Association between multiple trigger fingers, systemic diseases and carpal tunnel syndrome: A multivariate analysis.

INTRODUCTION: Trigger finger (TF) is a frequent pathology depending on several factors. The objective of this study was to assess the relationship between multiple TF and systemic or musculoskeletal disorders in a sample of young patients. MATERIAL AND METHOD: A retrospective study was performed of all patients with TF operated in our hospital between 2011 and 2015. Multiple or single TF diagnosis and pathologies such as diabetes mellitus (DM), thyroid dysfunction, carpal tunnel syndrome (CTS), epicondylalgia or DeQuervain's disease were collected. Statistical results included a bivariate ana

Musculoskeletal balance of the human wrist elucidated using intraoperative laser diffraction.

This review describes a series of experiments in which sarcomere length was measured in human wrist muscles to understand their design. Sarcomere length measurements were combined with studies on cadaveric extremities to generate biomechanical models of human wrist function and to provide insights into the mechanism by which wrist strength balance is achieved. Intraoperative measurements of the human extensor carpi radialis brevis (ECRB) muscle during wrist joint rotation reveal that this muscle appears to be designed to operate on the descending limb of its length-tension curve and generates

The Effect of Impactful Articles on Clinical Practice in the United States: Corticosteroid Injection for Patients with Lateral Epicondylitis.

BACKGROUND: Following publication of high-level evidence demonstrating that it is not an effective treatment for lateral epicondylitis, a reduction in the corticosteroid injection rate would be expected. The authors aimed to clarify current clinical practice pattern for lateral epicondylitis and identify factors that influence the introduction of evidence into clinical practice. METHODS: In this administrative claims analysis, the authors used 2009 to 2015 Truven MarketScan data to extract claims for corticosteroid injection, physical therapy, platelet-rich plasma injection, and surgery for l

Self-reported physical exposure association with medial and lateral epicondylitis incidence in a large longitudinal study.

INTRODUCTION: Although previous studies have related occupational exposure and epicondylitis, the evidence is moderate and mostly based on cross-sectional studies. Suspected physical exposures were tested over a 3-year period in a large longitudinal cohort study of workers in the USA. METHOD: In a population-based study including a variety of industries, 1107 newly employed workers were examined; only workers without elbow symptoms at baseline were included. Baseline questionnaires collected information on personal characteristics and self-reported physical work exposures and psychosocial mea

Understanding Motor Adaptation in the Transition to Sustained Pain: Protocol for a Longitudinal Experimental Study.

BACKGROUND: Chronic pain affects over 30% of the global population and remains a major public health issue due to limited treatment efficacy and the need for mechanism-based, personalized approaches. Motor behavior is theorized to play a role in pain persistence through altered movement patterns, muscle recruitment, and proprioception. While motor behavior is linked to chronic pain, empirical evidence on underlying mechanisms, particularly cortical dynamics, remains scarce. OBJECTIVE: This study aimed to investigate longitudinal changes in cortical sensorimotor excitability and their relation

Fresh Osteochondral Allograft Transplantation of the Capitellum for the Treatment of Osteochondritis Dissecans.

BACKGROUND: Osteochondritis dissecans (OCD) of the humeral capitellum is a rare and challenging condition to treat. Several surgical options exist, but in the last few years, the pendulum has swung from debridement and microfracture to restoration of the articular surface. Osteochondral autografts from the rib and knee have been described, but donor-site morbidity is a concern. PURPOSE: To expand the results of fresh osteochondral allograft transplantation (FOCAT) in a previously published report with inclusion of additional patients and a longer follow-up period. STUDY DESIGN: Case series;

Total extraperitoneal laparoscopic inguinal hernia repair with adductor tenotomy: a 10-year experience in the treatment of athletic pubalgia.

BACKGROUND: Athletic pubalgia, commonly referred to as a "sports hernia," is a disease process characterized by groin pain produced by physical exertion often occurring in patients whose athletic activities require them to make rapid changes in direction. The groin pain is due to the traction-countertraction relationship between the adductor muscles and the weaker abdominal muscles. Hence, a few studies have shown inguinal hernia repair with adductor tenotomy to be an effective treatment for this pathology (Brody in Hernia 21:139-147, 2016, https://doi.org/10.1007/s10029-016-1520-8 ; Rossidis

Elbow problems in the athlete.

Athletes expose their elbows to significantly high forces, in addition to multiple repetitive motions. Athletic injuries about the elbow include ligamentous, tendon, articular, and nerve injuries. The proper diagnosis requires a thorough knowledge of the anatomy, pathomechanics, and physical findings associated with these injuries. Treatment entails a methodical progression from nonoperative to operative intervention.

Sports medicine for the internist.

Because of increasing participation in leisure sports and exercise rehabilitation programs, many patients are developing nonsurgical, soft tissue overuse syndromes. Many of these musculoskeletal problems arise because of underlying biomechanical difficulties, training errors, or improper use of equipment. Epidemiologic observations reveal that these individuals frequently visit a general medical practitioner before seeking the advice of an orthopedic specialist. Prompt diagnosis simply requires an appreciation of anatomy and an analytic understanding of sports biomechanics. Although compulsive

CCL4L2 participates in tendinopathy progression by promoting macrophage inflammatory responses: a single-cell analysis.

BACKGROUND: Tendinopathy is very common in clinical practice, which is highly prevalent in athletes, sports enthusiasts and other people involved in high-load weight-bearing activities. Common types of tendinopathy include rotator cuff injury, Achilles tendinitis, tennis elbow and so on. Macrophages (Macs) are key immune cells in the pathogenesis of tendinopathy. In this study, CCL4L2+ M1-related signaling pathways were screened by combining single-cell RNA sequencing (scRNA-seq) to explore their significance in tendinopathy treatment. METHODS: Immune cell populations were screened by Uniform

Elbow tendinopathy.

Overuse injuries of the lateral and medial elbow are common in sport, recreational activities, and occupational endeavors. They are commonly diagnosed as lateral and medial epicondylitis; however, the pathophysiology of these disorders demonstrates a lack of inflammation. Instead, angiofibroblastic degeneration is present, referred to as tendinosis. As such, a more appropriate terminology for these conditions is epicondylosis. This is a clinical diagnosis, and further investigations are only performed to rule out other clinical entities after conventional therapy has failed. Yet, most patients

Surgical treatment of medial epicondylitis. Results in 35 elbows.

We reviewed 35 of 38 consecutive patients who had operative treatment for medial epicondylitis of the elbow after the failure of conservative management. Their mean age was 43 years and mean follow-up was 85 months. At operation residual tears with incomplete healing were consistently found in the flexor origin at the medial epicondyle and microscopy showed reactive fibrous connective tissue with varying degrees of inflammation. The mean subjective estimate of elbow function was improved from 38% to 98% of normal, while isokinetic and grip strength testing in 16 patients showed no significant

Public parks and physical activity among adolescent girls.

OBJECTIVES: Physical activity may be constrained or facilitated by local environments. The availability of neighborhood facilities for physical activity may be particularly relevant for youth, who are unable to drive and whose activity is often limited to the immediate distance they are able to walk or bicycle. Several studies have shown that proximity to recreational facilities and parks is one of the most important predictors of physical activity. Because the United States already has an extensive infrastructure of parks, with 70% of adults indicating that they live within walking distance o
