[Effect of local anesthetics on ciliary activity of the human tracheobronchial mucosa (studies in vitro)].

The local cellular toxicity of the local anaesthetics Tetracaine, Propipocain and Lidocaine were investigated in vitro using human tracheal and bronchial epithelium. The ciliary beat of the respiratory epithelium served as an index of the cellular activity. After bronchoscopic explanting the respiratory epithelium was incubated at 37 degrees C in tissue culture medium. The special local anaesthetic drug was added in the selected concentration. The evaluation of ciliary activity was done by light microscope. All local anesthetics tested showed local cytotoxic properties depending on their conce

Treatment of backache with spinal injections of local anesthetics, spinal and systemic steroids. A review.

Low back pain and sciatica have been treated with peridural local anesthetics for over 80 years and with epidural and subarachnoid steroid injections for a quarter of a century. This review surveyed the literature concerning the evolution, pathophysiology, complications and results of this type of therapy. The volume injected and the method used vary with different physicians and no standard has been established. Good results from this treatment which vary from 20 to 95% decrease on long-term follow up. Statistical significance is absent if compared with other forms of therapy. It is the autho

The effects of epidural injection of local anesthetics and corticosteroids on patients with lumbosciatic pain.

Although epidural cortisone injections are commonly used for treatment of lumbosciatic pain, insufficient critical analysis of the end result can be found in the literature. The present study is a retrospective critical analysis of 367 patients with leg pain who were engaged for a minimum of two weeks or an average of two months in multifaceted conservative management without relief of pain. Injections of 10 cm3 of 0.5% bupivacaine and 100 mg of methylprednisolone were given to inpatients treated by the same anesthesiologist. The average follow-up period was 21.4 months (range, 6-36 months). R

A Comparative Analysis of the Efficacy of Local Anesthetics and Systemic Anesthetics in the Red-Headed Versus Non-Red-Headed Patient Population: A Comprehensive Review.

Researchers have found that individuals with red hair often require higher doses of anesthetic medications to achieve the same level of pain relief or sedation compared to people with other hair colors. This review investigates the effects of local and systemic anesthetics in individuals with red hair compared to the general population. Focusing on both local and systemic anesthesia, this research aims to elucidate any distinctive responses or complications among the red-haired demographic. Utilizing a systematic review approach, we analyzed a wide array of previous research papers published o

Stability of the pH of local anesthetics after bicarbonation: an electrochemical assay of clinical mixtures.

BACKGROUND: Raising the pH of local anesthetics shortens their time to onset, and this is commonly done in clinical practice via addition of sodium bicarbonate. A prior study found that the pH of local anesthetics decreases over time after a factory-sealed vial has been opened, presumably due to hydrolysis of atmospheric carbon dioxide. We hypothesized that the alkalinization effect achieved by adding bicarbonate to local anesthetics may similarly be unstable, potentially affecting the drug's time-to-onset if not used immediately. METHODS: We prepared solutions of chloroprocaine 3% and lidoca

Health risks of cocaine adulteration: local anesthetics as modulators of monoamine and organic cation transporters.

INTRODUCTION: Local anesthetics (LAs) are frequently used as adulterants in cocaine sold on the illicit market, sometimes in higher quantities than cocaine itself. These agents can mimic cocaine's anesthetic effect, masking the products reduced purity. While reports suggest that LAs influence monoaminergic neurotransmission, systematic evidence remains limited. We examined three LAs commonly detected in cocaine samples submitted for drug checking from Vienna, Austriaprocaine, benzocaine, and lidocainefor their activity on uptake-1 monoamine transporters (DAT, NET, SERT) and uptake-2 organic ca

-Effects of Local Anesthetics on Smooth Muscle Tissue in Rat Trachea: An In Vitro Study.

OBJECTIVES: We evaluated the muscle responses of rat trachea to LA drugs, such as lidocaine and prilocaine, in terms of airway spasms. MATERIALS AND METHODS: A total of 16 male rats were used. After ketamine anesthesia, the tracheal ring of each rat was removed and placed in the organ bath in the Krebs solution. The rat tracheal veins were randomly divided into two groups based on the LA applied at the basal tonus level: group 1 (n=8), lidocaine; group 2 (n=8), prilocaine. Second, the baths were washed. Supramaximal contraction was obtained by applying acetylcholine to the tracheal rings (n=1

Amide-linked local anesthetics preferentially target leukemia stem cell through inhibition of Wnt/β-catenin.

The anti-cancer activities of amide-linked local anesthetics have been demonstrated in various types of bulky/differentiated cancer cells. However, whether these anesthetics also affect biological functions of cancer stem cells is largely unknown. In this study, we systematically investigated the effects of three commonly used amide-linked local anesthetics (ropivacaine, lidocaine and bupivacaine) on leukemia stem cell (LSC) derived from two different leukemia diseases (acute myeloid leukemia, n = 8 and chronic myeloid leukemia, n = 8) as well as normal hematopoietic stem cell (HSC) derived fr

Myotoxicity of local anesthetics is equivalent in individuals with and without predisposition to malignant hyperthermia.

PURPOSE: Malignant hyperthermia (MH) is an inherited muscle disorder caused by abnormal elevations of intracellular calcium (Ca2+) in skeletal muscle. There are several reports of myotoxicity caused by local anesthetics, and the increased intracellular Ca2+ is considered to be an important cause. However, there is insufficient evidence regarding myotoxicity in MH-susceptible individuals when large doses of local anesthetics are administered. This study investigated the effect of MH predisposition on myotoxicity. METHODS: Human skeletal muscle samples were obtained from 22 individuals to deter

Local Anesthetics and Antipsychotic Phenothiazines Interact Nonspecifically with Membranes and Inhibit Hexose Transporters in Yeast.

Action mechanisms of anesthetics remain unclear because of difficulty in explaining how structurally different anesthetics cause similar effects. In Saccharomyces cerevisiae, local anesthetics and antipsychotic phenothiazines induced responses similar to those caused by glucose starvation, and they eventually inhibited cell growth. These drugs inhibited glucose uptake, but additional glucose conferred resistance to their effects; hence, the primary action of the drugs is to cause glucose starvation. In hxt(0) strains with all hexose transporter (HXT) genes deleted, a strain harboring a single

Structural analysis of compounds with actions similar to local anesthetics and antipsychotic phenothiazines in yeast.

Local anesthetics and antipsychotic phenothiazines cause a rapid shutdown of both actin polarization and translation initiation in yeast cells, like some environmental stresses. These compounds all have an amphiphilic structure, surfactant activity and the ability to lyse yeast cells. To elucidate the structures responsible for the shutdown activity and cell lysis, we investigated a variety of amphiphiles. In the hydrophobic region, the straight alkyl structure was sufficient for the shutdown of actin polarization and translational initiation. In the hydrophilic region of the straight alkyl co

Allergic contact dermatitis from EMLA cream: concomitant sensitization to both local anesthetics lidocaine and prilocaine.

Local anesthetics are widely used drugs. In contrast to the local anesthetics of the ester group, the ones of the amide group (for example prilocaine and lidocaine) are considered to be rare sensitizers. Positive patch test results to both prilocaine and lidocaine in EMLA cream might indicate potential cross-reactivity.

[Comparative efficiency of local anesthetics from the group of complex amides during therapeutic stomatological interventions].

The efficiency of infiltration, conduction, and intraligamental anesthesia with different drugs for dental interventions was evaluated in 331 patients without concomitant somatic diseases. The efficiency of local anesthesia depends primarily on the drug; other essential factors are the route of this drug administration and type of intervention in the oral cavity. The most effective of the studied anesthetics was 4% artisane with epinephrine 1:100,000; 3% prolocaine with epinephrine 1:100,000 ranked second. These anesthetics maximally realized their effects at intraligamental injection during t

[Diphenhydramine is useful in a parturient with hypersensitivity to local anesthetics to manage her delivery].

We did an obstetric management of a parturient, aged 32 years, with a history of local anesthetic hypersensitivity. The results of histamine release test, and cellular antigen stimulation test with lidocaine, mepivacaine, bupivacaine and preservative-free lidocaine during her pregnancy showed that all local anesthetics studied were positive. We used diphenhydramine as an alternative local anesthetic for episiotomy to the parturient during the delivery. Infiltration with diphenhydramine provided adequate analgesia without noticeable adverse reaction. One month after the delivery, we performed p

Management of patients at risk for adverse reactions to local anesthetics: analysis of 386 cases.

Local anesthetics are among the most used drugs in clinical practice. Various types of reactions have been reported and many patients are told that they are allergic to caines or are at risk of allergic reactions. We report our experience with 386 patients using skin test and progressive subcutaneous challenge in order to select a local anesthetic that could be safely used. The patients underwent specific protocol including clinical history, prick and intradermal tests, and subcutaneous incremental challenge with a local anesthetic from the amide group which was free of preservatives and vasoc

Induction by psychotropic drugs and local anesthetics of DnaK and GroEL proteins in Escherichia coli.

We examined effects of psychotropic drugs and local anesthetics on the synthesis of heat shock proteins in Escherichia coli. Chlorpromazine, a phenothiazine derivative, was shown to induce DnaK and GroEL proteins, major heat shock proteins in E. coli. The inductions of these proteins were not observed in an rpoH (= htpR) amber mutant strain, indicating that the heat shock sigma factor sigma 32 was required for their inductions. Northern blot hybridization analysis revealed that chlorpromazine induced increases of messenger RNAs for the DnaK and GroEL proteins. Thus, the induction occurred at t
