Consideration for Health Disparities in Value Assessment Frameworks.

BackgroundCost-effectiveness analysis (CEA) compares interventions based on relative value and is an integral part of value assessment. Despite recommendations for economists to consider disparities in CEAs that impact health-care resource allocation decisions, the perception held by stakeholders is that value assessment frameworks are inconsistent in practice.MethodsWe reviewed value assessment reports produced by a United States (US)-based value assessment organization to identify how patients and caregiver input may contribute to how the organization considers health disparities. We purpose

The burden of illness in patients with paroxysmal nocturnal hemoglobinuria receiving treatment with the C5-inhibitors eculizumab or ravulizumab: results from a US patient survey.

Paroxysmal nocturnal hemoglobinuria (PNH) is a rare and life-threatening disease with symptoms of hemolysis and thrombosis. Current therapies for this complement-mediated disease rely predominantly on inhibition of the C5 complement protein. However, data on treatment responses and quality of life in C5-inhibitor (C5i)-treated PNH patients are scarce. The objective of this study was to determine C5i treatment effects on clinical parameters, PNH symptoms, quality of life, and resource use for PNH patients. This cross-sectional study surveyed 122 individuals in the USA receiving treatment for PN

Burden of asthma among patients adherent to ICS/LABA: A real-world study.

OBJECTIVES: Asthma is a chronic respiratory condition with a U.S. prevalence of 7.4%. Despite numerous treatment options, asthma remains poorly controlled in some patients. Uncontrolled asthma is associated with high healthcare resource utilization (HCRU) and reduced productivity. This study assessed symptoms, productivity, and HCRU of patients adherent to medium/high-dosage inhaled corticosteroid/long-acting beta2-agonist (ICS/LABA) treatment, and the relationship of asthma control with these parameters. METHODS: Data were collected in the U.S. in 2013-2016 in the Adelphi Respiratory Disease

The cost effectiveness of single-level instrumented posterolateral lumbar fusion at 5 years after surgery.

STUDY DESIGN: Cost effectiveness analysis for single-level instrumented fusion during a 5-year postoperative interval. OBJECTIVE: To determine the cost/quality-adjusted life year (QALY) gained for single-level instrumented posterolateral lumbar fusion for degenerative lumbar spine conditions during a 5-year period. SUMMARY OF BACKGROUND DATA: Cost/QALY has become a standard measure among healthcare economists because it is generic and can be used across medical treatments. Prior studies have reported widely variable estimates of cost/QALY for lumbar spine fusion. This variability may be rela

Value of Treatment for Combat Ocular Trauma.

INTRODUCTION: Combat ocular trauma (COT) represents a significant risk to U.S. warfighters, and the cost of COT can be substantial. We quantified the economic value associated with vision improvements from COT treatment and the potential value of treatment innovation or other interventions that result in better final vision following COT. Finally, we modeled the value associated with improved combat eye protection (CEP) compliance. MATERIALS AND METHODS: We developed an economic model to simulate COT injuries, treatment, and lifetime economic and vision outcomes for U.S. warfighters aged 25.

Symptoms, Diagnosis, and Treatment for Women and Girls With Hemophilia: A Narrative Review.

INTRODUCTION: Hemophilia is an X-linked bleeding disorder previously thought to present only in men. This sentiment is rapidly changing as the information around women and girls' experiences of bleeding symptoms has evolved, sparking intense discussion among researchers, clinicians, patients, and patient advocates regarding appropriate nomenclature. AIM: The primary objective of this narrative review is to build a comprehensive understanding of how women with hemophilia experience symptoms, how those symptoms are managed, navigate barriers to care, as well as discuss what can be done to allev

Resurgence of scarlet fever in China: a 13-year population-based surveillance study.

BACKGROUND: A re-emergence of scarlet fever has been noted in Hong Kong, South Korea, and England, UK, since 2008. China also had a sudden increase in the incidence of the disease in 2011. In this study, we aimed to assess the epidemiological changes before and after the upsurge. We also aimed to explore the reasons for the upsurge in disease in 2011, the epidemiological factors that contributed to it, and assess how these could be managed to prevent future epidemics. METHODS: In this observational study, we extracted the epidemiological data for all cases of scarlet fever between 2004 and 20

Twelve-month prevalence of DSM-5 mental disorders and the psychosocial correlates- a child and adolescent psychiatric epidemiologic survey in Hong Kong SAR.

BACKGROUND: Childhood and adolescence, as critically important developmental stages, set the premise for the lifelong trajectory of health and well-being. Epidemiologic evidence abounds in purporting that half of all lifetime mental disorders begin in adolescence. Epidemiologic data based on structured clinical diagnostic assessments are disproportionately scarce in Asian countries that host fast-growing youth population along a wide socioeconomic spectrum in the course of globalisation. METHODS: A cross-sectional school-based epidemiologic survey was conducted on a representative random samp

Rapidly Increasing Serum 25(OH)D Boosts the Immune System, against Infections-Sepsis and COVID-19.

Vitamin D deficiency is a global public health problem, a pandemic that commonly affects the elderly and those with comorbidities such as obesity, diabetes, hypertension, respiratory disorders, recurrent infections, immune deficiency, and malignancies, as well as ethnic minorities living in temperate countries. The same groups were worst affected by COVID-19. Since vitamin D deficiency weakens the immune system, it increases the risk of infections, complications, and deaths, such as from sepsis and COVID-19. Deficiency can be remedied cost-effectively through targeted food fortification, suppl

Efficacy of a Self-Help Web-Based Recovery Training in Improving Sleep in Workers: Randomized Controlled Trial in the General Working Population.

BACKGROUND: Sleep complaints are among the most prevalent health concerns, especially among workers, which may lead to adverse effects on health and work. Internet-delivered cognitive behavioral therapy for insomnia (iCBT-I) offers the opportunity to deliver effective solutions on a large scale. The efficacy of iCBT-I for clinical samples has been demonstrated in recent meta-analyses, and there is evidence that iCBT-I is effective in the working population with severe sleep complaints. However, to date, there is limited evidence from randomized controlled trials that iCBT-I could also be an ef
