Comparison of approaches for surface vessel diameter and pulsatility quantification.

Cerebral blood vessels change diameter with each heartbeat, in response to oxygen demand, and during pathological states. To study these dynamics, vessel diameters are often quantified from intravital microscope images. We compare three different image-based vessel diameter measurement algorithms: one that uses an intensity threshold (cross section threshold (CST)), one that performs a Radon transform then calculates its full width at half maximum (FWHM), and a newly developed algorithm that leverages the intensity gradient (find image edges (FIE)). Using synthetic data, we find that the avera

Advantages of temporary venoatrial shunt using centrifugal pump during bidirectional cavopulmonary shunt.

Single-ventricle palliation without the use of cardiopulmonary bypass carries advantages that reduce systemic edema and inflammatory responses; however, simple clamping of the superior vena cava (SVC) without a temporary shunt leads to increase in cerebral venous pressure and subsequent decrease in cerebral blood flow during bidirectional cavopulmonary shunt (BCPS). We report our experience of BCPS, using a centrifugal pump-assisted temporary shunt. The criteria included an unrestrictive interatrial communication, the absence of atrioventricular valve regurgitation, and the existence of an ant

Left Ventricular Assist Device Implantation to the Left Subclavian Artery With Distal Banding: A Novel Technique for Hostile Chest.

Patients with hostile chests are often precluded from the use of left ventricular assist device therapies for advanced heart failure. We describe a technique for left ventricular assist device implantation using a left thoracotomy approach with outflow graft anastomosis to the left subclavian artery and placement of a distal band to the left subclavian artery. Distal banding of the left subclavian artery prevents over-circulation to the left upper extremity. This technique provides an alternative approach for left ventricular assist device implantation in patients who otherwise would be deemed

The artificial heart - progress, problems, and further development.

The state of the art in the development of artificial heart research based on more than 100 own animal experiments and information from the other groups working in the same field is laid out. Consideration is given to a variety of special problems. The use of biomaterials (blood compatibility and mechanical strength and durability), the flow pattern in the pumps (clot formation), and special hemodynamical effects are discussed as well as different driving systems and percutane energy transmission (avoidance of infection). Respective measurements show the hemodynamics in the systemic and pulmon

Interpolation and Imputation Strategies for Missing Segments in Continuous Pressure-Flow Cerebral Bio-Signals: A Systematic Scoping Review.

Objective: Continuous pressure-flow cerebral bio-signals are critical for monitoring cerebrovascular dynamics but are often disrupted by missing data segments caused by artifacts from a variety of sources. These missing segments refer to segments of the signal that do not contain any valid physiological data. Such interruptions fragment the signals, resulting in discontinuities that compromise their overall integrity. Therefore, reconstructing missing values and preserving signal continuity are essential for ensuring the stable computation of signal trajectories and the accuracy of derived cer

In vivo Endocrine Secretion of Prostacyclin Following Expression of a Cyclooxygenase-1/Prostacyclin Fusion Protein in the Salivary Glands of Rats Via Nonviral Gene Therapy.

Pulmonary arterial hypertension (PAH) is a progressive disease that culminates in right heart failure and death. Prostacyclin (PGI2) and its derivatives are effective treatments for PAH when administered as continuous parenteral infusions. This treatment paradigm requires medical sophistication, and patients are at risk for complications from an indewelling catheter; drug interruptions may result in rebound pulmonary hypertension and death. We hypothesized that the salivary gland can be repurposed into an endogenous production site for circulating PGI2 through the expression of a fusion protei

Deaths on board ships assisted by the Centro Internazionale Radio Medico in the last 25 years.

Data on occupational diseases of seafarers and of causes of death during their career are sparse. The causes of deaths on board ships assisted by Centro Internazionale Radio Medico (CIRM), the Italian Telemedical Maritime Assistance Service (TMAS) were reviewed by examining 29,146 files of patients treated from 1986 to 2010. In the 25 years, 383 deaths occurred (1.31%). Diseases of the circulation were the most frequent, followed by external causes such as accidents and violence, infectious and parasitic diseases, alcohol and drug addiction, respiratory system diseases. Stent Assisted Coil Embolization: the Treatment of Wide-necked, Dissecting, and Fusiform Aneurysms.

Stent assisted coil embolization is a useful therapeutic modality for wide-necked and geometrically difficult aneurysms, as the stent provide a buttress that allows for coil deposition while preventing coil haerniation into the parent vessel lumen, and placement of an endovascular stent within the parent artery across the aneurysm neck may divert the blood from the aneurysm inflow tract and promote intra-aneurysm stasis and thrombosis. We report herein a 3 patients treated with endovascular stent-assisted coil embolization for symptomatic or enlarging wide-necked, dissecting, and fusiform aneur

Use of a left ventricular assist device as a bridge to transplantation in a pediatric patient.

Despite many advances in heart transplantation and in mechanical circulatory support, the benefits of staged cardiac transplantation have not been extended to the pediatric transplant recipient, chiefly because implantable circulatory assist devices are still too large. Extracorporeal devices, however, can overcome this impediment. Here we report the 1st case, to our knowledge, in which an extracorporeal left ventricular assist device has been used in a child to support circulation prior to cardiac transplantation. The patient was a 9-year-old boy in New York Heart Association functional class

Delayed deformation of the Neuroform Atlas Stent during follow-up: Observational findings and clinical implications.

OBJECTIVE: The Neuroform Atlas Stent (NAS) has become one of the most widely used stents for stent-assisted coil embolization of wide-necked intracranial aneurysms, owing to its low-profile design, flexible deliverability, and compatibility with 0.017-inch microcatheters. However, data on its long-term durability are limited. The purpose of this study is to evaluate the incidence, angiographic features, and clinical significance of NAS deformation or crumpling that occurs during the follow-up period. METHODS: We retrospectively reviewed 174 patients with unruptured intracranial aneurysms who

Treatment of patients older than 60 years with symptomatic vertebrobasilar artery stenosis.

BACKGROUND: Vertebrobasilar artery stenosis is an important cause of ischemic posterior circulation strokes. This study aimed at evaluating the safety and efficacy of treatment including conservative therapy alone and conservative plus endovascular therapy for elderly patients with symptomatic vertebrobasilar artery stenosis. METHODS: Patients older than 60 years with symptomatic vertebrobasilar artery stenosis (> or = 50%) confirmed by cerebral angiography were enrolled. All of them were treated with medical therapy and some with additional stent-assisted angioplasty (the stenting subgroup).

Pre-clinical evaluation of a novel, pneumatic, ventricular assist device (Medos HIA-VAD) under pathophysiological conditions.

To evaluate a new cardiac assist system, the Medos HIA-VAD, we studied the effects of mechanical unloading on regional and global myocardial dysfunction. As a model for the regional temporary contractile dysfunction we chose an anesthetized, open chest preparation in sheep. We occluded the diagonal coronary artery for 15 minutes and reperfused for 90 minutes. Hemodynamic parameters and wall thickening were monitored. Unloading with the 60-ml Medos HIA-VAD was performed either during ischemia (group II) or during reperfusion (group III). The recovery of non-uniformity indicated by post-ejection
