.

Hydrogel-Based Depot Systems in Psychiatric Pharmacotherapy: A Narrative Review Rethinking Long-Acting Drug Delivery.

Poor adherence to pharmacotherapy remains a major challenge in the long-term management of severe psychiatric disorders. Although long-acting injectable (LAI) antipsychotics can provide effective and sustained drug exposure while supporting treatment continuity, their pharmaceutical and clinical characteristics vary considerably according to the active pharmaceutical ingredient and formulation technology. Specific formulations may be associated with limitations such as complex release kinetics, injection-site reactions, or the need for oral supplementation during treatment initiation. Hydrogel

Detection of clinically significant drug-drug interactions in serotonin syndrome: a multisource real-world data and pharmacovigilance study.

BACKGROUND: Serotonin syndrome (SS) is a potentially life-threatening condition increasingly driven by complex drug-drug interactions (DDIs) as serotonergic agents expand beyond psychiatry into pain management and infectious disease treatment. Current clinical decision support tools often exhibit inconsistencies in detecting these risks, leading to potential under-recognition of severe interactions. OBJECTIVES: This study aimed to detect and validate clinically significant DDIs associated with SS by integrating multisource real-world data. We sought to identify high-priority drug combinations

Acute psychosis with convulsive episodes following total thyroidectomy induced by concurrent hypothyroidism and hypocalcemia: a case report.

BACKGROUND: Total thyroidectomy is a common surgical procedure, with postoperative anxiety and depression being recognized complications. However, the development of acute psychosis is a rare and severe manifestation. The etiology of such presentations is often complex, involving metabolic derangements that directly impact central nervous system function. We report a case of severe psychosis with convulsive features precipitated by the synergistic effects of two distinct postoperative complications: iatrogenic hypothyroidism and hypoparathyroidism-induced hypocalcemia. The unique monthly cycli

Comparison of Typical and Atypical Long-Acting Antipsychotics in a Real-World Sample in terms of Treatment Adherence and Factors Influencing Treatment Choice: A One-Year Follow-Up Study.

INTRODUCTION: Non-adherence to antipsychotics, which plays a critical role in the treatment of individuals with severe mental disorders, is the most significant predictor of relapse. The effectiveness of long-acting injectable antipsychotics (LAIs) in ensuring treatment adherence and preventing relapse is well established. However, studies examining the factors influencing the clinical choice between first-generation and second-generation LAIs and the adherence differences between them are limited. This study aimed to compare patients with schizophrenia and bipolar disorder, who were hospitali

Recognition and treatment of attention deficit-hyperactivity disorder in patients with treatment-resistant burning mouth syndrome: a retrospective case study.

INTRODUCTION: Burning mouth syndrome (BMS) is an idiopathic oral pain disorder characterized by burning sensations and dysesthesia, often complicated by psychosocial factors and psychiatric comorbidities, necessitating a multidisciplinary approach. BMS, classified as nociplastic pain (NcplP), frequently involves central sensitization. Attention-deficit/hyperactivity disorder (ADHD), a neurodevelopmental disorder, is commonly comorbid with NcplP, and ADHD-targeted treatment has shown efficacy in NcplP management. However, the role of ADHD diagnosis and treatment on BMS and associated brain func

An analysis of the association between atypical antipsychotic drugs and glaucoma risk: a disproportionality assessment based on the FAERS database.

Atypical antipsychotic medications are widely used in the treatment of schizophrenia and other psychiatric disorders; however, emerging evidence suggests a possible link between these agents and glaucoma. As glaucoma is a debilitating condition that can cause irreversible vision loss, clarifying its relationship with antipsychotic use is crucial to guide clinical decision-making. This study utilized the FDA Adverse Event Reporting System (FAERS), a spontaneous reporting database, to explore potential associations between atypical antipsychotic medications and glaucoma. Relevant adverse event r
