Direct evidence on the impact of air pollution on preterm birth (PTB) among advanced maternal age (AMA) primiparas is limited. This study examined the association between air pollution and PTB among 6,295 AMA primiparas in rural Henan Province. Logistic regression and distributed lag nonlinear models showed that prenatal exposure to carbon monoxide (CO), sulfur dioxide (SO2), nitrogen dioxide (NO2), and fine particulate matter (PM2.5) was associated with an increased PTB risk among AMA primiparas, while ozone (O3) exposure was inversely related to PTB risk (all P < 0.05). Moreover, the vulnera

The birth rates among women of advanced maternal age (AMA) have risen over the last two decades; yet, pregnancies with AMA are considered high-risk and are associated with a significant increase in pregnancy complications. Although the mechanisms leading to pregnancy complications in women with AMA are not fully understood, it has been well established in the literature that offspring exposed to unfavorable environmental conditions in utero, such as gestational diabetes, preeclampsia, and/or intrauterine growth restriction during the early stages of development are subject to long-term health

Women of advanced maternal age have a higher risk of having a child affected by a chromosomal disorder than younger childbearing women and are frequently offered invasive testing during pregnancy. The aim of our systematic review was to identify and analyse the current evidence base regarding factors that influence the uptake of invasive fetal testing by pregnant women of advanced maternal age. We conducted a systematic review. A search of The Cochrane Library, CINAHL, Embase and Medline databases was undertaken for papers published in English and Italian from January 2002 to May 2012. BACKGROUND: The increasing prevalence of advanced maternal age (AMA) is significantly associated with a higher risk of diverse pregnancy-related complications, posing a challenge to maternal and fetal health. However, personalized risk assessment tools specifically designed for this high-risk population remain limited. This study aimed to develop and validate a multi-factor nomogram to predict composite pregnancy complications in AMA women, facilitating early clinical intervention.
OBJECTIVE: To construct and validate a predictive nomogram integrating metabolic, immune-nutritional, and lifest

INTRODUCTION: Women with advanced maternal age (AMA) face a higher risk of pregnancy complications including preeclampsia, fetal growth restriction, and preterm birth. While placental dysfunction is implicated, the underlying mechanisms remain unclear. This study employs high-throughput sequencing-based transcriptomics to investigate AMA-associated dysregulation in placental angiogenesis, exploring links to redox imbalance. Our goal is to establish mechanistic and functional links between altered gene expression and perinatal complications.
METHODS: Placental pathology from 129 cases was anal

BACKGROUND: Infertility affects one in six couples worldwide, with advanced maternal age (AMA) posing unique challenges due to diminished ovarian reserve and reduced oocyte quality. Single vitrified-warmed blastocyst transfer (SVBT) has shown promise in assisted reproductive technology (ART), but success rates in AMA patients remain suboptimal. This study aimed to identify and refine predictive factors for live birth following SVBT in AMA patients, with the goal of enhancing clinical decision-making and enabling personalized treatment strategies.
METHODS: This retrospective cohort study analy

Advanced maternal age is a risk factor for female infertility, and placental dysfunction is considered one of the causes of pregnancy complications. We investigated the effects of advanced maternal aging on pregnancy outcomes and placental senescence. Female pregnant mice were separated into three groups: young (3 months old), middle (8-9 months old), and aged (11-13 months old). Although the body weights of young and middle dams gradually increased during pregnancy, the body weight of aged dams only increased slightly. The placental weight and resorption rate were significantly higher, and li

OBJECTIVE: to assess complications of very advanced maternal age (VAMA) pregnancies ≥ 45 years with and without egg donation (ED).
STUDY DESIGN: obstetric and neonatal complications were studied in 20,659 singleton pregnancies according to three maternal age groups: 20-39, 40-44 [advanced maternal age (AMA)] and ≥ 45 years (VAMA). Twenty pregnancies within the AMA/LAMA group that were achieved with ED were compared with age-matched controls.
RESULTS: AMA mothers were more likely to have higher rates of preterm deliveries (OR 1.25), cesarean sections (OR 1.84) hypertension (OR 1.71) and diabe

Advanced maternal age (AMA) is a major challenge in reproductive medicine, compounded by coexisting conditions such as adenomyosis and endometriosis, which impair oocyte competence and endometrial receptivity. Prolonged gonadotropin-releasing hormone agonist (GnRHa) protocols have been proposed to improve in vitro fertilization-embryo transfer (IVF-ET) outcomes. We report the case of a 41-year-old Indonesian woman with 13 years of primary infertility, AMA, adenomyosis, endometriosis, and male factor infertility. Following laparoscopic excision of endometriosis, she underwent a prolonged GnRH a

PURPOSE: To assess the impact of maternal age on the association between maternal basal FSH and aneuploidy.
METHODS: A retrospective study including data from 1749 blastocysts diagnosed as euploid or aneuploid by PGT-A (preimplantation genetic testing for aneuploidy). Aneuploidy incidence was compared between embryos from mothers with high vs. low basal FSH levels (above and below the group median, respectively) in total, pre-AMA (advanced maternal age; < 35 years, 198 embryos) and AMA (≥ 35 years, 1551 embryos) patient groups, separately. OBJECTIVES: Gestational diabetes (GDM) is a common complication during pregnancy that is strongly associated with adverse fetal and maternal outcomes. Advanced maternal age (≥35 years) is a known risk factor for GDM. Studies advocate that GDM comprises distinctive metabolic entities, suggesting an individualized approach based on early pregnancy characteristics (such as 75 g oGTT values, maternal age, obstetric history).
METHODS: The oGTT blood glucose levels of 1,664 women were categorized into isolated fasting hyperglycemia (GDM-IFH), isolated postprandial hyperglycemia (GDM-IPH) and combin
