Cor triatriatum: report of an unusual variant--triatrial heart. A case report.

Cor triatriatum is a rare cardiac malformation presenting with symptoms and signs of pulmonary venous obstruction. Although diagnosis of this defect has been facilitated by diagnostic procedures such as echocardiography and angiocardiography, it could be missed preoperatively. The authors report a case of cor triatriatum that is unique in having two separate atrial septal defects in each side of the fibromuscular membrane and that was undiagnosed preoperatively with transthoracic echocardiography and cardiac catheterization.

Familial skeletal cardiovascular syndrome (Holt-Oram) in a polygamous African family.

A polygamous Nigerian family with inherited skeletal cardiovascular anomalies (Holt-Oram syndrome) is described. Common to the affected individuals in this family were pectus excavatum and abnormalities of the thumbs. Atrial septal defect was proven by angiocardiography in one and clinically diagnosed in another member of the family. The mode of inheritance of the abnormalities in this family as in other reported cases is thought to be an autosomal dominant type which is unaffected by the polygamous situation.

Pulmonary atresia with double ductus arteriosus.

We present the case of a neonate with pulmonary atresia and persistent bilateral patent ductus arteriosus imaged by gated multidetector CT. Traditionally, these patients have been assessed preoperatively with invasive angiocardiography or with cardiovascular magnetic resonance under sedation. Our case illustrates that contemporary cardiovascular CT techniques can now be used for preoperative evaluation with minimal radiation penalty, obviating the risks of sedation or cardiac catheterization.

Infectious endocarditis caused by gas-producing Escherichia coli in a diabetic dog.

A 10-year-old, female West Highland white terrier was presented with poorly controlled diabetes mellitus and a previously undetected heart murmur. Emphysematous cystitis, emphysematous peritonitis and infective endocarditis of the tricuspid valve with gas accumulation were diagnosed with radiographs, including non-selective angiocardiography. The diagnoses were confirmed by post-mortem examination and positive cultures for Escherichia coli in blood, urine and tricuspid valve tissue samples.

[A severe respiratory distress in a new born infant with double aortic arch without cardiac anomalies].

A 39-day-old girl underwent operation for double aortic arch. She required endotracheal intubation at 23-day-old because of severe respiratory distress. Echocardiography, esophagography and angiocardiography revealed double aortic arch without other cardiac anomalies. At the operation the lesser arch and ductus arteriosus were divided. Soon after the operation her respiratory distress disappeared and fiberbronchoscopic examination showed rapid recovery of the deformed trachea.

Systolic prolapse of the mitral valve in Noonan's syndrome.

A thirty-five-year-old woman with Noonan's syndrome (Turner phenotype with normal chromosome pattern) had mitral valve prolapse and mitral insufficiency associated with the auscultatory findings of a midsystolic click and late systolic murmur. Selective left-ventricular angiocardiography also showed eccentric hypertrophy of the left ventricle. To our knowledge, this is the first reported instance of mitral valve prolapse occurring in association with Noonan's syndrome.

[Lipomyxosarcoma of the pulmonary veins extending into the left atrium. Repetitive surgical treatment].

The case reported here of a 54-year old woman with lipomyxosarcoma of the pulmonary veins successfully excised is the first in the literature. The initial symptoms were febrile left ventricular failure with pulmonary oedema and haemoptysis. The diagnosis was made by angiocardiography. The tumour was excised in two stages: cardiac first, under cardiopulmonary bypass, then thoracic with left pneumonectomy. Two years after surgery, the patient is in good condition without chemotherapy.

Congenital pulmonary arteriovenous fistula producing pulmonary arterial steal syndrome.

This report describes a congenital pulmonary arterial steal syndrome manifested as cyanosis and acidosis in a newborn. A fistulous connection between the right pulmonary artery and a large, anomalous right common pulmonary vein stole blood from the pulmonary arteries. The anomaly was suspected because of a pericardiac shadow on frontal and lateral chest films, substantiated by M-mode echocardiogram, confirmed at cardiac catheterization with angiocardiography, and analyzed at postmortem examination.

[Normal values of the growth of the aorta in children. An angiography study].

Angiocardiography is a standard procedure for quantification and evaluation of congenital heart disease. Knowledge of the diameters at the level of aortic valve ring, ascending and descending aorta can contribute to the decision in repair of congenital heart disease. Moreover, they are helpful for postoperative follow-up studies. In order to establish normal standards, analysis was performed on 51 cineangiograms obtained from infants, children and adolescents with no evidence of heart disease (n = 16) or only hemodynamically irrelevant cardiovascular malformations (n = 35). The individual diag

Cor triatriatum dextrum--an unusual variant of triatrial heart.

The present report describes a rare case of triatrial heart, cor triatriatum dextrum in a 31-year-old woman in whom the anomaly was incidentally detected by radionuclide angiocardiography. Subsequent assessment with two-dimensional echocardiography and right heart catheterization confirmed the presence of a membrane subdividing the right atrium into two distinct chambers. To the best of our knowledge, this is the only case in which this cardiac anomaly was demonstrated by radionuclide technique.

Stenosis of pulmonary veins with total left anomalous pulmonary venous return.

A case of congenital stenosis of the pulmonary veins and total left anomalous pulmonary venous return is reported. There was also obstruction between the left pulmonary veins and left vena cava superior. The diagnosis was based on 2-dimensional and Doppler echocardiography and was confirmed by angiocardiography. The 6-month-old baby with these anomalies was operated upon successfully. These cardiac malformations are very rare separately and their combination has not been previously reported.

[Ebstein's anomaly associated with pulmonary stenosis].

Although Ebstein's anomaly occurs mostly in isolation, in 2 to 5% of cases it is combined with pulmonary artery stenosis. This combination noticeably aggravates the natural course of the disease and complicates the diagnosis of the pathology. On the basis of five observations the authors present the specific differential-diagnostic signs which help to make the correct diagnosis. The key role in the identification of combined forms of Ebstein's anomaly is played by the catheterization of the cardiac chambers and by angiocardiography.

Anomalous drainage of the right superior vena cava into the left atrium.

A 22 year old man with asymptomatic hypoxemia was found to have a large right to left shunt due to a rare congenital anomaly: total drainage of the right superior vena cava into the left atrium. The anomaly was first suspected after radionuclide angiocardiography was performed using technetium-99m macroaggregated albumin and was confirmed by cardiac catheterization. Contrast echocardiographic and surgical findings are discussed. Other reports on this anomaly are reviewed.

Aortic orgin of the left pulmonary artery. Variant of tetralogy of Fallot.

A case of tetralogy of Fallot with aortic origin of the left pulmonary artery is presented. This rare malformation should be suspected in a child with stunted growth, cyanosis, a continuous murmur over the pulmonary area and a hypervascular left lung on chest roentgenogram. Final diagnosis depends upon cardiac catheterization and angiocardiography. During surgical correction of the tetralogy, the left pulmonary artery can be divided from the aorta and anastomosed to the main pulmonary artery.

Echocardiographic assessment of juxtaposition of the right atrial appendage in children with congenital heart disease.

The objective of this study was to analyze the echocardiographic characteristics of juxtaposition of the atrial appendages and to determine its prevalence in children with congenital heart disease. From June 1998 to December 2008, 10,880 children underwent selective angiocardiography, magnetic resonance imaging (MRI), and echocardiography for evaluation of congenital heart disease. Juxtaposition of the atrial appendages was diagnosed based on the results of angiocardiography and MRI; the echocardiographic characteristics of this anomaly were analyzed retrospectively. There were 33 patients dia

[Incidence and extent of right ventricular functional disorders following the 1st myocardial infarction].

A series of 63 patients were studied by gated radionuclide angiocardiography 51/2 weeks after their first acute myocardial infarction. Left (LVEF) and right (RVEF) ventricular ejection fraction were assessed in three groups of patients: 12 patients with transmural anterior infarctions, 35 patients with transmural inferior infarctions and 16 patients with non-transmural infarctions. LVEF was depressed only in the group of patients with anterior infarctions (34.4 +/- 16.3%) but remained within normal limits in the other two groups. RVEF was reduced in 64% of patients with inferior infarctions; t

[Left ventricular-right atrial communication detected by pregnancy; report of a operative case].

A 24-year-old pregnant female was admitted to our hospital because of congestive heart failure. When she was 3 month old, she underwent closure of patent ductus arteriosus. Echocardiography, cardiac catheterization and selective angiocardiography showed a left ventricular-right atrial shunt. At the operation, we found a left ventricular right-atrial communication (group I) and repaired the defect directly. The patient was discharged and returned to her social life.

Aortopulmonary window coexisting with tetralogy of Fallot: echocardiographic diagnosis.

Aortopulmonary window coexisting with tetralogy of Fallot (TF) was prospectively diagnosed by two-dimensional (2D) echocardiography and Doppler in an 18-month-old boy; the diagnosis was confirmed by cardiac catheterization and angiocardiography. Surgical correction was performed, but the patient died in the operating room from right ventricular dysfunction. The autopsy showed an adequate surgical repair, but the histologic examination of the lungs demonstrated severe pulmonary vascular disease, which was presumed to be the cause of death.

Angiocardiographic diagnosis of ventricular septal defect localization in double outlet right ventricle.

Children with double outlet right ventricle, aged 3 months to 23.5 years, were examined to assess the possibility of angiocardiographic diagnosis of defect localization. It was found angiocardiography allows satisfactory localization of the defect in question. To be able localize the defect, it is critical to determine the interaction between the defect and the conus septum, and the origin of large arteries. To establish the diagnosis of ventricular septal defect, right and left ventriculography in standard and axial projections must be performed.

[Enlargement of the lower mediastinum: pericardiac or extracardiac origin? (author's transl)].

The radiographic findings in 9 patients who suffered from a non-myocardiac enlargement of the lower anterior mediastinum are discussed. By means of several diagnostic procedures consisting of radiography, fluoroscopy, roentgen-kymography, angiocardiography and needle-biopsy under visible control it is often possible to distinguish between primary pericardiac lesions and growths of the mediastinum lying in close contact to the heart. In most cases, however, final histological diagnosis will only be reached by thoracotomy with removement of the tumor.
