Predicting survival from out-of-hospital cardiac arrest: a multivariate analysis.

From 954 attempts to resuscitate patients from out-of-hospital cardiac arrest two datasets were derived, namely 861 cases of cardiac arrest and 906 cases of either cardiac or primary respiratory arrest. For each dataset, multivariate analysis was performed by fitting a number of explanatory variables with respect to the outcomes of admission to hospital and discharge home in logistic regression models. There were numerous interactions between these variables. Being conscious at the time of the arrival of the ambulance crew and subsequently having cardiac arrest strongly predicted survival, as

Extracorporeal Membrane Oxygenation-Impella (ECPELLA) in a Patient With Recalcitrant Ventricular Tachycardia Undergoing Ablation.

In advanced heart failure, refractory hemodynamically unstable ventricular tachycardia (VT) can be life-threatening. The utilization of short-term temporary mechanical circulatory support (MCS) has been described. Still, it is limited to the intra-aortic balloon pump (IABP) or Impella 2.5/CP (Abiomed Inc., Danvers, MA, USA) which may only provide up to 1-2.5 L/min of added support. Escalation of MCS therapies should be considered. Referrals to advanced tertiary heart transplant centers should be done early to afford patients the best chance at an optimal outcome, with the option for heart tran

When the Female Heart Stops: Sex and Gender Differences in Out-of-Hospital Cardiac Arrest Epidemiology and Resuscitation.

Sex- and gender-based differences are emerging as clinically significant in the epidemiology and resuscitation of patients with out-of-hospital cardiac arrest (OHCA). Female patients tend to be older, experience arrest in private locations, and have fewer initial shockable rhythms (ventricular fibrillation/ventricular tachycardia). Despite standardized algorithms for the management of OHCA, women are less likely to receive evidence-based interventions, including advanced cardiac life support medications, percutaneous coronary intervention, and targeted temperature management. While some data s

Pulseless electrical activity in a pediatric patient: a case report and review of causative factors and treatment.

Pulseless electrical activity, an arrhythmia that leads to cardiac arrest, is defined as the presence of organized electrical activity without a palpable pulse or arterial blood pressure. When this arrhythmia presents during anesthesia, it has become routine practice to initiate advanced cardiac life support according to the American Heart Association guidelines. This arrhythmia is usually associated with a poor prognosis unless a reversible cause is investigated and treated immediately. The purpose of this article is to summarize the causative factors of pulseless electrical activity and its

Nurse-driven analgesia and sedation in pediatric patients with univentricular hearts requiring extracorporeal life support after first-stage palliation surgery: A pilot study.

BACKGROUND: Few data are available regarding requirements of sedation and analgesia in children during extracorporeal life support. AIMS: The aim of this study was to evaluate if children with functionally univentricular hearts on extracorporeal life support after first-stage palliation surgery have higher requirement of analgesics and sedatives compared with children without extracorporeal life support using a goal-directed nurse-driven analgesia and sedation protocol. METHODS: This prospective observational matched case-control pilot study was conducted at a cardiac pediatric intensive car

Reversible Causes in Cardiovascular Collapse at the Emergency Department Using Ultrasonography (REVIVE-US).

INTRODUCTION: Ultrasonographic evaluation of patients in cardiac arrest is currently not protocolised in the advanced cardiac life support (ACLS) algorithm. Potentially reversible causes may be identified using bedside ultrasonography that is ubiquitous in most emergency departments (EDs). This study aimed to evaluate the incidence of sonographically detectable reversible causes of cardiac arrest by incorporating an ultrasonography protocol into the ACLS algorithm. Secondary objectives include rates of survival to hospital admission, hospital discharge, and 30-day mortality. MATERIALS AND MET

Hypertrophic cardiomyopathy in the trauma patient: A case report and review of the literature.

INTRODUCTION: Hypertrophic cardiomyopathy (HCM), a prevalent genetic cardiac condition characterized by myocardial thickening, poses unique challenges in trauma care. Sparse evidence seems to agree that HCM patients have worse outcomes following non-cardiac surgeries, particularly emergent procedures. However, despite a prevalence of 1 in 500 in the US population, the intersection of HCM and Advanced Trauma Life Support (ATLS) remains undiscussed in current literature. CASE PRESENTATION: A 54-year-old female with unknown past medical history presented as a level 2 trauma alert following a mot

Cardiac xenotransplantation: from concept to clinic.

For many patients with terminal/advanced cardiac failure, heart transplantation is the most effective, durable treatment option, and offers the best prospects for a high quality of life. The number of potentially life-saving donated human organs is far fewer than the population who could benefit from a new heart, resulting in increasing numbers of patients awaiting replacement of their failing heart, high waitlist mortality, and frequent reliance on interim mechanical support for many of those deemed among the best candidates but who are deteriorating as they wait. Currently, mechanical assist

Prehospital cardiac arrest and the adverse effect of male gender, but not age, on outcome.

OBJECTIVE: To analyze the incidence and outcome of prehospital cardiac arrest as it correlated to gender and age as a secondary end point in an interventional clinical trial. METHODS: This prospective, randomized, double-blinded clinical intervention trial enrolled 874 prehospital cardiopulmonary arrest patients encountered by prehospital urban, suburban, and rural regional emergency medical service (EMS) areas. This trial evaluated outcome and profiled demographic predictors of cardiac arrest patients refractory to defibrillation with intravenous access who underwent standard advanced cardia

Successful use of methylene blue following out of hospital cardiac arrest: a case report.

BACKGROUND: Cardiac arrest and cardiogenic shock are medical emergencies with poor survival rates despite improvements in advanced life support and resuscitation techniques. CASE SUMMARY: We present the case of a 44-year-old woman with ischaemic cardiomyopathy who suffered a prolonged out-of-hospital cardiac arrest due to ventricular tachycardia. Despite high-dose catecholamines and vasopressin, the patient had persistent hypotension and elevated vasoactive-inotropic requirements. Haemodynamic monitoring revealed features consistent with post-arrest vasoplegia. Two low-dose boluses of methyle

Improving end-of-life care in the cardiac intensive care unit: Navigating the challenge of timing palliative care intervention.

BACKGROUND: Integrating end-of-life (EoL) care in cardiac intensive care units (CICUs) is particularly complex because it requires a shift from a purely curative approach to one that emphasizes symptom management, emotional and spiritual support and patient-centered care. Moreover, this transition is challenging due to the need to balance life-sustaining treatments with the goals of comfort and dignity. The concept of EoL care varies across countries and is influenced by cultural, ethical and legal factors. METHODS: This narrative review examines palliative and EoL care in critically ill card

Ventricular fibrillation due to overdose of loperamide, the "poor man's methadone".

Loperamide is an over-the-counter antidiarrheal agent that is considered by many patients to be safe, but has been used as a drug of abuse due to its opioid properties. However, cardiotoxicity has been reported, prompting the FDA to release a warning regarding the arrhythmogenic potential of loperamide. We present a case of a 38-year-old female presenting with cardiac arrest thought to be secondary to abuse of the loperamide that she was using to alleviate the heroin withdrawal symptoms. Cardiac ischemia and other drug toxicities were ruled out. Loperamide induces QTc prolongation and cardiac

Intra-arrest formation of right-heart thrombi -- a case illustrated by real-time ultrasonography.

The presence of free-floating right-heart thrombus has been reported in a cardiac arrest patient in the periarrest period. Free-floating right atrial thrombus is a rare phenomenon seen in patients developing severe pulmonary embolism, and is associated with increased mortality. However, there have been no previously reported cases of right-heart thrombus formation during a resuscitated cardiac arrest. We present the pre-hospital case of a woman in the clinical setting of cardiogenic shock due to acute myocardial infarction who developed asystolic cardiac arrest on the scene. Recent implementat

Predictors of survival and hospital outcome of prehospital cardiac arrest in southern Taiwan.

BACKGROUND AND PURPOSE: Despite recent improvements in emergency care medicine, outcome for prehospital cardiac arrest patients remains poor in southern Taiwan due to lack of training and authorization of emergency medical technicians to perform advanced life support. The purpose of this study was to analyze the characteristics of these patients and to identify possible predictive factors for final hospital discharge. METHODS: We retrospectively reviewed the characteristics of 361 prehospital cardiac arrest patients (male:female, 226:135; median age, 69 years) undergoing cardiopulmonary resus

ICU nurses' perceptions of potential constraints and anticipated support to practice defibrillation: a qualitative study.

AIM: The study examines the experience of intensive care nurses in caring for patients in cardiac arrest, and their perceptions of introducing nurse-led defibrillation. METHOD: This was a descriptive, exploratory and qualitative study at an intensive care unit (ICU) of an acute regional hospital in Hong Kong. Twelve registered nurses were purposefully selected for interview. RESULTS: Although all the participants were trained in basic life support, only 50% were trained in advanced cardiac life support (ACLS), and those trained in ACLS described having limited opportunities to apply their de

Imaging in patients after cardiac transplantation and in patients with ventricular assist devices.

The field of cardiac imaging and the management of patients with severe heart failure have advanced substantially during the past 10 years. Cardiac transplantation offers the best long-term survival with high quality of life for the patients with end stage heart failure. However, acute cardiac rejection and cardiac allograft vasculopathy (CAV) can occur post cardiac transplantation and these problems necessitate regular surveillance. The short-term success of mechanical circulatory support devices (MCSD), such as ventricular assist devices (VADs), in improving survival and quality of life has

From Continuous-Flow Mechanical Circulatory Support to Heart Transplantation: Hemodynamic, Immunometabolic, and Body Composition Determinants of Rehabilitation Outcomes.

Background: Continuous-flow left ventricular assist devices (LVADs) and heart transplantation (HTX) improve survival and quality of life in advanced heart failure. However, restoration of central hemodynamics does not consistently normalize exercise capacity, physical performance, or body composition. Persistent skeletal muscle dysfunction, endothelial abnormalities, metabolic disturbances, and adverse body composition changes frequently limit functional recovery. Methods: This narrative review examines determinants of rehabilitation outcomes across the transition from advanced heart failure t

Survival After Out-of-Hospital Cardiac Arrest and 85-Minute Resuscitation.

Refractory ventricular fibrillation (VF) during out-of-hospital cardiac arrest is associated with high mortality, particularly when resuscitation is prolonged. Early cardiopulmonary resuscitation (CPR), rapid defibrillation, and comprehensive post-return of spontaneous circulation (ROSC) care are critical determinants of survival. A 46-year-old previously healthy man collapsed during exercise. Immediate bystander CPR was initiated. Emergency medical services identified VF as the initial rhythm and delivered multiple defibrillation shocks along with antiarrhythmic therapy. On hospital arrival,
