Forearm Deformity and Radial Head Dislocation in Pediatric Patients with Hereditary Multiple Exostoses: A Prospective Study Using Proportional Ulnar Length as a Scale to Lengthen the Shortened Ulna.

BACKGROUND: Ulnar lengthening is a consensus treatment for hereditary multiple exostoses with radial head dislocation in pediatric patients; however, the optimal amount of ulnar lengthening remains unclear. It is necessary to look for a parameter to decide the amount of ulnar lengthening that will avoid recurrence and complications. The purpose of the present study was to confirm that proportional ulnar length (ulnar length/radial length) can be used as a scale for ulnar lengthening in pediatric patients. METHODS: The normal lengths of the ulna and radius in the pediatric population were meas

Alternative approaches to managing the cleft alveolus.

The cleft alveolus component of the oral cleft deformity is addressed with a separate surgical stage. Several host and operator related factors affect the surgical outcome. When factors that increase the likelihood of secondary alveolar bone graft failure are identified, alterative methods like dentoalveolar distraction (DAD) may be employed. In infants, molding of the alveolar segments is possible and when a synergistic surgical approach is used, the possibility of successful alveolar cleft repair is increased. The authors present two case reports wherein the use of nasoalveolar molding (NAM)

Late presentation of congenital dislocation of the knee: a case report.

Congenital dislocation of the knee is a rare disorder. Late presentation of congenital dislocation of the knee at an older age is a therapeutic challenge. A 12-year-old girl presented to us with congenital dislocation of the knee and with complaints of limp, short limb, and pain on weight bearing. Two-stage surgery was performed with quadricepsplasty followed by gradual distraction in the first stage and repeat quadricepsplasty, anterior capsular release, and open reduction in the second stage. The result was fair to good with a stable and painless knee on walking.

Orthopaedic splint treatment can reduce mandibular asymmetry caused by unilateral temporomandibular involvement in juvenile idiopathic arthritis.

Unilateral temporomandibular joint (TMJ) arthritis in juvenile idiopathic arthritis (JIA) patients often induces asymmetric dentofacial growth. This study evaluates the effects of an orthopaedic functional appliance worn full time to reduce asymmetric mandibular growth in JIA patients. Twenty-two JIA patients with unilateral TMJ involvement were included in the study (mean age 7.5 years, range: 3.8-13.8 years). They all received orthopaedic treatment with a functional appliance, a so-called distraction splint (mean treatment time 57 months). Panoramic tomograms or cone beam-computerized tomogr

Comparison of 39 post-traumatic tibia bone transports performed with and without the use of an intramedullary rod: the long-term outcomes.

Bone transport can be performed with an external fixator alone or with the monorail technique which entails the combination of a fixator and an intramedullary nail. The purpose of this study was to compare the complication rates and long-term outcomes of these methods. Two groups of patients, the external fixator (n = 21) and the monorail group (n = 18), were compared. The average follow-up period was 7.9 ± 5.6 years and the mean defect length 8.3 ± 3.1 cm. Healing was achieved in 19 (90%) and 13 (72%) of the fixator and monorail patients, respectively. Six patients underwent amputations becau

Use of dynamic distraction external fixation for unstable fracture-dislocations of the proximal interphalangeal joint.

PURPOSE: Unstable fracture-dislocations of the proximal interphalangeal (PIP) joint remain a difficult management problem, often leading to residual pain, stiffness, and recurrent instability. In a military setting, an easily applied, simple to operate, and inexpensive device becomes an attractive option. The purpose of this clinical investigation was to retrospectively review use of dynamic distraction external fixation (DDEF) for unstable fracture-dislocations and pilon injuries of the PIP joint in an active-duty population. METHODS: The fixator is assembled under a local anesthetic from th

Mechanical distraction for the treatment of posttraumatic stiffness of the elbow in children and adolescents.

BACKGROUND: Elbow contracture is a recognized sequela of elbow injuries in children and adolescents, but previous studies of operative treatment with formal capsular release have demonstrated unpredictable outcomes and unfavorable results. METHODS: Over a period of five years, fourteen children and adolescents with a mean age of fourteen years who had posttraumatic stiffness of the elbow were managed according to a prospective protocol. Eleven patients had undergone a mean of three previous operative procedures before the index operation. After intraoperative distraction with an external fixa

Occlusal difficulties after simultaneous mandibular and maxillary distraction in an adult case of hemifacial microsomia.

A 23-year-old woman with hemifacial microsomia type IIB was treated by simultaneous mandibular and maxillary distraction. During the course of the distraction, cephalometric analysis showed that the maxilla was not moving downward and that the upper edge of the mandibular condyle was moving upward into the space between the condyle and the glenoid fossa. This phenomenon could lead to early consolidation of the osteotomized maxilla, resulting in malposition and occlusal difficulties. This is thought to be a problem unique to bimaxillary distraction of hemifacial microsomia type IIB with severe

Acute shortening: modular treatment modality for severe combined bone and soft tissue loss of the extremities.

BACKGROUND: Acute shortening, using the Ilizarov technique followed by progressive lengthening, is one of the methods used to deal with complex fractures combined with severe soft tissue injuries. METHODS: We have summarized 12 patients who underwent acute shortening and stabilization using the Ilizarov frame. Nine of them underwent progressive lengthening to restore the length. For three patients, angulation of the bone segments was performed to save them from excessive bone debridement. RESULTS: Total wound closure and bone regeneration were achieved in all our patients. Five patients had

Orthodontic Airway Plate Therapy for Infants with Robin Sequence: Clinical Principles, Current Evidence, and Evolving Innovations.

BACKGROUND: Robin sequence (RS) is characterized by the triad of mandibular micrognathia, glossoptosis, and upper airway obstruction (UAO), posing significant airway and feeding challenges in affected infants. Although mandibular distraction osteogenesis is effective in selected cases, less invasive treatment approaches are emerging. The orthodontic airway plate (OAP), including the Tübingen palatal plate and pre-epiglottic baton plate, can be a reliable nonsurgical modality for infants across a broad spectrum of UAO severity, alleviating airway obstruction by anteriorly repositioning the tong

The Stanford Orthodontic Airway Plate: Design and Clinical Implementation in 64 Consecutively Treated Infants With Pierre Robin Sequence.

Mandibular distraction osteogenesis (MDO) is the established standard of care in the United States for severe upper airway obstruction in infants with Pierre Robin Sequence (PRS). While airway plates have been utilized effectively in Europe for decades, this approach was not adopted in the United States until 2020, when an institution-specific protocol was developed to coexist with surgical paradigms. The current study details the design and clinical implementation of the institution's Orthodontic Airway Plate (OAP). By applying orthodontic Visual Treatment Objective principles to maxillofacia
