Three-dimensional printed models of the rib cage in children with non-accidental injury as an effective visual-aid tool.

Three-dimensional (3-D) printing is gaining terrain in medical education, presurgical evaluation and recently as forensic evidence in court. Physicians, including radiologists, often provide expert testimony in court cases involving children with rib fractures and other injuries concerning for child physical abuse. Effectively communicating the complexities of fractures and other skeletal findings to nonmedical personnel using standard radiology studies can be challenging, especially during medical courtroom testimony. For this reason, we printed two 3-D models of the rib cage from the chest c

The pathogenesis of retinal and subdural haemorrhage in non-accidental head injury in infancy: assessment using Bradford Hill criteria.

Fatal non-accidental head injury (NAHI) in infancy is characterised by severe bilateral retinal haemorrhage (sbRH), thin film diffuse bilateral subdural haemorrhage (dbSDH) and severe fatal hypoxic ischaemic encephalopathy (sfHIE). The Geddes hypothesis is that sfHIE is the direct cause of sbRH and dbSDH. The conventional hypothesis (post-Geddes) is that trauma precipitates hypoxia to cause sfHIE and independently tears veins to cause sbRH and dbSDH. These hypotheses are assessed using Bradford Hill criteria of causation. The triad (sbRH, dbSDH and sfHIE) is a strong, specific and consistent a

Fatal facial-intracranial impalement injury in an accidental fall from a height: an autopsy case report with a review of the literature.

Impalement injury to the face and head is of clinical and forensic pathological significance because of its diagnostic pitfalls. This injury often penetrates the orbital, nasal or oral cavity into the skull, but impaling other sites of facial bone is rare. The present case was a rare type of facial-intracranial impalement injury with a direct cerebral arterial laceration. The victim was a 44-year-old man who fell from a height of about 2.5m onto a broken chair and died 6 days later despite surgical treatment. The clinical diagnosis was a 'small abrasion' of the face and 'traumatic intracerebra

Fatal respiratory thermal injury following accidental administration of carbon dioxide using the circle system for a cesarean delivery.

A 37-year-old parturient underwent emergency cesarean delivery because of severe preeclampsia. After induction of general anesthesia, the oxygen saturation decreased. Volatile anesthetics were discontinued and examination of the anesthetic circuit and machine revealed a soda lime canister that was extremely hot. The patient was detached from the anesthetic machine and hand-ventilated with an external oxygen cylinder. The surgery was cancelled and the patient was extubated. Analysis of the cylinder connected to the anesthesia machine displayed 100% carbon dioxide. The patient developed progress

Judicial outcomes in non-accidental head injury cases evaluated by the forensic medicine department of Angers University Hospital.

INTRODUCTION: The diagnosis of non-accidental head injury (NAHI) is based on specific traumatic injuries listed in national recommendations published by the Haute Autorité de Santé (HAS, French national authority for health). Forensic experts play a central role in these cases. However, they often have limited knowledge of judicial outcomes, providing the rationale for this study. MATERIALS AND METHODS: This retrospective single-center study covers 11 years of NAHI cases diagnosed and evaluated by the forensic medicine department of Angers University Hospital. Medical data were obtained from

Three Pediatric Cases of Accidental Lip Entrapment, Laceration, and Tongue Injury From Motorized Electric Nail Trimmers.

BACKGROUND Oral and perioral soft tissue injuries are common in early childhood, when oral exploration and imitation of adult grooming coincide with immature motor control. Motorized electric nail trimmers-battery-powered devices with a rotating element behind a small aperture-are commonly available; some reports have described entrapment of the tongue, facial skin, and eyelid in children. This report presents 3 pediatric cases of lip entrapment, laceration, and tongue injury caused by such devices. CASE REPORT Case 1: A 4-year-old girl presented with lower lip entrapment after activating a de

The Association Between Acute Pain and Posttraumatic Stress Symptoms in Children and Adolescents 3 Months After Accidental Injury.

Previous research suggests that acute pain is a risk factor for later posttraumatic stress symptoms (PTSS). In a prospective cohort study, we examined the association between acute pain from accidental injury and PTSS in children and adolescents, taking into account factors potentially related to pain or posttraumatic stress. Participants were 135 children and adolescents, 8-18 years old. We measured the worst experienced pain since the accident took place with a visual analogue scale. Three months after the accident, posttraumatic stress was assessed with a self-report measure. We found a pos

Predictors of increasing injury severity across suspected recurrent episodes of non-accidental trauma: a retrospective cohort study.

BACKGROUND: Little is known about how the severity of injury changes with recurrent events of suspected non-accidental trauma (NAT). Our objective was to determine risk factors for escalating severity of injury in children with multiple events of suspected NAT. METHODS: This retrospective longitudinal cohort study included children from a pediatric Medicaid accountable care organization with ≥ 1 non-birth related episode containing an International Classification of Diseases, Ninth Revision, Clinical Modification or Current Procedural Terminology code for NAT or a skeletal survey between 2007

Social interaction in young children with inflicted and accidental traumatic brain injury: relations with family resources and social outcomes.

Core social interaction behaviors were examined in young children 0-36 months of age who were hospitalized for accidental (n = 61) or inflicted (n = 64) traumatic brain injury (TBI) in comparison to typically developing children (n = 60). Responding to and initiating gaze and joint attention (JA) were evaluated during a semi-structured sequence of social interactions between the child and an examiner at 2 and 12 months after injury. The accidental TBI group established gaze less often and had an initial deficit initiating JA that resolved by the follow-up. Contrary to expectation, children wit

Multiple thoracic vertebral compression fractures caused by non-accidental injury: case report with radiological-pathological correlation.

We report a 21-month-old boy with multiple contiguous thoracic vertebral compression fractures involving eight vertebral bodies, attributable to non-accidental injury. No subluxation was associated, however, there was extensive injury to the upper cervical and lower lumbar regions of the spinal cord. Anterosuperior beaking, thought to represent a previous injury, was evident in a mid-lumbar vertebra. Clinical examination revealed bilateral retinal hemorrhages and retinoschisis. Death occurred as a result of severe brain edema with bilateral subdural and subarachnoid hemorrhages. Radiological-p

Seminal vesicle abscess with orchitis and emphysematous cystitis following traumatic urethral injury from accidental urinary catheter dislodgement.

BACKGROUND: Indwelling urinary catheters are widely used in hospitalized patients but can be inadvertently dislodged due to trauma, leading to complications such as catheter-associated urinary tract infections and urological injuries. CASE REPORT: An 87-year-old man developed a seminal vesicle abscess (SVA) caused by Pseudomonas aeruginosa following urethral injury due to accidental urinary catheter dislodgement. The patient presented with hematuria and penile bleeding following dislodgement of the urinary catheter, and subsequently developed fever and a urinary tract infection. Computed tomo

Documenting non-accidental injury patterns in a dog abuse investigation: A collaborative approach between forensic anthropology and veterinary pathology.

This case study demonstrates how animal abuse investigations can benefit from collaboration between forensic anthropologists and veterinary pathologists, especially in investigations involving trauma assessment of skeletal remains. In 2009, four deceased dogs were excavated from a residential property in conjunction with a case of suspected animal abuse. The decomposed remains were skeletonized to permit assessment of biological profiles, individualization, and trauma analysis. Each dog was identified based on their sex, age-at-death, fur coloration patterns, and through investigative informat

Screening for posttraumatic stress symptoms after accidental injury: Implementation of the young child PTSD screen in the acute trauma setting.

BACKGROUND: Post-traumatic stress symptoms (PTSS) in young children are underrecognized following unintentional injury, and validated screening tools for early childhood are rarely implemented in acute trauma settings. We evaluated the Young Child PTSD Screen (YCPS) administered at hospital discharge to determine screening positivity, associated clinical factors, and persistence of symptoms at one month. METHODS: We conducted a retrospective cohort study of children aged 2-6 years admitted to a Level I pediatric trauma center following unintentional injury (November 2020-November 2024). Careg

Accidentally ingested toothpicks causing severe gastrointestinal injury: a practical guideline for diagnosis and therapy based on 136 case reports.

BACKGROUND: Ingested toothpicks are a relatively rare event, but may cause serious gut injuries with peritonitis, sepsis, or death. Numerous case reports describing the clinical course in this setting are available but there is no concise guideline. The aim of the present study was to develop practical guidelines to aid clinicians in the diagnosis and management of acute tooth pick ingestion. METHOD: Our Medline search identified 116 publications containing case reports of ingested toothpicks. We then performed a retrospective analysis of patients' characteristics, medical history, diagnostic
