Overlapping type 2, barrier, and remodeling endotypes shape clinical expression in eosinophilic esophagitis.

Eosinophilic esophagitis (EoE) is a chronic, immune-mediated disease of the esophagus in which symptoms, histologic activity, endoscopic severity, epithelial dysfunction, and fibrostenotic remodeling frequently diverge. Peak eosinophil density remains central to diagnosis and response assessment, but it represents only one component of a broader epithelial-immune and structural process. This review integrates epithelial alarmins, local type 2 immunity, IL-13-driven epithelial programming, eosinophil-mast-cell effector activity, and stromal remodeling within an endotype-to-phenotype framework.

Immunological Mechanisms Underlying Allergy Predisposition After SARS-CoV-2 Infection in Children.

As the pediatric COVID-19 landscape evolves, it is essential to evaluate whether SARS-CoV-2 infection predisposes children to allergic disorders. This narrative review synthesizes current epidemiological and immunological evidence linking pediatric COVID-19 with new-onset atopy. Epidemiological data remain heterogeneous: large Korean and multinational cohorts report increased risks of asthma and allergic rhinitis following COVID-19, whereas U.S. cohorts show neutral or protective associations, highlighting geographic and methodological variability. Mechanistic insights provide biological plaus

Necrotic Change of Tunica Media Plays a Key Role in the Development of Coronary Artery Lesions in Kawasaki Disease.

BACKGROUND: Alarmins resulting from cell death or oxidative stress are involved in the development of Kawasaki disease (KD) vasculitis. In a previous study, we demonstrated the potential role of interleukin (IL)-33 as an alarmin in the development of KD vasculitis. Although edematous dissociation (necrotic change) of the tunica media is thought to be a major source of IL-33 in KD vasculitis, it has not yet been elucidated. METHODS AND RESULTS: We investigated the impact of IL-33 released from necrotic human coronary artery smooth muscle cells (HCASMCs) on human coronary artery endothelial cel

Genetic variation in the TSLP promoter is associated with Hashimoto's thyroiditis comorbidity in patients with chronic spontaneous urticaria.

BACKGROUND: Chronic spontaneous urticaria (CSU) is a systemic immune-mediated disorder frequently comorbid with autoimmune thyroid disease, particularly Hashimoto's thyroiditis (HT). Emerging evidence implicates shared immunogenetic pathways, yet the role of thymic stromal lymphopoietin (TSLP), a key epithelial alarmin in this comorbidity remains unclear. OBJECTIVE: To investigate the association between TSLP SNPs and the co-occurrence of CSU and HT. METHODS: We enrolled 45 patients with CSU alone and 41 with CSU and concomitant HT from a reference center in the Colombian Caribbean. Diagnosi

Inhibition of the NLRP3 inflammasome can prevent sterile intra-amniotic inflammation, preterm labor/birth, and adverse neonatal outcomes†.

Sterile intra-amniotic inflammation is commonly observed in patients with spontaneous preterm labor, a syndrome that commonly precedes preterm birth, the leading cause of perinatal morbidity and mortality worldwide. However, the mechanisms leading to sterile intra-amniotic inflammation are poorly understood and no treatment exists for this clinical condition. Herein, we investigated whether the alarmin S100B could induce sterile intra-amniotic inflammation by activating the NLRP3 inflammasome, and whether the inhibition of this pathway could prevent preterm labor/birth and adverse neonatal out

Endogenous IL-33 Accelerates Metacestode Growth during Late-Stage Alveolar Echinococcosis.

During the course of the infectious disease alveolar echinococcosis (AE), the larval stage of Echinococcus multilocularis develops in the liver, where an initial Th1/Th17 immune response may allow its elimination in resistant individuals. In patients susceptible to infection and disease, the Th2 response initiates later, inducing tolerance to the parasite. The role of interleukin 33 (IL-33), an alarmin released during necrosis and known to drive a Th2 immune response, has not yet been described during AE. Wild-type (WT) and IL-33-/- C57BL/6J mice were infected by peritoneal inoculation with E.

Neutrophil-Leishmania infantum Interaction Induces Neutrophil Extracellular Traps, DAMPs, and Inflammatory Molecule Release.

Neutrophils, the first cells to arrive at infection sites, release neutrophil extracellular traps (NETs) comprising nuclear and/or mitochondrial DNA webs decorated with proteins. Similar to other parasites, Leishmania infantum induces NET extrusion. However, our understanding of NET formation and neutrophil fate after NET release in a Leishmania infection context is limited. Our study aimed to determine the DNA origin of the NET scaffolds released by human neutrophils in response to chemical or L. infantum stimulation. Neutrophils were incubated with PMA, PHA, LPS, or L. infantum, followed by

When Our Best Friend Becomes Our Worst Enemy: The Mitochondrion in Trauma, Surgery, and Critical Illness.

Common for major surgery, multitrauma, sepsis, and critical illness, is a whole-body inflammation. Tissue injury is able to trigger a generalized inflammatory reaction. Cell death causes release of endogenous structures termed damage associated molecular patterns (DAMPs) that initiate a sterile inflammation. Mitochondria are evolutionary endosymbionts originating from bacteria, containing molecular patterns similar to bacteria. These molecular patterns are termed mitochondrial DAMPs (mDAMPs). Mitochondrial debris released into the extracellular space or into the circulation is immunogenic and

PAMPs and DAMPs in Sepsis: A Review of Their Molecular Features and Potential Clinical Implications.

Sepsis is a serious organ dysfunction caused by a dysregulated immune host reaction to a pathogen. The innate immunity is programmed to react immediately to conserved molecules, released by the pathogens (PAMPs), and the host (DAMPs). We aimed to review the molecular mechanisms of the early phases of sepsis, focusing on PAMPs, DAMPs, and their related pathways, to identify potential biomarkers. We included studies published in English and searched on PubMed® and Cochrane®. After a detailed discussion on the actual knowledge of PAMPs/DAMPs, we analyzed their role in the different organs affecte

Allergic Aspects of IgG4-Related Disease: Implications for Pathogenesis and Therapy.

IgG4-related disease (IgG4-RD) is a rare systemic fibroinflammatory disease frequently associated with allergy. The pathogenesis of IgG4-RD is poorly understood, and effective therapies are limited. However, IgG4-RD appears to involve some of the same pathogenic mechanisms observed in allergic disease, such as T helper 2 (Th2) and regulatory T cell (Treg) activation, IgG4 and IgE hypersecretion, and blood/tissue eosinophilia. In addition, IgG4-RD tissue fibrosis appears to involve activation of basophils and mast cells and their release of alarmins and cytokines. In this article, we review all

Tackling Sepsis Using Host Defense Peptides That Reduce Proinflammatory Cytokine Production.

Sepsis is defined as a life-threatening condition that arises when the body's dysregulated inflammatory response to infection causes injury to its own tissues and organs. Early and late mediators of sepsis are lipopolysaccharide (LPS) from bacteria and the alarmin high mobility group box 1 (HMGB1), respectively. In this Perspective, the role of host defense peptides (HDPs) in reducing proinflammatory cytokines tied to sepsis is presented. The multimodal bactericidal and immunomodulatory activities of HDPs imbue these molecules with tremendous therapeutic potential to tackle the leading cause o

Using Biologics in the Prevention of Anaphylaxis.

PURPOSE OF REVIEW: Until recently, the management of anaphylaxis has been dependent on the avoidance of known triggers combined with the emergent treatment of immediate and, possibly, fatal hypersensitivity reactions. The approval of omalizumab for the prevention of food-induced anaphylaxis heralds a new era for the use of biologics to curtail anaphylactic reactions in susceptible individuals. RECENT FINDINGS: As more knowledge is gleaned about the immunologic mechanisms of anaphylaxis, the application of biologics for prophylaxis of anaphylaxis has accelerated. While omalizumab is currently

Microglia and astrocyte responses to neuropathogenic protozoan parasites.

Cerebral toxoplasmosis and cerebral malaria are two important neurological diseases caused by protozoan parasites. In this review, we discuss recent findings regarding the innate immune responses of microglia and astrocytes to Toxoplasma and Plasmodium infection. In both infections, these tissue-resident glial cells perform a sentinel function mediated by alarmin crosstalk that licenses adaptive type 1 immunity in the central nervous system. Divergent protective or pathogenic effects of type 1 activation of these astrocytes and microglia are revealed depending on the inherent lytic potential o

The Role of Damage-Associated Molecular Patterns in Perioperative Neurocognitive Disorders: A Narrative Review.

Perioperative neurocognitive disorders (PND), such as postoperative delirium (POD) and cognitive dysfunction (POCD), frequently affect older surgical patients and significantly impair postoperative quality of life. During surgical procedures, damage-associated molecular patterns (DAMPs) such as high-mobility group box 1 (HMGB1), mitochondrial DNA, and S100 proteins are released from injured cells and implicated in the pathogenesis of PND. These molecules activate innate immune pathways through pattern recognition receptors (PRRs) such as toll-like receptors (TLRs) and receptors for advanced gl

Targeting DAMPs with nucleic acid scavengers to treat lupus.

Systemic lupus erythematosus (SLE) is a chronic and often progressive autoimmune disorder marked clinically by a variable constellation of symptoms including fatigue, rash, joint pains, and kidney damage. The lungs, heart, gastrointestinal system, and brain can also be impacted, and individuals with lupus are at higher risk for atherosclerosis, thrombosis, thyroid disease, and other disorders associated with chronic inflammation . Autoimmune diseases are marked by erroneous immune responses in which the target of the immune response is a "self"-antigen, or autoantigen, driven by the developmen

Benzo[a]pyrene exacerbates allergen-induced airway inflammation through NLRP3-dependent dendritic cell activation and pathogenic T helper cell polarization.

BACKGROUND: Environmental pollutants are known to aggravate allergic diseases, but the molecular mechanisms by which polycyclic aromatic hydrocarbons such as benzo[a]pyrene (BaP) potentiate allergic airway inflammation remain poorly understood. OBJECTIVE: We investigated how BaP co-exposure modifies house dust mite (HDM)-driven allergic airway responses, focusing on the role of the NLRP3 inflammasome in dendritic cells (DCs). METHODS: Mice were sensitized and challenged intranasally with HDM with or without BaP. Airway hyperresponsiveness (AHR), bronchoalveolar lavage (BAL) cell counts, lung

Current knowledge on biomarkers for contact sensitization and allergic contact dermatitis.

Contact sensitization is common and affects up to 20% of the general population. The clinical manifestation of contact sensitization is allergic contact dermatitis. This is a clinical expression that is sometimes difficult to distinguish from other types of dermatitis, for example irritant and atopic dermatitis. Several studies have examined the pathogenesis and severity of allergic contact dermatitis by measuring the absence or presence of various biomarkers. In this review, we provide a non-systematic overview of biomarkers that have been studied in allergic contact dermatitis. These include

Pulmonary neuroendocrine cells in pediatric asthma and bronchopulmonary dysplasia: neuroimmune-developmental mechanisms and translational implications.

Pediatric asthma, bronchopulmonary dysplasia (BPD), and viral-induced wheeze arise from distinct clinical settings, but all occur in the context of a maturing pediatric lung, where epithelial injury, inflammation, neural inputs, and repair or remodeling programs are closely interconnected. This intersection between pediatric lung maturation and inflammatory injury provides a rationale for focusing on pulmonary neuroendocrine cells (PNECs), developmentally enriched airway epithelial sensor cells that translate environmental and injury-related cues into local neuroimmune, repair, and remodeling

NOD2 Agonism Counter-Regulates Human Type 2 T Cell Functions in Peripheral Blood Mononuclear Cell Cultures: Implications for Atopic Dermatitis.

Atopic dermatitis (AD) is known as a skin disease; however, T cell immunopathology found in blood is associated with its severity. Skin Staphylococcus aureus (S. aureus) and associated host-pathogen dynamics are important to chronic T helper 2 (Th2)-dominated inflammation in AD, yet they remain poorly understood. This study sought to investigate the effects of S. aureus-derived molecules and skin alarmins on human peripheral blood mononuclear cells, specifically testing Th2-type cells, cytokines, and chemokines known to be associated with AD. We first show that six significantly elevated Th2-r
