Lung Infection Due to Species Infrequently Isolated in the Community Setting: Klebsiella variicola and Acinetobacter baumannii.

The authors report a case of pulmonary infection caused by two typically nosocomial bacterial agents, Klebsiella variicola and Acinetobacter baumannii, simultaneously evidenced in a 65-year-old male patient from the community. Both diagnoses were established after culture and matrix-assisted laser desorption/ionization time-of-flight mass spectrometry (MALDI-TOF MS) analysis of sputum, and the patient was successfully treated with oral levofloxacin, with microbiological eradication subsequently confirmed by three negative follow-up sputum cultures. The singularity of this case challenges the c

Successful Treatment of Carbapenem-Resistant Acinetobacter baumannii Infection with Sulbactam-Durlobactam: A Two-Case Report.

BACKGROUND: Sulbactam-Durlobactam (SUL-DUR), a novel β-lactam/β-lactamase inhibitor combination, exhibits potent activity against Acinetobacter baumannii isolates, including carbapenem-resistant Acinetobacter baumannii (CRAB). Current guidelines recommend SUL-DUR in combination with imipenem-cilastatin or meropenem for the treatment of CRAB pneumonia. However, clinical evidence regarding its use in CRAB bloodstream infections remains limited, and the role of SUL-DUR monotherapy in CRAB pneumonia remains unclear. METHODS: We describe two critically ill patients with severe CRAB infections who

Live attenuated hfq-deleted Acinetobacter baumannii vaccine provides protection against systemic infection.

Acinetobacter baumannii is a multidrug-resistant (MDR) nosocomial pathogen with high mortality rates, necessitating alternative preventive strategies. The RNA chaperone Hfq plays a key role in bacterial virulence, and its deletion attenuates pathogenicity. In this study, we evaluated an hfq deletion mutant (Δhfq) of A. baumannii ATCC 17978 as a live-attenuated vaccine candidate. Mice vaccinated intraperitoneally with 2.3 × 107 CFU of Δhfq, followed by a booster on day 28, exhibited 90 % survival after an intraperitoneal lethal challenge with MDR A. baumannii AB5075, whereas all unvaccinated co

Positron Emission Tomography Imaging of Acinetobacter baumannii Infection: Comparison of Gallium-68 Labeled Siderophores.

Acinetobacter baumannii (AB) is an opportunistic pathogen with growing clinical relevance due to its increasing level of antimicrobial resistance in the last few decades. In the event of an AB hospital outbreak, fast detection and localization of the pathogen is crucial, to prevent its further spread. However, contemporary diagnostic tools do not always meet the requirements for rapid and accurate diagnosis. For this reason, we report here the possibility of using gallium-68 labeled siderophores, bacterial iron chelators, for positron emission tomography imaging of AB infections. In our study,

Emergence of co-infection with Cryptococcus neoformans and Acinetobacter baumannii in older people with systemic lupus erythematosus.

Systemic lupus erythematosus (SLE) is a common autoimmune disease affecting various organs. Cryptococcus neoformans, an environmental yeast that is heavily dependent on the complement cascade for natural host defense, is produced via inhalation in immunocompromised patients. The fungal pathogen, C. neoformans, can cause life-threatening infections in patients with SLE. Acinetobacter baumannii is a nosocomial pathogen that causes healthcare-associated infections, particularly in immunocompromised and critically ill patients. However, reports of co-infection with C. neoformans and A. baumannii i

Dietary zinc deficiency promotes Acinetobacter baumannii lung infection via IL-13 in mice.

Dietary zinc deficiency is a major risk factor for pneumonia. Acinetobacter baumannii is a leading cause of ventilator-associated pneumonia and a critical public health threat due to increasing rates of multidrug resistance. Patient populations at increased risk for A. baumannii pneumonia are also at increased risk of zinc deficiency. Here we established a mouse model of dietary zinc deficiency and acute A. baumannii pneumonia to test the hypothesis that host zinc deficiency contributes to A. baumannii pathogenesis. We showed that zinc-deficient mice have significantly increased A. baumannii b

Biomimetic Neutrophil Nanotoxoids Elicit Potent Immunity against Acinetobacter baumannii in Multiple Models of Infection.

Acinetobacter baumannii is a leading cause of antibiotic-resistant nosocomial infections with high mortality rates, yet there is currently no clinically approved vaccine formulation. During the onset of A. baumannii infection, neutrophils are the primary responders and play a major role in resisting the pathogen. Here, we design a biomimetic nanotoxoid for antivirulence vaccination by using neutrophil membrane-coated nanoparticles to safely capture secreted A. baumannii factors. Vaccination with the nanotoxoid formulation rapidly mobilizes innate immune cells and promotes pathogen-specific ada

BACKGROUND: Carbapenem-resistant Acinetobacter baumannii (CRAB) tops the list of threats to human health. Studies exploring predictors of mortality in patients with CRAB infection produced conflicting results. METHODS: A systematic search of the PubMed, Embase, and the Cochrane Library databases was performed from inception to June 2018 to identify studies reporting mortality predictors in patients infected with CRAB. Two authors independently assessed trials for inclusion and data extraction. RESULTS: A total of 19 observational studies were enrolled in this study. Factors associated with m

Multidrug-Resistant Acinetobacter baumannii Ventriculostomy-Related Infection, Treated by a Colistin, Tigecycline, and Intraventricular Fibrinolysis.

BACKGROUND: Acinetobacter baumannii meningitis and ventriculitis are difficult issues, because of the low diffusion of antibiotics in the cerebrospinal fluid and bacterial multidrug resistance. The presence of an infected intraventricular hematoma, constituting an equivalent of undrained abscess, may promote biofilm formation and failure of medical treatment. CASE DESCRIPTION: In this case of ventriculostomy-related infection after ventricular hemorrhage, Acinetobacter baumannii was sensitive only to colistin and tigecycline. Despite a combination therapy involving intraventricular injections

A case report of intraventricular tigecycline therapy for intracranial infection with extremely drug resistant Acinetobacter baumannii.

RATIONALE: Intracranial infection with Acinetobacter baumannii is a tough problem due to the presence of multiresistance and drugs poor penetration through the blood brain barrier (BBB). Tigecycline is effective to cure A baumannii, but it can only be used intravenously which is also difficult to pass BBB. So, it will be a breakthrough if intraventricular (IVT) tigecycline is used in the clinical therapy. However, this treatment has been reported quite rarely until now. PATIENT CONCERNS: We described a case of a 50-year-old male worker whose clinical futures were high fever and cerebral rigid

Biofilm formation by Acinetobacter baumannii strains isolated from urinary tract infection and urinary catheters.

Fifty Acinetobacter isolates were obtained from urinary tract infections and urinary catheter samples. Analytical profile index assays identified 47 isolates as Acinetobacter baumannii and three as Acinetobacter lwoffii. Six A. baumannii isolates (A1-A6) displayed hydrophobicity indices >70%. Twenty isolates exhibited lectin activity. Biofilm formation by these isolates was compared with those with low hydrophobicity index values (A45-A50). Biofilms on different surfaces were confirmed by light microscopy, epifluorescence microscopy and by obtaining scanning electron microscope images. Biofilm

BACKGROUND: Carbapenem-resistant Acinetobacter baumannii (CRAB) continues to pose significant public health in clinical settings due to its remarkable genomic plasticity and resistance to available therapeutic drugs, including carbapenems. Bacteriophage has emerged as an optimistic solution capable of addressing such drug resistance dilemma. This study represents a comprehensive characterization of a novel Acinetobacter phage with potential application against CRAB-associated wound infections. METHODS: Sewage sample was obtained, processed, and enriched with A. baumannii M13 phage(s) for the

A requirement for Acinetobacter baumannii purine biosynthesis during lung infection is exacerbated by host zinc deficiency.

Acinetobacter baumannii is a leading cause of hospital-acquired infections, including ventilator-associated pneumonia. Dietary zinc deficiency is a major risk factor for pneumonia, and hospitalized patients at risk for A. baumannii infection have increased rates of zinc deficiency. We previously showed that dietary zinc deficiency enhanced A. baumannii pneumonia pathogenesis via an IL-13-dependent mechanism in mice. Here, we identified A. baumannii genes required for proliferation in the lungs of zinc-deficient mice using a genome-wide transposon sequencing (Tn-seq) screen. In zinc-deficient m

Daily rhythms modulate Acinetobacter baumannii physiology impacting infection outcome and antibiotic-inactivating capacity.

We recently demonstrated that the human pathogen Acinetobacter baumannii exhibits light-regulated daily and circadian rhythms, suggesting that the physiological state of the bacterium varies along the day. Because this temporal dimension may influence host-pathogen interactions, we investigated here whether key bacterial processes relevant to pathogenesis fluctuate according to light-dark-induced rhythms and whether these fluctuations impact infection outcome. In particular, β-lactamase activity displayed significant daily variation in several multidrug-resistant strains, with higher activity

Site-specific and time-varying Acinetobacter colonization and risk of invasive infection in critically ill patients.

PURPOSE: Colonization by Acinetobacter baumannii is common among critically ill patients. However, the relative contribution of colonization at different anatomical sites and the impact of dynamic changes in colonization status over time remain incompletely defined. METHODS: We conducted a retrospective cohort study of adult intensive care unit (ICU) patients with at least one surveillance culture positive for carbapenem-resistant Acinetobacter spp. (CRAB) between January 2022 and January 2026. Active surveillance cultures were obtained at ICU admission and weekly thereafter from respiratory,

Integrating genomic and transcriptomic analyses reveals virulence factors and bloodstream infection mechanisms of Acinetobacter junii.

Acinetobacter junii, an emerging pathogen of the Acinetobacter genus, exhibits concerning environmental persistence and clinical virulence potential. Despite its growing significance in bloodstream infections (BSIs), the genetic determinants distinguishing pathogenic from environmental strains remain poorly characterized, limiting our understanding of its transmission dynamics and host adaptation mechanisms. We explored the diversity and population structure of A. junii using a comprehensive genome dataset, including four strains isolated from China, one of which (acj0210) was derived from BSI

Human bloodstream infection caused by a Colistin multidrug-resistant Acinetobacter baumannii strain: a case report.

Multidrug-resistant Acinetobacter baumannii is an important pathogen causing healthcare-associated infections. The onset of Pan Drug Resistant (PDR) strains of A. baumannii, which are also resistant to colistin due to its excessive consumption, makes these strains difficult to eradicate. The appearance of new aggressive phenotypes among sentinel microorganisms in the control of healthcare-associated infections (HAI) requires the molecular approach to define a database of local nosocomial strains. Whole Genome Sequencing (WGS) is a useful tool to monitor the emergence of new aggressive bacteria
