[Clinical study of spinal intradural arachnoid cyst].

Spinal intradural arachnoid cysts (S-IAC) are rarely found and only 80 cases have been reported since 1915. We report two of them. A 68-year-old male had slowly progressive dull pain in both thighs for about 6 years prior to admission. Neurological examination and plain x-ray on admission showed no abnormality. Myodil myelography showed a S-IAC at T4 vertebral level. Surgical treatment of cyst wall excision resulted in success. The other case, a 39-year-old male suddenly noticed monoparesis and sensory disturbance of left leg in the morning. On admission were observed spastic monoparesis of th

Arachnoid Cyst in Middle Cranial Fossa With Intraorbital Cyst (Orbital Meningocele).

Intracranial arachnoid cysts are extra-axial non-enhancing cerebrospinal fluid (CSF) density lesions. These are usually incidental findings on radiological investigations. Usually, the patients with arachnoid cysts are asymptomatic until the cyst grows large while symptomatic patients present with headaches, seizures, and focal neurological deficits. The adjacent calvarial bone may show remodeling and scalloping. Magnetic resonance imaging (MRI) stands superior in soft-tissue contrast and multiplanar imaging in excluding other lesions from the arachnoid cyst. Arachnoid cysts follow CSF signals

Intracranial arachnoid cysts: Review of natural history and proposed treatment algorithm.

BACKGROUND: With a prevalence of 1.4%, intracranial arachnoid cysts are a frequent incidental finding on MRI and CT. Whilst most cysts are benign in the long-term, clinical practice, and imaging frequency does not necessarily reflect this. METHODS: A literature review was conducted searching the Medline database with MESH terms. This literature was condensed into an article, edited by a consultant neurosurgeon. This was further condensed, presented to the neurosurgery department at Princess Alexandra Hospital for final feedback and editing. RESULTS: This review advises that asymptomatic pati

Middle fossa arachnoid cysts and inner ear symptoms: Are they related?

BACKGROUND: Arachnoid cysts most frequently occur in the middle cranial fossa and when they are symptomatic, patients present with central nervous symptoms. Nevertheless, a large proportion of arachnoid cysts are incidentally diagnosed during neuroimaging in cases with nonspecific symptoms. REPORT OF CASES: The cases of two males with middle cranial fossa arachnoid cysts with nonspecific inner ear symptoms were retrospectively reviewed. The first patient presented with mild headache, nausea, vertigo, unsteadiness, and tinnitus on the left ear while the second patient's main complaint was left

CSF overdrainage in shunted intracranial arachnoid cysts: a series and review.

BACKGROUND: Cerebrospinal fluid (CSF) overdrainage in hydrocephalus is well recognized. Overshunting following cysto-peritoneal (CP) drainage in patients with arachnoid cysts (AC) is rarely documented. AIM: We report five patients with acquired Chiari malformation I and three with posterior fossa overcrowding due to excessive CSF drainage in shunted intracranial ACs. We review our observations and discuss the current knowledge on the pathogenesis and management of this complication. PATIENTS AND METHODS: The medical records of the eight patients were analyzed in regard to clinical manifestat

Retrocerebellar arachnoid cysts in siblings with mental retardation and undescended testis.

Arachnoid cysts comprise approximately 1% of all intracranial space-occupying lesions and usually occur sporadically. We report retrocerebellar arachnoid cysts in two male siblings with mental retardation and undescended testis, suggesting the possibility of a genetic basis for at least some cases of retrocerebellar arachnoid cysts.

Familial arachnoid cysts in association with autosomal dominant polycystic kidney disease.

Autosomal dominant polycystic kidney disease (ADPKD) is a systemic disorder well-known for its association with intracranial aneurysms. Recently, intracranial arachnoid cysts have also been reported to be associated with ADPKD. We describe a father and daughter who each has a posterior fossa arachnoid cyst and asymptomatic ADPKD. To our knowledge, this is the first report of familial occurrence of arachnoid cysts in association with ADPKD.

[Intracranial arachnoid cysts. A study of a series of 35 cases].

INTRODUCTION: Arachnoid cysts are cavities with a content similar to cerebrospinal fluid, frequently communicating with the subarachnoid space. They make up 1% of the intracranial space occupying lesions, and although typically seen in children (13%), they may be undiagnosed until the patient has become adult. PATIENTS AND METHODS: We review a series of 35 cases of congenital intracranial arachnoid cysts in children, which had been investigated in the Neuropaediatric Department during 1987 1999. The patients had had cranial CAT and MR, and transcranial echography studies. RESULTS: In 85.7% t

Arachnoid cysts: how do postsurgical cyst size and seizure outcome correlate?

Arachnoid cysts (ACs) are congenital cystic brain malformations associated with epilepsy. The purpose of this study was to determine the effect of surgical intervention of ACs on cyst size and seizure outcome. We reviewed the world's medical literature dealing with surgically treated ACs in epilepsy patients. Our study included only cases, in which the relationship between pre-and postoperative CT-size of the AC and seizure outcome was described. We also included six patients with ACs and epilepsy treated surgically at the University of Mainz. We analyzed postoperative AC size and seizure outc

Progesterone receptors in arachnoid cysts. An immunocytochemical study in 2 cases.

We report 2 cases of arachnoid cysts, one with a retrocerebellar and the other with a left temporal localization, in which immunohistochemical studies had been conducted. The results of the immunohistochemistry on the presence of carcino-embryonic antigen (CEA) and glial fibrillary acidic protein (GFAP), and of the scanning- and transmission electron microscopy revealed the cyst lining to be identical to subdural neurothelium. Progesterone receptors were found in the nuclei of cells lining the cyst, which also suggests the similarity of the cyst lining to arachnoid granulations and meningiomas

[The radiological findings in surgically and conservatively treated intracranial arachnoid cysts].

PURPOSE: To determine which appearances on CT or MRI are indications for surgical treatment of intracranial arachnoid cysts. METHOD: The images obtained from 26 patients treated either by surgery or conservatively were compared retrospectively. The features evaluated were the size of the cyst, evidence of a space-occupying lesion, cerebral abnormalities near the cyst and skull deformities. RESULTS: In 6 patients (23%), surgical treatment was indicated because of the presence of obstructive hydrocephalus. The presence of all other radiological findings did not differ significantly between the

Macrocephaly, dystonia, and bilateral temporal arachnoid cysts: glutaric aciduria type 1.

Two siblings presented with macrocephaly, psychomotor delay, and progressive dystonia. The initial diagnosis was of hydrocephalus and bilateral temporal cerebrospinal fluid collections. Following ventriculoperitoneal shunting, the patients showed only modest neurological improvement. Metabolic investigations performed later in the course of the disease disclosed increased levels of glutaric acid in the urine and decreased levels of serum carnitine, which were confirmatory of glutaric aciduria type 1. The association of macrocephaly, dystonia, and bilateral temporal arachnoid cysts, shown eithe

Different Neuroendoscopic Modalities for Management of Suprasellar Arachnoid Cysts: Comparative Study.

BACKGROUND: Arachnoid cysts account for approximately 1% of all intracranial mass lesions, with suprasellar arachnoid cysts (SACs) representing a rare subset. Hydrocephalus is the most common presentation, occurring in nearly 90% of the cases. Due to their rarity, no universal consensus exists regarding classification, optimal surgical management, or outcome assessment. OBJECTIVE: To compare outcomes of ventriculocystostomy (VC) and ventriculocystocisternostomy (VCC) in the endoscopic management of suprasellar arachnoid cysts. METHODS: This retrospective study reviewed patients with SACs who

Surgical outcomes for spinal arachnoid cysts and webs: a case series.

BACKGROUND: Spinal arachnoid cysts (SAC) and spinal arachnoid webs (SAW) are intradural pathologies, often presenting with pain and/or myelopathic symptoms. Reports on postoperative outcomes for SAC and SAW are scarce. This study aims to assess the surgical outcomes of SAC and SAW. METHODS: Patients who underwent surgical treatment for SAC or SAW at our institution between 2011 and 2023 were retrospectively reviewed. Demographic data, radiological findings, surgical management, and clinical outcomes were evaluated using the modified McCormick (mMC) scale. Outcomes were categorized as "favorab

Outcomes of neuroendoscopy for suprasellar arachnoid cysts: a single center experiences.

BACKGROUND: Arachnoid cysts are extra parenchymal, intra-arachnoid fluid collections of unknown origin, similar in content to cerebrospinal fluid. Suprasellar arachnoid cysts are a rarer localization resulting from abnormalities of the Liliequist membrane or cystic dilatation of the interpeduncular cisterna. We aimed to contribute to the literature by presenting a series of suprasellar arachnoid cyst cases with endoscopic intervention and long-term results. METHODS: This study analyzed radiographic data, clinical symptoms, surgical records, and videos of giant arachnoid cysts that underwent e

Understanding intracranial arachnoid cysts: a review of etiology, pathogenesis, and epidemiology.

Arachnoid cysts (AC) are one of the common pathologies encountered in pediatric neurosurgical settings. It comes to clinical attention more often incidentally but also more often than previously due to the wide availability of diagnostic imaging. However, the accurate incidence and prevalence remain unknown. There is an ongoing debate on symptomatology, indications of treatment, and best treatment modalities. In this review, we aimed to discuss the current general knowledge about the arachnoid cyst, focusing on the etiology, pathogenesis, and epidemiology. Specific findings related to location
