Validity of the Foot and Ankle Ability Measure in athletes with chronic ankle instability.

CONTEXT: The Foot and Ankle Ability Measure (FAAM) is a region-specific, non-disease-specific outcome instrument that possesses many of the clinimetric qualities recommended for an outcome instrument. Evidence of validity to support the use of the FAAM is available in individuals with a wide array of ankle and foot disorders. However, additional evidence to support the use of the FAAM for those with chronic ankle instability (CAI) is needed. OBJECTIVE: To provide evidence of construct validity for the FAAM based on hypothesis testing in athletes with CAI. DESIGN: Between-groups comparison.

The HINTEGRA ankle: rationale and short-term results of 122 consecutive ankles.

The HINTEGRA ankle was developed as an attempt to specifically address the needs of minimal bone resection, extended bone support, proper ligament balancing, and minimal contact stresses within and around the prosthesis. The purpose of this study was to determine the short-term results in a consecutive series of 116 patients (122 ankles). Preoperative diagnoses were posttraumatic osteoarthrosis in 91 ankles (75%), primary osteoarthrosis in 16 ankles (13%), and systemic arthritis in 15 ankles (12%). Eight ankles had to be revised. Four were revised because of loosening of at least one component

Effect of ankle-foot orthosis on active ankle moment in patients with hemiparesis.

The paper investigates the effect of dorsi/plantar rigidity and the initial angle of ankle-foot orthoses (AFOs) on the moment generated by ankle musculature (referred to as active ankle moment) during gait in patients with hemiparesis. In the early stance phase, the active ankle moment in the direction of dorsiflexion is negligible, and AFOs play an important role in supplementing weak dorsiflexion. In mid to late stance, the moment generated by AFOs is very small compared with the active ankle moment in the direction of plantarflexion. AFOs therefore play only a limited role in assisting plan

Ankle Coordination in Chronic Ankle Instability, Coper, and Control Groups in Running.

PURPOSE: Coordination and coordination variability have been used as a measure of the function and flexibility of the sensorimotor system during running. Chronic ankle instability (CAI) is associated with altered sensorimotor system function compared with individuals without CAI. Copers may have adopted protective sensorimotor adaptations to prevent repeated ankle sprains; however, their coordination strategies between the foot and shank have not been investigated. We compared joint coupling angles and coordination variability using vector coding between individuals with CAI, copers, and contr

Active Ankle Circumduction to Identify Mobility Deficits in Subacute Ankle Sprain Patients.

Assessment of ankle mobility is complex and of clinical relevance after an ankle sprain. This study develops and tests a biomechanical model to assess active ankle circumduction and its reliability. The model was then applied to compare individuals' ankle mobility between injured and noninjured ankles after a sprain episode. Twenty patients with subacute unilateral ankle sprain were assessed at 4 weeks and 10 weeks after the injury. They underwent a clinical exam and an ankle circumduction test during which the kinematics were recorded with an optoelectronic device. A biomechanical model was a

Transepiphyseal (Type VII) Ankle Fracture Versus Os Subfibulare in Pediatric Ankle Injuries.

BACKGROUND: Pediatric ankle injuries are common, giving rise to ∼17% of all physeal injuries. An os subfibulare in a child with an ankle sprain may be confused with a type VII transepiphyseal fracture. Here, we evaluate the clinical and radiographic features of type VII transepiphyseal fractures to those of os subfibulare presenting with acute ankle trauma with the hypothesis that radiographs are necessary for final diagnosis and neither clinical history nor examination would be diagnostic. METHODS: We performed an internal review board-approved, retrospective chart review of patients identif

Management of Osseous and Soft-Tissue Ankle Equinus During Total Ankle Replacement.

Obtaining functional alignment of a total ankle replacement, including physiologic sagittal plane range of motion, is paramount for a successful outcome. This article reviews the literature on techniques available for correction of osseous and soft-tissue equinus at the time of index total ankle replacement. These techniques include anterior tibiotalar joint cheilectomy, posterior superficial muscle compartment lengthening, posterior ankle capsule release, and release of the posterior portions of the medial and lateral collateral ligament complexes. The rationale for these procedures and the o

Severe periprosthetic osteolytic lesions after the Ankle Evolutive System total ankle replacement.

Between 2002 and 2008, 130 consecutive ankles were replaced with an hydroxyapatite (HA) and titanium-HA-coated Ankle Evolutive System total ankle prosthesis. Plain radiographs were analysed by two independent observers. Osteolytic lesions were classified by their size and location, with cavities > 10 mm in diameter considered to be 'marked'. CT scanning was undertaken in all patients with marked osteolysis seen on the plain radiographs. Osteolytic lesions were seen on the plain films in 48 (37%) and marked lesions in 27 (21%) ankles. The risk for osteolysis was found to be 3.1 (95% confidence

The relationship between lateral ankle sprain and ankle tendinitis in ballet dancers.

The lateral ligament complex of the ankle is the most frequently injured structure in the body. Although most simple ankle sprains do not result in long-term disability, a significant number do not completely resolve, leading to residual symptoms that may persist for years. The most commonly reported symptoms, particularly among athletes, include instability, re-injury, and tendinitis. Ballet dancers are a combination of artist and high-performance athlete; consequently, they are subjected to the same types of injuries as other athletes, including lateral ankle sprains and their sequelae. Accessory navicular bone: when ankle pain does not originate from the ankle.

A young girl suffering from ankle pain occurring after gymnastics classes was referred to the rheumatology department by an orthopedic surgeon because a rheumatological condition was suspected to cause her symptoms. MRI was useful in pointing to the correct diagnosis of accessory navicular bone (AN). The morphological classification of ANs is discussed and the imaging modalities for diagnosis are presented.

Relationship between ankle-foot swelling and self-assessed function after ankle sprain.

PURPOSE: Decreasing swelling after ankle sprain is a main focus of treatment, as it is believed that swelling is related to function. The purpose of this study was to evaluate the relationship between ankle-foot swelling and self-assessed ankle function in the early period after ankle sprain. METHODS: Thirty-six individuals were tested for ankle-foot volume and self-assessed ankle function within days of spraining their ankle. Volumetric measurements of the injured and uninjured ankle-foot segment were taken with a foot and ankle volumeter. Stepwise linear regression analysis was performed, w

Can Chronic Ankle Instability Be Prevented? Rethinking Management of Lateral Ankle Sprains.

OBJECTIVE: To pose the question, "Can chronic ankle instability be prevented?" The evaluation and treatment of chronic ankle instability is a significant challenge in athletic health care. The condition affects large numbers of athletes and is associated with reinjury and impaired performance. The management of acute injuries varies widely but in athletic training has traditionally focused on initial symptom management and rapid return to activity. A review of practice strategies and philosophies suggests that a more detailed evaluation of all joints affected by the injury, correction of hypom

Long-term Functional Results of Total Ankle Arthroplasty in Stiff Ankles.

BACKGROUND: Total ankle arthroplasty (TAA) is advocated over ankle arthrodesis to preserve ankle motion (ROM). Clinical and gait analysis studies have shown significant improvement after TAA. The role and outcomes of TAA in stiff ankles, which have little motion to be preserved, has been the subject of limited investigation. This investigation evaluated the mid- to long-term functional outcomes of TAA in stiff ankles. METHODS: A retrospective study of prospectively collected functional gait data in 33 TAA patients at a mean of 7.6 (5-13) years postoperatively used 1-way analysis of variance a

Hysteresis and stiffness of the lateral ankle complex in those with chronic ankle instability.

The role of mechanical laxity and viscoelastic tissue properties in chronic ankle instability (CAI) is unclear, but may influence repeated injury. The purpose was to determine if lateral ankle complex stiffness and hysteresis was altered in CAI individuals with and without mechanical laxity, compared to copers and uninjured controls. Thirty-five recreational athletes (19 females, 22.1 ± 2.7 years, 69.7 ± 15.7 kg, 168.4 ± 10.7 cm) were assessed for ankle injury history and self-reported instability. An instrumented arthrometer was applied and laxity, stiffness and hysteresis values were capture
