Raltegravir-intensified initial antiretroviral therapy in advanced HIV disease in Africa: A randomised controlled trial.

BACKGROUND: In sub-Saharan Africa, individuals infected with HIV who are severely immunocompromised have high mortality (about 10%) shortly after starting antiretroviral therapy (ART). This group also has the greatest risk of morbidity and mortality associated with immune reconstitution inflammatory syndrome (IRIS), a paradoxical response to successful ART. Integrase inhibitors lead to significantly more rapid declines in HIV viral load (VL) than all other ART classes. We hypothesised that intensifying standard triple-drug ART with the integrase inhibitor, raltegravir, would reduce HIV VL fast

Atopy is inversely related to schistosome infection intensity: a comparative study in Zimbabwean villages with distinct levels of Schistosoma haematobium infection.

BACKGROUND: The hygiene hypothesis suggests that parasitic infections protect against allergic diseases by modulating the host's immune responses. Experimental studies indicate that this protection depends on the intensity of parasitic infection, but this observation has not been tested in human populations. The aim of this study is to investigate whether the intensity of Schistosoma haematobium infection is related to atopic responses and whether this relationship differs between populations with distinct parasite transmission dynamics. METHODS: The study was conducted in two villages with d

The complex epidemiology of Rhodesian sleeping sickness in Kenya and Uganda. III. The epidemiology in the endemic areas along the lake shore between the Nile and the Yala swamp.

The endemic Rhodesian sleeping sickness belt along the shore of Lake Victoria between the Nile and the Yala swamp consists of two ecologically different areas: the Busoga belt and the Samia belt. The Busoga belt is a country of forest and forest relics, full of game, with a high density of Glossina fuscipes fuscipes but also inhabited by G. pallidipes, and with a people encroaching on the northern border who pass the forest on their way to the lake to catch and buy fish, but who seldom penetrate deep in the forest to hunt. The Samia belt is a country of dense bush in savanna, with less and sma

Schistosoma haematobium infection levels determine the effect of praziquantel treatment on anti-schistosome and anti-mite antibodies.

Field studies show an association between schistosome infection and atopy, but the effects of anti-helminthic treatment on this association have not yet been investigated in human populations with different schistosome endemicity levels. This study aimed to compare the effects of anti-helminthic treatment on responses directed against the house dust mite Dermatophagoides pteronyssinus (Derp1) and Schistosoma haematobium in Zimbabwean populations living in high and low schistosome infection areas. Derp1- and schistosome-specific IgE and IgG4 antibodies were quantified by ELISA before and 6 week

Performance of an artificial intelligence-based tool for cervical precancer screening in five countries in Africa: a prospective, observational, diagnostic accuracy study.

BACKGROUND: Cervical cancer kills 300 000 women annually, almost entirely in low-income and middle-income countries. Visual inspection of the cervix using acetic acid (VIA) remains a common method of screening but has suboptimal accuracy. Automated visual evaluation (AVE) is an internally validated artificial intelligence (AI)-based tool designed to assist health workers to improve VIA and to aid diagnostic accuracy. We aimed to measure sensitivity and specificity of AVE for detection of histology-confirmed cervical intraepithelial neoplasia 2 or greater (CIN2 +) in real-world clinical setting

MICRORNAS AS REGULATORS OF DRUG METABOLISM AND TRANSPORT IN PREGNANT AND LACTATING WOMEN.

BACKGROUND: Physiological changes during pregnancy result in altered maternal drug metabolism that impacts efficacy and safety of therapeutics in pregnant women. Pregnancy induced hormonal, immunologic, or metabolic changes may also influence and alter drug disposition. Despite research efforts focused on pharmacokinetics of medications used in pregnant women in the past decade, knowledge gaps exist in understanding how pregnancy influences drug disposition and placental drug transporters. Moreover, there is a scarcity of research in understanding the safety and effectiveness of therapeutics i

National tuberculosis prevalence surveys in Africa, 2008-2016: an overview of results and lessons learned.

OBJECTIVE AND METHODS: Worldwide, tuberculosis (TB) is the leading cause of death from a single infectious agent. In many countries, national TB prevalence surveys are the only way to reliably measure the burden of TB disease and can also provide other evidence to inform national efforts to improve TB detection and treatment. Our objective was to synthesise the results and lessons learned from national surveys completed in Africa between 2008 and 2016, to complement a previous review for Asia. RESULTS: Twelve surveys completed in Africa were identified: Ethiopia (2010-2011), Gambia (2011-2013

Impact of contraceptive initiation on vaginal microbiota.

BACKGROUND: Data evaluating the impact of contraceptives on the vaginal microbiome are limited and inconsistent. OBJECTIVE: We hypothesized that women initiating copper intrauterine device use would have increased bacterial vaginosis and bacterial vaginosis-associated microbes with use compared to women initiating and using hormonal contraceptive methods. STUDY DESIGN: Vaginal swabs (N = 1047 from 266 participants seeking contraception) for Nugent score determination of bacterial vaginosis and quantitative polymerase chain reaction analyses for assessment of specific microbiota were collecte

Enhanced Prophylaxis plus Antiretroviral Therapy for Advanced HIV Infection in Africa.

BACKGROUND: In sub-Saharan Africa, among patients with advanced human immunodeficiency virus (HIV) infection, the rate of death from infection (including tuberculosis and cryptococcus) shortly after the initiation of antiretroviral therapy (ART) is approximately 10%. METHODS: In this factorial open-label trial conducted in Uganda, Zimbabwe, Malawi, and Kenya, we enrolled HIV-infected adults and children 5 years of age or older who had not received previous ART and were starting ART with a CD4+ count of fewer than 100 cells per cubic millimeter. They underwent simultaneous randomization to rec

Assessing the feasibility and acceptability of a pilot peer led intervention to reduce stigma and improve ART adherence among young men having sex with men and transgender women living with HIV in low resource settings in Southern Africa.

BACKGROUND: Young men who have sex with men (MSM) and transgender women (TGW) in Southern Africa experience a disproportionate burden of HIV, compounded by stigma related to HIV status and sexual orientation or gender identity (SOGI). These stigmas undermine mental health and antiretroviral therapy (ART) adherence, yet few integrated, community-informed interventions exist in low-resource settings. This study assessed the feasibility, acceptability, and preliminary impact of a peer-led pilot intervention to reduce stigma, improve mental health, and strengthen ART adherence among young MSM and

Colorimetric RT-LAMP for SARS-CoV-2 detection from nasopharyngeal swabs or crude saliva: a multicountry diagnostic accuracy study in Africa.

BACKGROUND: Reverse-transcription loop-mediated isothermal amplification (RT-LAMP) with colorimetric readout is a rapid, robust, and cost-effective one-step amplification assay that we previously trialled for the identification of SARS-CoV-2 in nasopharyngeal swabs in four countries. Here, we expanded our assessment of RT-LAMP for SARS-CoV-2 detection to several other African countries and evaluated its operational performance with crude saliva as a pragmatic approach for outbreak surveillance and response in resource-limited settings. METHODS: We conducted a multicountry diagnostic accuracy
