Depressive symptoms and association with prevalent cardiovascular disease: A cross-sectional analysis in Port-au-Prince, Haiti.

Cardiovascular disease (CVD) is the leading cause of mortality in low- and middle-income countries, and depression remains an underexplored CVD risk factor in Haiti. We assessed the association between depressive symptoms (DS) and prevalent CVD in urban Haiti and examined sex differences using a cross-sectional analysis of enrollment data from the Haiti Cardiovascular Disease Cohort (adults ≥18 years; March 2019-August 2021). DS were measured using the Patient Health Questionnaire-9 and categorized as none-mild (<10) versus moderate-severe (≥10). Prevalent CVD (angina, myocardial infarction, t

Improving Primary Healthcare: Applying Grapevine (Vitis vinifera) Principles to Lebanon's Vision 2030.

Background/Objectives: While Lebanon's Vision 2030 prioritizes strengthening Lebanon's chronically constrained, fragmented, and shocked primary healthcare system, implementation is hindered by weak sensing mechanisms, misaligned spatial distribution, and poorly integrated referral pathways. This study develops a preliminary, context-specific, systems-level healthcare policy assessment and framework for primary healthcare reform by applying biomimicry principles derived from the drought-resistant root architecture of grapevine (Vitis vinifera). Methods: Publicly available data from the Lebanese

Predicting the burden of acute malnutrition in drought-prone regions of Kenya: A statistical analysis.

In drought-prone regions, timely and granular predictions of the burden of acute malnutrition could support decision-making. We explored whether routinely collected and/or publicly available data could be used to predict the prevalence of global and severe acute malnutrition, as well as the mean weight-for-height Z-score and middle-upper-arm circumference for age Z-score, in arid- and semi-arid regions of Kenya, where drought is projected to increase in frequency and intensity. The study covered six counties of northern Kenya and the period 2015-2019, during which a major drought occurred. To

Perceived stress and allostatic load: Results from the All of Us Research Program.

The rising prevalence of individuals reporting extreme stress has major public health implications as it increases vulnerability to accelerated premature biological aging, thus increasing risk of chronic disease. To examine the impact of stress on premature biological aging, we assessed the association between exposure to increased stress, quantified by the Perceived Stress Scale, and odds of high allostatic load (AL). To illuminate previously unexplored socio-contextual factors, we controlled for self-reported individual and neighborhood social determinants of health that included discriminat

Why Are They Leaving? Factors Associated With Attrition After Testing Among Young South Africans Recently Diagnosed With HIV: An Observational Study.

PURPOSE: Young people in South Africa face unique challenges in initiating HIV treatment. There has been limited research on sociobehavioral factors influencing attrition from care among recently diagnosed adolescents and young adults (AYA). METHODS: We enrolled 100 AYA who recently tested positive for HIV between April 2018 and October 2019 at two community testing sites in Cape Town. We administered a sociobehavioral survey within 2 weeks of diagnosis and 6 months of follow-up and used electronic health records to confirm linkage to care, antiretroviral therapy (ART) initiation, and viral l

Maternal and early childhood health and social outcomes of migrants in high-income countries and the impact of policies that restrict access to healthcare; a systematic review and meta-analysis.

BACKGROUND: The "healthy migrant effect" suggests migrants experience better health than local populations despite socioeconomic disadvantage. Its relevance to maternal and child health is uncertain. This systematic review and meta-analysis examined outcomes among migrant women and children in high-income countries (HICs), and the impact of restrictive healthcare policies. METHODS: Studies published between 2014 and 2024 comparing outcomes for foreign-born migrant women and children (up to five years) with local-born populations were included. Quality was assessed using the Newcastle-Ottawa S

Navigating DR-TB Treatment care: a qualitative exploration of barriers and facilitators to retention in care among people with history of early disengagement from drug-resistant tuberculosis treatment in Johannesburg, South Africa.

BACKGROUND: Despite advances in drug-resistant tuberculosis (DR-TB) diagnosis, treatment, and service delivery, individuals with DR-TB often face significant socioeconomic and psychosocial challenges due to limited resources. These challenges can hinder retention in care, undermining the progress made in DR-TB management. As a consequence, advances in DR-TB diagnostics and treatment have not resulted in DR-TB programs meeting the 75% treatment success targets set by the World Health Organization (WHO). METHODS: We interviewed people with DR-TB who had disengaged from care and their family mem

Service restrictions from emergency shelters among people experiencing homelessness: Uncovering pathways into unsheltered homelessness and institutional circuitry.

Service restrictions refer to temporary or permanent bans of individuals from a program or an organization's services, and are widely used in emergency shelter systems. Limited research exists on how service restrictions unfold and their impacts on people experiencing homelessness. This qualitative study used in-depth interviews with timeline mapping to examine the antecedents and consequences of service restrictions from emergency shelters among people experiencing homelessness in two cities in Ontario, Canada. A total of 49 people experiencing homelessness who had been restricted from an eme

Medically Tailored Meals for Patients With Cirrhosis and Hepatic Encephalopathy: The BRAINFOOD Proof-of-concept Trial.

BACKGROUND AND AIMS: Guidelines recommend that patients with hepatic encephalopathy (HE) receive a high-protein diet (roughly 1 g/kg actual body weight). Concommitant sodium restriction, low health literacy, and food insecurity limit patients' ability to meet this goal. We aimed to determine the feasibility of home-delivered high-protein medically tailored meals (MTMs) for patients with a recent episode of overt HE. METHODS: We enrolled patients with prior overt HE on active HE therapy in a 6-month trial of MTM. All received 21 home-delivered meals/week with protein snacks (mid-day and bedtim

Racial and ethnic differences and the role of unfavorable social determinants of health across steatotic liver disease subtypes in the United States.

BACKGROUND: The global liver community established a more precise criteria to characterize steatotic liver disease (SLD), specifically metabolic dysfunction-associated steatotic liver disease (MASLD) and metabolic dysfunction-associated and alcohol-associated liver disease (MetALD). We aimed to estimate the burden of SLD subtypes and unfavorable social determinants of health (SDOH) in US adults and whether clinical and social factors drive disparities across racial/ethnic subgroups. METHODS: We evaluated 4263 persons aged 20 years or older from the National Health and Nutrition Examination Su

Contextualizing and optimizing novel strategies to improve the latent TB continuum of care: Insights from people living with HIV and health care providers in Brazil.

Tuberculosis (TB) causes 1 in 3 deaths among people living with HIV (PLHIV). Diagnosing and treating latent tuberculosis infection (LTBI) is critical to reducing TB incidence and mortality. Blood-based screening tests (e.g., QuantiFERON-TB Gold Plus (QFT+)) and shorter-course TB preventive therapy (TPT) regimens such as 3HP (3 months weekly isoniazid-rifapentine) hold significant promise to improve TB outcomes. We qualitatively explored barriers and solutions to optimizing QFT+ and 3HP among PLHIV in three cities in Brazil. We conducted 110 in-depth interviews with PLHIV, health care providers

'Back to better': amplifying health equity, and determinants of health perspectives during the COVID-19 pandemic.

INTRODUCTION: Equity and social justice have long been key tenets of health promotion practice, policy and research. Health promotion foregrounds the pertinence of social, economic, cultural, political and spiritual life in creating and maintaining health. This necessitates a critical structural determinants of health perspective that actively engages with the experiences of health and wellbeing among diverse peoples. The inequitable impacts of pandemics are well documented, as are calls for improved pandemic responses. Yet, current pandemic and emergency preparedness plans do not adequately a
