Development of figurative language skills following central nervous system-directed chemotherapy delivered in early childhood.

Central nervous system (CNS)-directed chemotherapy is delivered for the treatment of childhood acute lymphoblastic leukaemia (ALL). Figurative language deficits have been described in children following CNS-directed chemotherapy; however, comprehensive analysis of figurative interpretation errors, potentially providing clinical utility to assist with intervention planning, has never been performed. The present study aimed to compare the figurative language skills of seven children treated with CNS-directed chemotherapy for ALL before the age of 6 years (mean age at diagnosis 3 years 10 months)

Language growth and genetics of specific language impairment.

Behavioural studies of children with specific language impairment (SLI) have reported long-term growth outcomes across different dimensions of language. Genetic studies of children with SLI have identified candidate genes and putative associations of gene variants with SLI. The aims of this review are to summarize these two lines of investigation and to highlight the possible role of underlying growth timing mechanisms that influence the trajectory of language outcomes throughout childhood and into adolescence. Behavioural growth trajectories demonstrate that children with SLI have notable str

Consonant and syllable complexity of toddlers with Down syndrome and mixed-aetiology developmental delays.

This study examines whether speech sound production of toddlers with Down syndrome (DS) is on par with or more severely impaired than that of mental age (MA) peers with developmental delay due to aetiologies other than Down syndrome at two points within an 18-month period near the onset of spoken word production. The utterances of 26 children with DS, aged 24-33 months, with a mean MA of 14.3 months, originally studied by Fey et al. and Warren et al. were compared to those of a group of 22 children with similar intellectual and communication delay but no DS (NDS). Phonological measures include

Predicting reading outcomes in the classroom using a computer-based phonological awareness screening and monitoring assessment (Com-PASMA).

The screening and monitoring of phonological awareness (PA) in the classroom is of great importance to the early identification and prevention of reading disorder. This study investigated whether a time-efficient computer-based PA screening and monitoring assessment (Com-PASMA) could accurately predict end-of-year reading outcomes for 5-year-old children in the first year of schooling. A longitudinal design was employed where the Com-PASMA was used to measure the PA ability of 95 5-year-old children at the start, middle, and end of the first year of school. Of this group, 21 children presented

Communicative gesture use in infants with and without autism: a retrospective home video study.

PURPOSE: The authors aimed to compare gesture use in infants with autism with gesture use in infants with other developmental disabilities (DD) or typical development (TD). METHOD: Children with autism (n = 43), DD (n = 30), and TD (n = 36) were recruited at ages 2 to 7 years. Parents provided home videotapes of children in infancy. Staff compiled video samples for 2 age intervals (9-12 and 15-18 months) and coded samples for frequency of social interaction (SI), behavior regulation (BR), and joint attention (JA) gestures. RESULTS: At 9-12 months, infants with autism were less likely to use

The feasibility of a structured cognitive training protocol to address progressive cognitive decline in individuals with vascular dementia.

PURPOSE: Cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy, better known as CADASIL, is a rare, genetic form of early-onset vascular dementia. The purpose of this study was to use a modified version of Attention Process Training--II (APT-II; Sohlberg, Johnson, Paule, Raskin, & Mateer, 2001) with an individual with early-stage CADASIL. METHOD: APT-II, modified to include strategy training, was applied in an A-B, multiple-probe design for an individual who had been diagnosed with early-stage CADASIL. Outcome measures included pre-post neuropsychological

Investigating the use of traditional and spectral biofeedback approaches to intervention for /r/ misarticulation.

PURPOSE: Misarticulation of /r/ is among the most challenging developmental speech errors to remediate. Case studies suggest that visual biofeedback treatment can establish perceptually accurate /r/ in clients who have not responded to traditional treatments. This investigation studied the response of children with persistent /r/ misarticulation to a course of traditional treatment and a course of biofeedback treatment. METHOD: Eleven children with /r/ misarticulation completed 10 weeks of individual treatment consisting of 4-6 weeks of traditional treatment followed by 4-6 weeks of biofeedba

Feedback frequency in treatment for childhood apraxia of speech.

PURPOSE: To examine the role of feedback frequency in treatment for childhood apraxia of speech (CAS). Reducing the frequency of feedback enhances motor learning, and recently, such feedback frequency reductions have been recommended for the treatment of CAS. However, no published studies have explicitly compared different feedback frequencies in this population. METHOD: Using an alternating treatments single-subject design with multiple baselines across behaviors, retention and transfer of learning were compared following high-frequency feedback and low-frequency feedback in 4 children with

Interarticulatory Coordination of the Lips and Jaw in Childhood Apraxia of Speech.

Childhood apraxia of speech (CAS) is often characterized by an ability to produce phonemes in isolation, but not in more complex phonetic sequences. This has led to the hypothesis that articulator coordination is impaired in CAS, This study explored whether coordination between the lips and jaw during speech production is impaired in this group. We used two methods to investigate interarticulatory relationships. Cross-correlation analysis directly measures spatial-temporal coupling of articulator movements. The spatiotemporal index (STI; Smith, Goffman, Zelaznik, Ying, & MeGillem, 1995) measur

Complex contextual influences on the communicative interactions of students with multiple and severe disabilities.

The aim of this study was to explore teachers' perceptions and experiences of supports and obstacles to engaging students with multiple and severe disabilities (MSD) in communicative interactions at school. Eleven teachers of students with MSD participated in two in-depth interviews. Interview transcripts were analysed for narrative structure and content themes. Inter-coder reliability for coding of content themes was 87.5%. Participants identified a broad range of factors, including: characteristics of individual students, attitudes, perceptions and beliefs of teachers and other staff, class

The use of analogy in speech motor performance.

The acoustic correlates of pitch variation were examined in 40 participants who received analogy instructions or explicit instructions that required them to modulate their intonation during speech production. First, using focus group methodology, professional speech-language pathologists were asked to identify analogies that best described minimum pitch variation (monotone), moderate pitch variation (normal intonation), and maximum pitch variation (exaggerated intonation) in speech. The focus group established that an appropriate pitch variation metaphor may be related to imagery of "waves at

Rethinking aphasia therapy: a neuroscience perspective.

This article explores how consideration of acquired speech and language disorders from the perspective of neuroscience permits new insights into the content and design of therapy for people with aphasia. Key proposals are that aspects of current therapies often neglect the sensory-motor components of speech and language processing, and the interconnectivity of sensory-perceptual and motor systems. Furthermore, current therapy regimes are often administered at too low an intensity to stimulate neural reorganization. Neuroscientific perspectives on learning are explored and in particular the iss

Investigating parent delivery of the Lidcombe Program.

The Lidcombe Program is an early childhood stuttering treatment delivered by parents in the child's everyday environment, under the guidance of a speech-language pathologist (SLP). Given the parents' central role in the treatment delivery, the way it is implemented away from the clinic and away from the SLP's input is very important. And yet, to date there has been very little investigation into this process. This study investigated to what extent parents deliver contingencies for stuttering and stutter-free speech, in structured and unstructured conversations, as directed in the treatment man

The importance of production frequency in therapy for childhood apraxia of speech.

PURPOSE: This study explores the importance of production frequency during speech therapy to determine whether more practice of speech targets leads to increased performance within a treatment session, as well as to motor learning, in the form of generalization to untrained words. METHOD: Two children with childhood apraxia of speech were treated with an alternating treatment AB design, with production frequency differing in the 2 treatments. The higher production frequency treatment required 100+ productions in 15 min, while the moderate-frequency treatment required 30-40 productions in the
