Blunt abdominal aortic trauma in children: case report.

Injuries to the abdominal aorta as a result of blunt trauma in children are extremely rare. We encountered one such injury and a review of the literature revealed seven additional cases. Lower extremity ischemia, abdominal bruit, or paraplegia may suggest that diagnosis at the time of injury. Delayed presentations are characterized by abdominal pain or a pulsatile mass, with or without bruit. A high index of suspicion with early aortography is indicated to diagnose blunt aortic trauma.

Traumatic rupture of ascending aorta and left main bronchus.

A 14-year-old male pedestrian was hit by a truck and admitted with respiratory distress and subcutaneous emphysema. Aortography revealed disruption of the ascending aorta. Bronchoscopy revealed rupture of the left main bronchus at the carina. Both lesions were repaired using a median sternotomy, cardiopulmonary bypass, and a transpericardial approach for the bronchial repair. To our knowledge, this is the first report of successful repair of a combined rupture of a major bronchus and the ascending aorta.

Echographic follow-up of an abdominal aortic aneurysm treated with a prosthetic bypass.

The history of a patient with two thoracoabdominal aneurysms and lumbar Pott's disease is reported. A bypass was implanted from the ascending to the abdominal aorta. In the postoperative course the patient experienced acute low back pain. Ultrasonography suggested patency of the vascular graft. Therefore the symptoms were attributed to the vertebral pathology, and so the correct treatment could be given. The utility of ultrasonography in the early postoperative course, when aortography is contraindicated, is stressed.

Imaging-Based Diagnosis of a Ruptured Isolated Dissecting Abdominal Aortic Aneurysm: A Case Report.

Background and Clinical Significance: Acute aortic dissection is the most common and most severe manifestation of acute aortic syndrome. An isolated dissecting aneurysm of the abdominal aorta is defined as a dissecting aneurysm distal to the diaphragm and is an extremely rare disease. Detection of an intimal flap between two lumens using different imaging methods is a definitive diagnostic sign of aortic dissection. A number of studies have validated ultrasound, including point-of-care ultrasound, as the standard initial imaging modality for the diagnosis of aortic dissection. Case Presentatio

Percutaneous pulmonary valve implantation in patients after Ross procedure: role of intravascular ultrasound.

Coronary compression exclusion during right ventricle outflow tract stenting is recommended and potential oversight may be fatal. Balloon inflation in right ventricle outflow tract with simultaneous aortography can be inconclusive or falsely negative. We present a case of 27-year-old male post Ross operation qualified for percutaneous pulmonary valve implantation. Neither of the conventional views obtained provided a definite exclusion of coronary compression, therefore an intravascular ultrasound of the left coronary artery before and during balloon inflation in right ventricle outflow tract

Imaging of thoracic aortic dissection.

Acute thoracic aortic dissection has a high mortality if untreated, so the diagnosis must be rapidly made if mortality is to be lowered significantly. Multiple imaging techniques are often used. This retrospective study from 1988 to 1993 assesses the usefulness in diagnosis of chest X-rays, computed tomography (CT) scanning, aortography, magnetic resonance imaging (MRI), trans-thoracic (TTE) and trans-oesophageal (TOE) echocardiography. Forty-two patients with a final clinical diagnosis of dissection were studied. The diagnosis was confirmed in 16 (13 at surgery and three at autopsy). Three di

Usefulness of transesophageal echocardiography in assessment of aortic dissection.

BACKGROUND: The value of transesophageal echocardiography in the assessment of patients with aortic dissection was studied. METHODS AND RESULTS: Group 1 (34 patients) represented all patients studied at our institution with this technique in whom aortic dissection was proven by aortography, surgery, or autopsy. Group 2 (27 patients) represented all patients studied with this technique at our institution in whom aortic dissection was excluded by aortography. Transesophageal echocardiography made a correct diagnosis of aortic dissection in 33 of 34 patients (sensitivity, 97%; specificity, 100%)

[25 cases of traumatic rupture of the thoracic aorta: current diagnostic elements].

Traumatic rupture of the descending thoracic aorta is lethal within 3 weeks in 95% of patients who do not undergo operation. In this series of 25 patients who were operated on, 84% have survived for 6 years and there have been no cases of paraplegia. The mechanism of injury is most important in the investigation of patients with traumatic injuries and must be sought either from the patient or from witnesses. A history of rapid deceleration (more than 60 km/h) following a highway collision was present in all our cases. Failure to wear seat-belts resulted in 70% of patients being ejected from a

Left pulmonary artery originating from the right coronary artery in a patient with pulmonary atresia and ventricular septal defect.

We describe a patient with pulmonary atresia and ventricular septal defect in whom the left pulmonary artery was originating from the dilated right coronary artery. Successful division of the left pulmonary artery from the right coronary artery and the connection with the right ventricular outflow tract was carried out with closure of the end of the origin. The preoperative accurate diagnosis of this anomaly with ascending aortography is the most important in the surgical management of patients with pulmonary atresia and ventricular septal defect.

Intralobar lung sequestration with systemic coronary arterial supply.

Pulmonary sequestration is a rare anomaly. An accurate pre-operative evaluation of its vascular supply is essential for the surgeon's operative approach. We describe here an intrapulmonary sequestration with vascular arterial supply via the left circumflex and the right coronary artery. This case demonstrates that if aortography is unrevealing, then a coronary source should be considered in the preoperative search for the arterial supply to a pulmonary sequestration. Moreover, pulmonary sequestration should be listed in the differential diagnosis of aberrant coronary arteries.

Coronary artery discrete aneurysms, ectasia, and occlusive disease ane extensive arteriomegaly. Correlation, constellation, or coincidence? A case report.

The case of a sixty-five-year-old man with discrete coronary aneurysms, associated with coronary artery ectasia, stenoses, and total occlusion of the left anterior descending artery, that resulted in an acute anterior myocardial infarction, is presented. Aortography and digital subtraction angiography revealed marked aortic and iliac elongation, tortuosity, and aneurysmal dilatation as well as dilatated femoral and popliteal arteries with aneurysmal dilatation in places. It is considered that the case reflects the broad spectrum of atherosclerosis manifestations.

Mesenteric vascular insufficiency and claudication following acute dissecting thoracic aortic aneurysm.

Mesenteric vascular insufficiency and claudication of the left leg were observed in a patient 6 weeks after intraluminal aortic prosthesis placement for acute type III thoracic aortic dissection. Aortography revealed double-channel deformity of the thoracoabdominal aorta and complete occlusion of the celiac axis, inferior mesenteric artery, and left common iliac artery. An aortobi-iliac interposition graft placed end to end with an additional limb to the common hepatic artery was curative.

Subclavian and axillary artery injuries.

Experience with 36 Black patients with disruption of the subclavian or axillary arteries is reported. The clinical presentation and operative approaches are described. The average time interval between injury and diagnosis of arterial injury was 10 days. Two case reports which illustrate the adverse consequences of delayed treatment are presented. Careful clinical assessment of penetrating injuries in the neck and shoulder region is necessary to exclude major arterial injury. Arch aortography is indicated if arterial injury is suspected.

Aldosteronoma coexisting with renal artery stenosis in secondary hypertension.

A case of secondary hypertension due to an aldosteronoma coexisting with renal artery stenosis is reported. Tumor resection resulted in an immediate but short-lived clinical relief. Follow-up aortography revealed progressive arterial stenosis and infrarenal aortic occlusion. Differential renal venous renin and renal scintigraphy suggested renovascular hypertension. It is necessary to search for a second curable lesion if hypertension remains uncontrollable after surgical correction of an apparent cause.

Traumatic rupture of the thoracic aorta: diagnosis on fibreoptic bronchoscopy.

The diagnosis of thoracic aortic rupture can be difficult, especially in the presence of other life-threatening injuries. We present a case in which the presence of major intraabdominal trauma shifted clinical attention away from the mediastinum. During bronchoscopy, which was performed to investigate high intraoperative airway pressures, unexpected tracheal and carinal compression were discovered. Thoracic aortography revealed aortic rupture distal to the left subclavian artery. Successful repair was subsequently achieved.

Ebstein's anomaly with non-patent ductus arteriosus aneurysm and bradycardiac atrial fibrillation in an 80-year-old man.

An 80-year-old man with Ebstein's anomaly and ductus arteriosus aneurysm is reported. He was admitted with bradycardiac atrial fibrillation and right ventricular failure. For the control of brady-arrhythmia, a permanent pacemaker was implanted. Two-dimensional echocardiogram revealed distal displacement of the septal tricuspid valve. Aortography and computed tomography showed ductus arteriosus aneurysm. This is the first report of the association of Ebstein's anomaly and non-patent ductus arteriosus aneurysm.

Repair of isolated, symptomatic, sinus of Valsalva aneurysm in a patient with Marfan's syndrome.

A 29-year-old man with Marfan's syndrome presented with sudden onset of central chest pain radiating to the back. Aortography revealed a massively dilated right sinus of Valsalva aneurysm, with moderate aortic regurgitation. Urgent surgery in the form of a Bentall procedure was successfully carried out. There was no evidence of either aortic rupture or dissection. Acute expansion of the aneurysm must have precipitated the symptoms. The patient was alive and well three months later.

Acquired atherosclerotic aortopulmonary fistula presenting as new-onset congestive heart failure.

A middle-aged patient with new-onset congestive heart failure, a loud precordial systolic murmur, and a widened mediastinum on chest radiography is described. Noninvasive studies in addition to the clinical examination suggested an unusual complication of atherosclerotic aortic disease, which was demonstrated at aortography and confirmed at surgery. This case stresses the importance of a careful bedside examination in addition to selective noninvasive studies in patients with new-onset or atypical congestive heart failure.

Anomalous systemic arterial supply to the basal segments of the lung presenting with a murmur.

A murmur was discovered on routine preschool physical examination of a 5 year and 7 month-old boy. A lower lobe lesion was detected by chest X-ray. Aortography demonstrated that a large systemic artery from the descending thoracic aorta supplied the basilar segments of the left lower lobe, which had no normal pulmonary arterial supply. A clinical diagnosis of intralobar sequestration of the lung was made, and left lower lobectomy was performed.
