Mapping the mortality niche of chronic lower respiratory diseases complicated by respiratory failure: a multiscale spatial analysis of environmental and social drivers.

BACKGROUND: Chronic lower respiratory diseases complicated by respiratory failure as a multiple cause of death, denoted as RF-CLRD, represent a critical public health challenge with significant spatial heterogeneity across the United States. Although bioclimatic factors, air pollution, and social vulnerability are known determinants of respiratory health, traditional global models assume these risk factors operate uniformly across space, obscuring their spatial non-stationarity and region-specific impacts. METHODS: We utilized county-level data from the contiguous United States (2010-2019) to

Likelihood of using a harm reduction vending machine among rural people who inject drugs in Appalachian Kentucky.

BACKGROUND: Harm reduction vending machines (HRVMs) that dispense safe injection equipment and other supplies have operated globally for more than 30 years, yet few operate in the U.S., particularly in the rural epicenters of drug-related harms. This study explores likelihood of using HRVMs and correlates thereto among people who inject drugs in rural Appalachian Kentucky. METHODS: Respondent-driven sampling and outreach were used to recruit participants who were age 18 or older, resided in an Appalachian Kentucky county, and had used opioids and/or injected drugs to get high in the past 30 d

Factors Associated With Myocardial Infarction in a Rural Population With Peripheral Arterial Diseases.

Peripheral arterial disease (PAD) studies in rural populations are limited. The incidence of myocardial infarction (MI) is higher in patients with PAD. This study examined the association between sociodemographic and clinical risk factors and MI in patients with PAD in Central Appalachia, comprising of 230 counties across six states in the United States. Data from electronic medical records of 13,455 patients with PAD were extracted from a large health system in Central Appalachia. Bivariate and logistic regression analyses were conducted. The final sample consisted of 5574 patients with PAD,

Barriers to and facilitators of buprenorphine dispensing for opioid use disorder: Evidence from focus groups in Appalachian Kentucky.

BACKGROUND: As efforts to increase buprenorphine prescribing continue to intensify, pharmacy dispensing plays an important role in access to essential medications for opioid use disorder. Existing survey research suggests stigma and regulatory concerns are barriers to pharmacists dispensing buprenorphine, but facilitators are less clear, as are in-depth perceptions of pharmacists regarding buprenorphine. OBJECTIVE: To assess barriers and facilitators to buprenorphine dispensing in Appalachian Kentucky using a series of focus groups with pharmacy employees. METHODS: Between September and Octo

Rural residency as a risk factor for severe maternal morbidity.

PURPOSE: The goal of this study was to evaluate how rural/urban status and other risk factors alter women's odds of severe maternal morbidity (SMM) at delivery. METHODS: This study used 48,608 Kentucky resident delivery hospitalization records from 2017. We used multiple logistic regression with interaction terms to evaluate the moderating effect of rural/urban residence with other risk factors. We reported adjusted odds ratios (aORs) and 95% confidence intervals (CIs) as measures for association with the outcome of SMM at delivery. FINDINGS: The percentage of delivery hospitalizations with

Emergency Department Burden of Mental Health and Substance Misuse After Hurricane Helene: Insights From Rural Appalachia.

Objectives. To quantify temporal changes in mental health and substance use emergency department utilization following Hurricane Helene (September 2024) in Western North Carolina and identify disparities across vulnerable populations. Methods. We conducted a cross-sectional analysis using emergency department surveillance data from Western North Carolina counties. We calculated incidence rate ratios (IRRs) with 95% confidence intervals (CIs) by comparing 3 recovery periods (immediate, early, short-term) with their corresponding calendar dates in 2023. Results. Alcohol-related visits increased

High Prevalence of Moyamoya Syndrome in Appalachia.

INTRODUCTION: Moyamoya is a chronic cerebrovascular condition of unclear etiology characterized by progressive occlusion of 1 or both internal carotid arteries with neovascular collateral formation. With both an idiopathic form (moya-moya disease) and congenital condition-associated form (moyamoya syndrome), it can cause ischemic and hemorrhagic stroke. Recent findings in Kentucky have challenged traditional estimates of its incidence in US populations. Using the Kentucky Appalachian Stroke Registry (KApSR), our aim was to further characterize its incidence as a cause of stroke and to understa

Evaluation of Secondary Overtriage at a Rural Level 1 Trauma Center.

OBJECTIVE: To retrospectively assess the prevalence of secondary overtriage (SO) within a rural regional Appalachian health care system. METHODS: Trauma registry data was extracted for all trauma activation transfer patients from 2017 to 2022. Transferred patients were then stratified into two groups, non-secondary overtriage (non-SO) or SO. Patients were considered SO if they met three criteria following transfer: an Injury Severity Score (ISS) of less than 15, no required operative intervention, and discharge within 48 hours of arrival. Descriptive statistics were compared for age, length o

Factors Influencing Quality of Life Among Family Caregivers Supporting Heart Failure Home Care in Rural Appalachia.

Rural caregivers of older adults with heart failure experience substantial caregiving burden and reduced quality of life. This cross-sectional study examined how caregiver characteristics, demographics, psychological variables (anxiety, depression, loneliness, spiritual well-being), and palliative care concerns predict quality of life among rural family caregivers. Conducted in rural Appalachia, the study included 68 informal caregivers providing in-home care to adults aged 50 or older with advanced heart failure. Quality of life was measured using the World Health Organization Quality of Life

Facilitating factors and barriers for use of medications to treat opioid use disorder (MOUD) among justice-involved individuals in rural Appalachia.

The purpose of this qualitative study is to assess facilitating factors and barriers for medications to treat opioid use disorder (MOUD) initiation among justice-involved individuals in one rural Appalachian community, as well as how those factors may differ across the three types of Food and Drug Administration (FDA) approved medications. Qualitative interviews were conducted with rural justice-involved individuals (N = 10) with a history of opioid use in the target community. Overall, participants demonstrated knowledge of the different types of MOUD and their pharmacological properties, but

Clinical Trial Perceptions Among Patients Enrolled in a Precision Oncology Protocol at an Appalachian-Serving Cancer Center.

OBJECTIVE: Low rates of clinical trial participation, with notable disparities among rural populations, slow cancer treatment advances. Precision oncology protocols are designed to alleviate trial access barriers by consenting patients to sequencing with results matched to national databases of available trials. Precision oncology protocols represent a potential avenue for supporting patient informed trial decision-making before they are offered a specific trial but little is known about clinical trial perceptions and intentions of patients who join such protocols. METHODS: We administered su

Association between Breastfeeding and Childhood Obesity: Analysis of a Linked Longitudinal Study of Rural Appalachian Fifth-Grade Children.

INTRODUCTION: Although breastfeeding is associated with improving numerous health outcomes for the child, its role in reducing childhood obesity is contested. Despite this controversy, both the CDC and the US Department of Health and Human Services promote breastfeeding as one of the strategies for reducing childhood obesity. Rural Appalachia has one of the highest rates of childhood obesity and low rates of breastfeeding, compared to rest of the nation. The aim of this study was to examine the association between breastfeeding and childhood obesity at 11 years in the rural Appalachian state o

Phylogenetic systematics of the colorful, cyanide-producing millipedes of Appalachia (Polydesmida, Xystodesmidae, Apheloriini) using a total evidence Bayesian approach.

Here, we provide an exemplar-approach phylogeny of the xystodesmid millipede tribe Apheloriini with a focus on genus-group relationships-particularly of the genus Brachoria. Exemplars for the phylogenetic analysis were chosen to represent the maximum breadth of morphological diversity within all nominal genera in the tribe Apheloriini, and to broadly sample the genus Brachoria. In addition, three closely related tribes were used (Rhysodesmini, Nannariini, and Pachydesmini). Morphological and DNA sequence data were scored for Bayesian inference of phylogeny. Phylogenetic analysis resulted in po

Comparison of Machine Learning Models for Colon Cancer Survival: Predictive Modeling Approach.

BACKGROUND: Colon cancer is a leading cause of cancer-related deaths worldwide, with survival influenced by risk factors, treatment type, and patient characteristics. Traditional statistical models, such as Kaplan-Meier curves, have been widely used to estimate survival probabilities. However, these models often have difficulty handling complex interactions, covariates, and nonlinear relationships between risk factors. Recently, machine learning (ML) techniques have emerged as promising tools for improving survival prediction by handling large covariates and capturing complex patterns. OBJECT
