Pattern of lymph node pathology in a private pathology laboratory.

Lymph node excision biopsy is commonly carried out for the investigation of lymphadenopathy. The objective of this study is to elucidate the pattern of nodal pathology seen in a private pathology practice. A total of 137 nodal biopsies for primary investigation of nodal enlargement were retrieved from the files in a private diagnostic pathology laboratory in the year 1997. Lymph nodes excised for cancer staging were excluded from this study. The histology was reviewed based on H&E stained sections, and with additional histochemical and immunoperoxidase stains when deemed necessary. Cases of ma

Combination antibiotic therapy is required to eliminate Bartonella henselae in multiple microenvironments.

Bartonella are gram-negative, facultative intracellular bacteria. Infection by Bartonella manifests as different clinical syndromes collectively known as bartonellosis. The well-known diseases caused by these bacteria are cat scratch disease (Bartonella henselae), trench fever (Bartonella quintana) and Carrion's disease (Bartonella bacilliformis). Excluding B. bacilliformis, which is evolutionarily more distinct than the other species, Bartonella infections result in self-limiting disease that is often undiagnosed and untreated. However, individuals with compromised immune systems or other und

Comparison of different DNA fingerprinting techniques for molecular typing of Bartonella henselae isolates.

Seventeen isolates of Bartonella henselae from the region of Freiburg, Germany, obtained from blood cultures of domestic cats, were examined for their genetic heterogeneity. On the basis of different DNA fingerprinting methods, including pulsed-field gel electrophoresis (PFGE), enterobacterial repetitive intergenic consensus (ERIC)-PCR, repetitive extragenic palindromic (REP) PCR, and arbitrarily primed (AP)-PCR, three different variants were identified among the isolates (variants I to III). Variant I included 6 strains, variant II included 10 strains, and variant III included only one strain

Can Lymphadenopathy Guide the Diagnosis of Hepatosplenic Bartonella Infection in Pediatric Renal Transplant Recipients? What Is the Optimal Treatment and Follow-Up Duration?

BACKGROUND: Cat scratch disease (Bartonella henselae) typically presents with fever and granulomatous lymphadenopathy. Immunocompromised individuals, particularly organ transplant recipients, are at increased risk of severe infections with potential multisystem involvement. In renal transplant recipients, the optimal duration of treatment and antibiotic selection remain uncertain due to drug interactions and nephrotoxicity concerns. METHODS: We present a 13-year-old male renal transplant recipient who was diagnosed with hepatosplenic Bartonella infection after presenting to the pediatric infe

Bartonella henselae: etiology of pulmonary nodules in a patient with depressed cell-mediated immunity.

We describe an immunocompromised renal transplantation patient with opportunistic lung infection due to Bartonella henselae (formerly Rochalimaea henselae) and provide evidence suggesting transmission from a pet cat. Computed tomographic scans of the chest and lung biopsies provided material for diagnosis. The etiology was established by polymerase chain reaction and sequencing of a 16S ribosomal DNA segment from infected lung tissue. Histopathologic and serological evidence supported the molecular data. B. henselae was isolated from the blood of eight of the patient's many cats. The patient r

An Unusual Diagnostic Journey of a Spontaneously Resolving Breast Mass: When Rare Entities Cause Concerning Presentations.

This case report describes an atypical presentation of an enlarging breast mass over the course of 2.5 months in a 42-year-old African American woman, which initially raised suspicion for a serious condition but ultimately resolved spontaneously, emphasizing the importance of considering a broad differential diagnosis in complex breast pathologies. Her past medical history was significant for a similar mass in the same breast two years ago, which resolved spontaneously without medical intervention. Initial physical exam findings, including skin ulcerations and nipple retraction, raised suspici

Viability and Desiccation Resistance of Bartonella henselae in Biological and Non-Biological Fluids: Evidence for Pathogen Environmental Stability.

Pathogen environmental stability is an often-neglected research priority for pathogens that are known to be vector-transmitted. Bartonella henselae, the etiologic agent of Cat Scratch Disease, has become a "pathogen of interest" in several serious human illnesses, which include neoplastic, cardiovascular, neurocognitive, and rheumatologic conditions. Survival in the flea gut and feces as well as the association with a biofilm in culture-negative endocarditis provides insight into this organism's ability to adjust to environmental extremes. The detection of B. henselae DNA in blood and tissues

Effect of different drugs and drug combinations on killing stationary phase and biofilms recovered cells of Bartonella henselae in vitro.

BACKGROUND: Bartonella henselae is a Gram-negative bacterium transmitted to humans by a scratch from cat in the presence of ectoparasites. Humans infected with B. henselae can result in various clinical diseases including local lymphadenopathy and more serious systemic disease such as persistent bacteremia and endocarditis. The current treatment of persistent B. henselae infections is not very effective and remains a challenge. To find more effective treatments for persistent and biofilm Bartonella infections, in this study, we evaluated a panel of drugs and drug combinations based on the curr

Fine-needle aspiration cytology in children with superficial lymphadenopathy.

INTRODUCTION: In pediatric population Fine-Needle Aspiration Citology (FNAC) is slowly gaining acceptance in clinical management of Superficial Lymphadenopathy (SL). Our experience adds some data about the usefulness of this technique in diagnosing the cause of a SL and therefore guiding further treatment. PATIENTS AND METHODS: 238 FNAC were performed in 217 patients with SL, observed at our Institution from 2002 to 2006. The neck was the most frequent localization. The results were available within few hours. In cases of granulomatous findings, the samples were processed for microbiological

Proteomic and bioinformatic analysis of outer membrane proteins of the protobacterium Bartonella henselae (Bartonellaceae).

Bartonella henselae, an infectious agent causing cat-scratch disease and vasculoproliferative disorders in humans, is a fastidious facultative intracellular pathogen. The outer membrane proteins of B. henselae are key molecules that play a primary role in host-cell interactions. We isolated B. henselae outer membrane proteins, using the ionic detergent N-lauroyl sarcosine sodium salt and sodium carbonate, purification by two-dimensional (2-D) gel electrophoresis, and protein identification using mass spectrometry. Treatment with buffers containing ASB-14 and ZWITTERGENT 3-10 increased solubili

Use of Bartonella adhesin A (BadA) immunoblotting in the serodiagnosis of Bartonella henselae infections.

Bartonella henselae causes a variety of human diseases (e.g. cat scratch disease and the vasculoproliferative disorders, bacillary angiomatosis and peliosis hepatis). The laboratory diagnosis of B. henselae infections is usually based on the detection of anti-B. henselae antibodies by an indirect immunofluorescence assay (IFA) which, unfortunately, suffers from a significant amount of cross-reactivity and hence is prone to deliver false-positive results. In this pilot study, we evaluated the use of a potential two-step serodiagnosis of B. henselae infections by combining IFA and anti-Bartonell

Afipia felis induces uptake by macrophages directly into a nonendocytic compartment.

Afipia felis is a Gram-negative bacterium that causes some cases of human Cat Scratch Disease. A. felis can survive and multiply in several mammalian cell types, including macrophages, but the precise intracellular compartmentalization of A. felis-containing phagosomes is unknown. Here, we demonstrate that, in murine macrophages, most A. felis-containing phagosomes exclude lysosomal tracer loaded into macrophage lysosomes before, as well as endocytic tracer loaded after, establishment of an infection. Established Afipia-containing phagosomes possess neither early endosomal marker proteins [ear

Genotypic characteristics of two serotypes of Bartonella henselae.

The study of 16S rRNA gene sequences of all isolates of Bartonella henselae obtained in our laboratory and others from human patients or cats has revealed two genotypes according to the sequence of the 16S rRNA gene. Two isolates of these genotypes have previously been related to two different serotypes, and lack of cross-protection of the two serotypes has been demonstrated in cats. We investigated the grouping of eight strains of B. henselae on the basis of 16S ribosomal DNA, 35-kDa protein, Pap 31 protein, and internal transcribed spacer (ITS) gene sequencing; sodium dodecyl sulfate-polyacr
