The selective influence of rhythmic cortical versus cerebellar transcranial stimulation on human physiological tremor.

The influence of central neuronal oscillators on human physiological tremor is controversial. To address this, transcranial alternating current stimulation (TACS) was delivered at peak tremor frequency to 12 healthy volunteers in a 2 × 2 crossover study. Two sites were stimulated [contralateral primary motor cortex (M1), vs ipsilateral cerebellum] while participants performed two types of tasks designed to probe the different manifestations of physiological tremor of the hand-kinetic and postural tremor. Tremor was measured by accelerometry. Cortical coherence with the accelerometry signal was

Ankle-worn accelerometers for gait analysis in people with Alzheimer's disease and mild cognitive impairment: reliability of a finite state machine approach.

Clinical gait analysis is essential for understanding motor and cognitive contributions to mobility impairment. Continued methodological advancement in gait analysis is needed to detect important and subtle changes in walking behaviour that will inform this understanding. This study evaluated the reliability of a finite state machine (FSM) algorithm for stride segmentation during preferred and dual-task walking (DTW) and examined changes to temporal and accelerometry kinematic outcomes between tasks. Participants diagnosed with Alzheimer's disease or mild cognitive impairment completed a gait

Validation of the General Practice Physical Activity Questionnaire after severe COVID-19.

BACKGROUND: Physical inactivity is a risk factor for severe COVID-19 and often worsens after hospitalisation. Clinicians need quick, accurate assessments to target interventions. We aimed to assess the validity of the General Practice Physical Activity Questionnaire (GPPAQ) in adults recovering 1 year after COVID-19 hospitalisation. METHODS: Post-hospitalisation for COVID-19, adults attended a 1-year-visit and completed the GPPAQ Physical Activity Index (PAI-4 active), 14-day wrist-worn accelerometry (moderate-vigorous physical activity (MVPA) active) and other health outcomes. Validity was e

Quantitative tremor monitoring before, during and after MR-guided focused ultrasound thalamotomy for essential tremor with MR compatible accelerometers.

BACKGROUND: MR-guided focused ultrasound (MRgFUS) has been developed to treat essential tremor effectively and noninvasively. Currently, clinical examination is used to identify therapeutic efficacy during treatment, but MRgFUS surgery could benefit from real-time, rater-independent quantitative monitoring of tremor, such as accelerometry data. METHODS: Fourteen patients with medically refractory essential tremor underwent MRgFUS thalamotomy. Patients were instructed to hold postures during treatment. Tremor was monitored during each ultrasonic thermal sonication with MR-compatible accelerome

Bidirectional associations of accelerometer-derived physical activity and stationary behavior with self-reported mental and physical health during midlife.

BACKGROUND: Moderate-to-vigorous intensity physical activity (MVPA) is associated with favorable self-rated mental and physical health. Conversely, poor self-rated health in these domains could precede unfavorable shifts in activity. We evaluated bidirectional associations of accelerometer-estimated time spent in stationary behavior (SB), light intensity physical activity (LPA), and MVPA with self-rated health over 10 years in in the CARDIA longitudinal cohort study. METHODS: Participants (n = 894, age: 45.1 ± 3.5; 63% female; 38% black) with valid accelerometry wear and self-rated health at

Wearable movement-tracking data identify Parkinson's disease years before clinical diagnosis.

Parkinson's disease is a progressive neurodegenerative movement disorder with a long latent phase and currently no disease-modifying treatments. Reliable predictive biomarkers that could transform efforts to develop neuroprotective treatments remain to be identified. Using UK Biobank, we investigated the predictive value of accelerometry in identifying prodromal Parkinson's disease in the general population and compared this digital biomarker with models based on genetics, lifestyle, blood biochemistry or prodromal symptoms data. Machine learning models trained using accelerometry data achieve

Tremor Frequency Assessment by iPhone® Applications: Correlation with EMG Analysis.

Tremor frequency analysis is usually performed by EMG studies but accelerometers are progressively being more used. The iPhone® contains an accelerometer and many applications claim to be capable of measuring tremor frequency. We tested three applications in twenty-two patients with a diagnosis of PD, ET and Holmes' tremor. EMG needle assessment as well as accelerometry was performed at the same time. There was very strong correlation (Pearson >0.8, p < 0.001) between the three applications, the EMG needle and the accelerometry. Our data suggests the apps LiftPulse®, iSeismometer® and Studymyt

The impact of pregabalin and gabapentin on essential tremor: a systematic review and meta-analysis.

INTRODUCTION: Essential tremor (ET) is a common neurological disorder characterized by involuntary rhythmic oscillations. Emerging evidence suggests that gabapentinoids, including gabapentin and pregabalin, may be effective in managing ET. This study aimed to assess the efficacy of gabapentinoids in treating ET by analyzing data from randomized controlled trials (RCTs). METHODS: A systematic search was conducted across various databases to identify relevant RCTs utilizing gabapentin or pregabalin. Tremor severity was evaluated using the total tremor score (TTS) and accelerometry. RESULTS: In

The use of accelerometer bracelets to evaluate arm motor function over a stroke rehabilitation period - an explorative observational study.

BACKGROUND: Assessments of arm motor function are usually based on clinical examinations or self-reported rating scales. Wrist-worn accelerometers can be a good complement to measure movement patterns after stroke. Currently there is limited knowledge of how accelerometry correlate to clinically used scales. The purpose of this study was therefore to evaluate the relationship between intermittent measurements of wrist-worn accelerometers and the patient's progression of arm motor function assessed by routine clinical outcome measures during a rehabilitation period. METHODS: Patients enrolled

Quantifying real-world upper limb activity via patient-initiated spontaneous movement in neonatal brachial plexus palsy.

BACKGROUND: Neonatal brachial plexus palsy (NBPP) results in muscle weakness and impaired somatosensory function of the arm. Current functional assessment is primarily based on clinician-elicited measurements including muscle strength and range of motion. To what extent these measures are representative of real-world arm movement is unclear. OBJECTIVE: To determine the feasibility of using body-worn accelerometers to remotely assess arm movements in children with NBPP. DESIGN: Prospective criterion validity study of accelerometry versus clinician assessment. SETTING: Academic medical center

Association of Changes in Activity Patterns With Brain Atrophy and Disability Progression in People With Multiple Sclerosis.

BACKGROUND AND OBJECTIVES: The purpose of this study was to evaluate whether accelerometry detects subtle changes in disability in people with multiple sclerosis (PwMS). Specifically, we tested how accelerometry measures change longitudinally in PwMS and assessed how these changes translate to brain atrophy and disability worsening at the individual level. METHODS: We included PwMS aged 40 years or older with approximately annual brain MRI who wore GT9X Actigraph accelerometers every 3 months over 3 years. Accelerometry-derived indices included total and 2-hour specific activity, sedentary ti

Quantifying observable movement reserve in multiple sclerosis via upper diurnal activity quantiles.

BACKGROUND: The ability to sustain high-effort motor activity throughout the day may serve as a marker of functional capacity in individuals with multiple sclerosis (MS). We refer to this ability as observable movement reserve. Conventional assessments, including the Expanded Disability Status Scale (EDSS), often miss subtle functional changes. While accelerometry captures 24-hour motor activity, it typically overlooks peak activity and its diurnal variability. OBJECTIVE: To evaluate whether upper diurnal activity quantiles from accelerometry capture observable movement reserve and are associ

Beyond MVPA minutes: physical activity phenotypes are associated with metabolic profiles in children with obesity.

Pediatric obesity is associated with early cardiometabolic risk, yet MVPA alone may not capture meaningful variation in movement behavior. We applied DDRTree to accelerometry data to identify multidimensional physical activity phenotypes in children and adolescents with obesity and assessed their associations with body composition and cardiometabolic. This cross-sectional study included 91 children and adolescents with obesity (43% girls), aged 6-17 years, who wore a thigh-mounted accelerometer for 7 days. Multidimensional accelerometry features were extracted and PA phenotypes identified usin

Out-of-hospital cardiac arrest detection by a wearable: the first real-life case.

INTRODUCTION: Automated cardiac arrest detection and alerting using wearable technology is an emerging innovation in resuscitation science. The DETECT program aims to develop a wristband that integrates photoplethysmography and accelerometry to identify out-of-hospital cardiac arrest events and alert the emergency medical services. We report the first case in which the algorithm successfully detected a cardiac arrest that occurred during daily life in an implantable cardioverter-defibrillator (ICD) patient. CASE DESCRIPTION: A 64-year old male with a secondary prevention ICD participated in t

Alzheimer's disease polygenic risk, APOE ε4 dose, and accelerometer-derived circadian rest-activity rhythms in relation to incident dementia and Alzheimer's disease in UK Biobank: a prospective cohort study.

BACKGROUND: Circadian rest-activity rhythm disruption and sleep-wake fragmentation are common in Alzheimer's disease (AD) and may precede clinical diagnosis. Wrist-worn accelerometry enables objective, scalable circadian phenotyping in population cohorts. We examined whether AD polygenic risk and APOE ε4 dose are reflected in accelerometry-derived circadian phenotypes and whether these phenotypes predict subsequent incident dementia/AD. METHODS: We analyzed 94,241 UK Biobank participants with 7-day wrist accelerometry and genetic data. Circadian phenotypes included relative amplitude (RA), L5

Prediction of activity-related energy expenditure using accelerometer-derived physical activity under free-living conditions: a systematic review.

BACKGROUND/OBJECTIVES: Activity-related energy expenditure (AEE) might be an important factor in the etiology of chronic diseases. However, measurement of free-living AEE is usually not feasible in large-scale epidemiological studies but instead has traditionally been estimated based on self-reported physical activity. Recently, accelerometry has been proposed for objective assessment of physical activity, but it is unclear to what extent this methods explains the variance in AEE. SUBJECTS/METHODS: We conducted a systematic review searching MEDLINE database (until 2014) on studies that estima

The effects of TPA023, a GABAAα2,3 subtype-selective partial agonist, on essential tremor in comparison to alcohol.

Essential tremor (ET) is a relatively frequent neurological disorder that responds in some patients to gamma-aminobutyric acid A (GABA(A)) agonists such as the benzodiazepines. Partial subtype-selective GABA(A) agonists may have an improved side effect profile compared to non-selective GABA(A) agonists. However, it is unknown which GABA(A) subtypes are involved in the therapeutic effects of benzodiazepines in ET. The effects of 2 mg TPA023, a GABA(A) α2,3 subtype-selective partial agonist, on ET were compared to the effects of a stable alcohol level (0.6 g/L) and placebo in nine patients with

A double-blind placebo-controlled trial of zonisamide (zonegran) in the treatment of essential tremor.

Medical therapy for essential tremor (ET), a common movement disorder, is often inadequate. We performed a double-blind placebo-controlled randomized trial to evaluate the efficacy and tolerability of zonisamide (ZNS), an antiepileptic agent, in treating ET. Twenty patients (mean age, 60 +/- 15 years) with ET were randomized to receive ZNS or placebo. ZNS was initiated at a dosage of 100 mg/day and escalated to 200 mg/day at day 14. Patients were evaluated by accelerometry and the Fahn-Tolosa-Marin (FTM) rating scale at baseline and days 14 and 28, as well as the Clinical Global Impression (CG
