Diffusion tensor imaging in hydrocephalus--findings before and after shunt surgery.

BACKGROUND: To evaluate changes in diffusion tensor imaging (DTI)-derived parameters in patients with hydrocephalus (HC) before and several weeks after shunt surgery. METHODS: Thirteen HC patients were examined with DTI before and after shunt surgery. In a combined region of interest and whole brain voxel-based analysis, different DTI parameters were compared with an age-matched control group. RESULTS: Alteration of DTI parameters in HC patients and changes after shunt surgery are regionally different. HC patients show an increase in fractional anisotropy values based on increases in paralle

Etiology and cranial CT scan profile of nontumoral hydrocephalus in a tertiary black African hospital.

OBJECT: Hydrocephalus is a common condition in the pediatric population. The cause of hydrocephalus, Evans ratio, ventricular index, and cerebral mantle thickness are some of the factors associated with poor surgical outcome. This study was conducted to evaluate the profile of these factors in the authors' patient population. METHODS: The authors conducted a prospective study from the August 1, 2006, to May 30, 2010. The consecutive patients were all 6 years of age or younger. The demographic information, cause of hydrocephalus, and cranial computerized measurements were taken (including widt

Technical considerations of endoscopic septostomy using a biportal approach in a case of hydrocephalus associated with tuberous sclerosis.

INTRODUCTION: We report herein the technical considerations for endoscopic septostomy in a case of hydrocephalus associated with tuberous sclerosis. CASE REPORT: A 17-year-old boy presented with visual and gait disturbances. Computed tomography revealed an intraventricular mass obstructing the foramen of Monro bilaterally and marked hydrocephalus. First, we planned a ventriculo-peritoneal shunt with endoscopic septostomy using a biportal approach to resolve the hydrocephalus. Guidance by a rigid endoscope inserted into the anterior horn of the left lateral ventricle allowed us to easily and s

Hemangiopericytoma in the lateral ventricle.

A 31-year-old female presented with a particularly rare hemangiopericytoma (HPC) in the right lateral ventricle manifesting as a 6-month history of visual disturbance and headache. Left hemianopsia and choked disc were identified by an ophthalmologist who referred her to us. Magnetic resonance imaging demonstrated a 5-cm homogeneously enhanced mass in the trigone of the right lateral ventricle. The tumor was totally removed by two stage surgery. The histological findings were consistent with HPC. HPC is very important to differentiate from meningioma and solitary fibrous tumors because HPC is

Reevaluating routine imaging: clinical utility of postoperative CT after stereotactic brain biopsy.

OBJECTIVE: Stereotactic brain biopsy (SBB) is a widely used and generally safe diagnostic procedure. However, the utility of routine postoperative CT scans to screen for hemorrhage remains controversial, and reported postbiopsy hemorrhage rates vary widely (1%-60%). This study aimed to identify factors associated with postbiopsy hemorrhage and determine whether a selective, symptom-driven imaging strategy could safely replace routine imaging. METHODS: The data of 751 patients who underwent 753 SBBs between 1993 and 2021, all of whom received a postoperative CT within 48 hours, were retrospect

Assessing neurodevelopmental outcome in children with hydrocephalus in Malawi. A pilot study.

INTRODUCTION: Congenital and infantile hydrocephalus are assumed to be major contributors to pediatric morbidity, mortality and functional disability in low-income countries. Despite this, epidemiologic data and the overview of neurodevelopmental outcomes in these regions is very limited. We aimed to pilot the use of a wide range of more locally suitable tools to assess neurodevelopment to understand whether they were feasible for use and could provide estimates of developmental delay and poor functioning in a population of children with hydrocephalus in Malawi. METHODS: We conducted a prospe

Patterns of Health Care Costs Due to External Ventricular Drain Infections.

BACKGROUND: External ventricular drain (EVD) infections are a significant cause of morbidity among neurosurgical patients and have been correlated with increased length of hospital stay and longer requirements for intensive care. To date, no studies have examined the financial impact of EVD infections. METHODS: Patients who underwent EVD placement between December 2010 and January 2016 were included in the study. Clinical records were retrospectively reviewed and health care cost data were obtained from the hospital's finance department. Clinical information included patient demographics, det

[Trapped temporal horn, an unusual form of obstructive hydrocephalus: 5 case-reports].

OBJECTIVE: "Entrapped temporal horn" is an unusual form of obstructive hydrocephalus which is due to an obstacle at the trigone of the lateral ventricle that seals off the temporal horn that may act as a space occupying process. In this study, our aim was to assess the clinical presentation, imaging, pathophysiology and the management of this entity. METHODS: The medical records of patients with entrapped temporal horn diagnosed between January 2003 and December 2012 were reviewed retrospectively. RESULTS: Five patients were identified. In four cases, the condition developed after cranial su

Endoscopic treatment of intraventricular cerebrospinal fluid cysts: 10 consecutive cases.

INTRODUCTION: The use of the endoscope for lesions of the central nervous system has been progressively widened in the past decades. Among these lesions, the intraventricular cyst is one of the most attractive targets for this less invasive procedure. METHODS: Between 2003 and 2007, ten consecutive patients with IVC underwent endoscopic surgery in our department. The location of the cyst was the lateral ventricle in nine, the lateral and third ventricles in one. The cyst was resected or fenestrated according to the degree of adhesion of the cyst wall with the ventricular wall. The follow-up r

Decompressive craniectomy and postoperative complication management in infants and toddlers with severe traumatic brain injuries.

OBJECT: Infants with severe traumatic brain injury represent a therapeutic challenge. The internal absence of open space within the infant cranial vault makes volume increases poorly tolerated. This report presents 7 cases of decompressive craniectomy in infants with cerebral edema. METHODS: The authors reviewed the medical charts of infants with brain injuries who presented to Albany Medical Center Hospital between January 2004 and July 2007. Variables that were examined included patient age, physical examination results at admission, positive imaging findings, surgery performed, complicatio

The occurrence of obstructive vs absorptive hydrocephalus in newborns and infants: relevance to treatment choices.

BACKGROUND AND OBJECTIVE: The classification of hydrocephalus in newborns and in infants is different from the classification in adulthood. This difference exists due to disparity in the source pathologies that produce the hydrocephalus, and the practical distinctions in prognosis and treatment choices. The objective of this paper is to present the spectrum of obstructive-communicating hydrocephalus, which is more complex in the pediatric group, and to propose the relevance of this particular classification to treatment options. MATERIALS AND METHODS: The authors categorized infants with acti

Withdrawal of shunt systems--clinical use of the programmable shunt system and its effect on hydrocephalus in children.

OBJECTS: The most important function of the programmable valve (PV) is to limit the shunt-dependent flow of the cerebrospinal fluid by upgrading valve pressure. This activates the regular circulation of cerebrospinal fluid, which may make successful removal of the shunt possible once sufficient cerebral development has been achieved. The purpose of this paper is to indicate the possibility of shunt removal using the programmable Medos and Sophy valves (one programmable Sophy valve was specially designed for this situation). METHODS: Prior to regular use of the PV, removal of existing shunt sy

Cerebellar herniation and infarction as a complication of an occult postoperative lumbar dural defect.

A patient who underwent lumbar spine surgery sustained an occult dural injury. After unrecognized suction drainage of cerebrospinal fluid through a surgical drain, caudal herniation of the cerebellum with superior cerebellar infarction developed. This mechanism should be considered in patients in whom acute mental status changes develop after spinal surgery.

Killer amoebas: Primary amoebic meningoencephalitis in a changing climate.

Primary amoebic meningoencephalitis (PAM) caused by Naegleria fowleri is a rare and deadly disease that requires prompt treatment with multiple therapies. Although N. fowleri previously was only found in warmer areas, climate change appears to be contributing to its geographic spread. Clinicians should consider PAM when faced with a patient with meningitis, especially if the patient participates in outdoor water activities or practices nasal rinsing.
